El cáncer gástrico es uno de los más frecuentes a nivel mundial. Las metástasis cerebrales por cáncer gástrico son poco frecuentes y se diagnostican en menos del 1% de los pacientes. Presentamos el caso de una mujer de 61 años con historia de disminución de la agudeza visual, cefalea y movimientos involuntarios. Le realizan una resonancia magnética que muestra una lesión de apariencia extraparenquimal occipital izquierda. El PET scan revela una zona hipermetabólica en curvatura menor del estómago, y la endoscopia evidencia una lesión sugerente de neoplasia maligna en fondo gástrico Borrmann I. Se decide realizar una excéresis tumoral por neurocirugía, cuyo estudio de anatomía patológica reveló adenocarcinoma metastásico a cerebro. Se le realiza una gastrectomía total D2, no se evidencian otras metástasis. La paciente evoluciona favorablemente en el postoperatorio. El estudio anatomopatológico revelo adenocarcinoma pobremente diferenciado. En Perú y en el mundo, aún no se han establecido recomendaciones estándar sobre cómo tratar a estos pacientes, aunque se sabe que la resección quirúrgica de metástasis cerebrales puede disminuir significativamente la morbilidad y prolongar la supervivencia en comparación con los enfoques no quirúrgicos. Hasta donde sabemos, es el primer reporte de este tipo que se presenta en el país.

Despite his wide-ranging and incisive engagement with Heidegger's thought across his career, Derrida seems to have written very little about Heidegger's Ereignis manuscripts, which, according to many commentators, constitute the place where Heidegger's thinking comes closest to Derridean deconstruction. Taking up Derrida's comments in Hospitality 1 on the figure of ‘selfhood’ ( Selbstheit) in Heidegger's Contributions to Philosophy, this essay argues that this dense but important moment of engagement with the Ereignis manuscripts reveals the extent to which Heidegger's thinking of selfhood, in spite of its fundamentally relational character, remains thoroughly determined by ipseity, the philosopheme that links selfhood, possibility, and sovereignty within the metaphysics of presence. Beginning with a reconstruction of the link between power and selfhood in Derrida's thinking of ipseity and a close-reading of the key passage in Hospitality 1, the essay then turns to Heidegger's engagement with Hölderlin to show both the depth of Heidegger's commitment to a relational thinking of selfhood and the philosophical and rhetorical safeguards by which he ensures that the relations of difference that constitute the self continue to function in the name of the ipseity, understood as the very Ur-form of sovereign power.

Osteosarcoma of the skull is a rare primary malignant bone tumour in children, representing 1–2% of all cranial tumours. We describe a case of a 17-year-old adolescent with chondroblastic osteosarcoma of the parietal-occipital bone and no distant metastases at presentation treated with neoadjuvant chemotherapy, surgery and, later, concurrent chemoradiotherapy. The patient suffered progressive disease and died 15 months after diagnosis. There are at least 15 paediatric cases of osteosarcoma of the skull described in the literature. Due to its rarity, there are no broad prospective studies on this entity, which has distinctive features when compared to other craniofacial tumours, carrying a worse prognosis. Complete surgical resection is needed for long-term survival, whereas chemotherapy and radiotherapy have still questionable indications.

Aquest article elabora algunes de les implicacions conceptuals de l’exigencia de Derrida d’un «altre pensament de la vida» a Voyous: Deux essais sur la raison. L’article soste en primer lloc que la desconstruccio de Derrida de l’oposicio entre els usos metaforics i literaris del discurs de la vida a La vie la mort es radicalitza a Voyous quan s’afirma que el «suicidi autoimmunitari» de la democracia hauria de ser el punt de partida per repensar la vida en general. Per comprendre millor aquestes tendencies autoimmuno-suicides, l’article valora el compromis de Derrida amb la figura conceptual del primer motor d’Aristotil a Voyous. L’article soste que Derrida torna a Aristotil perque el primer motor il·lustra la manera en que el concepte de vida roman historicament i estructuralment informat pel valor filosofic de la sobirania ipseitocratica, que es al·lergica a les qualitats disseminants, proliferants i impropies que la tradicio sempre ha atribuit a la democracia. Data on response and survival outcomes of Latin American patients with diffuse Large B- cell lymphoma (DLBCL) are limited. We describe the clinical, inflammatory and immunohistochemical features of a cohort of DLBCL Peruvian patients treated with chemoimmunotherapy between 2010 and 2015. Logistic models were fitted for complete response (CR), and Cox proportional-hazard regression for progression-free survival (PFS) and overall survival (OS). Seventy-three patients were included in this analysis, 41 % had high/high-intermediate IPI and 48 % had high/high-intermediate NCCN-IPI scores, 41 % had non-germinal center (NGC) profile and 36 % were double expressors. CR was attained in 63 % of patients, median PFS was 53 months and median OS was 80 months. Both IPI and NCCN-IPI scores were statistically associated with PFS and OS. Neutrophil/lymphocyte ratio (NLR) ≥4 was associated with lower odds of CR (OR 0.19, p = 0.007), worse PFS (HR 2.67, p = 0.02) and worse OS (HR 2.77, p = 0.02). This essay seeks to account for Derrida's striking claims towards the end of The Death Penalty, Volume I concerning the survivability of the death penalty. Why will the death penalty live on, even beyond its end? How should we understand this claim? And what are the implications of the death penalty's survival for any attempt to continue Derrida's on-going efforts to deconstruct sovereignty in the name of what he often calls ‘the unconditional’? I suggest that the survivability of the death penalty is a crucial element in Derrida's attempt to articulate a different abolitionism, an abolitionism that has parted ways with all certainty concerning the death penalty's ‘end’.

Sovereignty: An Infrapolitical Question

Competition is a very important preconditionwhich affects the effectiveness of development of national economy under the conditions of globalization. In classical economics, the competitiveness of countries is determined through production inputs. In the modern era of globalization, it appears that, besides quantifiable factors, qualitative influences or ‘soft’ factors such as political stability, government policies, quality of education, etc., are all important in determining competiveness. The World Economic Forum’s global competitiveness index and the IMD World Competitiveness Yearbook (WCY) are the two most widely used competitiveness indices. Using the same data as the WCY, Principal Components Analysis (PCA) is used in this analysis to develop indices of countries’ competitiveness. The procedure deals with first transforming the original variables to a new set of uncorrelated variables called Principal Components (PC) . The new variables are linear combinations of the original variables, independent, and are derived in order of decreasing importance--the first PC accounts for as much as possible of the variation in the original data. We find that the WCY data collection methods could be simplified without compromising quality--which may encourage more countries to participate in the survey. Moreover, the approach developed in this study does not suffer from the same empirical limitations of past attempts to develop indices of the competitiveness of nations.

The past thirty years or so has seen microfinance take off from small group-based lending experiments to several thousand financial service providers (FSPs) serving a growing portion of the developing world today. Nevertheless, the challenge to improve broadbased access to financial services—going beyond credit and into other products such as savings, insurance and money transfer services—remains. Where is the microfinance industry headed? This essay reviews the available evidence, and argues that both the public and private spheres are crucial to the continued dynamism and expansion of the microfinance industry—the private sector as a continued source of product and process innovations; and the public sector taking on a strong market enabling and development role.
