1. Direct medical costs of type 2 diabetes mellitus in the Philippines: findings from two hospital databases and physician surveys

Abstract

Objective To estimate the annual direct medical cost of type 2 diabetes mellitus (T2DM) in hospitals and outpatient care clinics from a healthcare payer perspective in the Philippines.Design and participants (1) A review of electronic hospital records of people with T2DM in two tertiary hospitals—Ospital ng Makati (OsMak) and National Kidney and Transplant Institute (NKTI) and (2) a cross-sectional survey with 50 physicians providing outpatient care for people with T2DM.Setting Primary, secondary and tertiary healthcare facilities in Metro Manila.Outcome measures Cost of managing T2DM and its related complications in US dollars (USD) in 2016.Results A total of 1023 and 1378 people were identified in OsMak and NKTI, with a complication rate of 66% and 74%, respectively. In both institutions, the average annual cost per person was higher if individuals were diagnosed with any complication (NKTI: US$3226 vs US$2242 and OsMak: US$621 vs US$127). Poor diabetes control was estimated to incur higher per person cost than good control in both public outpatient care (poor control, range: US$727 to US$2463 vs good control, range: US$614 to US$1520) and private outpatient care (poor control, range: US$848 to US$2507 vs good control, range: US$807 to US$1603).Conclusion The results highlight the high direct medical cost resulting from poor diabetes control and the opportunity for cost reduction by improving control and preventing its complications.

Abstract

Introduction Diabetes and its complications are a major cause of morbidity and mortality in the Philippines. The prevalence of diabetes in the Philippines has increased from 3.4 million in 2010 to 3.7 million in 2017. The government has formulated strategies to control this increase, for example, through its non-communicable disease prevention and control plan. However, there is scarce research on the financial burden of diabetes. Filling this gap may further help policymakers to make informed decisions while developing and implementing resource planning for relevant interventions. The primary objective of the current study is to estimate the direct medical costs associated with type 2 diabetes mellitus (T2DM).Methods and analysis This is a 1-year retrospective cohort study of patients with T2DM in 2016. Data will be collected from: (1) hospital databases from public institutions to estimate the cost of diabetes treatment and (2) physician interviews to estimate the cost of management of diabetes in outpatient care. We will perform descriptive and comparative analyses on direct medical costs and healthcare resource utilisation, stratified by the presence of diabetes-associated complications.Ethics and dissemination Research ethics board approval has been obtained from the Department of Health Single Joint Research Ethics Board and Cardinal Santos Medical Center Research Ethics Review Committee. Findings from the study will be reported in peer-reviewed scientific journals and local researcher meetings.

Abstract

Growth hormone - secreting pituitary adenomas are the cause of acromegaly in 95% of patients. In rare circumstances, a pituitary adenoma on magnetic resonance imaging cannot be found; hence, a search for an ectopic source of GH production is done. Even rarer is an acromegalic patient without an ectopic source and without imaging evidence of pituitary adenoma. We report a case of acromegaly with no evidence of a pituitary adenoma and no evidence of an ectopic source after imaging studies; who underwent medical therapy with improving biochemical and clinical parameters.

Abstract

This is a case report of a 22-year-old Filipino male, morbidly obese, not known to have diabetes mellitus (DM) who presented with diabetic ketoacidosis on initial diagnosis. He had a phenotype of type 2 DM (T2D) but an initial presentation consistent with type 1 DM (T1D). Insulin therapy was eventually discontinued but he maintained good glycemic control with diet alone. C-peptide showed adequate increase after a mixed-meal diet and GAD65Ab was negative, thus he was diagnosed with ketosis-prone DM (KPDM). The increasing prevalence of obesity challenges the classic phenotype of patients with DM, with many patients presenting as an obese type T1D, and being diagnosed with T2D at a younger age. This complicates how to classify the patient’s diabetes, and the clinical profile is sometimes insufficient to make the proper diagnosis. In these cases, immunologic markers and assessment of β-cell function are important tools to differentiate between T1D and T2D, to direct management plans and to anticipate complications.

Abstract

Cardiovascular disease is a leading cause of mortality in the Philippines with an upward trend in the prevalence of atherosclerosis-related risk factors. In the national survey done in 2008, the prevalence of dyslipidemia is also increasing. As such, there is a need to increase awareness of the risk factors for cardiovascular disease among health care professionals. The development of Philippine guidelines were part of the strategy to improve overall cardiovascular risk and dyslipidemia management that is relevant to the Philippine setting. This article summarizes the 2005 Dyslipidemia guidelines, discusses some updates since its release and emphasizes equity issues when considering management of dyslipidemia in a Filipino patient.

Abstract

Objective. The main objective of this study is to determine if epicardial fat thickness can be an early marker of insulin resistance. The specific objectives are to determine the specific thickness of epicardial fat that will correlate with insulin resistance and to correlate epicardial fat thickness with co-morbidities, anthropometric measurements and other clinical variables. Methodology. Patients were enrolled into the study by purposive sampling. Insulin assay, fasting blood sugar (FBS) and 2D echocardiogram measuring the epicardial fat were requested. HOMA-IR was computed and correlated with epicardial fat thickness. SPSS version 19 and Epi info v3.5.1 were used for statistical analysis. Linear regression analysis was performed on all variables to identify correlates with epicardial fat thickness. Results. A total of 22 subjects were included in the study. Abstract

Objective. To present the results from the Philippine cohort of the A1chieve study receiving insulin detemir (IDet) ± oral antidiabetic drugs. Methodology. A1chieve is a multinational, 6-month, observational study of 66,726 people with type 2 diabetes mellitus (T2DM), both insulin users and non-insulin users, started on IDet, insulin aspart or biphasic insulin aspart in 28 countries across four continents. This subgroup analysis evaluates the safety and effectiveness of IDet in 988 subjects from the Philippines. Results. At baseline the mean age, duration of diabetes and mean BMI were found to be 57±11.9 yrs, 6.9±5.3 yrs and 26±4.8 kg/m2 respectively. Majority of subjects were insulin-naive (84.3%) and glycemic control was poor in all the groups at baseline. Abstract

Objective. To present the results from the Philippine cohort of the A 1 chieve study receiving insulin detemir (IDet) ± oral antidiabetic drugs. Methodology. A 1 chieve is a multinational, 6-month, observational study of 66,726 people with type 2 diabetes mellitus (T2DM), both insulin users and non-insulin users, started on IDet, insulin aspart or biphasic insulin aspart in 28 countries across four continents. This subgroup analysis evaluates the safety and effectiveness of IDet in 988 subjects from the Philippines. Results. At baseline the mean age, duration of diabetes and mean BMI were found to be 57±11.9 yrs, 6.9±5.3 yrs and 26±4.8 kg/m 2 respectively. Majority of subjects were insulin-naive (84.3%) and glycemic control was poor in all the groups at baseline. Abstract

This article briefly reviews the obesity practice recommendations of the Philippine Association for the Study of Overweight and Obesity (PASOO) and the obesity guidelines of the Family Medicine Research Group (FMRG) of the UP-Philippine General Hospital. The two treatment recommendations showed their focus on the primary care setting and several limitations in the development process. The implementation strategies centered on their dissemination among health care professionals although the PASOO included food and activity pyramid guides useful both as patient educational material and as a treatment tool. In spite of their limitations, both sets of recommendations are valuable resources because they effectively promote obesity awareness in the Philippine setting. However, the current sets of obesity recommendations need to be modified and updated to fulfill important requirements for high-quality recommendations backed by a strong Philippine evidence base. Collaboration among important stakeholders in the prevention and control of obesity and other noncommunicable diseases (NCD) is essential to arrive at an integrated approach to obesity.

Abstract

Our objective was to determine the association of serum adiponectin levels with the presence of IFG or DM in Filipinos. This case control study used sera of adult participants in the Philippines’ NNHeS: 2003—04. Subjects were divided into: normoglycaemic control, impaired fasting glucose, and type 2 diabetes mellitus. Seventy-seven prediabetic and 83 diabetic subjects were included in the prediabetic and diabetic groups, respectively. There was no significant difference in adiponectin values between control and prediabetic subjects. Diabetic subjects had significantly lower mean serum adiponectin levels (10.7 versus 14.2 µg/ml, p=0.0198) compared with age- and BMI-matched control subjects. Diabetic subjects were found most frequently (43.53%) in the lowest tertile (1.6—7.2 µg/ml) and least frequently (20%) in the highest tertile (14—84 µg/ml) of adiponectin values. We conclude that Filipinos with diabetes mellitus had significantly lower adiponectin levels compared with normoglycaemic subjects.

Abstract

Background: The accuracy and convenience of pen devices for insulin injection have improved quality of life for patients with insulin-treated diabetes mellitus (DM). Prefilled, disposable pens have the advantage of simplicity, with minimal training and attention required and these offer improvements in compliance, freedom and flexibility. Objective: The survey assessed doctor preference in prescribing insulin in a FlexPen™ in diabetes patients in the Philippines under normal clinical practice conditions. The data was based from observations on the use of NovoMix® 30 FlexPen™ (Biphasic Insulin Aspart 30). Data was collected from 256 doctors who were specialised in treating patients with diabetes and had their clinics in the Philippines. A Doctor Preference Questionnaire was developed to assess doctor preference in specific terms of ease of learning and teaching use of FlexPen™, time spent for patient training in use of FlexPen™, ease of setting up the required dose of FlexPen™, DoctorEs satisfaction of shifting from any other device to FlexPen™, confidence that the correct amount of insulin will be injected, feedback about the NovoFine® 30G needles, overall experience with FlexPen™. Results: In total, 256 doctors participated and completed the doctor satisfaction questionnaires. Majority of doctors 62.1% felt learning/ teaching the use of FlexPen™ to the patients was „very easy‰; and 66.8% healthcare professionals (doctor or their staff) spent less than 10 minutes to train the patients on how to use FlexPen™. About 96% doctors responded that it is very easy or easy to set up the required dose and dose correction using FlexPen™. About (98.8%) doctors were confident or very confident that their patients will inject the correct dosage of insulin using FlexPen™. Almost all doctors (98.4%) satisfied or very satisfied about the patient shifting from other insulin devices to FlexPen™; 39.5% doctors rated the design of FlexPen™ as excellent; 55.5% rated it as good; while 74.6% doctors rated the overall experience of FlexPen™ as excellent. Conclusion: The participating doctors showed high degree of acceptance of Flexpen™. The overall experience of Flexpen™ was excellent.
