Abstract

ObjectiveTo describe clinical and radiologic features associated with myelin oligodendrocyte glycoprotein antibodies (MOG-Ab) in a large French nationwide adult cohort, to assess baseline prognostic features of MOG-Ab-associated diseases after a first acute demyelinating syndrome, and to evaluate the clinical value of MOG-Ab longitudinal analysis.MethodsClinical data were obtained from 197 MOG-Ab-positive patients ≥18 years of age. Complete imaging data were available in 108, and 54 serum samples were eligible for longitudinal evaluation. For survival analysis comparison, 169 aquaporin-4 antibody (AQP4-Ab)-positive patients from the NOMADMUS database were included.ResultsMedian age at onset was 36.46 (range 18.0–76.8) years, and patients were predominantly white (92.9%) with male:female ratio, 1.1. Clinical phenotype at onset included optic neuritis or myelitis in 90.86%, isolated brainstem or encephalopathy syndromes in 6.6%, and a combination of syndromes in 2.5%. Distinctive brain MRI findings in MOG-Ab-positive patients were thalamic and pontine lesions. Cortical and leptomeningeal lesions were found in 16.3% and 6.1%, respectively. The probability of reaching a first relapse after 2 and 5 years was 44.8% and 61.8%, respectively. MOG-Ab-positive patients were at lower risk at presentation of further clinical relapse (hazard ratio [HR] 0.45, 95% confidence interval [CI] 0.26–0.79) compared to AQP4-Ab-positive individuals. MOG-Ab-positive individuals had a lower risk of reaching Disability Status Scale score of 3.0 (HR 0.46, 95% CI 0.22–0.94) and visual acuity of 20/100 (HR 0.23, 95% CI 0.07–0.72). Finally, MOG-Ab titers were higher at relapse than in remission (p = 0.009).ConclusionIn adults, MOG-Ab-associated disease extends beyond clinical and radiologic abnormalities in the optic nerve and spinal cord. Despite the relapsing course, the overall visual and motor outcome is better compared with AQP4-Ab-positive patients.

Abstract

International audience; Background: Mortality is higher in patients with multiple sclerosis (MS) compared with general population. Even if excess mortality has been consistently shown, little is known about the dynamics of these excess mortality rates (EMRs), that is, how these EMRs vary according to current age.Objective: Using an innovative statistical approach applied on French data, our main goal was to describe the dynamics of the EMRs and to study how initial phenotype and age at onset impact this dynamics. We were also interested in assessing the effects of sex and year of MS onset.Methods: In 18 OFSEP centers, all patients from metropolitan France with incident MS over the period 1960-2014 were included. Vital status on 1st January 2016 was obtained through linkage to the national deaths registry. Entry time in the cohort was set as the first visit of the patient in the OFSEP center and follow-up was censored at 55 years. Dynamics of EMRs according to disease duration and age were evaluated using multidimensional penalized hazard models. Penalized splines offered the opportunity to model the effects of covariates in a flexible way, accounting for non-linearity and interactions.Results: Overall, 37,524 patients were included (71% women, median age at onset 31.7 years). Clinical course at onset was relapsing (R-MS) in 33,005 and progressive (PPMS) in 4,519 patients. Over a total of 340,708 patients-years of follow-up, 2,883 deaths occurred (2,142 in R-MS and 741 in PPMS).In R-MS, no excess mortality was observed in the first 10 years of MS. Up to the age of 70, younger patients at MS onset had higher EMRs than older patients at onset. However, from age 70, EMRs were the same whatever the age at MS onset, meaning, in other words, that excess mortality was the same whatever the disease duration. Furthermore, we found that recent years of MS onset were associated with much lower EMRs, and that EMRs were higher in men than in women.In PPMS, the EMRs increased rapidly from the very start of the disease and depended on age at MS onset whatever the current age.Conclusion: Excess mortality is a relevant concept to capture the mortality due directly and indirectly to MS and our statistical approach has been specifically designed to describe the dynamics of the EMRs. For the first time, we showed that, in R-MS, current age has a stronger impact on MS mortality than disease duration, suggesting an amnesic process in MS survival, as previously raised about disability progression.

Abstract

International audience; Context: The OFSEP registry in multiple sclerosis (MS) recently proposed a high-definition (HD) cohort to better appreciate the causal factors of disease progression and develop reliable predictive tools to help in medical decision making and informing patients at different key moments in the disease course.Objectives: To describe the characteristics of the first patients included in the HD cohort and to analyse their cross-sectional relationship with quality of life (QoL) measures.Methods: MS patients are followed annually. Data collection includes OFSEP core minimal clinical data, MFSC, brain MRI, comorbidities, serum biomarkers and patient-reported outcomes including QoL. Inclusions in the HD cohort started in July 2018. Patients were described according to demographic, clinical and therapeutic characteristics at inclusion. Several QoL indicators were available: the visual analogue scale (VAS) of the EQ5D-5L, the physical (PCS) and mental (MCS) component summary scores of the SF12, and the summary index of the MuSiQoL. Associations between patients’ characteristics and QoL indicators were analysed with multiple linear regression (p< 0.05 was considered as significant).Results: In March 2019, 621 patients were included, mainly women (73%), with an age at inclusion of 43±11 years and disease duration of 12±9 years. Disease course was relapsing-remitting (80%), secondary (12%) and primary progressive (8%). Majority of patients had a low disability (EDSS [0-2.5], 57%; [3-3.5], 12%; [4-5.5], 18.2%; [6-7], 13%). Twenty two percent were untreated; others were mainly treated with natalizumab (22%), fingolimod (14%) and rituximab (10%). In multivariate analysis, VAS was significantly lower in patients with progressive course and high levels of EDSS. The same correlations were observed for the PCS, which was additionally higher in first line treated patients. Regarding the MusiQoL, its level was significantly lower in patients with high levels of EDSS. In contrast, MCS was higher for EDSS [0-2.5] and lower for [4-5.5].Discussion: The first patients included in the HD cohort had a low disability and were mostly treated with second line drugs. A low QoL was associated to higher EDSS but not to disease activity; clinical course and treatment were associated with some scales but not for others. The relationship observed between QoL and disability validates the usefulness of the QoL collection of data in the perspective of a patient-centred research program.

Abstract

Background: Despite a high incidence of oestrogen receptor-positive (ER+) HER2-negative (HER2-) breast carcinoma (BC) in elderly women, rationale and modalities of adjuvant chemotherapy (CT) vary greatly and remain heterogeneous after 70 years, precluding evaluation of its benefit and impact on functional status and outcome. As for trials investigating innovative treatments, elderly patients are often excluded from programs evaluating new modern prognosis classifiers. Trial design: ASTER 70s is a prospective multicentre trial that investigates the impact on overall survival (OS) of adjuvant CT in elderly ER+ HER2- BC patients selected with a modern prognosis classifier while taking into account competing risks for mortality (EudraCT 2011-004744-22). After surgery, 2,000 patients over 70 years will have genomic grade (GG) assessed centrally by qRT-PCR on formalin-fixed paraffin-embedded samples (Ipsogen). Only those with a high GG or Equivocal (estimation ~700, group I) will be randomized between endocrine treatment alone and CT followed by endocrine treatment. CT regimen is left to the choice of investigators amongst 3 regimen of same duration [4 q3w cycles, docetaxel+cyclophosphamide (TC), doxorubicin or non pegylated liposomal doxorubicin (Myocet®)+cyclophosphamide (AC or MC), all with G-CSF], as well as endocrine treatment (aromatase inhibitor±tamoxifen). Radiotherapy will follow standard guidelines. Patients with low GG or not randomized for any reason (estimation ~1,300, group II) will be followed up as an observational cohort, with endocrine treatment only. Eligibility criteria: Any ER+ HER2- BC after complete surgery, M0, any pT or pN. Normal organ functions. No specific BC treatment before surgery. No exclusion for underlying medical condition unless potential contra-indication to study drugs. Specific aims: OS is the primary endpoint. Secondary endpoints address competing risks for mortality, include cost-effectiveness and QTWiST analysis, geriatric questions, acceptability and quality of life (QLQ C30 and specific elderly scale ELD15). The Lee's 4-year mortality score is calculated. The G8 screening tool is also assessed and used for stratification. Translational research will focus on ageing and prognostic biomarkers, and pharmacogenetic. Statistical methods: Sample size based on 4-year OS (87.5 vs 80%), bilateral test, a=0.05, ß=0.20 and HR= 0.60. Abstract

Background and Objectives: Although rituximab failed to demonstrate a significant effect on disability progression in primary progressive multiple sclerosis (PPMS), ocrelizumab succeeded. Our main objective was to analyze confirmed disability progression (CDP) in a cohort of patients with PPMS treated with anti-CD20 therapies compared with a weighted untreated control cohort., Methods: This was a retrospective study using data from the French MS registry (Observatoire Français de la Sclérose En Plaques). We included patients with PPMS treated or never treated with anti-CD20 therapies from 2016 to 2021, with an Expanded Disability Status Scale score of ≤6.5 at baseline. The primary outcome was time to first CDP. The secondary outcomes were time to first relapse, MRI activity at 2 years, identification of risk factors associated with CDP, and serious infection incidence rates (IIRs). Each outcome was studied using an inverse probability of treatment weighting method. The outcomes were modeled using a weighted proportional Cox model for the time-to-event outcomes and by a logistic regression regarding the MRI activity., Results: A total of 1,184 patients (426 treated and 758 untreated) fulfilled the inclusion criteria. Median age (Q1-Q3) was 56 years (49.3-63.8), and 52.7% were female. Among treated patients, 295 received rituximab, whereas 131 received ocrelizumab. At baseline, anti-CD20-treated patients were younger (median 51.9 vs 58.6 years, Cohen d = 0.683) and had more active disease (54.5 vs 27.8%, Cohen d = 0.562). 91.6% were drug-naive at inclusion. In time to first CDP analysis, no statistical significance was observed (hazard ratio [HR], 1.13; 95% CI 0.93-1.36, p = 0.2113). In time to first relapse analysis, a nonsignificant trend toward fewer patients relapsing in the treated group was observed (HR 0.83; 95% CI 0.48-1.28, p = 0.0809). For MRI activity, no significant difference was found between the 2 groups. Risk factors associated with CDP in the treated group were male sex and MS duration. IIR was 6.67 (95% CI 3.12-14.25) per 100 person-years in the treated group vs 2.67 (95% CI 0.80-8.86) in the untreated group., Discussion: Time to first CDP was not different between anti-CD20 treated and untreated patients with PPMS. Although our study is retrospective and mainly included patients treated by rituximab, our results indicate that there should be a constant evaluation of all available data to ascertain the best risk/benefit ratio for patients with PPMS., Classification of Evidence: This study provides Class III evidence that anti-CD20 therapy of previously untreated patients with PPMS was not superior to no therapy in delaying time to first CDP.
