Objetivou-se analisar o perfil dos usuários de um serviço de nutrição e a percepção dos mesmos sobre a qualidade do atendimento, em uma pesquisa quali-quantitativa, utilizando entrevistas semiestruturadas, conduzidas logo após o atendimento nutricional a 32 idosos de um programa para a terceira idade no município de Viçosa - MG. Predominou o sexo feminino, com idades entre 60 e 78 anos; a hipertensão arterial foi a doença de maior ocorrência; a maioria está satisfeita com o atendimento; 44,8% têm dificuldades para mudar a alimentação e 100% foram otimistas quanto ao resultado do acompanhamento nutricional; a maioria é portadora de doenças crônicas não-transmissíveis, possui autonomia, compreende o serviço como instrumento de auxílio na qualidade de vida e associa a satisfação com o atendimento a aspectos de cuidado e atenção. Estimular pesquisas com programas de nutrição na terceira idade se torna cada vez mais necessário, para que se busque qualidade nutricional e melhores estratégias de atendimento aos idosos. This study analyzes the profile of users of nutrition services and their perception about the quality of care, in qualitative and quantitative research, using semi-structured interviews, conducted shortly after the nutritional care to 32 elderly of a program for the aged in Viçosa city, state of Minas Gerais, Brazil. The majority were female, aged between 60 and 78 years old; hypertension was the most frequent disease; most were satisfied with the service; 44.8% reported some difficulties in changing their diet and 100% were optimistic about the outcome of counseling; most have chronic non-communicable diseases, are autonomous, understand the service as an instrument of quality of life and derives satisfaction from the care and attention. Encouraging researches on nutrition programs for the elderly becomes increasingly necessary, in order to seek nutritional quality and better strategies of care for the elderly.

OBJETIVOS: Investigar a habilidade de indicadores bioquímicos do perfil lipídico plasmático em identificar resistência à insulina (RI), avaliada pelo índice HOMA-IR (Homeostasis Model Assessment - Insulin Resistance). MÉTODOS: Foram avaliados 138 homens saudáveis (20-59 anos). Analisaram-se os seguintes indicadores bioquímicos do perfil lipídico: triglicérides (TG), colesterol total (CT), HDL-C, LDL-C, e as relações CT/HDL-C e TG/HDL-C. Considerou-se o percentil 75 como ponto de corte para o índice HOMA-IR. A análise estatística constou do cálculo do coeficiente de correlação de Spearman e da construção de curvas ROC, com o cálculo das áreas abaixo da curva (AUC). RESULTADOS: A relação TG/HDL-C (r = 0.334 e AUC = 0.724 ± 0.046 p 0,05). CONCLUSÃO: A relação TG/HDL-C apresentou melhor habilidade em identificar RI, representando um instrumento alternativo e de fácil acesso para a avaliação da RI na prática clínica, proporcionando intervenções de caráter preventivo de doenças na população do sexo masculino. PURPOSE: To investigate the effectiveness of biochemical indicators from the plasmatic lipid profile to identify the insulin resistance (IR), assessed by the HOMA-IR index (Homeostasis Model Assessment -Insulin Resistance). METHODS: 138 healthy men (20-59 years) were evaluated. The lipid profile biochemical indicators analyzed were the following: triglycerides (TG), total cholesterol (TC), HDL-C, LDL-C, and TC/HDL-C and TG/HDL-C ratios. The percentile 75 was considered as the cut-off point for IR. Statistical analysis consisted of Spearman correlation coefficient, ROC curves and calculation of the areas under the curve (AUC). RESULT: The TG/HDL-C ratio showed the strongest correlation and the greatest AUC (r = 0.334 and AUC = 0.724 ± 0.046, p < 0.001) respectively, followed by the HDL-C (r = -0.313 and AUC = 0.716 ± 0.052, p < 0.01), the TG (r = 0.261 and AUC = 0.048 ± 0.674, p < 0.01) and CT/HDL-C ratio (r = 0.259 and AUC = 0.655 ± 0.05, p < 0.01). Results for TC and LDL-C showed no statistical significant (p > 0.05). CONCLUSION: The TG/HDL-C ratio showed the greatest ability to identify IR, proved to be an alternative and easy access instrument to assess IR in clinical practice, therefore providing diseases prevention for the adult male population.

O objetivo deste estudo foi avaliar a composição centesimal e o perfil de ácidos graxos da carne de novilho precoce de quatro grupos genéticos: Nelore (R1), Nelore x Canchin (R2), Nelore x Limousin (R3) e Nelore x Aberdeen Angus (R4), alimentados com dieta normal (D1) ou por outra constituída por lipídios protegidos (D2). Determinaram-se os teores de umidade, proteína, lipídios totais, resíduo mineral fixo e o perfil de ácidos graxos. Não foram encontradas diferenças para os teores de umidade, proteína, lipídios totais, Ácidos Graxos Saturados (AGS) e Ácidos Graxos Monoinsaturados (AGMI). A dieta protegida ocasionou aumento do teor total de Ácidos Graxos Poliinsaturados (AGPI), quando comparada à D1. Por sua vez, animais que receberam D1 apresentaram maior teor de AGPI w-3 que os da D2. Verificou-se que as técnicas de nutrição animal utilizadas neste estudo garantiram um produto com maior teor de AGPI, característica esta desejável na saúde humana. The correlation between the increase in visceral fat and insulin resistance makes the sagittal abdominal diameter and the waist perimeter as potential tools for the prediction of insulin resistance. To assess the reproducibility of different measurements of the sagittal abdominal diameter and the waist perimeter and analyze the discriminating power of the measurements when predicting insulin resistance. A total of 190 adult males were studied. The sagittal abdominal diameter (smallest girth, larger abdominal diameter, umbilical level and midpoint between the iliac crests) and the waist perimeter (umbilical level, smallest girth, immediately above the iliac crest and midpoint between the iliac crest and the last rib) were measured at four different sites. Insulin resistance was assessed by the homeostasis model of assessment-insulin resistance (HOMA-IR) index. All measurements presented an intraclass correlation of 0.986-0.999. The sagittal abdominal diameter measured at the smallest girth (r=0.482 and AUC=0.739±0.049) and the waist perimeter measured at the midpoint between the last rib and the iliac crest (r=0.464 and AUC=0.746±0.05) presented the highest correlations with the HOMA-IR and the best discriminating power for HOMA-IR according to the ROC analysis (p

The increase in the prevalence of excess weight among people of increasingly younger age groups may lead to the early development of cardiovascular risks. To investigate the prevalence of the metabolic syndrome among young adults and the impact of birth conditions and nutritional status during adolescence on this disorder. One hundred individuals were evaluated at three different stages of life. By consulting the database of people enlisted in the army reserve military service, data were collected on weight, height, and waist circumference of all draftees of 1996, 1997, and 1999 in order to analyze their nutritional status during adolescence. Using the same database, information was sought at the maternity on their birth conditions and the adult individuals were identified. Anthropometric and biochemical data were obtained from these adults. The Metabolic Syndrome (MS) was classified according to ATP III NCEP. Body fat percentage was assessed with bioelectrical impedance and the statistical analysis was performed using the Sigma Stat 2.0 software. Prevalence of the MS was detected in 13% of the individuals. The birth conditions showed no relationship with the syndrome. Orientador: Valdemiro Carlos Sgarbieri Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Engenharia de Alimentos Resumo: Os objetivos desse trabalho foram investigar as propriedades imunomoduladoras e anticâncer apresentadas por concentrados de proteínas de leite bovino, com atenção especial às proteínas do soro. Para tal, foi estabelecida uma metodologia adequada para a produção de concentrados de caseínas e de proteínas de soro, que possibilitasse preservar ao máximo suas propriedades estruturais e funcionais. Os concentrados protéicos produzidos foram caracterizados quanto à composição centesimal e o perfil de aminoácidos. Para a obtenção do caseinato de sódio (CNa) e do concentrado protéico de soro ácido (CSA), empregou-se a precipitação da caseína no pH isoelétrico (pH 4,6, 20°C), usando-se ácido lático (85%). O soro ácido obtido foi concentrado por ultrafiltração, diafiltração e liofilização para obtenção do CSA. O caseinato foi obtido por neutralização da caseína isoelétrica (NaOH, pH 7-7,5) e desidratado em spray dryer. A obtenção do coágulo de caseína (CoC) e do concentrado protéico de soro doce foi por coagulação do leite desnatado e pasteurizado (quimosina, 34°C, 45 min.), usando-se coágulo comercial (Há-La, Christian Hansen Brasil). O coágulo de caseína foi lavado, neutralizado e seco em spray dryer. O soro doce foi concentrado e desidratado pelo mesmo processo do soro ácido. O emprego de tecnologias de membranas (ultrafiltração/diafiltração) permitiu a obtenção de concentrados protéicos de soro de leite com mais de 80% de proteínas, sem que ocorresse desnaturação que prejudicasse as propriedades imunológicas das proteínas mais termolábeis. Para monitorar a desnaturação protéica e as propriedades imunomoduladoras apresentadas por concentrados protéicos de leite bovino utilizou-se o teste de CFP (células formadoras de placas), segundo modificação da metodologia de CUNNINGHAM e SZENBERG (1968). Utilizaram-se camundongos isogênicos machos de 6-8 semanas, da linhagem A/J. Esse teste permite dosar a resposta imune humoral ou p ou primária. A técnica foi padronizada e a contagem de células formadoras de placa foi efetuada em suspensão de células de baço, 5 dias após a imunização, quando acontece o pico de resposta. Após a produção de cada lote de concentrado protéico os testes de CFP foram sistematicamente efetuados, encontrando-se bons resultados para os concentrados protéicos de soro obtidos. A irradiação do CSD com doses de 3 e 5kgy promoveu efeitos distintos na resposta imune, observando-se valores mais elevados de CFP/baço na dose menor de irradiação. A existência de propriedades imunomoduladoras e anticâncer em proteínas de soro de leite foi avaliada em comparação com a caseína e o isolado protéico de soja, comerciais. Utilizaram-se 132 fêmeas de camundongos da linhagem A/J, sendo 33 animais para cada tratamento, recebendo a dietas contendo 20% de proteína e 15% de lipídios (óleo de milho, banha de porco e gordura de coco), segundo AIN-76 (1977), durante 32 semanas. Como fonte de proteína foram utilizados o concentrado protéico de soro doce (CSD), o ImmunocalTM (IM), a caseína comercial (CC) e o isolado protéico de soja (IPS). Após 4 semanas em dieta, os animais foram injetados com azoximetano (5mg/Kg de peso corporal), durante 6 semanas consecutivas, para indução de câncer de cólon. Os animais foram sacrificados em 3 tempos diferentes, quando foram efetuadas as análises da resposta imune humoral (CFP) em baços, dosagens de glutationa nos fígados e monitoramento do desenvolvimento da doença (focos de criptas aberrantes - FCA), quantificação e análise histológica de tumores no cólon. Não houve diferença entre os ganhos de peso, para todos os tratamentos. Verificou-se correlação positiva entre as concentrações de glutationa no fígado e o número de células produtoras de anticorpos (CFP), no baço dos camundongos (r = 0,998 e r = 0,995), com 4 semanas e 30-32 semanas, respectivamente. Maior número de tumores foi observado nos camundongos alimentados com IPS (18,20 ± 5,46), seguido daqueles em dieta contendo CC (14,33 ± 3,43), diferindo estatisticamente (p < 0,05) do CSD e IM (9,33 ± 2,96 e 8,56 ± 3,09), respectivamente. Nas condições utilizadas neste trabalho, as proteínas de soro de leite ofereceram considerável proteção ao animal contra a indução de tumores, em relação às outras fontes protéicas analisadas Abstract: The objective of the present work was the investigation of immunomodulation and anticancer properties presented by bovine milk proteins with special attention to the whey proteins, in the form of whey protein concentrate (WPC). For such a task, adequate methodology was established for the production of concentrates of caseins and whey proteins, which permitted maximum preservation of their structural and functional properties. The protein concentrates produced were characterized for percentual components composition and their amino acid profiles. For the obtention of sodium caseinate (NaC) and of acid whey protein concentrate (aWPC) precipitation of casein at the isoeletric pH (pH 4.6, 20°C) was used. The acidic whey was concentrated by ultrafiltration/diafiltration and liophilization to obtain the WPC. The caseinate was obtained by neutralization of the isoeletric casein clot with sodium hydroxide solution, then dehydrated in spray dryer. Coagulated casein (CoC) and the ¿sweet¿ whey were obtained by treating defatted and pasteurized milk (72°C , 20 sec) with commercial preparation of coagulum (Há-La, Chr Hansen Brasil) containing chymosin, for approximately 45 min at 34°C. Coagulum and ¿sweet¿ whey were separated by filtration. The coagulum was washed several times with water, homogenized (hammer mill and colloidal mill), neutralized and the suspension was spray dried. The whey was concentrated by ultrafiltration, submitted to diafiltration, freeze-dried and lyophilized. Whey protein concentrates (WPC) containing 80-85% protein was obtained with minimum denaturation of the most labile proteins. Protein denaturation was monitored by solubility tests in aqueous solutions at pH 4.6. Immunomodulating activity of the WPC was monitored by the plaque forming cells method (PFC), according to modification of Cunningham and Szenberg (1968) procedure. Isogenic male mice of the A/J line (6-8 weeks of age) were used in the tests. The test measures humoral or primary immune response by the spleen cells. The technique was standardized and the PFC counting was done in spleen cells suspension five days after immunization when the immune response was at maximum. PFC tests were performed for each lot of WPC produced. The immunomodulating and anticancer properties of WPC and WPI (IM) from cows milk were studied in comparison with casein and soy protein isolate, commercial preparations. One hundred and thirty two A/J female mice were used in the experiments, 33 for each treatment. They were maintained in isolators and fed diets containing 20% to protein and 15% lipids (corn oil, lard and cocoa fat), modified according to AIN-76 (1977), during 32 weeks. The following protein sources were utilized: whey protein concentrate (WPC), ImmunocalTM (IM), commercial casein (CC) and soy protein isolate (SPI). After 4 weeks on the diets the animals were subcutaneously injected with azoxymethane (5mg/kg bw) for 6 consecutive weeks, for colon cancer induction. The animals were sacrificed at 3 different time intervals, when spleen immune response (PFC), liver glutathione concentration, monitoring of carcinogenesis (aberrant crypt foci, ACF), quantification and histological analysis of colon tumors were evaluated. No statistical differences were found in body weight gain among treatments. A strong positive correlation was found between liver glutathione concentration and antibody producing cells in the spleen (PFC), r=0.998, r=0.995, after 4 weeks and 32 weeks, respectively, ther beginning the experiment. The largest number of tumors was found in the animals fed the SPI diet (18.20 ± 5.46), compared to CC (14.33 ± 3.43) and the groups fed WPC and IM, respectively 9.33 ± 2.96 and 8.56 ± 3.09. No statistical difference was found between WPC and IM and both were superior and statistically different from CC and SPI. This study aimed to evaluate the possibility of occurrence of burnout syndrome in high school teachers statewide, in Viçosa - MG, and its correlation with the working conditions, health and nutritional status of these teachers. In addition to identifying the signs of the syndrome, we sought to evaluate the environmental, technological and organizational workplace, nutritional status and health of teachers. We conducted an epidemiological study qualitative and quantitative, cross-sectional sample was 100 teachers, who subsequently underwent changes due to the difficulty of scheduling and availability of the volunteers. For sample characterization we used a questionnaire and demographic information about work and health of research participants. The assessment of working conditions was made by means of ergonomic work analysis proposed by Guerin (2001). Were also carried out noise, temperature and relative humidity measurements. Noise was evaluated using a sound pressure level DEC 460, while lighting, temperature and relative humidity were assessed through a digital thermo-higro-anemometer luxmeter. Evaluated were pain complaints by questionnaire, using the body chart proposed by Corlett & Manenica (1980), and the height reached by the arm above the head to identify the vertical reach of the hand necessary for the writing table in the room class. The risks for musculoskeletal disorders from workrelated upper limb were evaluated by means of check-list proposed by Couto (2000). The shifts made during the working day were recorded by pedometer. We identified the burden of teaching materials transported by teachers at work using a digital scale accurate to 5 g. The presence of burnout syndrome was assessed by applying the Maslach Burnout Inventory, with a version translated into portuguese. The weight and body composition were assessed by bioimpedance bipolar and its height measured using a portable anthropometer. The body mass index was used to classify nutritional status, according to data obtained from weight and height, and the fat percentage estimated according to the manual of the equipment manufacturer. The waist perimeter runs obtained by measuring the waist at the midpoint was used to identify metabolic risks associated with obesity. To assess the quality dietary survey was conducted the frequency of food consumption. In biochemical evaluation, lipid profile, fasting blood glucose and full blood count were evaluated by means of biochemistry laboratory. For clinical evaluation was performed to evaluate blood pressure through blood pressure gauge, certified by the Brazilian Society of Hypertension, and the evaluation of cardiovascular disease risk based on Framingham risk score. Statistical analysis was performed using the SPSS 17.0 software, Minitab 14.0, Epi Info 6.0 and Excel 2007. Tests were used: Kolmogorov-Smirnov, chi-square, Fisher and Mann-Whitney test. The Spearman correlation was observed using a qualitative assessment of Callegari-Jacques (2003). In the sample found a prevalence of female teachers, with a mean of 40.4 ± 10.4 years, mostly married with children. Less than half of teachers or 46% showed complete specialization, while 40% do not have post-graduation. About 50% were under 10 years of service, and more than half were effective through public tender. More than half of teachers ministered over 18 weekly classes. The health check-ups showed a higher prevalence among women. Hoarseness after the work was reported mainly among women. The number of days leave to health care among women showed a decrease in the years 2008 and 2009. Noise, lighting, air humidity and temperature prevailed with inappropriate values in the classroom. The moisture had a strong correlation with temperature. The presence of lower limb pain was especially high among women. The risk for musculoskeletal disorders in upper limbs was not statistically significant. Among teachers evaluated, we identified an average distance of 1.43 + 0.5 km, with an average of 2281.9 + 878.3 steps in a work shift. The median transport weight reached 4,340 kg and burnout syndrome had a high score for depersonalization especially among men. Emotional exhaustion showed a statistically significant positive association with the fact that teachers have graduate and the number of classes over 18. Depersonalization was associated in both sexes. The regular noise correlated withe motional exhaustion, which also correlated with depersonalization regular and weak correlation with personal accomplishment. The age and length of service correlated with regular depersonalization, while the number of weekly classes correlated with emotional exhaustion on a regular basis. It is notable prevalence of overweight among men. Waist circumference and BMI were strongly correlated among them, besides regular correlation with the percentage of fat. Unhealthy BMI was associated with inadequate waist circumference and body fat percentage. As for food habits, there is among teachers regular consumption of fruits, vegetables, milk, rice, beans, bread and coffee. Association was found between sex and HDL-C in women showed higher frequencies of fitness than men, as did the relationship LDL-c/HDL-c. The prevalence of 8.6% of hypertension among teachers. Waist circumference showed greater distribution to very increased risk among women, which includes a potential risk group for cardiovascular disease due to accumulation of body fat located in the abdominal region. BMI change and the level of HDL-C were associated with inappropriate high level of depersonalization. Therefore, it was found that the burnout syndrome was present among the teachers in this study, especially regarding the item depersonalization, had the attention of government officials and schools, and teachers themselves. The health and nutritional status was inadequate in the sample plots, requiring a larger health care. The environmental conditions, organization and technology must be followed to maintain proper working conditions with teachers, with attention being paid to physical and mental health in the workplace. O presente trabalho objetivou avaliar a possibilidade de ocorrência da Síndrome de Burnout em professores do ensino médio estadual, na cidade de Viçosa MG, e sua correlação com as condições de trabalho, estado nutricional e de saúde desses professores. Além de identificar os sinais da síndrome, buscou-se avaliar as condições ambientais, tecnológicas e organizacionais no ambiente de trabalho, o estado nutricional e de saúde dos professores. Realizou-se um estudo epidemiológico quali-quantitativo, transversal, cuja amostra inicial foi de 100 professores, que posteriormente sofreu alterações devido à dificuldade de agendamento e disponibilidade de parte dos voluntários. Para a caracterização da amostra utilizou-se de questionário sociodemográfico e informações sobre trabalho e condições de saúde dos participantes da pesquisa. A avaliação das condições de trabalho fez-se por meio da Análise Ergonômica do Trabalho proposta por Guérin (2001). Também foram realizadas medições de ruído, temperatura e umidade relativa do ar. O ruído foi avaliado utilizando um Medidor de Nível de Pressão Sonora DEC 460. Já a iluminação, a temperatura e a umidade relativa do ar foram avaliadas por meio de um termo-higro-anemômetro luxímetro digital. Avaliaram-se queixas de dores por meio de questionário, utilizando o diagrama corporal proposto por Corlett & Manenica (1980), além da altura alcançada pelo braço acima da cabeça para identificar o alcance vertical da mão, necessário para a escrita no quadro em sala de aula. Os riscos para distúrbios músculo-esqueléticos de membros superiores relacionados ao trabalho foram avaliados por meio de check-list proposto por Couto (2000). Os deslocamentos realizados durante a jornada de trabalho foram registrados por meio de pedômetro. Identificou-se a carga de material didático transportada pelos professores durante o trabalho com a utilização de uma balança digital, com precisão de 5 g. A presença da Síndrome de Burnout foi avaliada por meio da aplicação do Maslach Burnout Inventory, com versão traduzida para o português. O peso e a composição corporal foram avaliados por meio da bioimpedância bipolar, sendo a altura aferida com a utilização de antropômetro portátil. O índice de massa corporal foi utilizado para classificar o estado nutricional, de acordo com os dados de peso e altura obtidos, sendo o percentual de gordura estimado segundo dados do manual do fabricante do aparelho. O perímetro da cintura obtido pela medida executada no ponto médio da cintura foi utilizado para identificar riscos metabólicos associados à obesidade. Para avaliação da qualidade dietética foi realizado o inquérito de frequência do consumo alimentar. Na avaliação bioquímica, o perfil lipídico, a glicemia de jejum e o hemograma completo foram avaliados por meio de exame bioquímico laboratorial. Para avaliação clínica foi realizada a aferição da pressão arterial por meio do medidor de pressão arterial, certificado pela Sociedade Brasileira de Hipertensão, além da avaliação do risco de doenças cardiovasculares com base no Escore de Risco de Framingham. As análises estatísticas foram realizadas com a utilização dos softwares SPSS 17.0, MiniTab 14.0, Epi Info 6.0 e Excel 2007. Foram utilizados os testes: Kolmogorov-Smirnov, Qui-Quadrado, Fisher e Mann-Whitney. A correlação de Spearman foi analisada através da avaliação qualitativa de Callegari-Jacques (2003). Na amostra encontrou-se prevalência de professoras, com média de 40,4 + 10,4 anos, sendo a maioria casada e com filhos. Menos da metade dos professores ou 46% apresentou especialização completa, enquanto 40% ainda não possuem pós-graduação. Cerca de 50% estava com menos de 10 anos de serviço, sendo que mais da metade era efetiva por meio de concurso público. Mais da metade dos professores ministrava mais de 18 aulas semanais. O controle de saúde periódico apresentou maior prevalência entre as mulheres. A rouquidão após o trabalho foi relatada principalmente entre as mulheres. O número de dias de licença para tratamento de saúde demonstrou queda entre as mulheres nos anos de 2008 e 2009. O ruído, a iluminação, a umidade relativa do ar e a temperatura prevaleceram com valores inadequados nas salas de aula. A umidade apresentou forte correlação com a temperatura. A presença da dor nos membros inferiores foi elevada principalmente entre as mulheres. O risco para distúrbios músculo-esqueléticos nos membros superiores não apresentou significância estatística. Entre os professores avaliados, identificou-se uma distância média percorrida de 1,43 + 0,5 Km, com média de 2281,9 + 878,3 passos, em um turno de trabalho. A mediana do transporte de peso chegou a 4,340 Kg. A Síndrome de Burnout apresentou alto score para despersonalização principalmente entre os homens. A exaustão emocional apresentou associação positiva estatisticamente significativa com o fato dos professores possuírem pós-graduação e com o número de aulas superior a 18. A despersonalização apresentou associação em ambos os sexos. O ruído apresentou correlação regular com a exaustão emocional, a qual também apresentou correlação regular com a despersonalização e correlação fraca com a realização pessoal. A idade e o tempo de serviço apresentaram correlação regular com a despersonalização, enquanto o número de aulas semanais correlacionou-se com a exaustão emocional de maneira regular. Destaca-se predomínio do sobrepeso entre os homens. O perímetro da cintura e o IMC apresentaram forte correlação entre eles, além de correlação regular com o percentual de gordura. O IMC inadequado associou-se com inadequado perímetro de cintura e percentual de gordura. Quanto ao hábito alimentar, há entre os professores consumo regular de frutas, hortaliças, leite, arroz, feijão, pão e café. Constatou-se associação entre sexo e níveis de HDL-c, nas mulheres apresentando maiores frequências de adequação do que nos homens, o mesmo acontecendo na relação LDL-c/HDL-c. Observou-se prevalência de 8,6% de hipertensão arterial entre os professores. O perímetro da cintura apresentou maior distribuição para risco muito aumentado entre as mulheres, o que as inclui num potencial grupo de risco para doenças cardiovasculares, devido ao acúmulo de gordura corporal localizada na região abdominal. O IMC alterado e o nível do HDL-c inadequado apresentaram associação com o nível elevado de despersonalização. Portanto, constatou-se que a Síndrome de Burnout esteve presente entre os professores da amostra, principalmente quanto ao quesito despersonalização, merecendo atenção de gestores governamentais e das escolas, além dos próprios professores. O estado nutricional e de saúde esteve inadequado em parcelas da amostra, necessitando um maior cuidado com a saúde. As condições ambientais, organizacionais e tecnológicas precisam ser acompanhadas para manutenção de adequadas condições de trabalho junto aos professores, com atenção sendo dispensada para a saúde física e mental no ambiente de trabalho.

Fundação de Amparo a Pesquisa do Estado de Minas Gerais This study aimed to evaluate the ability of anthropometric indicators and body composition in predicting of blood pressure disturbance in adults in Viçosa, MG, and check the agreement between methods of assessing body composition. Was conducted cross-sectional study, whose sample consisted of 161 volunteers of both sexes, aged between 20 and 59 years. Anthropometric measures included weight, height, waist circumference (WC), hip and thigh and sagittal abdominal diameter (SAD). We calculated the waist-hip ratio (WHR), waist / height (WHtR) and waist / thigh (WTR) and conicity index (CI), the abdominal diameter (ADI) and body mass index (BMI). To check the total body fat percentage (%TBF) was used the tetrapolar bioelectrical impedance analysis (BIA). To compare the predictive methods of body fat (bipolar bioelectrical impedance (Tanita ®) and the equations of body fat estimates proposed by Deuremberg et al. (1991), Gallagher et al. (2000) and Lean et al. (1996)) was used BIA as the reference method. The blood pressure (BP) was measured using a sphygmomanometer, considering as BP disturbance the 75th percentile as the cut-off point. Statistical analysis was performed using the intraclass correlation analysis, analysis of variance with Tukey post-hoc test, the Kruskal-Wallis test with post hoc Dunn's, the correlation coefficients of Spearman and Pearson construction of ROC curves (Receiver Operating Characteristic Curve) and the analysis of Bland-Altman agreement. We identified 32.3% and 43.5% of men with disturbance SBP and DBP, respectively, and 37.4% and 43.4% of women with SBP and DBP above the 75th percentile, respectively. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior This study had as goal to evaluate the association between anthropometric and body composition measures and biochemical parameters in serum concentrations of inflammatory markers high-sensitivity C-reactive protein (hs-CRP) and fibrinogen in adult men. It was a cross-sectional study, realized in the Nutrition Section of the Division of Health, Universidade Federal de Viçosa, during May to November of 2008. 130 individuals (20 to 59 years) were assessed, having measurement of weight, height, waist (WP), hip and thigh perimeters, sagittal abdominal diameter (SAD) and coronal diameter (CD). The WP (umbilical level, midpoint between the iliac crest and last rib, the lower perimeter between the chest and hip and highest point of iliac crest) and SAD (umbilical level, point of greater abdominal diameter, lower waist circumference between the chest and hip , midpoint between the iliac crests) were measured at four different anatomical points. The body composition was assessed by electrical bioimpedance. Several anthropometric and body composition indicators were calculated and analyzed: indicator of central obesity (WP, SAD, CD, conicity index and waist/ height ratio), indicators of general obesity (body mass index, body fat percentage and fat mass in kg) and indicators of body fat distribution (waist/ hip ratio, waist/ thigh ratio and sagittal index). The biochemical parameters analyzed were complete blood count, lipid profile (total and fractions cholesterol and triglycerides), fasting glucose, uric acid and inflammatory markers hs-CRP and fibrinogen. The ratios total cholesterol/ HDL and LDL/ HDL were calculated. It was considered as the cutoff point for the hs-CRP values 0.12 mg/dL and for the fibrinogen it was used the 50th percentile of the evaluated sample. The statistical analysis consists in the analysis of intraclass correlation, Student s t test, Mann Whitney test, analysis of variance with Tukey s posthoc test, Kruskal-Wallis test with Dunn's post-hoc test, Spearman and Pearson s correlation coefficient and construction of ROC curves (Receiver Operating Characteristic Curve). The WP measured at lower perimeter of the chest and the hips showed itself different from the others anatomical points of measurement, while the SAD was similar for the four anatomical points assessed. The WP and the SAD presented high reliability, with intraclass correlation coefficients ranging from 0.998 to 0.999 (p < 0.001). Among the anatomical points assessed, the WP measured at lower perimeter of the chest and hip and the SAD measured at the umbilical level were the locations chosen to be used in the prediction of cardiovascular risk through changes in serum levels of hs-CRP and fibrinogen. Among the anthropometric and body composition indicators assessed the SAD (area under the ROC curve = 0.698 ± 0.049, p < 0.001) and the conicity index (0.658 ± 0.048, p < 0.01) were the most indicated in the assessment of cardiovascular risk, as they had greater ability to discriminate changes in serum levels of hs-CRP and fibrinogen, respectively. The cut-off points of 89.9 cm and 20.5 cm were the ones that reached largest sum between sensitivity and specificity values for the WP and SAD, respectively, showing therefore, to be more accurate in predicting cardiovascular risk through changes in serum levels of hs-CRP. Evaluating the traditional biochemical indicators, it was found that uric acid showed the best correlation (0.325, p < 0.01) and the largest area under the ROC curve (0.704 ± 0.054, p < 0.001), indicating more effectiveness in predicting high levels of hs-CRP. While the ratios total cholesterol/ HDL (0.222, p < 0.05) and LDL/ HDL (0.235, p < 0.01) showed the best correlations and largest areas under the ROC curve (0.624 ± 0.049 and 0.624 ± 0.049 p < 0.05) in the evaluation of higher serum levels of fibrinogen. It is suggested the use of anthropometric parameters (SAD and conicity index) and biochemical parameters (uric acid and the ratios total cholesterol/ HDL and LDL/ HDL) in predicting cardiovascular risk in clinical practice. However, it is relevant to assess the effectiveness of anthropometric and body composition indicators, and biochemical parameters evaluated in this study in other population segment in both sexes, since they may have different behavior depending on sex and the age range considered. O presente estudo teve como objetivo avaliar a associação entre medidas antropométricas e de composição corporal, e parâmetros bioquímicos com as concentrações séricas dos marcadores inflamatórios proteína C reativa de alta sensibilidade (PCR-as) e fibrinogênio em homens adultos. O estudo foi do tipo transversal, realizado no Setor de Nutrição da Divisão de Saúde da Universidade Federal de Viçosa, durante os meses de maio a novembro de 2008. Foram avaliados 130 indivíduos (20 a 59 anos), com aferição de peso, estatura, perímetros da cintura (PC), do quadril e da coxa, diâmetro abdominal sagital (DAS) e diâmetro coronal (DC). O PC (nível umbilical, ponto médio entre a crista ilíaca e a última costela, menor perímetro entre o tórax e o quadril e ponto mais alto da crista ilíaca) e o DAS (nível umbilical, ponto de maior diâmetro abdominal, menor cintura entre o tórax e o quadril, ponto médio entre as cristas ilíacas) foram aferidos em quatro pontos anatômicos diferentes. A composição corporal foi avaliada por bioimpedância elétrica. Foram calculados e analisados diversos indicadores antropométricos e de composição corporal: indicadores de obesidade central (PC, DAS, DC, índice de conicidade (ICO) e relação cintura/ estatura), indicadores de obesidade geral (índice de massa corporal, percentual de gordura corporal e massa gorda em kg) e indicadores de distribuição de gordura corporal (relação cintura/ quadril, relação cintura/ coxa e índice sagital). Os parâmetros bioquímicos analisados foram hemograma completo, perfil lipídico (colesterol total e frações e triglicerídios), glicemia de jejum, ácido úrico e os marcadores inflamatórios PCR-as e fibrinogênio. As relações colesterol total/ HDL e LDL/ HDL foram calculadas. Considerou-se como ponto de corte para a PCR-as valores 0,12 mg/ dL e para o fibrinogênio utilizou-se o percentil 50 da amostra avaliada. A análise estatística constitui-se da análise de correlação intraclasse, teste t de Student, teste de Mann Whitney, análise de variância com o teste post-hoc de Tukey, teste de Kruskall-Wallis com o teste post-hoc de Dunn s, coeficientes de correlação de Spearman e Pearson e construção de curvas ROC (Receiver Operating Characteristic Curve). O PC aferido no menor perímetro entre o tórax e o quadril mostrou-se diferente dos demais pontos anatômicos de aferição, ao passo que o DAS foi semelhante para os quatro pontos avaliados. O PC e o DAS apresentaram elevada confiabilidade, com coeficientes de correlação intraclasse de 0,998 a 0,999 (p < 0,001). Dentre os pontos anatômicos avaliados, o PC aferido no menor perímetro entre o tórax e o quadril e o DAS aferido ao nível umbilical foram os locais de escolha para serem utilizados na predição do risco cardiovascular por meio de alterações nos níveis séricos de PCR-as e fibrinogênio. Entre os indicadores antropométricos e de composição corporal avaliados o DAS (área abaixo da curva ROC = 0,698 ± 0,049; p < 0,001) e o ICO (0,658 ± 0,048; p < 0,01) foram os mais indicados na avaliação do risco cardiovascular, uma vez que apresentaram maior capacidade em discriminar alterações nos níveis séricos de PCR-as e fibrinogênio, respectivamente. Os pontos de corte de 89,9 cm e 20,5 cm foram os que atingiram maior soma entre os valores de sensibilidade e especificidade para o PC e o DAS, respectivamente, apresentando portanto, maior acurácia na predição do risco cardiovascular por meio de alterações nos níveis séricos de PCR-as. Avaliando-se os indicadores bioquímicos tradicionais, verificou-se que o ácido úrico apresentou a melhor correlação (0,325; p < 0,01) e a maior área abaixo da curva ROC (0,704 ± 0,054; p < 0,001), evidenciando maior eficácia para predizer níveis elevados de PCR-as. Enquanto que, as relações colesterol total/ HDL (0,222; p < 0,05) e LDL/ HDL (0,235; p < 0,01) mostraram as melhores correlações e maiores áreas abaixo da curva ROC (0,624 ± 0,049 e 0,624 ± 0,049; p < 0,05) na avaliação de maiores níveis séricos de fibrinogênio. Todavia, julga-se relevante a avaliação da eficiência dos indicadores antropométricos e de composição corporal, e dos parâmetros bioquímicos, avaliados neste estudo, em outros extratos populacionais e em ambos os sexos, uma vez que os mesmos podem apresentar comportamentos distintos dependo do sexo e da faixa etária considerada.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior The enteral nutrition consists in the administration of nutrients (food and/or nutritional supplements) into the gastrointestinal tract by using a probe when the oral ingestion is inadequate. Handmade preparations represent an option of enteral diet, which consist in solutions in which the foods are used freshly, or food products and/or modules of nutrients. The handmade enteral diets exhibit some advantages, such as possibility of changing the composition and the volume of nutrients depending on the patient needs, besides the lower cost, in comparison with the manufactured one. This study aimed at developing inexpensive handmade enteral formulations, with known chemical composition, control of stability, fluidity and osmolarity. In order to meet this goal, the foods were submitted to selection, hygiene, cooking (if required), liquefying, homogenizing and passage through a sieve to prepare the formulas. Four different formulations were prepared: porridges, fruit milkshakes, cocktails and preparations containing vegetables plus a source of protein. The osmolarity was assessed by cryoscopy. The fluidity of the formulations was evaluated considering their flow through pediatric probe (2.1 mm internal diameter) and the nutritional composition was determined using tables of chemical composition of foods. Fifty-two formulations were in a suitable range of fluidity, stability and osmolarity (< 450 mOsm/l). Among them, 4 were porridges, 12 milkshakes, 8 cocktails and 28 preparations containing vegetables. The vegetables-based preparations yielded the lowest values of osmolarity. The porridges yielded caloric density of approximately 1 Kcal/ml, which represents the highest value when compared with the other formulations. The milkshakes with fruits yielded osmolarity values lower than those found for porridges, but the caloric density was lower. The amounts of some ingredients were reduced to adjust the osmolarity of the formulations, such as orange or watermelon juice, flour milk powder and manufactured food supplements. The cocktails yielded very low caloric density, having carbohydrates as main source of energy, and representing a good source of fibers. The cocktails and the vegetables- based preparations are free of lactose. The formulations yielded overall good predictable content of fibers. There is difference in fluidity depending upon the food used, even from the same group. The handmade enteral preparations had at least 3.7 times lower cost than the manufactured powder products, representing a choice for patients who have lower purchasingpower to afford domestic nutritional support, thus reducing the duration of hospitalization and the risk of malnutrition after leaving hospital. A nutrição enteral consiste na administração de nutrientes (por meio de alimentos e ou suplementos nutricionais) através do trato gastrointestinal, com auxílio de sonda, quando a ingestão oral não está adequada. Alternativa de dieta enteral são as preparações artesanais, que consistem em soluções nas quais são utilizados alimentos in natura, produtos alimentícios e ou módulos de nutrientes. As dietas enterais artesanais apresentam vantagens, tais como a individualização da fórmula quanto à composição de nutrientes e ou volume, além de menor custo em relação à industrializada. Este trabalho objetivou elaborar fórmulas enterais artesanais, quimicamente definida, de baixo custo, com controle de estabilidade, fluidez e osmolalidade. Para tal procedeu-se a seleção dos alimentos, higienização, cocção (dos que requeriam este processo), liquidificação, homogeneização e passagem por peneira para produção das fórmulas. Foram elaborados 4 tipos de formulações: mingaus, coquetéis de fruta com leite, coquetéis e preparações à base de vegetais adicionadas de uma fonte protéica. A osmolalidade foi determinada pelo método crioscópico. A fluidez das fórmulas foi avaliada considerando seu fluxo por sonda naso-gástrica pediátrica de 2,1 mm de diâmetro interno e a composição nutricional foi obtida utilizando-se tabelas de composição química de alimentos. Obteve-se 52 formulações adequadas quanto à fluidez, estabilidade e osmolalidade (< 450 mOsm/ L), sendo 4 denominadas mingaus, 12 coquetéis de fruta com leite, 8 coquetéis e 28 preparações à base de vegetais. As preparações à base de vegetais foram as que tiveram menores valores de osmolalidade. Os mingaus apresentaram densidade calórica em torno de 1Kcal/ mL, o que representou a maior densidade calórica dentre todas as fórmulas. Os coquetéis de fruta com leite tiveram valores de osmolalidade menores que os mingaus, mas a densidade calórica apresentou-se mais baixa. As quantidades de alguns ingredientes foram reduzidas para adequar a osmolalidade das formulações, como suco de laranja, melancia, farinha láctea e suplementos alimentares industrializados. Os coquetéis apresentaram densidade calórica muito baixa, tendo como principal fonte energética os carboidratos, sendo estes também ricos em fibras. As preparações denominadas coquetéis e à base de vegetais são isentas de lactose. As formulações em geral apresentaram bons teores preditivos de fibras. Ocorre diferença de fluidez dependendo do alimento utilizado, mesmo sendo do mesmo grupo. As preparações enterais artesanais tiveram custo pelo menos 3,7 vezes menor que as dietas industrializadas em pó, representando uma alternativa aos pacientes com menor poder aquisitivo para receber suporte nutricional domiciliar, reduzindo assim o tempo de hospitalização e o risco de se desnutrirem após alta hospitalar.

Conselho Nacional de Desenvolvimento Científico e Tecnológico This study aimed to evaluate the effectiveness of anthropometric and body composition indicators and lipid profile biochemical indicators in predicting the HOMA-IR index in men. The study was conducted in a cross-sectional design, in which 138 healthy adults (20 - 59 years) were evaluated. The anthropometric evaluation consisted of determining weight, height, sagittal abdominal diameter (SAD) and waist (WP), hip and thigh perimeters. The WP and SAD were measured in four distinct anatomical sites. The body composition was assessed by bioelectrical impedance. The nine anthropometric and body composition indicators studied were analyzed by the type of obesity that is intended to assess: central obesity indicators (WP, SAD, conicity index and waist/height ratio), general obesity indicators (body mass index and body fat percentage) and body fat distribution indicators (waist/hip ratio, waist/thigh ratio and sagittal index). The lipid profile biochemical indicators examined were: total cholesterol (TC), HDL-C, LDL-C and triglycerides (TG). The ratios TC/HDL-C and TG/HDL-C were calculated. The HOMA-IR index (Homeostasis Model Assessment - Insulin Resistance), an indicator of insulin resistance (IR), was calculated by the formula: HOMA-IR = fasting insulin (μU/mL) x fasting plasma glucose (mmol/L)/22.5, considering for the analyses the percentile 75 as the cut-off point. Statistical analysis consisted of intraclass correlation, analysis of variance with Tukey post-hoc test, Kruskall-Wallis test with Dunn's post-hoc test, the Spearman and Pearson correlation coefficients and ROC (Receiver Operating Characteristic) curves. There was high reproducibility for all WP and SAD measures, with an intraclass correlation coefficient ranging from 0.986 to 0.999 (p < 0.001). The WP measured in the lower waist and the SAD measured in the largest diameter differed from other locations. Among the anatomical sites tested, the lower waist between the chest and hip, for the SAD, and the midpoint between the iliac crest and the last rib, for the WP, were the sites that showed the strongest correlations with HOMA-IR (r = 0.482 and 0.464, p < 0.001) and the largest areas under the ROC curves (0.716 ± 0.051 and 0.746 ± 0.049, p < 0.001) respectively, and therefore, showed better performance in predicting IR risk. Among the nine indicators of obesity tested, the WP and SAD were the most promising for assessing the IR risk. The values of 89.3 cm (sensitivity = 80% and specificity = 66%) for the WP and 20.0 cm (sensitivity = 77.1% and specificity = 68%) for the SAD were the cut-off points that showed the most accurate prediction for HOMA-IR higher levels. For the lipid profile biochemical indicators, it was found that the TG/HDL-C ratio presented the strongest correlation (r = 0.334, p < 0.001) with HOMA-IR and largest area under the ROC curve (0.724 ± 0.046, p < 0.001), resulting in better performance for the prediction of the HOMA-IR index. The use of these three indicators as instruments for the IR prediction in clinical practice is advisable. However, the necessity of a greater number of investigations about the performance of these indicators in the IR prediction in larger samples should be pointed out, reaching other extracts of the Brazilian population, including women, adolescents and elderly, which would allow the use of these IR indicators in population screenings and in clinical practice, in a standardized way, respecting our population s characteristics. O presente estudo objetivou avaliar a eficácia de indicadores antropométricos e de composição corporal e de indicadores bioquímicos do perfil lipídico plasmático em predizer o índice HOMA-IR em homens. Realizou-se um estudo de delineamento transversal, no qual foram avaliados 138 indivíduos adultos (20 59 anos) saudáveis. A avaliação antropométrica constou da determinação do peso, da estatura, do diâmetro abdominal sagital (DAS) e dos perímetros da cintura (PC), do quadril e da coxa. O PC e o DAS foram aferidos em quatro locais anatômicos distintos. A composição corporal foi avaliada por bioimpedância elétrica. Os nove indicadores antropométricos e de composição corporal estudados foram analisados segundo o tipo de obesidade que se propõe a avaliar: indicadores de obesidade central (PC, DAS, índice de conicidade e relação cintura/estatura), indicadores de obesidade geral (índice de massa corporal e percentual de gordura corporal) e indicadores de distribuição de gordura corporal (relação cintura/quadril, relação cintura/coxa e índice sagital). Os indicadores bioquímicos do perfil lipídico analisados foram o colesterol total (CT), o HDL-C, o LDL-C e os triglicérides (TG). As relações CT/HDL-C e TG/HDL-C foram calculadas. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior The present work analyzed available equations in the literature for height estimate and evaluation of the percentile of children's body fat from 6 to 10 years old. The sample consisted of 404 children from public schools, whose were eutrophic for the Body Mass Index /Age and Stature/Age of the Center for Disease Control and Prevention National Center for Health Statistics (CDC/NCHS 2000). To analyze the equations of height estimate, were collected height data, knee height with the sitting and lying position, leg length, arm height and length and ulna length of 404 children from 6 to 10 incomplete years old. For the equations of body composition, there were analyzed 119 children, whom it was obtained data of age, body height, weight, triceps, biceps, subscapular, suprailic and the calf skinfold and analyzed leg-to-leg (Tanita) and tetrapolar bioelectrical impedance, being this last one considered as reference method. The statistical analysis consisted of Coefficient of Correlation of Pearson, paired t-test, Test t of Student, calculation of the Standard Error of the Estimate and test of Bland-Altman. It was verified that the equations of stature estimate that better were correlated with the real stature of the appraised infantile population (r > 0,97; p 35 mm]}, and for the male sex the one that uses the triceps and calf skinfold [%GC = 0,735(PCT + PCPan) + 1,0]. However, it is worth to emphasize the need of validation of the equations of estimate of the percentile of corporal fat in comparison with more accurate methods of evaluation of the corporal composition, because the tetrapolar bioelectrical impedance is a method validated for the clinical practice and not for population studies. O presente trabalho analisou equações disponíveis na literatura para estimativa de estatura e avaliação do percentual de gordura corporal de crianças com idade entre 6 a 10 anos. Foram avaliadas 404 crianças matriculadas na rede pública de ensino, eutróficas pelos parâmetros Índice de Massa Corporal/Idade e Estatura/Idade do Center for Disease Control and Prevention National Center for Health Statistics (CDC/NCHS 2000). Para analisar as equações de estimativa de estatura, coletaram-se dados de estatura, altura do joelho com o indivíduo sentado e deitado, comprimento da perna, altura e comprimento do braço e comprimento da ulna de 404 crianças com idade entre 6 e 10 anos incompletos. Para as equações de composição corporal, foram avaliadas 119 crianças, das quais obtiveram-se dados de idade, estatura, peso, pregas cutâneas tricipital, bicipital, subescapular, supra-ilíaca e da panturrilha e avaliadas a bioimpedância bipedal (Tanita) e tetrapolar, última considerada método de referência. A análise estatística constou de Coeficiente de Correlação de Pearson, Teste t-pareado, Teste t de Student, cálculo do Erro Padrão de Estimativa e análise de Bland-Altmam. Verificou-se que as equações de estimativa de estatura que melhor se correlacionaram com a estatura real da população infantil avaliada (r > 0,97; p < 0,001), cujas diferenças entre as médias estimada e real não foram estatisticamente significativas, foram aquelas desenvolvidas por Chumlea et al. (1994) para as crianças de raça branca [Meninos: E=40,54 + (2,22.AJ); Meninas: E=43,21 + (2,15.AJ)], utilizando a medida da altura do joelho direito com o indivíduo sentado ou a altura do joelho esquerdo com o indivíduo deitado, obtida com auxílio de um paquímetro. É importante cautela em relação à classificação do estado nutricional utilizando-se índices a partir dessas estimativas, como o Índice de Massa Corporal/Idade, pois podem desencadear condutas inadequadas dos profissionais de saúde devido à classificação errônea das crianças como apresentando sobrepeso ou baixo peso. Assim, é importante a avaliação de outros parâmetros de avaliação do estado nutricional bioquímicos, dietéticos e clínicos - para efetuar o diagnóstico nutricional. Observou-se que dentre todas as equações de estimativa da composição corporal analisadas neste estudo, aquelas que apresentaram melhor correlação com a bioimpedância método de comparação utilizado - foram as equações de Slaugther et al. (1988) que utilizam pregas cutâneas tricipital, subescapular e da panturrilha como variáveis preditoras. Para o sexo feminino, recomendase a equação que utiliza as pregas cutâneas tricipital e subescapular {[%GC = 1,33 (PCT + PCSe) 0,013 (PCT + PCSe)2 2,5] se ∑ PCT + PCSe < 35 mm; e [%GC = 0,546 PCT + PCSe) + 9,7; se ∑ PCT + PCSe > 35 mm]} e para o sexo masculino a que utiliza pregas cutâneas tricipital mais a da panturrilha [%GC = 0,735(PCT + PCPan) + 1,0]. Entretanto, vale ressaltar a necessidade de validação das equações de estimativa do percentual de gordura corporal em comparação com métodos mais precisos de avaliação da composição corporal, pois a bioimpedância é um método validado para a prática clínica e não para estudos populacionais.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior An epidemiological study with cross sectional was carried out to diagnose the metabolic syndrome and its possible risk factors in adolescents who have already shown menarche. The sample consisted of 100 adolescents at the age range from 14 to 17 years, who were selected at public schools in Viçosa- MG. The following were the inclusion criteria: to be studying at high school have already shown the menarche for at least one year. Four criteria proposed for diagnosis of the metabolic syndrome in adults (WHO, 1998; EGIR, 1999; NCEP/ATPIII, 2001; IDF, 2005) were compared, as well as a criterion proposed for adolescents (Alvarez et al., 2006). Because most criteria are addressed to adults, the evaluation of the risk markers for the metabolic syndrome was accomplished by adaptations for the age range under study. The body composition parameters (weight, stature, IMC and derived indexes (IMCG and IMCLG), percent fat, fat mass and fatless mass (MG and MLG), circumferences of both waist (CC) and hip (CQ) and waist/hip relationship (RCQ)), as well as the biochemical ones (total and cholesterol and fractions, triglyceride, glucemia, fast insulin and homocysteine) and clinical ones (both systolic and diastolic blood pressure). The determination of the insulin resistance was based on insulin levels and fast glucemia by HOMA-IR. The evaluation of the nutritional state was based on the Body Mass Index (BMI), by using the cut points preconized by CDC/NCHS (2000). The percent body fat was estimate by the horizontal electric bioimpedance device and was classified according to Lohman (1992). The cut points were used for the dyslipidemia classification, preconized by the III Diretrizes Brasileiras sobre Dislipidemias (2001) and I Diretriz de Prevenção da Aterosclerose na Infância e na Adolescência (2005). The levels of the fast insulin and the insulin resistance by HOMA-IR were classified according to the I Diretriz de Prevenção da Aterosclerose na Infância e na Adolescência (2005). For the altered fast glucemia, the recommendation by American Diabetes Association (2006) was used. The arterial hypertension was characterized according to V Diretrizes Brasileiras de Hipertensão Arterial (2006). Two tools for dietary evaluation were applied: Questionnaire of the Alimentary Consumption Frequency (QFCA) and Twenty-four hour Recordatory (R24H). From the dietary analysis data, the following variables were evaluated: energy, proteins, carbohydrates, lipids, iron, C vitamin, calcium, fibers, monounsaturated fatty acids (AGMI), polyunsaturated fatty acids (AGPI), saturated fatty acids (AGS) and cholesterol. In relation to nutritional state, 83, 11 and 6% showed eutrophia (ME), overweight / overweight risk (RS/SP) and low weight (BP) (CDC/NCHS, 2000), whereas 61% showed high percent body fat (% GC) high. The total cholesterol presented the highest percent inadequacy (57%), followed by HDL (50%), LDL (47%) and triacylglycerols (22%). Inadequacy was observed in 11, 9, 5 and 4% respectively, in relation to HOMA-IR, insulin, blood pressure and glucemia. The syndrome prevalence ranged from 1-28%, depending on the criteria and cut points. The criterion by WHO (1998) adapted to the age range showed higher positive predictive values, whereas the criterion including all the components used in the different proposals showed higher sensibility and specificity values. Higher levels were observed for insulin, to HOMA-IR and for most body composition variables in the adolescents with excessive weight or body fat. With the increased HOMA-IR quartiles, an increase also occurred in the weight, IMC, IMCG, CC, CQ, MG, MLG, central and peripheral fat, % GC, triglyceride, VLDL, CT/HDL and glucose. Fundação de Amparo a Pesquisa do Estado de Minas Gerais Aiming at the investigation of the influence from either birth conditions and nutritional state in adolescence on the individuals' nutritional situation at adult life, a cohort-type study with retrospective and prospective data. was carried out. One hundred male individuals residing in Viçosa county- MG were evaluated at three life phases: at birth, at the final adolescence phase, and at beginning of the adult life. The selection of the sample was performed from the data file registers relative to the military enlistment for 1996, 1997 and 1998 in Viçosa county. The measures of weight, stature, and waist circumference (CC) at the adolescence phase were collected. Then, both weight and length data at birth were obtained in the registers of the unique maternity available in this county during the investigated years. The Roher´s Growth Index was calculated from those data. The gestational age was obtained, by applying the equation proposed by Branches in 1986. The evaluation of the birthweight relative to the gestation age was accomplished, by using the percentile 10 of the Williams curve preconized by WHO in 1995 as cutoff for the Intrauterine Growth Retardation (RCIU). The adult individuals were searched at the addresses contained in the data file of the Tiro de Guerra and/or by telephone directory from their own names or their parents´. From those adult individuals who accepted to participate in the study, the checking were accomplished for weight, stature, waist circumference (CC) and hip circumference (HC), from which IMC and its derivatives (fat IMC and fat-free IMC) and waits/hip relationship (RCQ), by using the cut-off points proposed by WHO (1998). The percent body fat (% GORD) was checked by electric bioimpedance, by considering the cutoff points proposed by Lohman in 1992. The bicipital (PCB), tricipital (PCT), subscapular (PCSE) and supra-iliac (PCSI) from which the central fat (PCSE + PCSI) and peripheral fat (PCB + PCT) were determined. To verify the biochemical profile, some blood samples were collected for analyzing the total cholesterol, triglycerides (TG), HDL, LDL, VLDL, cholesterol-T/HDL relationship, LDL/HDL relationship, and fasting glucemia. For investigating the energetic intake, three feeding registrations were used. As a result from the characterization of the adult life, the sample was found to be well distributed for per capita income. Concerning to the nutritional situation, the prevalence of excessive weight (33%) and percentage of high body fat (39%) were obtained. Relative to the biochemical analysis, 19% showed tolerance to the reduced glucose, whereas the low HDL (62%) was the most frequent alteration observed in the lipid profile. The variables at birth were also observed to be weakly correlated with the nutritional situation at both adolescence phase and adult life. Though, the newborns with birthlength above 50cm showed weight, IMC, RCQ, percent body fat, PCES and fat weight statistically superior to the other ones in the adulthood. In addition, a correlation was found between the Roher growth index and body fat percent in the adulthood, whereas those ones who were small at birth for the gestational age showed two-times higher risk to have excessive body fat (RR = 2.00; IC = 1.07 3.73). At the final adolescence, the nutritional state behaved as determinant factor of the adult individuals' nutritional situation. The correlation between weight and IMC was found in adolescence, as well as among all anthropometric variables in adulthood, except for stature and both systolic and diastolic blood pressure. From the relative risk analysis, the following results were found for those individuals presenting IMC above the percentile 85 of the IMC/age curve (CDC/2000) at adolescence: risk 3.67 times higher to be excessively weighty at adulthood (RR = 3.67; IC = 2.46 3.74); risk of 2.53 times higher to have increased percentage for body fat (IC = 1.71 3.74) and risk 9.17 times higher to have increased CC (IC = 4.52 18.6). A significant correlation between IMC at adolescence and lipidic profile in adulthood. Prevalence of the metabolic syndrome (SM) was also observed in 13% sample, that showed higher values for almost all anthropometrical variables as well as for both percent body fat and peripheral fat. Concerning to biochemical analysis, significant results were observed for HDL, VLDL, triglyceride, Cholesterol/HDL relationship and LDL/HDL relationship. It was also found that those individuals diagnosed with SM showed statistically higher values for weight, CC, IMC at adolescence phase. Thus, those results demonstrate the influence of the nutritional state during the adolescence phase as a determinant of the nutritional situation at adulthood and reinforces the need for more investigations on the relationship occurring between birth conditions and the subsequent nutritional state. Com o objetivo de investigar a influência das condições de nascimento e estado nutricional na adolescência sobre a situação nutricional dos indivíduos na vida adulta, procedeu-se um estudo do tipo de coorte, com dados retrospectivos e prospectivos. Foram avaliados 100 indivíduos do sexo masculino residentes na cidade de Viçosa MG, em três fases da vida: ao nascer, no final da adolescência e início da vida adulta. A amostra foi selecionada a partir dos registros do banco de dados referente ao alistamento militar de Viçosa MG, nos anos de 1996, 1997 e 1999, de onde foram coletadas medidas de peso, estatura e circunferência da cintura (CC) na adolescência. A partir daí, buscou-se nos registros disponível na única maternidade do município os dados sobre peso e comprimento ao nascer, a partir do qual calculou-se o índice de Crescimento de Roher. A idade gestacional foi obtida a partir da equação proposta por RAMOS (1986). A avaliação do peso ao nascer em relação à idade gestacional foi realizada tendo como ponto de corte para retardo do crescimento intra-uterino (RCIU) o percentil 10 da curva de WILLIAMS (1982), preconizada pela WHO (1995). Os indivíduos adultos foram procurados a partir dos endereços, contidos no banco de dados do Tiro de Guerra e,ou por lista telefônica a partir de seus próprios nomes ou de seus pais. Nos indivíduos adultos, que aceitaram participar do estudo, aferiu-se peso, estatura, circunferência da cintura (CC) e circunferência do quadril, a partir dos quais se calculou IMC e seus derivados (IMC gordura e IMC livre de gordura) e relação cintura/quadril (RCQ), utilizando os pontos de cortes propostos pela WHO (1998). O percentual de gordura corporal (% GORD) foi aferido por meio de bioimpedância elétrica, sendo considerados os pontos de corte propostos por LOHMAN (1992). Aferiu-se pregas cutâneas bicipital (PCB), tricipital (PCT), subescapular (PCSE) e supra-ilíaca (PCSI); a partir das quais se determinou gordura central (PCSE + PCSI) e gordura periférica (PCB + PCT). Para o perfil bioquímico, amostras de sangue foram coletadas para análise de colesterol total, triglicerídeos (TG), HDL, LDL, VLDL, relação colesterol-T/HDL, relação LDL/HDL e glicemia de jejum. Para investigação da ingestão energética, foram utilizados três registros alimentares. Como resultado da caracterização da vida adulta, encontrou-se que a amostra é bem distribuída quanto à renda per capita. Quanto à situação nutricional, obteve-se prevalência de excesso de peso (33%) e elevado percentual de gordura corporal (39%). Em relação à análise bioquímica, 19% apresentavam tolerância à glicose diminuída e quanto ao perfil lipídico, a alteração mais freqüente foi o HDL baixo (62%). Observou-se também que as variáveis ao nascer se correlacionaram de forma fraca com a situação nutricional na adolescência e na vida adulta. Os recémnascidos que apresentaram comprimento superior a 50 cm, entretanto, apresentaram peso, IMC, RCQ, percentual de gordura corporal, PCES e peso de gordura, na vida adulta, estatisticamente, superiores aos demais. Além disso, encontrou-se correlação entre o índice de crescimento de Roher e percentual de gordura corporal na vida adulta, sendo que os nascidos pequenos, para idade gestacional, apresentaram risco 2 vezes maior para excesso de gordura corporal (RR = 2,00; IC= 1,07 3,73). O estado nutricional no final da adolescência se comportou como fator determinante da situação nutricional de indivíduos adultos. Foi encontrada correlação entre peso e IMC na adolescência e todas as variáveis antropométricas na vida adulta, com exceção da estatura e pressão arterial sistólica e diastólica. A partir da análise de risco relativo, encontrou-se ainda que aqueles indivíduos, que na adolescência possuíam IMC acima do percentil 85 da curva de IMC/ idade (CDC/2000), apresentaram risco 3,67 vezes maior para excesso de peso quando adulto (RR= 3,67; IC = 2,46 3,74), bem como risco de 2,53 vezes maior de terem percentual de gordura corporal aumentado (IC = 1,71 3,74) e risco 9,17 vezes maior de terem CC aumentada (IC = 4,52 18,6). Obteve-se, ainda, correlação significante entre IMC na adolescência e perfil lipídico na vida adulta. Foi observada prevalência de síndrome metabólica (SM) em 13% da amostra, sendo que estes apresentaram valores superiores para quase todas as variáveis antropométricas avaliadas, bem como para o percentual de gordura corporal e gordura periférica. Quanto à análise bioquímica, resultados significantes foram observados em relação ao HDL, VLDL, triglicerídeo, relação colesterol/HDL e relação LDL/ HDL. Encontrou-se ainda que os indivíduos que foram diagnosticados com SM, apresentaram valores estatisticamente maiores de peso, CC e IMC, na adolescência, portanto, estes resultados demonstram a influência do estado nutricional na adolescência como determinante da situação nutricional na vida adulta, ressaltando a necessidade de maiores investigações sobre a relação existente entre condições de nascimento e estado nutricional posterior.

This study aimed to study the profile of pregnant women and newborn infants attended by the national health system SUS in Viçosa-MG and microregion, as well as to evaluate the predicting factors of weight at birth and placental characteristics of this population. This was a cross-sectional study using 244 puerperium women, their newborn infants (NB) and respective placentas, from October 2005 to March 2006. Delivery type, sex of NB infant, anthropometric data of puerperium women and NB infant as well as placenta weight were verified. The puerperium mothers were classified according to the pre-gestational Body Mass Index as low weight, normal weight, overweight and obese, according to the Institute of Medicine IOM (1990). The NB infants were classified according to weight at birth and gestational age, according to the World Health Organization (WHO) (1995). The Röhrer Index was used to evaluate NB weight/length ratio. The placenta with the umbilical cord and membranes were weighed according to the method described by Margotto (1992), and the placenta index was calculated (placenta weight /fetus weight ratio). A mean pre-gestational Body Mass Index (BMI) of 22.05 was observed within the normality limits. However, an expressive number of gestational women with low pre-pregnancy BMI were confirmed. As for ponderal gain during the course of pregnancy, 43.8% gained weight below the recommended, which was associated with NB low weight. It was observed that the pregnant women from the rural area had 3.73 more chance to have low weight newborn infants and those with per capita income lower than 1/3 of the current minimum wage had 4.35 times more chance to give birth to low weight NB infants. The pregnant women that had less than six prenatal appointments had 7.87 times more chance of occurrence of low weight at birth and those with low weight in previous pregnancies had 4.47 times more chance to give birth to low weight NB infants during the current pregnancy. It was confirmed that 38.37% of the NB infants had weight below the adequate average. Out of these, 8.98% were low weight and the others were underweight. Low weight at birth percentage found did not differ from the national average. Although most the NB infants (61.63%) presented a favorable weight (above 3.000 g), there was a great difference in this indicator when compared to countries such as Sweden, where 88.88% of the NB infants presented favorable weight according to Puffer and Serrano (1988). No abnormality was confirmed for the other anthropometrical data. As for the Rohrer index, 86.5% of the NB infants were within the normalcy range. Such normalcy was also confirmed when placenta weight and placenta index were analyzed. In the case of gestational age, the majority was of term and in the case of the preterm births, all were low weight. It was observed that 14% of the NB infants were small for gestational age or SGA (34 NB infants); 5.4% were big for gestational age or BGA (13 NB infants) and 80.6% were adequate for gestational age or AGA (195 NB infants). Thus, it was confirmed that a significant proportion of SGA NB infants suffered restriction in their intrauterine growth, with a slight predominance of the SGA group over the preterm group in the category low weight at birth. Factors such as height and pre-gestational weight were only associated with the SGA group and not with the preterm group, as expected, since nutritional factors are more associated with uterine growth restriction than with pregnancy duration. Associations with lower maternal ponderal gain, shorter maternal height, lower pre-pregnancy weight, fewer prenatal appointments, lower Rohrer indices and lower placenta weights were observed in the SGA group. It can be inferred that the association between lower placenta weight and SGA occurrence is true, since no shorter pregnancy duration occurred as confusion factor as in the preterm infants case, what may indicate loss of placenta function in this group. It was concluded that the factors associated with low weight at birth in Viçosa-MG were ponderal gain below the recommended during pregnancy, fewer prenatal appointments, shorter pregnancy duration, lower placenta weight, lower per capita income, low schooling, and lower Rohrer and placenta indices. Este estudo objetivou conhecer o perfil das gestantes e dos recémnascidos atendidos pelo Sistema Único de Saúde (SUS) em Viçosa-MG e microrregião, bem como avaliar os fatores preditores do peso ao nascer e as características das placentas nesta população. Trata-se de um estudo transversal realizado com 244 puérperas, seus recém-nascidos (RN) e respectivas placentas, durante o período de outubro de 2005 a março de 2006. Foram verificados tipo de parto, sexo dos RN, dados antropométricos da puérpera e do RN e peso das placentas. As puérperas foram classificadas de acordo com o Índice de Massa Corporal pré-gestacional, em gestantes de baixo peso, de peso normal, com sobrepeso e com obesidade, segundo o Institute of Medicine IOM (1990). Os RN foram classificados segundo o peso ao nascer e quanto à idade gestacional, de acordo com a Organização Mundial da Saúde OMS (1995). Utilizou-se o índice ponderal ou de Röhrer para avaliar a relação entre o peso e o comprimento do recém-nascido. A placenta foi pesada com cordão umbilical e membranas, conforme o método descrito por Margotto (1992), e, em seguida, calculou-se o índice placentário (relação peso placentário/peso fetal). Observou-se um índice de massa corporal (IMC) pré-gestacional médio de 22,05, dentro dos limites da normalidade. Entretanto, constatou-se um contingente expressivo de gestantes com baixo índice de massa corporal pré-gestacional. Quanto ao ganho ponderal durante a gestação, 43,8% ganharam peso abaixo do recomendado, o que foi associado com o baixo peso do recém-nascido. Observou-se que as puérperas provenientes da zona rural tinham 3,73 vezes mais chance de terem recém-natos de baixo peso e aquelas com renda per capita menor que 1/3 do salário-mínimo vigente tiveram 4,35 vezes mais chance de darem à luz a RN de baixo peso. As gestantes que fizeram menos de seis consultas durante o pré-natal tiveram 7,87 vezes mais chance de ocorrência do baixo peso ao nascer e aquelas com história de ocorrência de baixo peso em gestações anteriores tiveram 4,47 vezes mais chance de darem à luz a RN de baixo peso na gestação atual. Constatou-se que 38,37% dos RN possuíam peso abaixo da média adequada. Destes, 8,98% tinham baixo peso, e o restante, peso insuficiente. O percentual encontrado de baixo peso ao nascer, não diferenciou da média nacional. Embora a maioria dos recémnascidos (61,63%) apresentasse peso favorável (acima de 3.000 g), houve grande diferença desse indicador quando comparado com o de países como a Suécia, onde 88,88% dos RN apresentaram peso favorável, segundo Puffer e Serrano (1988). No caso dos outros dados antropométricos dos recém-nascidos, não se constatou nenhuma anormalidade. Em relação ao índice ponderal, 86,5% dos recém-natos se encontravam dentro da faixa de normalidade. Essa normalidade também foi constatada quando foram analisados o peso da placenta e o índice placentário. No caso da idade gestacional, a maioria foi a termo e, no caso dos prematuros, todos apresentaram baixo peso. Observou-se que 14% de RN eram pequenos para a idade gestacional ou PIG (34 RN); 5,4% de RN eram grandes para a idade gestacional ou GIG (13 RN) e 80,6% de RN eram adequados para a idade gestacional ou AIG (195 RN). Dessa forma, constatou-se uma proporção significativa de PIG ou RN que sofreu restrição no seu crescimento intra-uterino, havendo uma leve predominância do grupo de PIG em relação ao grupo de prematuros na categoria de baixo peso ao nascer. Fatores como altura e peso pré-gestacional só tiveram associação com os PIG e não com os prematuros, o que era esperado, uma vez que fatores nutricionais associam-se mais à restrição do crescimento uterino do que à duração da gestação. No grupo dos PIG, observaram-se associações com menor ganho ponderal materno, menor altura materna, menor peso pré-gestacional, menor número de consultas no pré-natal, menores índices ponderais e menores pesos das placentas. Pode-se inferir que a associação entre o menor peso placentário e a ocorrência de PIG é verdadeira, uma vez que não houve menor duração da gestação como fator de confusão, como no caso dos prematuros, o que pode indicar que houve prejuízo da função placentária neste grupo. Conclui-se que os fatores associados ao baixo peso ao nascer em Viçosa-MG foram ganho ponderal abaixo do recomendado durante a gestação, menor número de consultas durante o pré-natal, menor duração da gestação, menor peso da placenta, menor renda per capita, baixa escolaridade e menores índices ponderais e placentários.

This work had as general objective to study the nutritional state, the corporal composition, the aspects dietary, socioeconomic and of health of people with Down s syndrome, between 4 and 10 years, residents in Viçosa - MG. The study was composed by 3 groups, each one with 10 participants. Socioeconomic questionnaire was applied, of past health and relative subjects to the gestation and the individuals' birth. The weight, stature, was checked brachial perimeter, cutaneous pleats circumferences of the arm, waist and hip; blood pressure, and body composition for apparel of bioelectrical impedance. Reminding of 24 hours, selective readiness for olive oil, oil and sugar, and questionnaire of frequency of the alimentary consumption was used to evaluate the macronutrients ingestion, calories, vitamins, minerals, fibers and fatty acids. They were determined levels serics of total cholesterol and fractions (triacilglycerides, HDL, LDL); hormones of thyroid (T3 and T4) and TSH; hemoglobin, hematocrite and other indexes (VCM, HCM, CHCM); vitamin E for High Performance Liquid Chromatography (HPLC) and profile of fatty acids for gas chromatographic. The groups came homogeneous as for the social class and they didn't differ in relation to the age bone nor chronological. The people with trissomia of the 21 studied in the age group from 4 to 10 years presented larger amount of thin mass, however the location of the fat was predominantly central; it was not found in this population tied factor the genesis of the cardiovascular diseases, nor to the stress oxidativo, however, indications of fetal programming taking to the weight excess if it made present. A Síndrome de Down é uma mutação genética, resultante da tríplice cópia do cromossomo 21, com prevalência de um para cada 600 nascidos vivos. Este trabalho teve como objetivo geral estudar o estado nutricional, a composição corporal, os aspectos dietéticos, socioeconômicos e de saúde de portadores de Síndrome de Down, entre 4 e 10 anos, residentes em Viçosa MG. O estudo foi composto por 3 grupos, cada um com 10 participantes. Aplicou-se questionário socioeconômico, de saúde pregressa e questões relativas à gestação e nascimento dos indivíduos. Aferiu-se o peso, estatura, perímetro braquial, pregas cutâneas (tricipital, bicipital, subescapular e suprailíaca), circunferências do braço, cintura e quadril; pressão arterial, e composição corporal por aparelho de bioimpedância elétrica tetrapolar. Recordatório de 24 horas, disponibilidade seletiva para azeite, óleo e açúcar, e questionário de freqüência do consumo alimentar foram utilizados para avaliar a ingestão de macronutrientes, calorias, vitaminas, minerais, fibras e ácidos graxos. Determinaram-se níveis séricos de colesterol total e frações (triacilgliceróis, HDL, LDL); hormônios tireoidianos (T3 e T4) e TSH; hemoglobina, hematócrito e demais índices hematimétricos (VCM, HCM, CHCM); vitamina E por High Performance Liquid Chromatography (HPLC) e perfil de ácidos graxos por cromatografia gasosa. Os grupos apresentaram-se homogêneos quanto à classe social e não diferiram em relação à idade óssea nem cronológica. Os portadores da trissomia do 21 estudados na faixa etária de 4 a 10 anos apresentaram maior quantidade de massa magra, porém a localização da gordura foi predominantemente central; não foi encontrado nesta população fator ligado a gênese das doenças cardiovasculares, nem ao estresse oxidativo, contudo, indícios de programação fetal levando ao excesso de peso se fez presente.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior The study had as objective to compare the results, of the nutritional evaluation, from anthropometric and body composition measurements in adult men. The study was cross-sectional and conducted at the Division of Health of the Federal University of Viçosa during the months of February and October 2005. The sample it was constituted of 98 healthy men with age between 20 and 58 years. Anthropometric and body composition measurements were performed in the morning after the subjects fasted for at least 10h. All subjects abstained from vigorous exercise for at least 12h and from alcoholic beverage for least 48h. The anthropometric evaluation included measured of weight, height, circumferences and skinfolds. The body composition was evaluated by anthropometry-based equations, near infrared interactance (Futrex-5000®), legto-leg bioelectrical impedance (Tanita Tbf-531®) and tetrapolar bioelectrical impedance (Biodynamics model 310®). Tetrapolar bioelectrical impedance was used as a frame of reference for comparison with the other methods used in the evaluation of the body composition. Indirect estimates of weight and height were calculated using prediction equations based on knee height, arm span, half arm span. Statistical analysis included paired t-test for means, correlation coefficient analysis, test of Bland & Altman (1986) and Odds Ratio (OR). For all analysis, significance level was set at p 0.05. Este estudo foi realizado com crianças na faixa etária entre 4 e 9 anos, de ambos os sexos, portadoras de hipertrofia de adenoide, atendidas por otorrinolaringologista no Consórcio Intermunicipal de Saúde de Viçosa, MG, durante o período de setembro a dezembro de 2004. Teve como objetivos caracterizar os aspectos socioeconômicos, o perfil alimentar e o estado nutricional de portadores de hipertrofia de adenoide; identificar alterações no padrão alimentar de portadores de hipertrofia de adenoide; comparar a função mastigatória em crianças com e sem hipertrofia de adenoide; avaliar e comparar as características da respiração, deglutição e fala de crianças com e sem hipertrofia de adenoide. Para a avaliação socioeconômica, utilizou-se questionário semiestruturado visando obter informações sobre renda, estrutura familiar, escolaridade e ocupação dos pais das crianças. O diagnóstico de hipertrofia de adenoide foi feito utilizando o exame de nasofibroscopia, e a avaliação nutricional das crianças foi realizada usando os indicadores antropométricos Altura/Idade (A/I) e Peso/Idade (P/I), em que foi considerado o padrão de referência do National Center of Health Statistics (NCHS) de 2000 (CDC, 2000). Para a avaliação e comparação dos aspectos orofaciais e das funções estomatognáticas (avaliação fonoaudiológica), foi formado outro grupo de crianças com hipertrofia de adenoide. Ambos os grupos foram compostos por 24 crianças, correspondentes ao número de crianças com hipertrofia de adenoide recrutadas no período descrito. Durante a avaliação fonoaudiológica, as crianças foram avaliadas quanto aos aspectos orofaciais e às funções estomatognáticas. Foi observada alta prevalência de crianças com alterações nutricionais (66,7%) no grupo com hipertrofia de adenoide. A prevalência de desnutrição/risco de desnutrição encontrada foi de 33,3% para P/I, e a prevalência de sobrepeso/risco de sobrepeso encontrada foi de 33,3% para o mesmo índice. Verificou-se que a maioria das crianças (75%) realiza cinco ou mais refeições/dia e seus hábitos alimentares não diferiram muito do encontrado na população geral em relação à quantidade e qualidade, mas houve alta frequência de preferência por alimentos de consistência alterada. Houve significativo maior número de queixas do trato gastrointestinal no grupo de crianças com hipertrofia de adenoide. Todas as crianças do grupo com hipertrofia de adenoide apresentaram respiração oral ou mista, enquanto 83,3% das crianças sem hipertrofia de adenoide apresentaram respiração nasal. Houve maior prevalência de alterações de deglutição e fala nas crianças do grupo com hipertrofia de adenoide, enquanto se observou menor prevalência estatisticamente significativa de hábitos de sucção não nutritiva nesse grupo. Durante a mastigação, a maioria das crianças de ambos os grupos apresentou incisão anterior do alimento e mastigação bilateral alternada, e 91,7% das crianças do grupo com hipertrofia de adenoide realizaram a mastigação com lábios abertos. Houve maior frequência de alterações na velocidade (tempo) mastigatória nas crianças do grupo com hipertrofia de adenoide e 75% das crianças desse grupo utilizaram líquido durante a mastigação, enquanto somente 12,5% das crianças do outro grupo o fizeram. Houve diferença significante estatisticamente quando se comparou a presença de obesidade com o uso ou não de líquido durante a mastigação. Os resultados deste estudo evidenciam a importante prevalência de alterações no estado nutricional (principalmente tendência a sobrepeso/risco de sobrepeso) de crianças com hipertrofia de adenoide sem indicação cirúrgica. This study was conducted with children aged between 4 and 9 years old, both sexes, suffering from adenoid hypertrophy, seen by the otolaryngologist Intermunicipal Consortium of Health Viçosa, Minas Gerais, during the period September to December 2004. Aimed to characterize the socioeconomic, dietary profile and nutritional status of patients with adenoid hypertrophy; identify changes in the dietary pattern of patients with adenoid hypertrophy; compare the masticatory function in children with and without adenoid hypertrophy; evaluate and compare the characteristics of breathing, swallowing and speech of children with and without adenoid hypertrophy. Socioeconomic evaluation was conducted using semi-structured questionnaire to obtain information on income, family structure, education and occupation of parents of children. The diagnosis of adenoid hypertrophy was done using nasal endoscopy examination of the nutritional assessment of children was performed using anthropometric indices height/age (H/A) and weight/age (W/A), considering the standard reference the National Center for Health Statistics (NCHS), 2000 (CDC, 2000). For the evaluation and comparison of orofacial aspects and stomatognathic functions (speech evaluation) was formed another group of children adenoid hypertrophy. Both groups were composed of 24 children, which corresponded to the number of children with adenoid hypertrophy recruited in the period described. During the speech assessment, children were assessed for orofacial aspects and how to stomatognathic functions. High prevalence of children with nutritional changes (66.7%) in the group with adenoid hypertrophy was observed. The prevalence of malnutrition/risk of malnutrition was 33.3% for P/I, and the prevalence of overweight/risk of overweight was 33.3% for the same index. It was found that most children (75%) perform five or more meals/day and their eating habits did not differ much from that found in the general population in relation to quantity and quality but there was a high frequency preferably by modified food consistency. There was a significant higher number of complaints of the gastrointestinal tract in the group of children with adenoid hypertrophy. All children in the group with adenoid presented orally or mixed breathing, while 83.3% of children without adenoid hypertrophy showed nasal breathing. There was a higher prevalence of abnormal swallowing and speech in children with adenoid hypertrophy group, while statistically significant lower prevalence of non-nutritive sucking habits in this group was observed. During chewing, the majority of children in both groups had food and chewing alternating bilateral anterior incision, with 91.7% of children in the group with adenoid conducted chewing with mouth open. There was a higher frequency of changes in the speed (time) chewing in children with adenoid hypertrophy group and 75% of children in this group used liquid during mastication, while only 12.5% of children in the other group they do. There was no statistically significant difference when comparing the presence of obesity with or without use of liquid during chewing. The results of this study demonstrate the significant prevalence of changes in nutritional status (especially tendency to overweight/at risk for overweight) in children with adenoid hypertrophy without surgery.

Este trabalho foi realizado no município de Cajuri, MG, no período de maio a outubro de 2004, com o objetivo de avaliar os efeitos da reabilitação oral no consumo alimentar de pessoas idosas (60 anos ou mais). Foram selecionados, dentre os idosos do município, 60 indivíduos edêntulos totais ou parciais, com necessidade de tratamento prostodôntico e que aceitaram participar efetivamente desta pesquisa. Após a identificação e caracterização socioeconômica e da saúde bucal e geral dos idosos, foi realizado o exame clínico oral da amostra. Em seguida, fez-se a avaliação do consumo alimentar, por meio de inquérito de Freqüência Alimentar e Recordatório 24 h. Uma escala de autopercepção de bem-estar oral foi aplicada aos idosos. Procedeu-se ao tratamento prostodôntico dos idosos, e após 15 dias da instalação das próteses foram avaliados novamente o consumo alimentar e a auto-percepção do bem-estar oral. A maioria dos idosos estudados foi classificada como do extremo jovem (60– 69 anos), sendo 65% deles mulheres. Entre os medicamentos identificados foi mais freqüente o uso dos anti-hipertensivos entre os idosos. Grande parte destes (56,6%) era eutrófica (IMC entre 22–27 kg/m2), porém foi detectada uma parcela importante (33,3%) com baixo peso (IMC < 22 kg/m2). Quanto ao risco de complicações metabólicas em conseqüência da circunferência da cintura e sexo, constatou-se que as mulheres estavam em maior risco que os homens. O edentulismo total esteve presente em 75% dos casos, e muitos idosos relataram usar a prótese por mais de seis anos. Verificou-se diferença estatisticamente significativa e entre a percepção da cirurgiã-dentista e dos idosos quanto à adaptação da prótese e ao seu padrão estético antes da reabilitação oral. As causas mais freqüentes dos relatos negativos sobre a capacidade mastigatória foram a falta de dentes e a prótese mal-ajustada. Houve diferença estatisticamente significante entre o consumo de verduras cruas, legumes crus e queijo antes e depois da reabilitação oral, considerando-se como hábito alimentar o consumo do alimento quatro vezes ou mais por semana. O modo de preparo dos alimentos foi modificado após a reabilitação oral, conferindo aos idosos maior liberdade de escolha dos alimentos. A escala de autopercepção de bem-estar oral mostrou-se um instrumento eficaz para avaliar a percepção da condição oral pelos pacientes, e, ainda, detectou-se melhora na saúde oral dos idosos após a reabilitação oral, bem como na sua auto- estima. This work was accomplished in the city of Cajuri, MG, from May to October 2004. Its objective was to evaluate the effect of the oral rehabilitation on the food intake in elderly people (aged 60 and more). Sixty totally or partially edentulous individuals were selected amongst the elderly of the city, needing a prosthodontic treatment that accepted to participate effectively in this research. After the identification of social-economic characteritics and the elderly’s oral and general health an oral clinical examination was done. Afterwards a FFQ (Food Frequency Questionnaire) and 24 h Dietary Record had been applied to assess the food consumption of the participants. A scale of self-perception of oral well-being was also applied to the elderly. The prosthodontic treatment was carried out and fifteen days after the prothesis installation, the food intake and the self-perception of oral condition were assessed again. The majority of the elderly were aged 60-69 years old and 65% were women. Among the drugs identified, the most frequently used among the elderly was mainly antihypertensives. Great part of the elderly (56,6%) was in an eutrofic state (BMI 22 - 27 kg/m2), however an important group (33,3%) with low weigth was detected (BMI < 22 kg/m2). When verified the risk of metabolic complications considering the waist circumference and sex, the women were in greater risk than the men. Total edentulism was present in 75% of the cases and many elderly reported using their prothesis for more than six years. A significant difference was detected between the dentist’s and elderly’s perception about the prothesis adaptation and esthetic before the oral rehabilitation. The most frequent causes of the negative reports about the masticatory ability were tooth loss and badly-adjusted prothesis. The food preparation was modified after the oral rehabilitation, conferring to the elderly more freedom of food election. The scale of self-perception of oral condition showed to be an efficient instrument to evaluate the perception of oral health of patients, and besides it detected improvement in the elderly oral health after oral rehabilitation, as well as on their self-esteem.

Com o objetivo de diagnosticar o estado nutricional da Equipe Olímpica Permanente de Levantamento de Peso do Comitê Olímpico Brasileiro (COB), composta por 24 atletas, na faixa etária entre 16 e 23 anos, sendo 12 do sexo masculino e 12 do feminino, foram realizados os seguintes procedimentos: análise da adequação da ingestão de energia, dos macro e micronutrientes (vitamina C, retinol, cálcio e ferro), usando a Ingestão Diária de Referência (IDR), de 2001, por meio dos métodos Recordatório de 24 horas e Questionário de Freqüência de Consumo Alimentar; caracterização do perfil antropométrico, utilizando medidas de peso corporal, estatura, sete dobras cutâneas e composição corporal, por meio de fórmulas específicas para determinar o percentual de gordura corporal; e avaliação do estado nutricional de ferro por meio de parâmetros bioquímicos. Os resultados da avaliação dietética indicaram que 83% dos atletas estavam com ingestão energética abaixo dos valores recomendados; 96% com a ingestão de carboidratos adequada de acordo com o padrão de referência; 89% dos da classe 1 (adequação de 10 a 30%) e 93% dos da classe 2 (adequação de 10 a 35%) com a ingestão de proteínas dentro do padrão de referência; 87% dos da classe 1 (adequação de 20 a 35%) com a ingestão de lipídios adequada e 13% com excesso, e os da classe 2 (adequação de 25 a 35%), 45% estavam com ingestão adequada, 33% com deficiência e 22% com excesso, em comparação com o padrão de referência; 33% com deficiência de vitamina C e 67% com excesso; 50% com deficiência de retinol e 50% com excesso; 79% com deficiência de cálcio; e 25% com deficiência de ferro e 75% com excesso. O resultado do percentual de gordura corporal, segundo fórmulas específicas, indicou que todos os atletas do sexo masculino estavam abaixo do padrão de referência, e 58% dos atletas do sexo feminino estavam com excesso, 17% com deficiência e 25% com os valores adequados. Os dados da análise bioquímica do sangue não revelaram nenhuma alteração no perfil bioquímico dos atletas dentro dos critérios preestabelecidos. The objective of this work was to diagnose the nutritional status of the Weightlifting Permanent Olympic Team of the Brazilian Olympic Committee (COB), consisting of 24 athletes, 12 male and 12 female, from 16 to 23 years old. The following procedures were carried out: the energy ingestion adequacy, macro and micronutrients (vitamin C, retinol, calcium and iron) were analyzed and verified using the Reference Daily Ingestion (RDI) (2001), through the methods 24-hour Reminding and Food Consumption Frequency Questionnaire; the anthropometric profile was characterized, through body weight, height, seven skinfold caliper testing and body composition by using specific formulas in order to determine percent body fat; and the iron nutritional status was evaluated via biochemical parameters. The results of the dietary evaluation showed that 83% of the athletes had their energy ingestions below the recommended values; 96% had adequate carbohydrate ingestion, according to the reference pattern; 89% of class 1 (adequacy from 10 to 30%) and 93% of class 2 (adequacy from 10 to 35%) had protein ingestion within the reference pattern; 87% of class 1 (adequacy from 20 to 35%) had adequate lipid ingestion and 13% had excessive, as for class 2 (adequacy from 25 to 35%), 45% had adequate ingestion, 33% had insufficient and 22% had excessive, compared to the reference pattern; 33% had vitamin C deficiency and 67% had excess; 50% had retinol deficiency and 50% had excess; 79% had calcium deficiency; and 25% had iron deficiency and 75% had excess. The results for percent body fat, according to specific formulas, indicated that all male athletes were below the reference patterns, whereas the females, 58% had excess, 17% had deficiency and 25% had adequate values. The blood biochemical analysis data did not reveal any alteration in the athletes' biochemical profile within the preset criteria.

A obesidade é um dos principais fatores de risco para o desenvolvimento das doenças crônicas não transmissíveis como infarto, hipertensão arterial, doenças cardiovasculares, diabetes e alguns tipos de câncer. Presente em idade precoce, a obesidade infantil pode persistir e aumentar a incidência de morbidade e mortalidade em adultos. As causas da obesidade envolvem uma combinação da predisposição genética com um estilo de vida caracterizado pela inatividade física e ingestão excessiva de alimentos de alta densidade energética, provocando um balanço energético positivo e conseqüentemente o ganho de peso corporal. Renda familiar, escolaridade dos pais, hábitos nutricionais inadequados e sedentarismo têm papel importante no desenvolvimento de sobrepeso e obesidade infantil. A atividade física é fator protetor contra a obesidade e sobrepeso em crianças, promove alterações na composição corporal e reduz o risco de desenvolvimento das doenças crônicas não transmissíveis. O objetivo deste trabalho foi avaliar o estado nutricional e associação do mesmo com atividade física e fatores sócioeconômicos (renda familiar e escolaridade dos pais) em crianças de 7 a 10 anos de idade. Foram estudados 403 escolares da 1a a 4a série do Ensino Fundamental da Rede Municipal de Ensino de Corumbá (MS), sendo a maioria do sexo masculino e com média de idade igual a 8,8 ± 1,12 anos. Os dados foram obtidos a partir de medidas antropométricas (Índice de Massa Corporal, percentual de gordura e circunferência de cintura), clínicas (pressão arterial sistólica e diastólica) e bioquímica (glicemia capilar de jejum), e por questionários de caracterização da atividade física, dos hábitos alimentares e do nível socioeconômico. Verificou-se uma freqüência de 3% de hipertensos, 3,8% em situação limítrofe e 93,2% normotensos. Foi verificado correlação positiva da Pressão Arterial Sistólica com as medidas antropométricas, e com a atividade física. A maioria das crianças (94,4%) apresentaram glicemia normal e 5,6% glicemia de risco ( 110 mg/dL). A prevalência de risco de sobrepeso e sobrepeso foi igual a 6,2% e 6,5% respectivamente; 78,2% das crianças foram consideradas eutróficas e 9,2% estavam abaixo do peso. A prevalência de risco de sobrepeso e sobrepeso nas crianças do sexo feminino foi 39% maior do que no sexo masculino, sendo essa prevalência maior também nas famílias com melhor poder aquisitivo. Foi constatado no estudo 9,4% de crianças com percentual de gordura corporal maior do que 30%, sendo a maioria (60,5%) meninas e 39,5% meninos. A maior prevalência de percentual de gordura acima da mediana, foi observada nas crianças de famílias com maior poder aquisitivo, em ambos os sexos. Quanto à escolaridade, 58,9% dos pais tinham 7 anos de estudo. Não houve associação entre escolaridade dos pais e estado nutricional. Entre os sexos, não houve diferenças significantes quanto ao tempo dispendido nas atividades físicas, no entanto, os meninos apresentaram maior gasto energético do que as meninas em função da maior predisposição em participar de atividades mais intensas do que as meninas. Quanto à idade, as crianças mais velhas brincaram menos e gastaram mais tempo assistindo televisão e/ou videogame, do que as crianças mais novas, em ambos os sexos. Foi constatado efeito inverso entre renda e atividade física, sendo que as crianças menos ativas pertenciam às famílias de maior renda. As crianças eutróficas praticaram atividades físicas de mais intensidade do que as crianças com sobrepeso, e não houve diferença quanto à duração das atividades. Na atividade de se deslocar para a escola, constatou-se que as crianças com sobrepeso caminharam menos do que as crianças eutróficas. Nas atividades físicas sedentárias, as crianças com sobrepeso gastaram mais tempo assistindo televisão e/ou videogame do que as crianças eutróficas. De acordo com os resultados encontrados, conclui-se que menor atividade física favorece o excesso de peso, e conseqüentemente a incidência de doenças e/ou distúrbios endócrino-metabólicos, devido às associações com os parâmetros antropométricos, clínico e bioquímico observados neste estudo. As estratégias de prevenção da obesidade infantil devem enfocar primeiramente o incentivo à prática de atividades físicas regulares e a redução de hábitos sedentários. The obesity is one of the main factors of risk for the development of noncommunicable chronic diseases as stroke, arterial hypertension, cardiovascular diseases, type 2 diabetes and certain types of cancer. Present at early age, the childhood obesity can persist and increase the incidence of morbidity and mortality in adults. The causes of the obesity involves a combination of the genetic pre- disposition with a lifestyle characterized by the physical inactivity and excess intake of high energy density foods, provoking positive energy balance and consequently the gain of body weight. Familiar income, parents schooling, inadequated nutritional habits and sedentarysm have an important role in the overweight development and childhood obesity. The physical activity is a protective factor against the obesity and overweight in children, it promotes alterations in the body composition and it reduces the risk of development of noncommunicable chronic diseases. The objective of this work was to evaluate the nutritional status and its association with physical activity and the socioeconomic factors (familiar income and parents schooling) in children of 7 to 10 years of age. 403 students from the 1st to 4th series of the BASIC TEACHING of the MUNICIPAL NET OF EDUCATION of Corumbá ( MS ) have been studied, being the majority of the masculine sex with the average of 8,8 to ± 1,12 years of age. The data were obtained from anthropometric measures (Body Mass Index, percentage of body fat and waist circumference), clinics (sistolic and diastolic arterial blood pressure) and biochemical (fasting capillary glicemy), and from questionnaires about of the physical activity characterization, the nutritional habits and the socioeconomic status. It was verified a frequency of 3% of hipertensive ones, 3.8% in bordering situation and 93.2% normo-tensive ones. It was verified a positive correlation of the sistolic arterial pressure with the anthropometric measures, and the physical activity. The majority of the children (94,4%) showed normal glicemy and 5.6% showed risk glicemy (110 mg/dL). The overweight and overweight prevalence risk was equal to 6.2% and 6.5% respectively; 78,2% of the children were considered normal-weight, and 9,2% were under- weight. The overweight and overweight prevalence risk in the children of the feminine sex was 39% greater than in the masculine sex, being this prevalence higher in families with a better purchasing power. It was evidenced in the study that 9.4% of children with a body fat percentage higher than 30%, being the majority (60,5%) girls and 39.5% boys. The higher prevalence of body fat percentage above of the medium one, was observed in the children of families with a greater purchasing power, in both genders. About schooling, 58.9% of the parents had 7 years of study. There was no association between schooling and nutritional status of the parents. Between the genders, there was no significant difference about the time spent in physical activities, however, the boys showed a greater energy expenditure than the girls due to the higher predisposition in participating of more intense activities than the girls. About to the age, the oldest children played less and spent more time watching television and/or playing video-games, than the youngest ones, in both genders. An inverse effect between income and physical activity was evidenced, where the less active children belonged to the families with a higher income. The normal-weight children practiced physical activities of a higher intensity than the overweight ones, and there was no difference on how long the activities lasted. In the activity like commuting to school, it was evidenced that the overweight children walked less than the ones with a normal-weight. In the sedentary physical activities, the overweight children spent more time watching television and/or playing video-games than the normal-weight ones. According to the results, it was concluded that less physical activities favors to the excess of weight, and consequently to the endocrine-metabolic incidence of diseases and/or disturbs, due to associations with the anthropometrics parameters, clinical and biochemist observed in this study. The strategies of prevention of the childhood obesity must first focus the incentive to the practice of regular physical activities and to the reduction of sedentary habits.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Este trabalho objetivou validar um questionário semiquantitativo de freqüência alimentar (QFAs) para avaliar, de forma relativa, a ingestão dietética de adultos na cidade de Viçosa, Minas Gerais, por meio da comparação dos valores de consumo entre o QFAs e a média de quatro recordatórios 24 horas, no período de quatro meses. A metodologia utilizada foi a aplicação de quatro recordatórios 24 horas, com intervalos de um mês, e um QFAs constituído de 58 itens alimentares no final do estudo a uma amostra de 94 adultos de ambos os sexos. Os dados dietéticos foram calculados pelo software Diet Pro e analisados segundo diferenças de médias e estimativas de correlação de Pearson, ajustados pela energia e corrigidos pela variância intrapessoal, bem como pela classificação cruzada dos métodos. A influência do gênero, escolaridade, renda per capita e Índice de Massa Corporal (IMC) no estudo de validação foi determinada. Os resultados foram: alta variabilidade no consumo alimentar dessa população; diferenças de médias ou medianas de nutrientes obtidos pela aplicação das metodologias referidas, que revelaram subestimação no QFAs de micronutrientes (vitamina C, retinol e cálcio), porém, quando avaliados pelas estimativas de correlação e corrigidos pela variância, apresentaram-se adequados; lipídio foi o único nutriente que indicou baixa consistência na avaliação dietética entre os métodos (r=0,33); e os demais nutrientes variaram de r=0,40 a r=0,76, com média de r=0,52. Contudo, a ingestão de lipídio apresentou-se bem correlacionada nas estimativas de ingestão do grupo com melhores condições de renda e escolaridade, bem como no sexo feminino e nos indivíduos não obesos e, ou, com sobrepeso. Igualmente, os grupos do sexo feminino, de indivíduos com IMC inferior a 25,0 e de melhores escolaridade e renda per capita apresentaram tendência para estimativas mais fortes dos nutrientes avaliados. Ao contrário, os grupos com baixo grau de estudo, menor condição econômica, sobrepeso/obesidade e do sexo masculino revelaram baixas estimativas correlacionais. Diante dos resultados, concluiu-se que o QFAs apresentou aceitável desempenho na avaliação do consumo alimentar habitual da maioria dos nutrientes pela população estudada. E cuidados devem ser estabelecidos na utilização do QFAs, dependendo do grupo e objetivo do estudo, indicando que deve ser dado atenção à construção da lista de alimentos, para que proporcionem a adequação dos nutrientes em análise. To validate a food frequency questionnaire semiquantitative (FFQs) to assessment of relative form the dietary consumption of adults in the Viçosa City - Minas Gerais, Brazil, by means of the comparison of the consumption ́s values between the FFQs and the average of four 24 hour-Recalls, during four months. With a sample of 94 adults of both gender was applied four 24 hour recalls with intervals of one month and a FFQs consisting of 58 food itens in the end of the study. The dietary data was calculated by DIET PRO Software and analyzes according to differences of means, correlation of Pearson, adjusted for the energy and corrected by the within-variability, as well for the cross classification of the methods. The influence of the gender, education, income and body mass of index (BMI) on the validation study was determined. Results: In this population, high variability in the food consumption was observed. Differences in the means of the methodologies was under-reported by the FFQs for micronutrients (vitamin C, retinol and calcium), however, when evaluated for the correlation of Pearson and corrected for the variance it was presented adequate. Lipid was the only nutrient that indicated little consistence in the dietary assessment between the methods (r=0,33) and the another nutrient ones varied of r=0,40 and r=0,76, with average of r=0,52. However, the lipid ingestion was presented well correlated in the esteems of the group with better conditions of income and education, as well as in the feminine sex and the not obesity individuals and/or with overweight. Equally, the groups of the feminine sex, individuals with BMI lower than 24,99, of better education and income was presented trend for more adequate esteems of the evaluated nutrients. In contrast, the groups with inferior degree of study, minor economic condition, with overweight/obesity and of the masculine sex was with values not so adequate. Conclusions: The results indicate that the FFQs showed acceptable performance to assessment habitual food consumption for the majority of the nutrients in the studied population. Cares must be established in the use of the FFQs to depend on the group and objective of the study, being suggested that attention must be given to the food list ́s construction so that provides the adequacy of the nutrients in analysis.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Este trabalho foi realizado no Programa de Atendimento a Diabéticos do Ambulatório de Especialidades da Prefeitura do município de Viçosa, MG, com portadores de diabetes mellitus tipo 2, no período de junho a setembro de 2002. O seu objetivo foi avaliar indivíduos portadores de diabetes mellitus tipo 2 quanto ao tratamento clínico-nutricional e risco de complicações. Para tal, foram avali ados 111 pacientes assistidos pelo Programa. Realizou-se avaliação nutricional pelos métodos antropométricos de Índice de Massa Corporal (IMC), Circunferência da Cintura (CC) e pelo método da Bioimpedância Elétrica (BIA); avaliação dietética utilizando a história dietética, questionário de freqüência alimentar e recordatório de 24 horas; e avaliação bioquímica, cujos dados foram obtidos dos prontuários médicos dos indivíduos. O tratamento adequado (dieta, atividade física e, se necessário, hipoglicemiantes), foi relatado por 64% dos pacientes. Trinta e dois deles usavam apenas insulina, 55 hipoglicemiante oral e 24 utilizavam os dois associados. A prática de ativida de física foi relatada por 64%, porém apenas 49% faziam-na de forma regular e adequada. A avaliação nutricional revelou que a maioria dos pacientes apresentou sobrepeso ou obesidade pelos três métodos utilizados. A avaliação dietética revelou consumo alime ntar adequado em termos qualitativos, porém, em termos quantitativos, observou-se inadequação de energia, fibras e cálcio, com valores inferiores ao recomendado. A avaliação bioquímica revelou níveis elevados de hemoglobina glicada, colesterol total e LDL, níveis baixos de HDL e níveis de triglicerídeos normais. As complicações decorrentes do diabetes verificadas foram disfunção erétil (68%), hipertensão arterial (50%), alterações oculares (39%), alterações circulatórias (29%), dislipidemia (19%), alterações cardíacas (18%), alterações renais (8%) e lesões nos pés (5%). Os principais fatores de risco associados estatisticamente (p< 0,05) com desenvolvimento das complicações foram idade, sexo, tempo de diabetes, consumo de fibras, IMC, hipertensão, tipo de tratamento, consumo de gordura monoinsaturada e alterações circulatórias. Conclui- se que a alta prevalência de hipertensão e alterações oculares é justificada pela soma dos fatores de risco modificáveis e fatores de risco não-modificáveis. As altas prevalências de disfunção erétil e alterações circulatórias podem ser justificadas por outros fatores de risco não avaliados neste estudo, como alterações metabólicas, uso de insulina e efeitos deletérios do cigarro, mesmo em ex-fumantes. As baixas prevalências de dislipidemia, alterações renais e lesão de pé podem ser justificadas pelo adequado manejo da doença pelos profissionais de saúde deste Programa , o que reforça a importância de se implantarem programas de atendimentos específicos à população diabética, com profissionais aptos a lutar, junto com os pacientes , por melhor qualidade de vida, caracterizada morbimortalidade. principalmente por redução da morbimortalidade. This work was accomplished at the Diabetics Attendance Program of the Clinic of Specialties of the City Hall of the municipal district of Viçosa, MG, with diabetes mellitus type 2 carries, from June to September 2002. The objective was to evaluate diabetes mellitus type 2 carries related to the clinical-nutritional treatment and risk of complications. For such, it was evaluated 111 patients attended in the program. It was done nutritional evaluation by anthropometric methods of Corporal Mass Index (IMC), Waist Circumference (CC) and by the Electric Bioimpedance method (BIA); dietary evaluation using the dietary history, including alimentary frequency questionnaire and 24 hours reminding; and biochemical evaluation, whose data were obtained from the individuals' medical handbooks. The appropriate treatment (diet, physical activity and, if necessary, hypoglycemiante medicine) was noticed by 64% of the patients. Thirty two of them just used insulin, 55 hypoglycemiante oral and 24 used the two associates. The physical activity practice was related by 64%, however, only 49% did it in a regular and appropriate way. The nutritional evaluation revealed that most of the patients presented overweight or obesity for the three used methods. The dietary evaluation revealed appropriate consumption in qualitative terms, however, in quantitative terms it was observed an inadequacy of energy, fibers and calcium, with lower values than the recommended. The biochemical evaluation revealed high levels of glicosiled hemoglobin, total cholesterol and LDL; low levels of HDL and normal level of triglycerics. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Estudos sobre a qualidade protéica e a biodisponibilidade de minerais têm a sua importância na manipulação dietética, visando a redução da prevalência de desnutrição energético-protéica e de carências nutricionais específicas, uma vez que componentes da dieta podem promover melhor utilização dos nutrientes. Nesse sentido, o presente trabalho visou avaliar os efeitos da adição de Lactobacillus acidophilus, uma bactéria probiótica, na qualidade protéica de uma bebida láctea à base de soro de queijo. Avaliou-se, ainda, o efeito dessa bactéria sobre a biodisponibilidade de cálcio e de zinco. No ensaio para avaliar a qualidade protéica foram utilizados 24 ratos machos recém-desmamados, separados em quatro grupos (n=6): um grupo controle com dieta à base de caseína, um segundo grupo com dieta aprotéica e dois grupos experimentais, sendo um à base da fórmula infantil e o outro à base da bebida láctea probiótica. Foram utilizados os métodos PER (coeficiente de eficiência protéica), NPR (razão protéica líquida), NPU (utilização protéica líquida) e digestibilidade. No estudo com cálcio foram utilizadas 60 ratas fêmeas adultas, sendo 30 intactas e 30 ovariectomizadas, divididas em 6 grupos (n=10), e cada grupo subdividido em três. Dentre as ratas intactas, um grupo recebeu probiótico, outro grupo recebeu leite sem probiótico e um terceiro grupo dose intraperitonial de 45 Ca. Da mesma forma ocorreu com as ratas ovariectomizadas. O cálcio foi avaliado utilizando-se a técnica do traçador bioquímico (45Ca), medindo-se sua absorção fracional no fêmur dos animais. Foi ainda avaliado o efeito do probiótico no íleo das ratas, medindo-se a superfície de absorção intestinal. A biodisponibilidade do zinco foi avaliada em 32 ratos machos recém-desmamados, divididos em quatro grupos: dieta padrão com 15 e 30 μg de zinco/g de dieta e dieta padrão com probiótico, também com 15 e 30 μg de zinco/g de dieta. Foi feita a determinação do teor do mineral no fêmur, nos rins, no fígado, no sangue e no cérebro, por espectrofotometria de absorção atômica. O ganho de peso foi monitorado e determinada a atividade da enzima fosfatase alcalina, indicativa de crescimento ósseo. A bebida láctea elaborada apresentou uma alta qualidade protéica, semelhante a da proteína padrão (caseína), o que pode ser atribuído ao elevado valor biológico das proteínas do soro de queijo, associado à presença de bactérias probióticas e outras culturas lácteas, que ajudam a promover uma melhor utilização da proteína. No experimento com zinco não foi observada diferença nos fêmures dos animais, porém, a presença de Lactobacillus acidophilus proporcionou maior ganho de peso, podendo estar relacionado com a melhor biodisponibilidade desse e de outros nutrientes promotores do crescimento, o que indica efeito benéfico da presença da bactéria. Quanto à retenção de cálcio nos fêmures, os resultados mostraram ser maior nos animais que receberam probiótico e naqueles não submetidos à ovariectomia. Através da análise estatística dos valores observados para os parâmetros ‘espessura’ e ‘superfície de absorção’ dos cortes de íleo, obtidos histologicamente, observou-se que os animais intactos (com ovário) que receberam probiótico apresentaram os valores mais elevados, resultado semelhante ocorrido na retenção no fêmur, sugerindo a interferência do hormônio estrogênio nessas funções. Dessa forma, pode-se concluir que microrganismos probióticos são fatores de promoção da saúde, interferindo positivamente na biodisponibilidade de minerais como o cálcio e o zinco. A elevada qualidade protéica da bebida com probiótico e soro de leite demonstra não só benefícios dos probióticos, como também da utilização do soro de leite na elaboração de produtos lácteos. Studies on protein quality and mineral bioavailability are important on diet manipulation to reduce occurrence of protein-energy malnutrition and specific nutritional deficiencies, since diet components may improve nutrient utilization. The present work aimed at to evaluate the effects of Lactobacillus acidophilus, a probiotic bacteria, on protein quality of a whey-based milky beverage. The effect of this bacteria on calcium and zinc bioavailability was also evaluated. For protein quality assay, twenty-four weaning male rats were divided into four groups (n=6): control group fed a casein-based diet, protein-free group and two experimental groups, one of them fed a commercial probiotic infant formula, and the other fed the probiotic milky beverage. The following methods were utilized: protein efficiency ratio (PER), net protein ratio (NPR), net protein utilization (NPU) and digestibility. Sixty adult females rats, thirty intact and thirty ovariectomized, were utilized in the study on calcium bioavailability. They were divided into six groups (n=10). Among the intact female rats, one group was fed the probiotic, a second one received milk without probiotic, and a third one was fed an intraperitoneal dosis of 45 Ca. The ovariectomized rats were treated in the same manner. The calcium was evaluated by means of the method of biochemical tracer (45Ca), by measuring its fractional absorption in the animal femurs. The effect of the probiotic bacteria on the ileum of the animals was also evaluated, by measuring the intestinal absorption surface. The zinc bioavailability was evaluated in thirty-two weaning male rats divided into four groups, fed a standard diet with 15 or 30 μg zinc/g diet, or a standard diet plus probiotic with 15 or 30 μg zinc/g diet. The zinc content was determined by atomic absorption spectrophotometry in femur, kidneys, liver, blood and brain. The weight gain was monitored and the activity of alkaline fosfatase, used as a bone growth marker, was determined. The formulated milky beverage presented a high protein quality, similar to the standard protein (casein). This may be attributed to the high biological value of the whey proteins combined with the presence of probiotic bacteria and other dairy cultures that help to improve protein utilization. In the experiment with zinc, there was no difference among animal femurs, but the presence of Lactobacillus acidophilus provided an enhanced weight gain, which may be related to the better bioavailability of zinc and other growth-promoting nutrients, indicating a benefic effect of this bacteria. Calcium retention in the femurs was higher in the intact animals fed the probiotic. The statistical analysis of the values reported for the parameters ‘thickness’ and ‘absorption surface’ of the ileum cuts indicated that the intact animals fed the probiotic presented the highest values, similarly to what occurred with calcium retention in the femur, suggesting the influence of estrogen hormone on these functions. It may be concluded that probiotic microorganisms are health-promoting factors, improving calcium and zinc bioavailability. The high protein quality of the beverage indicates the benefits of both probiotics and whey addition to dairy products.

Conselho Nacional de Desenvolvimento Científico e Tecnológico Estudos sobre fatores dietéticos, relacionados à obesidade, têm sido concentrados no consumo de carboidratos e lipídios, sendo que alguns autores consideram a obesidade associada ao consumo em excesso de gordura, enquanto outros, aos carboidratos refinados. Uma grande variedade de novos produtos para controle de peso tem surgido no mercado, sendo extremamente atrativos para indivíduos que desejam perder ou manter o peso. Contudo, quando modificações de um produto alimentício convencional são feitas, implicando em redução do conteúdo calórico, alterações no metabolismo energético e no controle do apetite podem ocorrer, tornando a eficiência do uso destes produtos no controle da obesidade um tema de grande controvérsia. O presente estudo objetivou avaliar o efeito de alimentos modificados na ingestão alimentar e no metabolismo energético de homens de peso normal e com sobrepeso. Duas dietas isoenergéticas foram utilizadas, sendo compostas por três preparações convencionais, dieta padrão, e, três “análogos” diet, dieta modificada (maior quantidade de carboidratos complexos). A Escala de Analogia Visual (VAS) foi utilizada para avaliar diferenças na percepção sensorial entre os grupos, e sensações referentes a ingestão alimentar. Utilizou-se a calorimetria indireta, para obtenção dos dados referentes ao metabolismo energético. Não foi encontrada diferença (P>0,01) na percepção das características sensoriais avaliadas, aparência, aroma, sabor, gosto residual, e palatabilidade, entre os grupos. Diferenças entre o volume das duas dietas testadas (P0,05) entre os grupos, mas foi observada, no grupo com sobrepeso, uma menor saciedade (P0,01) on the sensorial characteristics assessed, visual appeal, smell, taste, aftertaste, and palatability between the groups. Conselho Nacional de Desenvolvimento Científico e Tecnológico As farinhas de soja integrais utilizadas neste trabalho foram obtidas, a partir de grupos de sementes (bulks) derivadas de linhagens que diferem entre si com relação à presença de lipoxigenases (LOX) e/ou inibidor de tripsina Kunitz (KTI). Para essa seleção e confirmação dos genótipos realizaram-se análises de lipoxigenases, de inibidores de tripsina e de quimotripsina. Realizou-se ainda, análise da composição química centesimal para caracterização das farinhas e atividade hemaglutinante nas sementes e nas farinhas de soja derivadas. Constatou-se , através da atividade hemaglutinante, que o tratamento térmico (89oC por 5 minutos) aplicado no processamento das farinhas foi suficiente para inativar as lectinas presentes na soja. Com o objetivo de avaliar a qualidade protéica e digestibilidade in vivo das farinhas de soja, procedeu-se aos ensaios biológicos com ratos, em que foram avaliados o PER (Relação da Eficiência Protéica), NPR (Razão Protéica Líquida), NPU (Utilização Protéica Líquida) e a digestibilidade. Os valores de PER, NPR, NPU para as variedades de soja foram inferiores aos valores obtidos para caseína, possivelmente em virtude da deficiência da proteína de soja em algum tipo de aminoácido essencial. Para as farinhas sem KTI foram obtidos valores de digestibilidade maiores que para as farinhas com KTI, e esses foram bem próximos aos da caseína. Isto comprova que a eliminação genética do KTI melhora consideravelmente a digestibilidade da proteína de soja. No entanto, essa diferença não refletiu em melhor qualidade protéica dessa variedade, visto que não houve diferença nos valores de PER, NPR e NPU entre as variedades estudadas. Procedeu-se a determinação da composição aminoacídica das farinhas e cálculo do PDCAAS (Escore Químico Corrigido pela Digestibilidade) , constatando-se que o aminoácido limitante dessa soja é lisina e não metionina, ao contrário do que aponta a literatura, possivelmente em decorrência do melhoramento genético que levou a essa alteração. Os resultados do PDCAAS não indicaram diferença entre as farinhas sem KTI das farinhas derivadas de linhagens com KTI, como foi observado pelos resultados da digestibilidade in vivo. Conclui-se que a retirada genética do inibidor de tripsina Kunitz melhora significativamente a digestibilidade da proteína de soja, eliminando a necessidade de se empregar altas temperaturas nos produtos de soja visando inativação desse inibidor e, conseqüentemente, evitando o superaquecimento com insolubilização de proteínas, perdas de características funcionais e destruição de alguns aminoácidos. The whole soy flours used in this work were obtained from seed bulks derived from strains differing in relation to presence of lipoxygenases (LOX) and/or Kunitz trypsin (KTI). Analyses for lipoxygenase and trypsin and chemotrypsin inhibition were performed in order to select and confirm the genotypes. Chemical content analysis was also performed so as to characterize the flours and the hemaglutinating activity in the seeds and the derived soy flour. It was observed from that activity that the thermal treatment (89 o C for 5 min) on the flour processing was enough to inactivate the lectins present in the soy. In order to evaluate the in vivo digestibility and protein quality for the soy flours, biological tests with rats were performed and from these tests, PER, NPR, NPU and digestibility were evaluated. PER (Protein Efficiency Ratio), NPR (Net Protein Ratio) and NPU (Net Protein Utilization) values were obtained below the values for casein. This result may be due to the deficiency of sulfured amino acids in soy protein. Flours without KTI showed higher values for digestibility than flours with KTI. These values were quite close to the ones for casein. This indicates that the KTI genetic elimination in soy improves the digestibility for the soy protein a lot. However, this difference did not reflect in better protein quality of this soy strain, since there was no differences in the values of PER, NPR and NPU between the strains. Amino-acidic content for the flours and PDCAAS (Protein Digestibility Corrected Amino Acid Score) values were determined showing that lysine rather than methionine is the limiting amino acid for this soy, different of that pointed by literature, possibly because the genetic enhancements. PDCAAS showed no difference between flours without KTI and the flours derived from KTI strains as it was observed from the in vivo digestibility results. It follows that the genetic withdrawal of Kunitz trypsin inhibitor significantly improves digestibility of soy protein, eliminating the need to deploy high temperatures in soy products to inactivate this inhibitor and, consequently, avoiding overheating with protein insolubilization, characteristics and loss of some aminoacids. loss of functional

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Foram enriquecidas amostras de leite tipo C com ferro aminoácido quelato e sulfato ferroso, em diferentes níveis de adição (0, 6, 12, e 24 mg/l). Metade delas foi homogeneizada e todas as amostras foram submetidas a análises físico-químicas de pH, acidez, teor de gordura e índice de TBA, além de teste sensorial com julgadores treinados para sabor oxidado, durante um período de estocagem de três dias, sob refrigeração a 5oC. Estudou-se, ainda, a biodisponibilidade relativa do ferro aminoácido quelato, por meio da regeneração dos níveis de hemoglobina em ratos anêmicos. Os resultados obtidos mostraram que o ferro aminoácido quelato, adicionado ao leite, apresentou uma biodisponibilidade que não diferiu estatisticamente (p>0,01) do sulfato ferroso, sendo capaz de restabelecer, em 14 dias, os níveis de hemoglobina em ratos anêmicos, quando adicionado nos níveis de 12 e 24 mg/l. O uso de 24 mg de ferro/l de leite, na forma de ferro aminoácido quelato, manteve as características físico-químicas do leite, não alterando o sabor, durante todo período de estocagem a 5oC, enquanto o sulfato ferroso, nas dosagens de 12 e 24 mg/litro, promoveu alterações na oxidação da gordura, influenciando no sabor do leite, que foi perceptível pelo painel de julgadores, sem contudo alterar as suas características físico-químicas de acidez e pH. Para ambas as fontes de ferro, o processo tecnológico de enriquecimento do leite mostrou-se simples e de fácil execução, sem modificar apreciavelmente a rotina de processamento do leite tipo C. Desta forma, o ferro aminoácido quelato mostrou-se superior ao sulfato ferroso no enriquecimento do leite tipo C, dadas as suas características nutricionais, físico-químicas e sensoriais satisfatórias, nos níveis de 12 e 24 mg/l, com potencial para uso em programas institucionais de suplementação alimentar, no combate à anemia ferropriva. Four different levels of ferrous aminoacid chelate and ferrous sulfate were added to grade C padronized (3,2%) milk. The influence of homogenization and the pH, acidity, fat, lipid oxidation (TBA test) , and sensorial analysis were evaluated during three days of storage at 5 o C. The bioavailability of both iron sources was determined by the hemoglobin depletion-repletion technique using rats. The results demonstrated that the bioavailbility did not present statistical difference (p>0,01) of both iron sources, since they were capable to restaure hemoglobin levels in anemic rats in 14 days, when added in the levels of 12 e 24 mg/l elemental iron. The highest concentration of ferrous aminoacid chelate (24 mg/l iron) presented no influence in the physical-chemical characteristics studied, and also in the flavor evaluation. However the ferrous sulfate at 12 and 24 mg/l , presented flavor and lipid oxidation changes, statistically detect by the judges panel, but without changing the pH and acidity. Homogenization, followed by LTLT pasteurization (65 o C for 30 min.) increase the TBA level from 0,019 (absorbance at 532 nm) to 0,033 at zero time. This difference in TBA was maintained during the three days of storage. The ferrous aminoacid chelate was then considered superior to ferrous sulfate at 12 and 24 mg/l because it did not change the flavor and the characteristics of milk during storage for three days at 5 o C. We therefore recommend that it can be used in institucional program for milk suplementation, to reduce iron deficiency.
