Summary Actinomycosis is a chronic, suppurative granulomatous infection, which is usually caused by Actinomyces israelii. These Gram-positive, anaerobic, saprophyte bacteria may form an abscess followed by sinus tract formation and cause a purulent discharge with yellowish sulfur granules. Actinomycosis can present itself with spinal cord compression, although this form is unusual and is seen in less than 5% of infections. This is a report of a patient in whom extensive actinomycosis developed in the thorax, with a sinus tract draining pus. The patient also had invasion into the spinal canal causing cord compression.

Histoplasmosis is caused by the dimorphic fungus Histoplasma capsulatum.Primary infection occurs through inhalation of spores from the air. Immunocompetent individuals are usually asymptomatic, but may develop pulmonary disease. Immunocompromised patients tend to present systemic histoplasmosis with cutaneous lesions occurring by secondary invasion. In this case report, we describe a probable primary cutaneous histoplasmosis (PCH) in a patient with adult onset Still's disease under immunosuppression.

Myocarditis is a rare but serious manifestation of patients with systemic lupus erythematosus (SLE). Left ventricular noncompaction (LVNC) is a disorder of myocardial morphogenesis frequently associated to neuromuscular diseases. Hypertrabeculation, a cardinal echocardiographic feature of LVNC, might represent a morphological expression of a number of morbidities, nevertheless. The relationship of LVNC with connective tissue disorders such as SLE is unknown. We aim to present a case of a patient with SLE who recently showed features compatible with an atypical LVNC.To report a case of a young female with a 10-year history of SLE who developed haematological disease activity and cardiac failure.Ecocardiography showed hypertrabeculation/noncompaction, a very low ejection fraction and pulmonary hypertension. Clinical and echocardiographic features reverted with standard treatment for SLE activity and cardiac insufficiency.The transitory aspect of the cardiomyopathy made unlikely a "true" LVNC for this patient, but she might have presented a lupus myocarditis with "LVNC-like" features. The occurrence of hypertrabeculated myocardium in patients with SLE warrants further studies.
