Abstract

Cosmetic dermatology is a key subspecialty of academic dermatology. As such, academic centers are expected to demonstrate excellence in the teaching of cosmetic dermatology skills to trainees, the clinical delivery of cosmetic dermatology services to patients, and the performance of clinical research that advances knowledge and uncovers new therapies in cosmetic dermatology. The Association of Academic Cosmetic Dermatology (AACD), a newly formed medical professional society, includes as its principal aims the support of all of these areas. AACD is comprised of group of board-certified dermatologists who teach cosmetic and laser dermatology at US dermatology residency programs. An expert panel constituted by the AACD recently convened a workshop to review gaps pertaining to academic cosmetic dermatology. This panel considered needs and potential corrective initiatives in three domains: resident education, patient experience, and clinical research. The work of the panel was used to develop a roadmap, which was adopted by consensus, and which will serve to guide the AACD moving forward.

Abstract

Cosmetic and laser procedures are increasingly popular among patients and are skills in which dermatologists are regarded as well trained. Most dermatology residents intend to incorporate cosmetic procedures into their practice and prefer to learn such procedures during residency through direct patient care. However, there are notable challenges in optimizing how residents are trained in cosmetic and laser dermatology. To address these barriers and elevate the practice of cosmetic dermatology in academic medicine, the Association of Academic Cosmetic Dermatology (AACD) was founded in 2021 as the lead professional society for dermatologists who direct the education of resident trainees in cosmetic and laser dermatology. The AACD, a group of board-certified dermatologists who teach cosmetic and laser dermatology to residents, aims to improve cosmetic dermatology education through collaboration, research, and advocacy.

Abstract

Abstract Background The risk of incident atrial fibrillation (AF) among breast cancer survivors, especially for younger women, and cancer treatment effects on the association remain unclear. This study aimed to investigate the risk of AF among breast cancer survivors and evaluate the association by age group, length of follow-up, and cancer treatment. Methods Using data from the Korean Health Insurance Service database (2010–2017), 113,232 women newly diagnosed with breast cancer (aged ≥ 18 years) without prior AF history who underwent breast cancer surgery were individually matched 1:5 by birth year to a sample female population without cancer (n = 566,160) (mean[SD] follow-up, 5.1[2.1] years). Sub-distribution hazard ratios (sHRs) and 95% confidence intervals (CIs) considering death as a competing risk were estimated, adjusting for sociodemographic factors and cardiovascular/non-cardiovascular comorbidities. Results BCS had a slightly increased AF risk compared to their cancer-free counterparts (sHR 1.06; 95% CI 1.00–1.13), but the association disappeared over time. Younger BCS (age 65 years. Use of anthracyclines was associated with increased AF risk among BCS (sHR 1.57; 95% CI 1.28–1.92), which was more robust in younger BCS (sHR 1.94; 95% CI 1.40–2.69 in those aged ≤ 50 years). Conclusions Our findings suggest that younger BCS had an elevated risk of incident AF, regardless of the length of follow-up. Use of anthracyclines may be associated with increased mid-to-long-term AF risk among BCS. Graphical Abstract

10. The Effects of Intentional Pairing of Teacher Candidates and Cooperating Teachers in Early Field Experiences

Abstract

Field experiences are critical components of educator preparation programs. The opportunity for a teacher candidate to collaborate with an experienced mentor in an authentic classroom environment can be of great benefit to the teacher candidate, the cooperating teacher, the students in the class, and the reciprocal partnership between the educator preparation program and the school district. When pairing teacher candidates with cooperating teachers, current practices are generally random and based on the number of candidates needing placement and the availability of qualified cooperating teachers willing to engage in the process of mentoring a pre-service teacher. This mixed-methods, pragmatic case study research investigates the effectiveness of an intentional, inverse pairing of teacher candidates and cooperating teachers based on their perceived competency levels in the Texas Teacher Standards. Surveys were sent to the participating teacher candidates and cooperating teachers, asking them to score themselves on a Likert scale indicating their level of competency in each area. Qualitative data were also collected from both the teacher candidates and the cooperating teachers to inform the pairing process. The data were analyzed, and pairings were made, matching teacher candidates with lower competency levels with cooperating teachers who reported higher competency in each standard. At the end of the ten-week field experience, teacher candidates were once again asked to score themselves on their perceived level of competency. Cooperating teachers scored the teacher candidate(s) with whom they worked in each standard based on what they had observed and experienced while collaborating over the ten-week period. Findings reveal that teacher candidates rated themselves higher at the conclusion of the field experiences after working with a cooperating teacher on tasks and experiences related to the Texas Teacher Standards. Ratings of the cooperating teachers were lower than that of the teacher candidates. Additional qualitative data suggest areas in which to strengthen the field experience and content covered in prerequisite courses. Implications for future research could include adding additional layers to the data collected to inform the pairings and professional development or training opportunities for teacher candidates and cooperating teachers to provide a framework for their collaboration. 15. Dermoscopy findings of a one mm lentigo maligna

Abstract

A 66-year-old man presented with a grayish 1mm macule on the left cheek. Dermatoscopic examination revealed annular-granular structures partially surrounding a single follicular ostium. Histopathologic examination demonstrated atypical, confluent single melanocytes at the basal layer with nesting, crowding, and upward migration descending along the hair follicle, consistent with the diagnosis of lentigo maligna (LM). This case presents evidence in support of the Schiffner progression model for LMs, starting with asymmetric pigmented follicular openings composed of annular-granular structures, which later coalesce into gray to dark black/brown streaks, and then form pseudo-networks or rhomboidal structures. The finding of annular-granular structures partially surrounding a single hair follicle may be the earliest recognizable dermatoscopic feature of an LM and may help clinicians diagnose facial LMs earlier.

22. Race differences in the impact of juvenile detention on likelihood of young adult incarceration of males aging out of care.

Abstract

Youth involved in the child welfare system are more likely to be involved in the juvenile justice system, and juvenile justice involvement is linked to increased likelihood of subsequent incarceration. For these reasons, overrepresentation of Black youth in the child welfare system contributes to racial disparities in incarceration rates. It is, therefore, important to study youth transitioning out of foster care to examine how their young adult outcomes differ by race. Employing the National Youth in Transition Database (NYTD), this study compared the incarceration experiences of non-Hispanic White and Black males aging out of foster care. Specifically, this study examined the impact of juvenile detention on the likelihood of incarceration ages 19 to 21and factors that mediated this relationship. Notably, 33.5% of White males and 30.7% of Black males in the sample experienced juvenile detention. Findings indicate that juvenile detention disproportionately impacts Black males who age out of foster care compared to their White counterparts. The direct effect of juvenile detention on reported incarceration between age 19–21 for Black males in the sample was OR = 2.31 compared to OR = 1.91 for White males. Reported incarceration between age 17–19 significantly mediated the relationship between juvenile confinement and incarceration age 19–21 for both Black and White males in the sample, although this effect was substantially stronger for Black males. 32. Culturally Responsive School Psychology Practice: A Study of Practitioners' Self-Reported Skills

Abstract

The American Psychological Association (APA) recognizes cultural competence as a defining feature of psychological practice, education, training, and research (Sue et al. "American Psychologist," 49, 792-796, 1999). The purpose of this study was to investigate the self-appraised cultural competence of school psychology practitioners using the Multicultural School Psychology Counseling Competency Scale (MSPCCS; Rogers & Ponterotto "Psychology in the Schools," 34(3), 211-217, 1997) as well as to investigate the convergent validity of the scale with the Cross-Cultural Counseling Inventory-Revised (CCCI-R; LaFromboise et al. "Professional Psychology: Research and Practice," 22(5), 380-388, 1991). Social desirability was also investigated to determine if there was a statistically significant correlation with self-report ratings of multicultural competence. A total of 161 participants were part of the sample that was recruited from 11 state professional school psychology associations across the USA. The results revealed that the MSPCCS demonstrates convergent validity with the CCCI-R and that on average school psychology practitioners report moderately high levels of multicultural competence. There was not a statistically significant positive correlation between total scores and social desirability, suggesting respondents gave fair estimates of their self-reported multicultural competence. Abstract

Cosmetic and laser procedures are increasingly popular among patients and are skills in which dermatologists are regarded as well trained. Most dermatology residents intend to incorporate cosmetic procedures into their practice and prefer to learn such procedures during residency through direct patient care. However, there are notable challenges in optimizing how residents are trained in cosmetic and laser dermatology. To address these barriers and elevate the practice of cosmetic dermatology in academic medicine, the Association of Academic Cosmetic Dermatology (AACD) was founded in 2021 as the lead professional society for dermatologists who direct the education of resident trainees in cosmetic and laser dermatology. The AACD,a group of board-certified dermatologists who teach cosmetic and laser dermatology to residents, aims to improve cosmetic dermatology education through collaboration, research, and advocacy.

43. Systematic review of photodynamic therapy for the treatment of hidradenitis suppurativa

Abstract

To perform a systematic review of available literature regarding the use of 5-aminolevulinic acid (ALA) and ALA derivative photodynamic therapy (PDT) in the treatment of hidradenitis suppurativa (HS) and provide recommendations on its use.A systematic review was performed of all published studies up to September 1, 2019 from nine databases, including PubMed, that evaluated PDT in the treatment of HS. For each study, quality of evidence and risk of bias was evaluated. Recommendations from the body of evidence were created based on Strength of Recommendation and Taxonomy (SORT) criteria.Eighteen studies met inclusion criteria. The majority of studies had a high risk of bias. Blue light PDT with 20% ALA and red light PDT with 16% methyl aminolevulinate (MAL) demonstrated some benefit based on a small number of poor-quality studies with a high risk of bias (Grade C, level III evidence). The most promising results were for 1%-5% ALA with intralesional diode, with good to complete response in 78%-94% of anatomic sites treated (Grade B, level II evidence).The majority of studies contained high levels of bias, with significant heterogeneity between studies. Conclusions are limited by small samples sizes, lack of randomized controlled trials, and differing protocols.Further studies are needed to determine the clinical efficacy of 20% ALA with blue light and MAL with red light. Intralesional diode PDT shows the most promise and warrants further investigation in larger, randomized controlled trials.

45. Revamping the broken electronic medical record in academic dermatology in the United States: An 'Epic' endeavor

Abstract

Many Healthcare institutions face continued challenges related to electronic medical record use (EMR). Within this viewpoint article, we seek to share with our dermatology colleagues our institutional experience on the creation of standardized dermatology records. In our experience, key elements are needed to implement a standardized record, including faculty buy-in, reconciliation with dictation, selection of a leader, and design of the standardized record template. Creation of a standardized record should not only account for the clinic but also patient handouts and support staff templates. We have obtained preliminary evidence that the use of an EMR template not only reduces the cost of dictation but also improves documentation. In addition, we have not seen evidence of over documentation. However, more studies are needed to understanding how a standardized EMR impacts billing, coding, teaching and overall, patient care.
