9. Career Development in Spain.

- Abstract
In Spain, the term "career development" refers to vocational or career guidance services. The 1983 Law on University Reform conferred on universities the freedom of teaching, endowing them with legal status and administrative powers. Thus, there are no regulations regarding guidance at this educational level. Guidance departments in secondary schools coordinate the work of tutors and provide counseling. Vocation or career guidance services are being transferred to autonomous communities. Services are organized according to a four-stage model that enables clients to follow an individual career path. The four stages are: (1) identification of employment skills; (2) positioning in the labor market; (3) matching qualifications with labor market requirements; and (4) active job search. A summary of current policy issues include: state and regional governments do not usually grant guidance the importance it deserves; the number of specialized guidance professionals is small considering the great demand for services; guidance initiatives are increasing; and policies are uneven concerning guidance in the different educational levels. Action steps that Spain must consider for the future include: identifying overlapping policies, improving the current provisions for career guidance; promoting research on the delivery of career development; and developing comprehensive lifelong career guidance programs. - Abstract

16. Linfoma Anaplásico de Células Grandes Asociado a Implantes Mamarios
- Author

- Abstract

HELENA study group., [Aim]: To assess if a healthy diet might attenuate the positive sedentary–inflammation relation, whereas an unhealthy diet may increase the effect of sedentary behaviors on inflammatory biomarkers., [Methods]: In 618 adolescents (13–17 years) of the European HELENA study, data were available on body composition, a set of inflammation markers, and food intake assessed by a self-administered computerized 24 h dietary recall for 2 days. A 9-point Mediterranean diet score and an antioxidant-rich diet z-score were used as dietary indices and tested as moderators. A set of low-grade inflammatory characteristics was used as outcome: several cytokines in an inflammatory ratio (IL-6, IL-10, TNF-α, TGFβ-1), C-reactive protein, three cell-adhesion molecules (sVCAM-1, sICAM-1, sE-selectin), three cardiovascular risk markers (GGT, ALT, homocysteine) and three immune cell types (white blood cells, lymphocytes, CD3). Sedentary behaviors were self-reported and analyzed as total screen time. Multiple linear regression analyses tested moderation by diet in the sedentary behaviors–inflammation association adjusted for age, sex, country, adiposity (sum of six skinfolds), parental education, and socio-economic status., [Results]: Both diet scores, Mediterranean and antioxidant-rich diet, were significant protective moderators in the effect of sedentary behaviors on alanine-transaminase enzyme (P = 0.014; P = 0.027), and on the pro/anti-inflammatory cytokine ratio (P = 0.001; P = 0.004), but not on other inflammatory parameters., [Conclusion]: A higher adherence to the Mediterranean diet or an antioxidant-rich diet may attenuate the onset of oxidative stress signs associated by sedentary behaviors, whereas a poor diet seems to increase inflammation., The HELENA Study was carried out with the financial support of the European Community Sixth RTD Framework Programme (Contract FOODCT-2005-007034).

- Abstract

Abstract Background The liver X receptors (LXR) α and β regulate lipid and carbohydrate homeostasis and inflammation. Lxrβ-/- mice are glucose intolerant and at the same time lean. We aimed to assess the associations between single nucleotide polymorphisms (SNPs) in LXRβ and risk of type 2 diabetes mellitus (T2DM), obesity and related traits in 3 separate cohort studies. Methods Twenty LXRβ SNPs were identified by sequencing and genotyped in the HUNT2 adult nested case-control study for T2DM (n = 835 cases/1986 controls). Five tag-SNPs (rs17373080, rs2695121, rs56151148, rs2303044 and rs3219281), covering 99.3% of the entire common genetic variability of the LXRβ gene were identified and genotyped in the French MONICA adult study (n = 2318) and the European adolescent HELENA cross-sectional study (n = 1144). In silico and in vitro functionality studies were performed. Results We identified suggestive or significant associations between rs17373080 and the risk of (i) T2DM in HUNT2 (OR = 0.82, p = 0.03), (ii) obesity in MONICA (OR = 1.26, p = 0.05) and (iii) overweight/obesity in HELENA (OR = 1.59, p = 0.002). An intron 4 SNP (rs28514894, a perfect proxy for rs17373080) could potentially create binding sites for hepatic nuclear factor 4 alpha (HNF4α) and nuclear factor 1 (NF1). The C allele of rs28514894 was associated with ~1.25-fold higher human LXRβ basal promoter activity in vitro. However, no differences between alleles in terms of DNA binding and reporter gene transactivation by HNF4α or NF1 were observed. Conclusions Our results suggest that rs17373080 in LXRβ is associated with T2DM and obesity, maybe via altered LXRβ expression.

Background: An adequate nutritional status of antioxidant vitamins (vitamins A, C, E) and β-carotene is essential especially during childhood and adolescence, because of their important roles in cell growth and development. Currently, there are no physiological reference values for blood concentration of these vitamins and β-carotene in apparently healthy European adolescents. The aim of the current study was to obtain reliable and comparable data of antioxidant vitamins and β-carotene in a cross-sectional study, within HELENA (Healthy Lifestyle in Europe by Nutrition in Adolescence), which was conducted in a representative sample of adolescents from ten European cities. Material and Methods: From a subsample of 1,054 adolescents (males= 501) of the HELENA Cross Sectional Study with an age range of 12.5 to 17.49 years, fasting blood samples were taken and analyzed for vitamins A, E, C, and β-carotene status. As specific reference values for adolescents are missing, percentile distribution by age and sex is given. Results: Mean concentrations were the following: Retinol: 356.4 ± 107.9 cm/mL; α-tocopherol: 9.9 ± 2.1 μg/mL; vitamin C: 10.3 ± 3.3 mg/L; and β-carotene: 245.6 ± 169.6 cm/mL. Females showed higher α-tocopherol and vitamin C values compared with males and 17-year-old boys had higher retinol levels than the same-aged girls (p = 0.018). Retinol serum concentrations increased significantly according to age in both gender, but girls had also significantly increasing β-carotene levels by age. Conclusions: For the first time, concentrations of antioxidant vitamins and pro-vitamin β-carotene have been obtained in a representative sample of apparently healthy European adolescents. These data can contribute to the establishment of reference ranges in adolescents. © 2011 Hans Huber Publishers, Hogrefe AG, Bern.

Diet quality is influenced by socio-economic and geographical factors. The present study sought to assess whether adolescents' diet quality is affected by their parents' socio-economic status and whether the relationship between these factors is similar in northern and southern Europe. Data collected in the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) study in eight European countries were analysed. Dietary intake data were recorded via repeated 24 h recalls (using specifically developed HELENA Dietary Intake Assessment Tool software) and converted into an adolescent-specific Diet Quality Index (DQI-AM). Socio-economic status was estimated through parental educational level (Par-Educ-Lev) and parental occupation level (Par-Occ-Lev) as reported by the adolescents in a specific questionnaire. The DQI-AM data were then analysed as a function of Par-Educ-Lev and Par-Occ-Lev in northern European countries (Austria, Belgium, France, Germany and Sweden) and southern European countries (Greece, Italy and Spain). We studied a total of 1768 adolescents (age 14·7 (sd 1·3) years; percentage of girls: 52·8 %; 1135 and 633 subjects from northern and southern Europe, respectively). On average, the DQI-AM score was higher in southern Europe than in northern Europe (69·1 (sd 0·1) v. 60·4 (sd 2·8), respectively; PP interaction = 0·004 for the mother and 0·06 for the father). The DQI was also positively correlated with Par-Occ-Lev (all P trends P interaction = 0·51 for the mother and 0·50 for the father). In conclusion, Par-Educ-Lev and Par-Occ-Lev are associated with diet quality in adolescents in Europe. However, this association differs between northern Europe and southern Europe.

This report aims (1) to examine the association between seasonality and physical activity (PA) and sedentary time in European adolescents and (2) to investigate whether this association was influenced by geographical location (Central-North versus South of Europe), which implies more or less extreme weather and daylight hours. Valid data on PA, sedentary time and seasonality were obtained in 2173 adolescents (1175 females; 12.5-17.5 years) included in this study. Physical activity and sedentary time were measured by accelerometers. ANCOVA was conducted to analyse the differences in PA and sedentary time across seasons. Results showed that girls had lower levels of moderate to vigorous PA (MVPA) and average PA, and spent more time in sedentary activities in winter compared with spring (all P0.05). Stratified analyses showed differences in PA and sedentary time between winter and spring in European girls from Central-North of Europe (P0.05 for sedentary time). There were no differences between PA and sedentary time across seasonality in boys. In conclusion, winter is related with less time spent in MVPA, lower average PA and higher time spent in sedentary activities in European adolescent girls, compared with spring. These differences seem to mainly occur in Central-North Europe.

Hereditary cancers are the result of germline mutations in specific genes that increase susceptibility to cancer. This susceptibility is spread among family members according to different patterns of inheritance. Inherited susceptibility to cancer, does not imply the certainty of developing it in all cases. In this sense many questions have come out in recent years, not only in the medical sense but also in the field of bioethics and the law.

Hereby we present a case of a 43-years-old male who complained of a three years history preauricular painful mass. Fine needle aspiration cytology was performed, diagnosing of compatible with acinar cell carcinoma, thus DNA quantification by image cytometry was carried out. Biological parameters studied (ploidy, S-phase, 5-c exceeding rate) showed that it is a low grade of malignancy lesion. Total parotidectomy conservative of facial nerve was recommended, without regional lymphadenectomy. Patient remains, one year later, asymptomatic and free of disease.

- Abstract
Ni-based oxygen carriers allow to work at high temperatures (900-1100ºC) in a Chemical-Looping Combustion (CLC) process with full CH4 conversion, although thermodynamic restrictions result in the presence of CO and H2 in the gas outlet of the fuel reactor. On the other hand, Cu-based oxygen carriers allow complete fuel combustion to CO2 and H2O, but the operating temperature is limited due to the low melting point of the metallic Cu. The objective of this research was to analyze the behavior of several Ni-Cu oxygen carriers to reduce or avoid CO and H2 emissions during a CLC process working at high temperatures. Commercial Al2O3 and Al2O3 were used as support to prepare by dry impregnation different oxygen carriers based on nickel and copper. The reactivity of these oxygen carriers was determined in a thermogravimetric analyzer (TGA). The effect of the mixture of NiO and CuO on the CO and H2 generation was analyzed in a fixed bed reactor, and the gas product distribution during reduction/oxidation reactions was studied in a batch fluidized bed reactor working with CH4 as fuel and diluted air for oxidation. The fluidization behavior of the oxygen carriers with respect to the attrition and agglomeration processes was also analyzed during the multi-cycle batch fluidized bed tests. The presence of CuO in the Ni-Cu oxygen carriers allows the full conversion of CH4 to CO2 and H2O in the batch fluidized bed reactor during the initial part of the reduction time, and this time depended on the CuO content of the oxygen carrier. TGA and X-ray diffraction studies indicated that CuO is used for the reduction reaction before NiO. In addition, it was observed that the presence of NiO stabilized the CuO and allowed to work at 950 ºC with Ni-Cu oxygen carriers with a high metal oxide utilization.

- Abstract
Chemical Looping Combustion (CLC) is nowadays an attractive option to decrease the greenhouse gas emissions affecting global warming, because it is a combustion process with inherent CO2 separation and, therefore, without energy losses. The CLC concept is based on the transfer of oxygen from the combustion air to fuel by means of an oxygen carrier in the form of a metal oxide. The system consists of two separate but interconnected reactors, normally fluidized bed type. In the fuel reactor the oxygen carrier particles react with fuel and generate a gas stream mainly composed by CO2 and H2O. The reduced metal oxide is later transported to the air reactor where oxygen from the air is transferred to the particles, obtaining in this way the original metal oxide ready to be returned to the fuel reactor for a new cycle. In this work, a 10 kW pilot plant composed of two interconnected bubbling fluidized bed reactors has been design and built to demonstrate the CLC technology. The prototype was run during 200 h, of which 120 h burning methane, and the effect of the operating conditions (oxygen carrier to fuel ratio, fuel gas velocity, oxygen carrier particle size and fuel reactor temperature) on fuel conversion was analyzed working with a CuO-Al2O3 oxygen carrier prepared by dry-impregnation. Also, the behavior with respect to attrition, agglomeration, and reactivity of the oxygen carrier was analyzed. It was found that the most important parameter affecting the CH4 conversion was the oxygen carrier to fuel ratio. Complete methane conversion, without CO or H2 emissions, was reached with this oxygen carrier working at 800ºC and oxygen carrier to fuel ratios higher than 1.4.

- Abstract
Chemical-looping combustion (CLC) is a two-step combustion process that produces a pure CO2 stream, ready for compression and sequestration. A CLC system is composed by two reactors, an air and a fuel reactor, and an oxygen carrier (OC) circulating between the reactors, which transfers the oxygen necessary for the fuel combustion from the air to the fuel. This system can be designed similar to a circulating fluidised bed, but with the addition of a bubbling fluidised bed on the return side. A mapping of the range of operational conditions, design values, and OC characteristics is presented for the most usual metal oxides (CuO, Fe2O3, and NiO) and different fuel gases (CH4, H2, and CO). The pressure operation of a CLC system is also considered. Moreover, a comparison of the possible use of three high reactive OCs (Cu10Al-I, Fe45Al-FG, Ni40Al-FG) previously characterised is carried out. It was found that the circulation rates and the solids inventories are linked, and the possible operating conditions are closely dependent on the reactivity of the OCs. The operational limits of the solids circulation rates, given by the mass and heat balances in the system, were defined for the different type of OCs. Moreover, a plot to calculate the solids inventories in a CLC system, valid for any type of OC and fuel gas, is proposed. The minimum solids inventories depended on the fuel gas used, and followed the order CH4 > CO > H2. Values of minimum solids inventories in a range from 40 to 133 kg / MWf were found for the OCs used in this work, excepting for the reaction of Fe45Al-FG with CH4, which needs a higher amount of solids because of its low reactivity. From the economic analysis carried out it was found the cost of the OC particles does not represent any limitation to the development of the CLC technology. © 2006 Elsevier Ltd. All rights reserved.

- Abstract
Chemical-looping combustion (CLC) is a combustion technology with inherent separation of the greenhouse gas CO2 that involves the use of an oxygen carrier, which transfers oxygen from air to the fuel avoiding the direct contact between them. An oxygen carrier in a CLC power plant must show high reaction rates and sufficient durability in successive cycle reactions. Furthermore, the oxygen carrier particles must not agglomerate. To produce materials with these characteristics, different Cu-based oxygen carriers with a range of CuO content between 10 and 26 wt % were prepared by impregnation using alumina as support. The particles were calcined at different temperatures in the range of 550-950 °C. The oxygen carriers were characterized by X-ray diffraction, scanning electron microscopy with energy-dispersive X-ray, and thermogravimetry. The samples were evaluated in a fluidized bed facility in order to determine their behavior in multicycle reduction-oxidation tests. A number of 100 cycles was done when the carrier did not agglomerate. The effects of metal content, calcination temperature, and method of preparation on the oxygen carriers reactivity, gas product distribution, attrition rate, and agglomeration were analyzed for the different oxygen carriers. It was observed that the CuO content in the oxygen carrier, the calcination temperature used in the preparation, and the conversion reached by the oxygen carrier during the reduction period affected the agglomeration process. In this work, the preparation conditions to produce Cu-based oxygen carriers with high reactivities, and small attrition rates were found. Moreover, the agglomeration of these oxygen carriers in the fluidized reactor, which is the main reason adduced in the literature to reject the Cu-based oxygen carriers for the CLC process, was avoided. © 2005 American Chemical Society.
