Abstract

RESUMEN Objetivo: Evaluar eficacia y seguridad de la traqueostomía percutánea, mediante dilatación única con asistencia fibrobroncoscópica, en pacientes críticos sometidos a ventilación mecánica. Métodos: Entre los años 2004 y 2014, se incluyeron prospectivamente 512 enfermos consecutivos con indicación de traqueostomía según los criterios clínicos de nuestro centro. Un tercio de los pacientes fueron de alto riesgo. Se registraron variables demográficas, puntaje APACHE II, y días de ventilación mecánica previo a traqueostomía percutánea. La eficacia del procedimiento fue evaluada mediante tasa de éxito en su ejecución y necesidad de conversión a técnica abierta. La seguridad fue evaluada por tasa de complicaciones operatorias y postoperatorias. Resultados: La edad media del grupo fue 64 ± 18 años (203 mujeres y 309 varones). El puntaje APACHE II fue 21 ± 3. Los pacientes permanecieron en promedio 11 ± 3 días en ventilación mecánica antes de la realización de la traqueostomía percutánea. Todos los procedimientos se completaron exitosamente, sin necesidad de convertir a técnica abierta. Dieciocho pacientes (3,5%) presentaron complicaciones operatorias. Cinco pacientes experimentaron desaturación transitoria, cuatro presentaron hipotensión relacionada a la sedación, y nueve presentaron sangrado menor, pero ninguno requirió transfusión. No se registraron complicaciones graves, ni muertes asociadas al procedimiento. Once pacientes (2,1%) presentaron complicaciones postoperatorias. Siete presentaron sangrado menor y transitorio del estoma de la traqueostomía percutánea, 2 sufrieron desplazamiento de la cánula de traqueostomía y 2 desarrollaron infección superficial del estoma. Conclusión: La traqueostomía percutánea mediante la técnica de dilatación única con asistencia fibrobroncoscópica, parece ser efectiva y segura en enfermos críticos sometidos a ventilación mecánica, cuando es realizada por intensivistas experimentados mediante un abordaje estandarizado.

Abstract

In this article, we describe a prototype photoreactor of which the geometrical configuration was obtained by Genetic Algorithms to maximize the residence time of the reactant gases. A gas reaction mixture of CO2:H2O (1:2 molar ratio) was studied from the fluid dynamic point of view. The two main features of this prototype reactor are the conical shape, which enhances the residence time as compared to a cylindrical shape reference reactor, and the inlet heights and position around the main chamber that enables turbulence and mass transfer control. Turbulence intensity, mixing capability, and residence time attributes for the optimized prototype reactor were calculated with Computational Fluid Dynamics (CFD) software and compared with those from a reference reactor. Turbulence intensity near the envisioned catalytic bed was one percentage point higher in the reference than in the optimized prototype reactor. Finally, the homogeneity of the mixture was guaranteed since both types of reactors had a turbulent regime, but for the prototype the CO2 mass fraction was found to be better distributed. Abstract: A kinetic study of the liquid phase naphthalene hydrogenation over a commercial and sulphided NiMo/γ-Al2O3 was performed using experimental data obtained in a Robinson–Mahoney reactor at 523–583K and 2.0–4.0MPa. The effect of H2S on the hydrogenation rate was investigated varying its partial pressure from 0.09 to 0.89MPa. Under these conditions, an exponential decrease in the hydrogenation rate was observed as the H2S partial pressure increased. The naphthalene conversion ranged from 6 to 89% with tetralin as the main hydrogenation product with selectivities of at least 89%. Reaction networks were formulated based on stepwise hydrogenation mechanisms assuming that both hydrogen and hydrogen sulphide could be dissociated either homolytically or heterolytically on both coordinatively unsaturated metal ion sites (CUS) and sulphur anions (SA) of the sulphided catalyst. Langmuir–Hinshelwood rate equations were derived and discriminated through model regression to experimental data. Two models best describe the experimental data according to a statistical analysis and the physical meaning of the parameter estimates. These models correspond to the third hydrogen addition chemisorbed on CUS and supplied from either homolytic or heterolytic hydrogen dissociation, as rate-determining step. The calculated catalyst surface species concentrations indicate that hydrogen and sulphydril groups are the most abundant species under the investigated operating conditions while the hydrocarbons surface concentration is almost negligible. Abstract

The effect of microwave radiation on the hydrothermal synthesis of the double perovskite Sr2FeMoO6 has been studied based on a comparison of the particle size and structural characteristics of products from both methods. A temperature, pressure, and pH condition screening was performed, and the most representative results of these are herein presented and discussed. Radiation of microwaves in the hydrothermal synthesis method led to a decrease in crystallite size, which is an effect from the reaction temperature. The particle size ranged from 378 to 318 nm when pH was 4.5 and pressure was kept under 40 bars. According to X-ray diffraction (XRD) results coupled with the size-strain plot method, the product obtained by both synthesis methods (with and without microwave radiation) have similar crystal purity. The Scanning Electron Microscopy (SEM) and Energy Dispersive X-ray Spectroscopy (EDS) techniques showed that the morphology and the distribution of metal ions are uniform. The Curie temperature obtained by thermogravimetric analysis indicates that, in the presence of microwaves, the value was higher with respect to traditional synthesis from 335 K to 342.5 K. Consequently, microwave radiation enhances the diffusion and nucleation process of ionic precursors during the synthesis, which promotes a uniform heating in the reaction mixture leading to a reduction in the particle size, but keeping good crystallinity of the double perovskite. Precursor phases and the final purity of the Sr2FeMoO6 powder can be controlled via hydrothermal microwave heating on the first stages of the Sol-Gel method. [ABSTRACT FROM AUTHOR]

43. Quantum Chemical Calculations in Stepwise Adrenaline Deprotonation

Abstract

Theoretical calculations of adrenaline structures formed during the in-vacuo deprotonation were performed using the DFT approach. The electronic properties of these chemical structures such as the atomic charge, the HOMO-LUMO electron distribution and its energy associated are presented and discussed. According to our results, the proton loss is more feasible during the deprotonation process of the amino group followed by cathecol aromatic ring deprotonation and then by the hydrogen loss of the aliphatic hydroxyl group. In fact, the results of the energy values of the latter deprotonation process indicate that it is very unlikely that the aliphatic OH group might loss its proton since it yields unstable chemical species. Under biological pH environment, adrenaline is a fully protonated species which avoids the amino group might be modified by oxidation-reduction reactions. This causes, however, that the cathecol aromatic ring be sensitive to chemical modification by the aforementioned reactions since adrenaline electronic properties are modified by the protonated amino group. This indicates that orbital re-distribution of the molecule might occur due to the proton presence.
