Abstract

Hepatocellular carcinoma (HCC) is one of the most common malignant tumors in China. Surgery is the most important way to treat HCC currently. The high recurrence rate after hepatectomy is the most important reason to affect its prognosis, and it is also the key clinical problem to be solved. According to the recurrence time, the recurrence can be divided into early recurrence and late recurrence. The prognosis of patients with early recurrence is worse than that of patients with late recurrence. Therefore, it is very important for surgical decision-making to identify the two kinds of recurrence. This article reviews the research progress of early recurrence cut-off time of hepatocellular carcinoma after radical hepatectomy.

Abstract

Background: As an indicator of tumor invasiveness, microvascular invasion (MVI) is a crucial risk factor for postoperative relapse, metastasis, and unfavorable prognosis in hepatocellular carcinoma (HCC). Nevertheless, the genetic mechanisms underlying MVI, particularly for Chinese patients, remain mostly uncharted. Methods: We applied deep targeted sequencing on 66 Chinese HCC samples. Focusing on the telomerase reverse transcriptase (TERT) promoter (TERTp) and TP53 co‐mutation (TERTp+/TP53+) group, gene set enrichment analysis (GSEA) was used to explore the potential molecular mechanisms of the TERTp+/TP53+ group on tumor progression and metastasis. Additionally, we evaluated the tumor immune microenvironment of the TERTp+/TP53+ group in HCC using multiplex immunofluorescence (mIF) staining. Results: Among the 66 HCC samples, the mutated genes that mostly appeared were TERT, TP53, and CTNNB1. Of note, we found 10 cases with TERTp+/TP53+, of which nine were MVI‐positive and one was MVI‐negative, and there was a co‐occurrence of TERTp and TP53 (p < 0.05). Survival analysis demonstrated that patients with the TERTp+/TP53+ group had lower the disease‐free survival (DFS) (p = 0.028). GSEA results indicated that telomere organization, telomere maintenance, DNA replication, positive regulation of cell cycle, and negative regulation of immune response were significantly enriched in the TERTp+/TP53+ group (all adjusted p‐values (p.adj) < 0.05). mIF revealed that the TERTp+/TP53+ group decreased CD8+ T cells infiltration (p = 0.25) and enhanced PDL1 expression (p = 0.55). Conclusions: TERTp+/TP53+ was significantly enriched in MVI‐positive patients, leading to poor prognosis for HCC patients by promoting proliferation of HCC cell and inhibiting infiltration of immune cell surrounding HCC. TERTp+/TP53+ can be utilized as a potential indicator for predicting MVI‐positive patients and poor prognosis, laying a preliminary foundation for further exploration of co‐mutation in HCC with MVI and clinical treatment. A total of 416 patients were included, divided into an early recurrence group (N = 169) and a non-early recurrence group (N = 247), taking 12 months as the cut-off point for early recurrence. Univariate and multivariate Cox analysis was adopted to screen for risk factors for recurrence, and independence of risk factors was determined by logistic regression analysis. All variables were included in the logistic regression analysis. As previous studies have shown that tumor diameter is a risk factor for recurrence, this was also included in the analyses. Abstract

This study aimed to assess the diagnostic value of the latent transforming growth factor-beta binding protein-1 (LTBP-1) in distinguishing hepatocellular carcinoma (HCC) from patients with hepatitis or liver cirrhosis. The protein levels of LTBP-1 or AFP in circulating plasma were measured by enzyme-linked immunosorbent assay (ELISA) or chemiluminescence in four cohorts: HCC (n = 167), liver cirrhosis (n = 50), chronic hepatitis B (CHB, n = 50), and healthy individuals (n = 104). Receiver operating characteristics (ROC) curves and area under the curves (AUC) of the proteins were calculated. Results showed that plasma levels of LTBP-1 were significantly higher in HCC patients than those in other three groups. LTBP-1 showed a better diagnostic performance (AUC = 0.74, 95% CI: 0.67-0.80) in distinguishing HCC from the CHB or cirrhosis patients, compared to AFP (AUC = 0.59, 95% CI: 0.52-0.65). In the early-stage HCCs investigated, diagnostic performance of LTBP-1 (AUC = 0.77, 95% CI: 0.70-0.84) remained better than that of AFP (AUC = 0.61, 95% CI: 0.52-0.69). Combination of LTBP-1 and AFP showed increased diagnostic efficiency than any of the two proteins performed alone, for both all HCC (AUC = 0.78, 95% CI: 0.72-0.83) and early-stage HCC (AUC = 0.80, 95% CI: 0.74-0.87). These findings proposed that LTBP-1 may be a promising biomarker for distinguishing HCC from the CHB or liver cirrhosis patients, especially for the early-stage HCC.
