Abstract

Background: Distinction of IDH mutant and wildtype GBMs is challenging on MRI, since conventional imaging shows considerable overlap. While few studies employed deep-learning in a mixed low/high grade glioma population, a GBM-specific model is still lacking in the literature. Our objective was to develop a deep-learning model for IDH prediction in GBM by using Convoluted Neural Networks (CNN) on multiparametric MRI. Methods: We included 100 adult patients with pathologically proven GBM and IDH testing. MRI data included: morphologic sequences, rCBV and ADC maps. Tumor area was obtained by a bounding box function on the axial slice with widest tumor extension on T2 images and was projected on every sequence. Data was split into training and test (80:20) sets. A 4 block 2D - CNN architecture was implemented for IDH prediction on every MRI sequence. IDH mutation probability was calculated with softmax activation function from the last dense layer. Highest performance was calculated accounting for model accuracy and categorical cross-entropy loss (CCEL) in the test cohort. Results: Our model achieved the following performance: T1 (accuracy 77%, CCEL 1.4), T2 (accuracy 67%, CCEL 2.41), FLAIR (accuracy 77%, CCEL 1.98), MPRAGE (accuracy 66%, CCEL 2.55), rCBV (accuracy 83%, CCEL 0.64). ADC achieved lower performance. Conclusion: We built a GBM-tailored deep-learning model for IDH mutation prediction, achieving accuracy of 83% with rCBV maps. High predictivity of perfusion images may reflect the known correlation between IDH, hypoxia inducible factor (HIF) and neoangiogenesis. This model may set a path for non-invasive evaluation of IDH mutation in GBM.

Abstract

Radiomic models have been shown to outperform clinical data for outcome prediction in glioblastoma (GBM). However, clinical implementation is limited by lack of parameters standardization. We aimed to compare nine machine learning classifiers, with different optimization parameters, to predict overall survival (OS), isocitrate dehydrogenase (IDH) mutation, O-6-methylguanine-DNA-methyltransferase (MGMT) promoter methylation, epidermal growth factor receptor (EGFR) VII amplification and Ki-67 expression in GBM patients, based on radiomic features from conventional and advanced MR. 156 adult patients with pathologic diagnosis of GBM were included. Three tumoral regions were analyzed: contrast-enhancing tumor, necrosis and non-enhancing tumor, selected by manual segmentation. Radiomic features were extracted with a custom version of Pyradiomics, and selected through Boruta algorithm. A Grid Search algorithm was applied when computing 4 times K-fold cross validation (K=10) to get the highest mean and lowest spread of accuracy. Once optimal parameters were identified, model performances were assessed in terms of Area Under The Curve-Receiver Operating Characteristics (AUC-ROC). Metaheuristic and ensemble classifiers showed the best performance across tasks. xGB obtained maximum accuracy for OS (74.5%), AB for IDH mutation (88%), MGMT methylation (71,7%), Ki-67 expression (86,6%), and EGFR amplification (81,6%). Best performing features shed light on possible correlations between MR and tumor histology.

Abstract

We study a family of McKean-Vlasov (mean-field) type ergodic optimal control problems with linear control, and quadratic dependence on control of the cost function. For this class of problems we establish existence and uniqueness of an optimal control. We propose an $N$-particles Markovian optimal control problem approximating the McKean-Vlasov one and we prove the convergence in relative entropy, total variation and Wasserstein distance of the law of the former to the law of the latter when $N$ goes to infinity. Some McKean-Vlasov optimal control problems with singular cost function and the relation of these problems with the mathematical theory of Bose-Einstein condensation is also established., Comment: Introduction has been modified. Abstract

A stochastic approach to the (generic) mean-field limit in Bose-Einstein Condensation is described and the convergence of the ground state energy as well as of its components are established. For the one-particle process on the path space a total variation convergence result is proved. A strong form of Kac's chaos on path-space for the $k$-particles probability measures are derived from the previous energy convergence by purely probabilistic techniques notably using a simple chain-rule of the relative entropy. The Fisher's information chaos of the fixed-time marginal probability density under the generic mean-field scaling limit and the related entropy chaos result are also deduced.

Abstract

While domestic horses can be trained to choose the highest numerosity, spontaneous choice tests led to contrasting results, which in part might have depended upon the adopted experimental protocols. To bring further evidence, we carried out two experiments based on binary choice tests (1:4, 1:2, 2:3, 4:6 apple halves). We tested both horses used for schooling (N = 25) and semi-wild bred horses (N = 10), using two different arenas. In both experiments, horses did not choose the highest quantity, irrespective of test difficulty. Horses preferentially chose the stimulus located at their left, while no other tested variable affected their choice. The consistency between the results suggests that the lack of discrimination did not depend on test protocols. The strong preference for a given side and high within-individual repeatability indicate that lateralization may affect the results of spontaneous tests. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Implantation failure after transferring morphologically "good‐quality" embryos in in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) may be explained by impaired endometrial receptivity. Analyzing the endometrial transcriptome analysis may reveal the underlying processes and could help in guiding prognosis and using targeted interventions for infertility. This exploratory study investigated whether the endometrial transcriptome profile was associated with short‐term or long‐term implantation outcomes (ie success or failure). Material and methods: Mid‐luteal phase endometrial biopsies of 107 infertile women with one full failed IVF/ICSI cycle, obtained within an endometrial scratching trial, were subjected to RNA‐sequencing and differentially expressed genes analysis with covariate adjustment (age, body mass index, luteinizing hormone [LH]‐day). Endometrial transcriptomes were compared between implantation failure and success groups in the short term (after the second fresh IVF/ICSI cycle) and long term (including all fresh and frozen cycles within 12 months). The short‐term analysis included 85/107 women (33 ongoing pregnancy vs 52 no pregnancy), excluding 22/107 women. The long‐term analysis included 46/107 women (23 'fertile' group, ie infertile women with a live birth after ≤3 embryos transferred vs 23 recurrent implantation failure group, ie no live birth after ≥3 good quality embryos transferred), excluding 61/107 women not fitting these categories. As both analyses drew from the same pool of 107 samples, there was some sample overlap. Additionally, cell type enrichment scores and endometrial receptivity were analyzed, and an endometrial development pseudo‐timeline was constructed to estimate transcriptomic deviations from the optimum receptivity day (LH + 7), denoted as ΔWOI (window of implantation). Results: There were no significantly differentially expressed genes between implantation failure and success groups in either the short‐term or long‐term analyses. Principal component analysis initially showed two clusters in the long‐term analysis, unrelated to clinical phenotype and no longer distinct following covariate adjustment. Cell type enrichment scores did not differ significantly between groups in both analyses. However, endometrial receptivity analysis demonstrated a potentially significant displacement of the WOI in the non‐pregnant group compared with the ongoing pregnant group in the short‐term analysis. Conclusions: No distinct endometrial transcriptome profile was associated with either implantation failure or success in infertile women. Abstract

Objective: Middle ear cholesteatoma may lead to the erosion of the bony structures of the temporal bone, possibly causing intra- and extracranial complications. Surgical treatment is mandatory, and due to possible residual/recurrent disease, the use of reliable diagnostic methods is essential. Our study aimed to evaluate the reliability of non-EPI DW-MRI for the follow-up of cholesteatoma after surgery. Methods: In a study group including 53 consecutive patients who underwent surgery for cholesteatoma at a tertiary university hospital, an imaging protocol was applied, including non-echo planar diffusion-weighted imaging magnetic resonance (MR) at 1 month after surgery and then at 6 and 12 months after surgery. Based on the combination of preoperative assessment and intraoperative findings, the study group was divided into 3 subgroups: petrous bone (PB) cholesteatoma, complicated cholesteatoma and uncomplicated cholesteatoma. PB cholesteatoma patients were treated by a subtotal petrosectomy, whereas complicated and uncomplicated cholesteatoma patients were treated either by a canal wall up procedure or a retrograde (inside-out) canal wall down technique with bone obliteration technique (BOT). Results: The results show that patients who had positive findings on non-EPI DW-MRI scans 1 month after surgery consequently underwent revision surgery during which residual cholesteatoma was noted. All the patients who displayed negative findings on non-EPI DWI-MRI scan at 1 month after surgery did not show the presence of a lesion at the 6- and 12-month evaluations. The 6 patients who displayed residual cholesteatoma at the 1-month follow-up presented dehiscence/exposure of the facial nerve canal at the primary surgery, mostly at the level of the labyrinthine segment. Conclusion: Non-EPI DW-MRI is a useful and reliable tool for follow-up cholesteatoma surgery, and when applied early, as was done in the protocol proposed in the present study, this tool may be used to detect the presence of residual cholesteatoma in some patients, prompting the planning of early revision surgery.
