- Abstract
Background/objectives: The aim of the present study was to validate a short questionnaire on habitual dietary salt intake, to quickly and easily identify individuals whose salt consumption exceeds recommended levels. Subjects/methods: A total of 1131 hypertensive subjects participating in the MINISAL-SIIA study were included in the analysis. Anthropometric indexes, blood pressure, and 24-h urinary sodium excretion (NaU) were measured. A fixed-sequence questionnaire on dietary salt intake was administered. Results: NaU was significantly associated with scores, with a linear association across categories (p for trend <0.0001). In addition, participants who achieved a total score above the median value (eight points) had significantly higher NaU than those whose score was below median (p < 0.0001). In the total sample, the prevalence of “high NaU” (NaU > 85 mmol/day) and “very high NaU” (NaU > 170 mmol/day) was 86 and 35%, respectively. The score of the questionnaire had a significant ability to detect both “high NaU”—with a specificity of 95% at the score of 10 points—and “very high NaU”—with a specificity of 99.6% at score of 13 points. Conclusions: The main results of the study indicates that a higher score of this short questionnaire is distinctive of habitual high salt consumption in hypertensive patients.

Il Circular Economy Network (CEN) è un progetto che si propone, sulla spinta della strategia europee, di stimolare nel nostro Paese uno sviluppo dell’economia circolare capace di sostenere le sfide climatiche, ecologiche e sociali proposte dalla green economy, accrescendo al tempo stesso la competitività delle nostre imprese sui mercati internazionali. Esso nasce da un’iniziativa della Fondazione per lo Sviluppo Sostenibile ed è aperto a tutte le imprese che intendono condividere queste finalità mediante assunzione di azioni e impegni concreti. Oggi vi partecipano come promotori ben 13 soggetti, a cui si affiancano alcune decine di aderenti. Il lavoro del Circular Economy Network si basa su alcuni assi principali: promuovere, raccogliere e divulgare studi, ricerche ed elaborazioni sull’economia circolare; definire gli indicatori chiave di circolarità e analizzare le performance nazionali; effettuare la ricognizione delle principali criticità e delle barriere da rimuovere, indicando le possibili soluzioni; elaborare strategie, policy e misure da proporre ai decisori politici, favorendo una positiva interlocuzione tra il mondo delle imprese e le istituzioni; valorizzare e contribuire alla diffusione delle buone pratiche e delle migliori tecniche.

Transcription of subordinate genes in distinct anterior domains of Drosophila is activated by the bicoid protein (bcd). Three genes are inactivated due to the action of the receptor tyrosine kinase torso (tor). The activated tor receptor stimulates signal transduction cascade, leading to the phosphorylation of a part of the bcd protein which participates in the transcriptional activation. The phosphorylation down-regulates the bcd morphogen's activation by inhibiting it. The protein bcd can be conferred resistant to tor-mediated repression by the addition of a heterologous transcription activation domain.

The aim of this study was to detect the prevalence of resistant hypertension (RH), allowing for adherence to appropriate lifestyle measures according to European Society of Hypertension-European Society of Cardiology (ESH-ESC) 2013 guidelines, in a sample of 1284 hypertensive subjects participating at the MINISAL-SIIA study. Hypertensive patients were recruited in 47 Italian centres, recognised by the Italian Society of Hypertension. Anthropometric indexes, blood pressure and 24-h urinary sodium (Na24h) and potassium (K24h) excretion were measured. Data on antihypertensive therapy were available for 1177 (92%) subjects. The population was divided into three groups (North, Central and South), according to their geographical location. Accounting only at the treatment criteria, the prevalence of RH was 8.2% (96/1177). Following the completion in May 2011 of the shutdown for the installation of the beryllium wall and the tungsten divertor, the first set of JET campaigns have addressed the investigation of the retention properties and the development of operational scenarios with the new plasma-facing materials. The large reduction in the carbon content (more than a factor ten) led to a much lower Zeff (1.2-1.4) during L- and H-mode plasmas, and radiation during the burn-through phase of the plasma initiation with the consequence that breakdown failures are almost absent. Gas balance experiments have shown that the fuel retention rate with the new wall is substantially reduced with respect to the C wall. The re-establishment of the baseline H-mode and hybrid scenarios compatible with the new wall has required an optimization of the control of metallic impurity sources and heat loads. Stable type-I ELMy H-mode regimes with H98,y2 close to 1 and βN ∼ 1.6 have been achieved using gas injection. ELM frequency is a key factor for the control of the metallic impurity accumulation. Pedestal temperatures tend to be lower with the new wall, leading to reduced confinement, but nitrogen seeding restores high pedestal temperatures and confinement. Compared with the carbon wall, major disruptions with the new wall show a lower radiated power and a slower current quench. The higher heat loads on Be wall plasma-facing components due to lower radiation made the routine use of massive gas injection for disruption mitigation essential. © 2013 IAEA, Vienna.

Background: The aim of this study was to identify which factors were associated with a risk of hospital readmission within 3 months after discharge of a sample of elderly patients admitted to internal medicine and geriatric wards. Methods: Of the 1178 patients aged 65 years or more and discharged from one of the 66 wards of the 'Registry Politerapie SIMI (REPOSI)' during 2010, 766 were followed up by phone interview 3 months after discharge and were included in this analysis. Univariate and multivariate logistic regression models were used to evaluate the association of several variables with rehospitalization within 3 months from discharge. Results: Nineteen percent of patients were readmitted at least once within 3 months after discharge. By univariate analysis in-hospital clinical adverse events (AEs), a previous hospital admission, number of diagnoses and drugs, comorbidity and severity index (according to Cumulative Illness Rating Scale-CIRS), vascular and liver diseases with a level of impairment at discharge of 3 or more at CIRS were significantly associated with risk of readmission. Multivariate logistic regression analysis showed that only AEs during hospitalization, previous hospital admission, and vascular and liver diseases were significantly associated with the likelihood of readmission. Conclusions: The results demonstrate the need for increased medical attention towards elderly patients discharged from hospital with characteristics such as AEs during the hospitalization, previous admission, vascular and liver diseases. © 2012 European Federation of Internal Medicine. - Abstract
Burning plasma experiments such as ITER and DEMO require diagnostics capable of withstanding the harsh environment generated by the intense neutron flux and to maintain stable operating conditions for times longer than present day systems. For these reasons, advanced control and monitoring (CM) systems will be necessary for the reliable operation of diagnostics. This paper describes the CM system of the upgraded magnetic proton recoil neutron spectrometer installed at the Joint European Torus focusing in particular on a technique for the stabilization of the gain of the photomultipliers coupled to the neutron detectors. The results presented here show that this technique provides good results over long time scales. The technique is of general interest for all diagnostics that employ scintillators coupled to photomultiplier tubes. © 2009 American Institute of Physics.

- Abstract
A time-of-flight (TOF) spectrometer for measurement of the 2.5-MeV neutron emission from fusion plasmas has been developed and put into use at the JET tokamak. It has been optimized for operation at high rates (TOFOR) for the purpose of performing advanced neutron emission spectroscopy (NES) diagnosis of deuterium plasmas with a focus on the fuel ion motional states for different auxiliary heating scenarios. This requires operation over a large dynamic range, including high rates of >100 kHz with a maximum value of 0.5 MHz for the TOFOR design. This paper describes the design principles and their technical realization. The performance is illustrated with recent neutron TOF spectra recorded for plasmas subjected to different heating scenarios. A true event count rate of 39 kHz has been achieved at about a tenth of the expected neutron yield limit of JET, giving a projected maximum of 400 kHz at peak JET plasma yield. This means that the count rate capability for NES diagnosis of D plasmas has been improved more than an order of magnitude. Another important performance factor is the spectrometer bandwidth, where data have been acquired and analyzed successfully with a response function for neutrons over the energy range 1 to >5 MeV. The implications of instrumental advancement represented by TOFOR are discussed. © 2008 Elsevier B.V.

- Abstract
A time-of-flight neutron spectrometer (TOFOR) has been developed to measure the 2.45 MeV d+d→ He3 +n neutron emission from D plasmas. The TOFOR design features the capability to operate at high rates in the 100 kHz range, data collection with fast time digitizing and storing, and monitoring of the signals from the scintillation detectors used. This article describes the principles of the instrument and its installation at JET and presents preliminary data to illustrate the TOFOR performance as a neutron emission spectroscopy diagnostic. © 2006 American Institute of Physics.

- Abstract
The magnetic proton recoil neutron spectrometer has been upgraded (MPRu) with a new focal plane hodoscope detector as a major part to improve the immunity to background and to extend the measuring range from 18 down to 1.5 MeV. The MPRu detector project has entailed the development of the phoswich technique for this application. This was done through tests of prototype scintillators to reach the final design. The article reports on the tests conducted, the projected specification, and demonstrated performance through the first MPRu results obtained at JET. © 2006 American Institute of Physics.

- Abstract
The measurement of fast ion populations is one of the diagnostic capabilities provided by neutron emission spectroscopy (NES). NES measurements were carried out during JET trace tritium campaign with the magnetic proton recoil neutron spectrometer. A favorable plasma scenario is (T)D where the resulting 14 MeV neutron yield is dominated by suprathermal emission from energetic tritons accelerated by radio frequency at their fundamental cyclotron frequency. Information on the triton distribution function has been derived from NES data with a simple model based on two components referred to as bulk (B) and high energy (HE). The HE component is based on strongly anisotropic tritium distribution that can be used for routine best-fit analysis to provide tail temperature values (THE). This article addresses to what extent the THE values are model dependent by comparing the model above with a two-temperature (bi-) Maxwellian model featuring parallel and perpendicular temperatures. The bi-Maxwellian model is strongly anisotropic and frequently used for radio frequency theory. © 2006 American Institute of Physics.

Background: Traditionally, pneumonia has been classified as either community- or hospital-acquired. Although only limited data are available, health care–associated pneumonia has been recently proposed as a new category of respiratory infection. “Health care–associated pneumonia” refers to pneumonia in patients who have recently been hospitalized, had hemodialysis, or received intravenous chemotherapy or reside in a nursing home or long-term care facility. Objective: To ascertain the epidemiology and outcome of community-acquired, health care–associated, and hospital-acquired pneumonia in adults hospitalized in internal medicine wards. Design: Multicenter, prospective observational study. Setting: 55 hospitals in Italy comprising 1941 beds. Patients: 362 patients hospitalized with pneumonia during two 1-week surveillance periods. Measurements: Cases of radiologically and clinically assessed pneumonia were classified as community-acquired, health care–associated, or hospital-acquired and rates were compared. Results: Of the 362 patients, 61.6% had community-acquired pneumonia, 24.9% had health care–associated pneumonia, and 13.5% had hospital-acquired pneumonia. Patients with health care–associated pneumonia had higher mean Sequential Organ Failure Assessment scores than did those with community-acquired pneumonia (3.0 vs. 2.0), were more frequently malnourished (11.1% vs. 4.5%, and had more frequent bilateral (34.4% vs. 19.7%) and multilobar (27.8% vs. 21.5%) involvement on a chest radiograph. Patients with health care–associated pneumonia also had higher fatality rates (17.8% [CI, 10.6% to 24.9%] vs. 6.7% [CI, 2.9% to 10.5%]) and longer mean hospital stay (18.7 days [CI, 15.9 to 21.5 days] vs. 14.7 days [CI, 13.4 to 15.9 days]). Logistic regression analysis revealed that depression of consciousness (odds ratio [OR], 3.2 [CI, 1.06 to 9.8]), leukopenia (OR, 6.2 [CI, 1.01 to 37.6]), and receipt of empirical antibiotic therapy not recommended by international guidelines (OR, 6.4 [CI, 2.3 to 17.6]) were independently associated with increased intrahospital mortality. Limitations: The number of patients with health care–associated pneumonia was relatively small. Microbiological investigations were not always homogeneous. The study included only patients with pneumonia that required hospitalization; results may not apply to patients treated as outpatients. Conclusion: Health care–associated pneumonia should be considered a distinct subset of pneumonia associated with more severe disease, longer hospital stay, and higher mortality rates. Physicians should differentiate between patients with health care–associated pneumonia and those with community-acquired pneumonia and provide more appropriate initial antibiotic therapy.

- Abstract
The diagnostics functions of neutron measurements as well as the roles played by neutron yield monitors, cameras and spectrometers are reviewed. The importance of recent developments in neutron emission spectroscopy (NES) diagnostics is emphasized. Results are presented from the NES diagnosis of the Joint European Torus (JET) plasmas performed with the magnetic proton recoil (MPR) spectrometer during the first deuterium tritium experiment of 1997 and the recent trace tritium experiment of 2003. The NES diagnostic capabilities at JET are presently being enhanced by an upgrade of the MPR (MPRu) and a new 2.5 MeV time-of-flight (TOF) neutron spectrometer (TOFOR). The principles of MPRu and TOFOR are described and illustrated with the diagnostic role they will play in the high performance fusion experiments in the forward programme of JET largely aimed at supporting the International Thermonuclear Experimental Reactor (ITER). The importance of the JET NES effort for ITER is discussed. © 2005 IAEA, Vienna.

A time-of-flight neutron spectrometer (TOFOR) has been developed to measure the 2.45 MeV d+d→ He3 +n neutron emission from D plasmas. The TOFOR design features the capability to operate at high rates in the 100 kHz range, data collection with fast time digitizing and storing, and monitoring of the signals from the scintillation detectors used. This article describes the principles of the instrument and its installation at JET and presents preliminary data to illustrate the TOFOR performance as a neutron emission spectroscopy diagnostic. © 2006 American Institute of Physics.

INTRODUCTION:The aim of the study was to assess the age-specific, sex-specific, and region-specific average sodium and potassium intake and its association with anthropometric characteristics in a sample of the Italian adult hypertensive population. METHODS:A total of 1232 hypertensive patients were recruited consecutively by 47 centers recognized by the Italian Society of Hypertension. The enrolled participants were on stable antihypertensive treatment. Anthropometric indices, blood pressure, 24-h urinary sodium, and potassium excretion were measured and used as proxy for the average daily sodium and potassium intake. RESULTS:The average sodium intake was 172 mmol (or 10.1 g of salt/day) among men and 138 (or 8.1) among women, with no difference among geographical areas. Over 90% of men and 81% of women had a consumption higher than the recommended standard dietary intake of 5 g/day. The average potassium intake was 63 and 56 mmol, respectively in men and women, again without geographical differences, nearly 92% of men and 95% of women having an intake lower than the recommended intake (100 mmol/day or 3.9 g/day). Background: As chronicity represents one of the major challenges in the healthcare of aging populations, the understanding of how chronic diseases distribute and co-occur in this part of the population is needed. Objectives: The aims of this study were to evaluate and compare patterns of diseases identified with cluster analysis in two samples of hospitalized elderly. Methods: Data were obtained from the multicenter ‘Registry Politerapie SIMI (REPOSI)' that included people aged 65 or older hospitalized in internal medicine and geriatric wards in Italy during 2008 and 2010. The study sample from the first wave included 1,411 subjects enrolled in 38 hospitals wards, whereas the second wave included 1,380 subjects in 66 wards located in different regions of Italy. To analyze patterns of multimorbidity, a cluster analysis was performed including the same diseases (19 chronic conditions with a prevalence >5%) collected at hospital discharge during the two waves of the registry. Results: Eight clusters of diseases were identified in the first wave of the REPOSI registry and six in the second wave. Several diseases were included in similar clusters in the two waves, such as malignancy and liver cirrhosis; anemia, gastric and intestinal diseases; diabetes and coronary heart disease; chronic obstructive pulmonary disease and prostate hypertrophy. Conclusion: These findings strengthened the idea of an association other than by chance of diseases in the elderly population.

BACKGROUND This study was undertaken to determine whether use of the direct renin inhibitor aliskiren would reduce cardiovascular and renal events in patients with type 2 diabetes and chronic kidney disease, cardiovascular disease, or both. METHODS In a double-blind fashion, we randomly assigned 8561 patients to aliskiren (300 mg daily) or placebo as an adjunct to an angiotensin-converting-enzyme inhibitor or an angiotensin-receptor blocker. The primary end point was a composite of the time to cardiovascular death or a first occurrence of cardiac arrest with resuscitation; nonfatal myocardial infarction; nonfatal stroke; unplanned hospitalization for heart failure; end-stage renal disease, death attributable to kidney failure, or the need for renal-replacement therapy with no dialysis or transplantation available or initiated; or doubling of the baseline serum creatinine level. RESULTS The trial was stopped prematurely after the second interim efficacy analysis. After a median follow-up of 32.9 months, the primary end point had occurred in 783 patients (18.3%) assigned to aliskiren as compared with 732 (17.1%) assigned to placebo (hazard ratio, 1.08; 95% confidence interval [CI], 0.98 to 1.20; P = 0.12). Effects on secondary renal end points were similar. Systolic and diastolic blood pressures were lower with aliskiren (between-group differences, 1.3 and 0.6 mm Hg, respectively) and the mean reduction in the urinary albumin-to-creatinine ratio was greater (between-group difference, 14 percentage points; 95% CI, 11 to 17). The proportion of patients with hyperkalemia (serum potassium level, = 6 mmol per liter) was significantly higher in the aliskiren group than in the placebo group (11.2% vs. 7.2%), as was the proportion with reported hypotension (12.1% vs. 8.3%) (P CONCLUSIONS The addition of aliskiren to standard therapy with renin-angiotensin system blockade in patients with type 2 diabetes who are at high risk for cardiovascular and renal events is not supported by these data and may even be harmful. Balloon flight in polar regions - Abstract
On June 17,h 2010 at 20:30 (UTC) a Long Duration Balloon has been launched from the Nobile Amundsen base (76° 31' 26.0" N 84° 30' 55.4" W, Svalbard Islands) carrying on board the most recent Iridium® telemetry module built by an Italian factory in collaboration with an Italian research institute. The payload landed successfully on June 20th 2010 at 21:00 58 (UTC), 76° 30' 44.2" N 84° 40' 31.1" W, about 280 m a.s.l. The Bi-directional Iridium Telemetry (BIT) represents the last application of a prototype module (MSITel) whose development began years ago. A small CZT detector has been used to allow full testing of the scientific input channel. The BIT telemetry provided a true real time control and monitoring of the whole payloads through a full duplex Iridium link, all over the flight, including the ballast release and the flight termination. This BIT unit was able even to take pictures using two onboard webcams that were packed and sent to ground in low resolution mode. All the data has been stored into on board memory sticks and recovered for complete post flight data analysis. The SURECA BIT-10 mission, that once again has demonstrated the capabilities and the reliability of the MSITel module, has accomplished thanks to the joint effort of the Italian Space Agency and the Andoya Rocket Range that made feasible Long Duration Balloon flights from Northern Polar Region.

Fusion power production is the ultimate goal of fusion research, and its determination is crucial in any fusion energy application. In this paper the principles of collimated neutron flux measurements for fusion plasma power determination are described. In this method, a high-resolution neutron spectrometer provides an absolutely calibrated neutron flux, and a neutron profile monitor ("camera") gives information on the neutron emission profile of the plasma. The total neutron flux seen by the spectrometer is discussed in terms of direct and scattered flux, and a model is set up to evaluate the magnitude of these different components. Particular care is taken to estimate the uncertainties involved, both in the model and the measurements. The method is put to practical use at JET, where a magnetic proton recoil spectrometer and a neutron profile monitor are available. Results from JET's trace tritium experimental campaign in 2003 are presented and show that the systematic uncertainties in fusion power measurements are reduced in comparison to what has been presented for foil activation systems. A systematic error of 6% is reported here. For ITER these results imply that the fusion power can be redundantly measured and with better accuracies than for traditional methods.
