5. Magmatism during the continent – Ocean transition

- Abstract
As continents break apart, the dominant mechanism of extension transitions from faulting and lithospheric stretching to magma intrusion and oceanic crust formation in a new ocean basin. A common feature of this evolution preserved at magmatic rifted margins worldwide are voluminous lava flows that erupted close to sea level during the final stages of development of the continent-ocean transition (COT). The mechanisms responsible for the generation of the melts that contribute to these voluminous flows, the so-called seaward dipping reflectors (SDR), and their significance in the context of COT development, are relatively poorly understood; they lie deep below post-rift strata along submarine rifted margins where they cannot be studied directly. Extensive coring of the Afar Stratoid Series - an a really-extensive sequence of Pliocene-aged basalts and intercalated sediments that lie atop the developing COT in the sub-aerial Afar Depression, northern Ethiopia - offers fresh scope to address this issue. We present a numerical model simulating the formation of enriched metasomes within the continental lithospheric mantle by the passage of magmas resembling modern axial basalts. Thermal destabilization of the metasome, caused by plate stretching, initiates melt formation within the metasome. These melts, when mixed with a depleted lithospheric mantle component, closely match the range of compositions of the Afar Stratoid Series lavas in this study. Metasomatic re-enrichment and subsequent melting of the lithospheric mantle during the COT may contribute to further plate thinning. These results demonstrate a novel mechanism by which large-volume flows may be erupted during the COT.

This article, published in the journal Neuropediatrics, discusses the use of A20 (TNFAIP3) as a potential biomarker to distinguish relapse from remission in pediatric patients with monophasic MOGAD (MOG-IgG-associated disorders). MOGAD is a type of acquired demyelinating syndrome associated with antibodies against myelin oligodendrocyte glycoprotein. The study found that A20 levels were significantly lower during clinical events compared to remission in monophasic MOGAD patients. However, there was no significant difference in A20 levels between clinical events and remission in multiphasic MOGAD patients. The authors suggest that further research is needed to investigate the diagnostic, prognostic, and therapeutic implications of A20 in pediatric MOGAD patients. [Extracted from the article]

PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

The vast majority of active volcanism that is located at plate boundaries can be easily explained by plate tectonic processes. Intraplate volcanism, which incorporates some of the smallest and largest volcanic events on Earth, cannot be successfully explained by a single process or model. The most volumetrically significant intraplate volcanic events are associated with the arrival of the head of a thermo-chemical anomaly rising from the deep mantle and impacting the base of the lithosphere. This event generates massive and short-lived magmatic activity over a wide area (up to 2000 km across), forming a large igneous province. A long-lived hotspot track can form over the mantle plume tail and is best illustrated by the formation of age progressive volcanic chains, such as the famous Hawaiian-Emperor chain. Millions of smaller solitary volcanic edifices, non-age progressive volcanic chains and provinces, on the other hand, have other potential mechanisms of origin. Potential models comprise decompression melting due to lithospheric extension, destabilization of fusible lithologies in the lithospheric mantle, small scale sub-lithospheric convection, or lithospheric delamination.

Objective We undertook this phase III study to evaluate baricitinib, an orally administered JAK‐1/JAK‐2 inhibitor, as monotherapy or combined with methotrexate (MTX) compared to MTX monotherapy in patients with active rheumatoid arthritis (RA) who had received no or minimal conventional synthetic disease‐modifying antirheumatic drugs (DMARDs) and who were naive to biologic DMARDs. Methods A total of 588 patients were randomized 4:3:4 to receive MTX monotherapy (once weekly), baricitinib monotherapy (4 mg once daily), or the combination of baricitinib and MTX for 52 weeks. The primary end point assessment was a noninferiority comparison of baricitinib monotherapy to MTX monotherapy based on the proportion of patients meeting the American College of Rheumatology 20% improvement criteria (achieving an ACR20 response) at week 24. Results The study met its primary objective. Moreover, baricitinib monotherapy was found to be superior to MTX monotherapy at week 24, with a higher ACR20 response rate (77% versus 62%; P ≤ 0.01). Similar results were observed for combination therapy. Compared to MTX monotherapy, significant improvements in disease activity and physical function were observed for both baricitinib groups as early as week 1. Radiographic progression was reduced in both baricitinib groups compared to MTX monotherapy; the difference was statistically significant for baricitinib plus MTX. The rates of serious adverse events (AEs) were similar across treatment groups, while rates of some treatment‐emergent AEs, including infections, were increased with baricitinib plus MTX. Three deaths were reported, all occurring in the MTX monotherapy group. Malignancies, including nonmelanoma skin cancer, were reported in 1 patient receiving MTX monotherapy, 1 receiving baricitinib monotherapy, and 4 receiving baricitinib plus MTX. Conclusion Baricitinib alone or in combination with MTX demonstrated superior efficacy with acceptable safety compared to MTX monotherapy as initial therapy for patients with active RA.

Background In clinical practice, temporary interruption of rheumatoid arthritis (RA) therapy is common for various reasons including side effects, non-compliance, or necessity for surgery. To characterize temporary interruptions of baricitinib and placebo-matched tablets in phase 3 studies of patients with moderate-to-severe rheumatoid arthritis (RA) and describe their impact on efficacy and safety. Methods During 4 baricitinib phase 3 studies, investigators documented timing, reason, and duration of investigator-initiated temporary interruptions of study drug. In 2 studies, patients recorded RA symptoms in daily diaries for 12 weeks. Post hoc analyses investigated changes in symptom scores during interruptions and resumption of treatment. Interruptions were evaluated for reoccurrence of adverse events or laboratory abnormalities after retreatment. Results Across the placebo-controlled studies, interruptions occurred in larger proportions of baricitinib- (2 mg, 18%; 4 mg, 18%) vs placebo-treated (9%) patients in only one study (bDMARD-inadequate responder patients, RA-BEACON). In the active comparator-controlled studies, the lowest rates of interruption were in the baricitinib monotherapy arm (9%) of RA-BEGIN (vs methotrexate monotherapy or combination therapy), and proportions were similar for baricitinib (10%) and adalimumab (9%) in RA-BEAM. Adverse events were the most common reason for interruption, but their reoccurrence after drug restart was infrequent. Most interruptions lasted ≤ 2 weeks. Daily diaries indicated modest symptom increases during interruption with return to pre-interruption levels or better after resumption. Interruptions had no impact on long-term efficacy outcomes. Conclusions Consistent with its pharmacologic properties, brief interruptions of baricitinib during phase 3 studies were associated with minor increases in RA symptoms that resolved following retreatment. This analysis provides useful information for clinicians, as temporary interruption of antirheumatic therapy is common in the care of patients with RA. 8 pages, 1 figure, Improvements in seismic imaging, computing capabilities, and analytical methods, as well as a number of industry deep-water wells sampling distal offshore settings, have underpinned new concepts for rifted margin evolution developed in the last two decades; these mark significant progress in our understanding of extensional systems. For example, the tectonic, sedimentary, and magmatic processes linked to the formation of rifted margins have been overhauled, giving rise to more quantitative approaches and new concepts. However, these processes cannot be understood in isolation, requiring consideration of the continuum in which inheritance and physical processes are integrated within a plate tectonic framework. The major progress and fundamental developments of past research in rifted margins have been made hand-in-hand with other domains of Earth Sciences and have fundamental implications for the understanding of key geological systems such as active rifts, the ocean lithosphere, subduction zones, and collisional orogens. The “IMAGinING RIFTING” workshop, organized in Pontresina-Switzerland in September 2017, gathered researchers from all disciplines working on rifts and rifted margins, and included participants from academia and industry. This contribution summarizes the workshop discussions, in addition to outlining our state-of-the-art knowledge of rifted margins. We highlight future challenges in unraveling the processes and conditions under which these extensional systems form and, ultimately, how tectonic plates rupture and new oceans are born. Our aims here are to provide a framework for future research endeavors and to promote collaboration not only within the rift and rifted margins communities, but across other Earth Science disciplines

- Abstract
Continental flood basalt provinces (CFBPs) are large igneous features formed by the extrusion of massive amounts of lavas that require significant evolution within the lithosphere. Although sequential lava flows are effective probes of magmatic systems, CFBPs are typically poorly preserved. We focus on lava flows from the well-preserved 1.1 Ga Keweenawan CFBP that erupted within the Midcontinent Rift System. We present a new geochemical, petrographic, and stratigraphic synthesis from the Main stage Portage Lake Volcanics (PLV). Flow-by-flow analysis of the PLV reveals that major element behaviour is decoupled from trace element behaviour; MgO exhibits limited variability, while compatible and incompatible trace elements deviate from high to low concentrations throughout the sequence. The concentrations of incompatible trace elements slightly decrease from the base of the sequence to the top. We investigate these observations by applying a recharge, evacuation, assimilation and fractional crystallization model to geochemical and petrographic data. Our modelling demonstrates a magmatic system experiencing increased evacuation rates while fractionation and assimilation rates decrease, indicating an increase in magmatic flux. The outcome of this modelling is a progressively more efficient magma system within the PLV. This study highlights the utility of joint petrographic and geochemical interpretation in constraining CFBP magma evolution.

Bromodomains bind to acetylated lysine residues 1 to mediate a wide range of biological processes, including the assembly of transcriptional machinery at modified histones. This thesis describes the design of small molecule inhibitors of bromodomains, with particular focus on the bromodomain of CREBBP. A fragment based approach was employed to investigate bicyclic amides as acetyl lysine mimics. Initially the benzoxazinone scaffold (BNZ) 2 was shown to be a novel, ligand efficient bromodomain inhibitor. Structure based elaboration of the BNZ scaffold was employed to direct substitutions towards the region of CREBBP with greatest variability compared to other bromodomains. Ultimately, the compounds in this series were limited to micromolar affinity for CREBBP, but provided useful structure activity relationships. Subsequently the dihydroquinoxalinone scaffold (DHQN) 3 was also shown to be a novel acetyl lysine mimic. Attachment of the optimum side group identified in the BNZ series led to the discovery of the first sub micromolar inhibitor of CREBBP. A co crystal structure with CREBBP revealed that the side group of this compound bound in a newly identified induced fit pocket, mediated by a cation π interaction. A combination of structural, functional and computational studies confirmed that the cation π interaction contributed significantly towards the binding affinity of these ligands. Further work to elaborate the DHQN core, or develop an alternative acetyl lysine mimic into a CREBBP inhibitor, did not lead to an improvement. However, the optimum compound 4 was shown to displace CREBBP from chromatin in a cell based assay. Overall, cyclic amide based fragments were developed as CREBBP inhibitors, providing some of the first bromodomain ligands with nanomolar affinity outside of the BET family. In the process, key structural information about binding of ligands to CREBBP was revealed. Compound 4 provides a tool with which to study the biological implications of aberrant CREBBP activity and to investigate the therapeutic potential of bromodomain inhibition.
