In a recent work [20], we predicted and experimentally validated a new physical mechanism for altering the propagation path of a monochromatic beam. Specifically, we showed that by properly tailoring the spatial distribution of the linear state of polarization transverse to the direction of propagation, the beam followed a curved trajectory in free space. Here we extend the model to the partially coherent, polychromatic case by redefining the beam amplitude, phase, and polarization angle as appropriate statistical quantities. In particular, we propose an entirely new definition of linear polarization gradient as an average over the third generalized Stokes parameter in the spatial frequency domain. In the new model, the beam curvature matches that of our previous work in the fully coherent case, but is predicted to gradually vanish as the beam loses coherence and becomes depolarized. The model also clearly predicts, however, that there does not exist a natural mechanism in free space or due to atmospheric turbulence that will cause significant depolarization. Simulated beam trajectories are shown for varying levels of initial partial coherence and for different polarization profiles. A new class of non-diffracting beams is also suggested by way of example. Lastly, as a byproduct of the derivation we generalize a previously known result concerning the free-space propagation of generalized Stokes parameters [13] by showing the result also holds for propagation in inhomogeneous media., Comment: 25 pages, 3 figures, 3 appendices

The emergent properties of quantum materials, such as symmetry-broken phases and associated spectral gaps, can be effectively manipulated by ultrashort photon pulses. Impulsive optical excitation generally results in a complex non-equilibrium electron and lattice dynamics that involves multiple processes on distinct timescales, and a common conception is that for times shorter than about 100 fs the gap in the electronic spectrum is not seriously affected by lattice vibrations. Here, we directly monitor the photo-induced collapse of the spectral gap in a canonical charge-density-wave material, blue bronze Rb0.3MoO3. We find that ultra-fast (about 60 fs) vibrational disordering due to efficient hot-electron energy dissipation quenches the gap significantly faster than the typical structural bottleneck time corresponding to one half-cycle oscillation (about 315 fs) of the coherent charge-density-wave amplitude mode. This result not only demonstrates the importance of incoherent lattice motion in the photo-induced quenching of electronic order, but also resolves the perennial debate about the nature of the spectral gap in a coupled electron-lattice system.

Abstract Background and methods Human metapneumovirus (hMPV) is a recently discovered respiratory virus associated with bronchiolitis, pneumonia, croup and exacerbations of asthma. Since respiratory viruses are frequently detected in patients with acute exacerbations of COPD (AE-COPD) it was our aim to investigate the frequency of hMPV detection in a prospective cohort of hospitalized patients with AE-COPD compared to patients with stable COPD and to smokers without by means of quantitative real-time RT-PCR. Results We analysed nasal lavage and induced sputum of 130 patients with AE-COPD, 65 patients with stable COPD and 34 smokers without COPD. HMPV was detected in 3/130 (2.3%) AE-COPD patients with a mean of 6.5 × 105 viral copies/ml in nasal lavage and 1.88 × 105 viral copies/ml in induced sputum. It was not found in patients with stable COPD or smokers without COPD. Conclusion HMPV is only found in a very small number of patients with AE-COPD. However it should be considered as a further possible viral trigger of AE-COPD because asymptomatic carriage is unlikely.

We employ time- and angle-resolved photoemission spectroscopy to study the spin- and valley-selective photoexcitation and dynamics of free carriers at the K and K' points in singly-oriented single layer WS$_2$/Au(111). Our results reveal that in the valence band maximum an ultimate valley polarization of free holes of 84$\,$% can be achieved upon excitation with circularly polarized light at room temperature. Notably, we observe a significantly smaller valley polarization for the photoexcited free electrons in the conduction band minimum. Clear differences in the carrier dynamics between electrons and holes imply intervalley scattering processes into dark states being responsible for the efficient depolarization of the excited electron population., Comment: 13 pages, 7 figures

Time- and angle-resolved extreme ultraviolet photoemission spectroscopy is used to study the electronic structure dynamics in BaFe$_2$As$_2$ around the high-symmetry points $\Gamma$ and $M$. A global oscillation of the Fermi level at the frequency of the $A_{1g}$(As) phonon mode is observed. It is argued that this behavior reflects a modulation of the effective chemical potential in the photoexcited surface region that arises from the high sensitivity of the band structure near the Fermi level to the $A_{1g}$ phonon mode combined with a low electron diffusivity perpendicular to the layers. The results establish a novel way to tune the electronic properties of iron pnictides: coherent control of the effective chemical potential. The results further suggest that the equilibration time for the effective chemical potential needs to be considered in the ultrafast electronic structure dynamics of materials with weak interlayer coupling., Comment: 6 pages, 3 figures

- Abstract
Inzidenz und Prävalenz eines Diabetes mellitus bei Mukoviszidose (zystische Fibrose, „cystic fibrosis“ [CF]) steigen mit zunehmender Lebenserwartung. Der CFRD („cystic fibrosis related diabetes“) unterscheidet sich von anderen bekannten Diabetestypen. Neben ihm können CF-Patienten auch unter Steroidtherapie bzw. im Rahmen akuter Infektionen oder während der Schwangerschaft (Gestationsdiabetes), dann meistens vorübergehend, einen ggf. auch insulinpflichtigen Diabetes mellitus entwickeln. Frauen mit CF erkranken häufiger an einem Diabetes mellitus als Männer mit CF. In der Regel begleitet der Diabetes mellitus die Betroffenen lebenslang. Deshalb sind ein umfangreiches Wissen und eine gute Patientenschulung von großer Bedeutung. Da CF-Patienten mit einem Diabetes mellitus einen schlechteren Krankheitsverlauf und eine höhere Mortalität aufweisen, ist die frühzeitige Diagnosestellung sehr wichtig, um von Anfang an eine adäquate Behandlung einzuleiten. Ein gut therapierter Diabetes mellitus wirkt sich auf den weiteren Krankheitsverlauf positiv aus. Die inzwischen guten Kenntnisse über die Pathogenese, den Verlauf und die Therapie erlauben eine gute Kontrolle des Diabetes mellitus. Dies führt zu einer Verbesserung der Lebensqualität und zu einer Erhöhung der Lebenserwartung. Patienten mit CF und einem Diabetes mellitus müssen keine strenge Diät einhalten. Wichtig sind vielmehr bewusstes Essen, ausgewogene Mahlzeiten und eine gute Kenntnis der Nahrungsmittel in Bezug auf die Verstoffwechselung von Kohlenhydraten, Fetten und Eiweiß. Die Therapie der Wahl ist die (intensivierte) Insulintherapie. In der Initialphase kann der Einsatz von Repaglinid temporär eine Alternative darstellen.

Zusammenfassung: Asthma ist eine chronisch entzündliche Erkrankung der Atemwege. Pathogenetisch steht die allergeninduzierte Atemwegsentzündung im Vordergrund. Infektionen spielen aber eine wesentliche Rolle als Auslöser akuter Exazerbationen und kommen auch in einer chronisch oder latenten Form als Verstärker der lokalen Entzündungsreaktion in Betracht. Ziel dieses Beitrags ist es, eine Übersicht über die unterschiedlichen Infektionserreger zu geben und den aktuellen Wissensstand zur Bedeutung von Atemwegsviren und atypischen Bakterien (C. pneumoniae und M. pneumoniae) in der Pathogenese von Asthma darzustellen.

ImportanceThe longer-term effects of therapies for the treatment of critically ill patients with COVID-19 are unknown.ObjectiveTo determine the effect of multiple interventions for critically ill adults with COVID-19 on longer-term outcomes.Design, Setting, and ParticipantsPrespecified secondary analysis of an ongoing adaptive platform trial (REMAP-CAP) testing interventions within multiple therapeutic domains in which 4869 critically ill adult patients with COVID-19 were enrolled between March 9, 2020, and June 22, 2021, from 197 sites in 14 countries. The final 180-day follow-up was completed on March 2, 2022.InterventionsPatients were randomized to receive 1 or more interventions within 6 treatment domains: immune modulators (n = 2274), convalescent plasma (n = 2011), antiplatelet therapy (n = 1557), anticoagulation (n = 1033), antivirals (n = 726), and corticosteroids (n = 401).Main Outcomes and MeasuresThe main outcome was survival through day 180, analyzed using a bayesian piecewise exponential model. A hazard ratio (HR) less than 1 represented improved survival (superiority), while an HR greater than 1 represented worsened survival (harm); futility was represented by a relative improvement less than 20% in outcome, shown by an HR greater than 0.83.ResultsAmong 4869 randomized patients (mean age, 59.3 years; 1537 [32.1%] women), 4107 (84.3%) had known vital status and 2590 (63.1%) were alive at day 180. IL-6 receptor antagonists had a greater than 99.9% probability of improving 6-month survival (adjusted HR, 0.74 [95% credible interval {CrI}, 0.61-0.90]) and antiplatelet agents had a 95% probability of improving 6-month survival (adjusted HR, 0.85 [95% CrI, 0.71-1.03]) compared with the control, while the probability of trial-defined statistical futility (HR >0.83) was high for therapeutic anticoagulation (99.9%; HR, 1.13 [95% CrI, 0.93-1.42]), convalescent plasma (99.2%; HR, 0.99 [95% CrI, 0.86-1.14]), and lopinavir-ritonavir (96.6%; HR, 1.06 [95% CrI, 0.82-1.38]) and the probabilities of harm from hydroxychloroquine (96.9%; HR, 1.51 [95% CrI, 0.98-2.29]) and the combination of lopinavir-ritonavir and hydroxychloroquine (96.8%; HR, 1.61 [95% CrI, 0.97-2.67]) were high. The corticosteroid domain was stopped early prior to reaching a predefined statistical trigger; there was a 57.1% to 61.6% probability of improving 6-month survival across varying hydrocortisone dosing strategies.Conclusions and RelevanceAmong critically ill patients with COVID-19 randomized to receive 1 or more therapeutic interventions, treatment with an IL-6 receptor antagonist had a greater than 99.9% probability of improved 180-day mortality compared with patients randomized to the control, and treatment with an antiplatelet had a 95.0% probability of improved 180-day mortality compared with patients randomized to the control. Overall, when considered with previously reported short-term results, the findings indicate that initial in-hospital treatment effects were consistent for most therapies through 6 months.

Purpose The first SARS-CoV-2 cases in Europe were reported in January 2020. Recently, concern arose on unrecognized infections before this date. For a better understanding of the pandemic, we retrospectively analyzed patient samples for SARS-CoV-2 from the prospective CAPNETZ study cohort. Methods We used nasopharyngeal swab samples from a cohort of well characterized patients with community acquired pneumonia of the CAPNETZ study group, recruited from different geographic regions across Germany, Austria, the Netherlands, and Switzerland between 02nd December 2019 and 28th April 2020. Multiplex real-time RT-PCR for a broad range of respiratory pathogens and SARS-CoV-2 real-time RT-PCR were performed on all samples. Results In our cohort, respiratory pathogens other than SARS-CoV-2 were detected in 21.5% (42/195) of patients with rhinovirus as the most frequently detected pathogen. The detection rate increased to 29.7% (58/195) when SARS-CoV-2 was included. No SARS-CoV-2 positive sample was detected before end of March 2020. Conclusions Respiratory viral pathogens accounted for a considerable number of positive results but no SARS-CoV-2 case was identified before the end of March 2020.
