Abstract

The blood type of patients who receive organs from donors with different blood types may change temporarily to that of the donor. Blood type is determined by ABO antigens, proteins on the surface of red blood cells (RBC). The blood type of a five-year-old boy with liver failure changed from type O to type AB after he received a liver from a type AB donor. The boy's blood type later changed back to O. He developed graft-versus-host disease approximately three months after the transplant, but subsequently recovered. The transplanted liver may have produced A and B antigens, which could have become attached to the boy's own RBCs. Or liver enzymes may have transferred AB antigens from the donor's RBCs. The transplanted liver may have contained stem cells, the precursors of all blood cells, which would have developed into AB blood cells. The presence of AB antigens may explain why the boy never rejected the liver, even though it was incompatible with his blood type.

Abstract

Nitric oxide may effectively reduce pulmonary hypertension in affected patients awaiting liver transplantation. Doctors treated a 14-year-old girl with end-stage cirrhosis and high blood pressure in her lungs. Inhaled nitric oxide produced blood vessel dilation in her lungs, reducing her pulmonary blood pressure to within the normal range. Nitric oxide was administered in the operating room during the liver transplantation and continued for three days postoperatively. Six months after surgery, the patient continued to have normal pulmonary pressure without medical support.

29. A Randomized Clinical Trial of Prophylactic OKT3 Monoclonal Antibody in Liver Allograft Recipients

Abstract

• Seventy-nine hepatic allograft recipients were randomized to receive either conventional immunosuppression, including cyclosporine, azathioprine, and steroids (41 patients), or investigational therapy in which OKT3 replaced cyclosporine during the first postoperative week (38 patients). Early rejection occurred in 29 patients (71 %) in the conventional group and 15 patients (39%) in the OKT3 group. Posttransplantation renal dysfunction occurred in 12 patients (29%) in the conventional group and 6 patients (16%) in the OKT3 group. Mean initial hospital stay was 34.1 ±18.8 days in the conventional group compared with 29.1 ±16.8 days in the OKT3 group. Cumulative patient survival (mean follow-up, 17.8±7.1 months) was 73.2% (30/41) for the conventional group and 84.2% (32/38) for the OKT3 group. Prophylactic OKT3 is indicated especially for liver allograft recipients with other complicating conditions that make management of early rejection unusually difficult.(Arch Surg. 1990;125:781-785)
