Life care planning has emerged as an effective method for identifying and outlining future care needs and costs. By 1985, the life care plan was introduced to the health care industry in the Guide to Rehabilitation. Susan Riddick-Grisham was the first nurse to formally teach other nurses an organized series of life care planning classes when she was hired by Crawford & Company in the early 1990s to educate their consultants nationwide. At present, the life care planning specialty practice continues to grow, change, and modify the scope of practice associated with catastrophic case management. The International Academy of Life Care Planners is well established and has published the third edition of basic standards of practice. In addition to training programs, the Foundation for Life Care Planning Research (FLCPR), established in 2002, is a nonprofit research group that supports graduate students and other qualified research efforts in life care planning, including reliability and validity studies.

Acquired brain injury (ABI) is the leading cause of neurological impairment for individuals between the ages of 16 and 30 years of age. ABI technically includes brain injury as a result of cerebrovascular disease. The costs associated with treating traumatic brain injury (TBI) are estimated to be $48.3 billion annually. The outer surface of the brain is called the cerebral cortex. The cortex of the brain is divided into lobes that represent areas of specific functioning. The cerebellum is an area of the brain that facilitates coordinated motor movements. Brain injuries can be classified by a number of methods. ABIs are generally classified as traumatic, anoxic/hypoxic-ischemic, vascular, or other. Endocrinology is the study of hormones and their function. Common endocrine disorders following brain injury include syndrome of inappropriate diuretic hormone (SIADH), growth hormone deficiency, and irregularities of gonadal steroid production, and thyroid dysfunction. Typical neurological problems include weakness, sensory deficits, and the previously mentioned cranial nerve problems.

The life care plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis, and research, which provides an organized concise plan for current and future needs with associated costs for individuals who have experienced catastrophic injury or have chronic health care needs. Life care planning has a rich history of scrutiny and has become the most recognizable approach for estimating lifelong future care. This article will outline general procedures, topics, and suggestions for completing a detailed, comprehensive plan for persons with spinal cord injury. Resources for training and certification are also included.

The practice of retaining rehabilitation counselors as case consultants and forensic specialists (used interchangeably with expert witness throughout this article), in the private sector has grown significantly over the past 20 years. With that growth, the need for ethical guidelines for individuals practicing within these areas also has grown. Yet not until 2001, did certified rehabilitation counselors have a professional code of ethics that truly addressed some of the issues unique to this group of practitioners. The revised CRCC Professional Code of Ethics for Rehabilitation Counselors attempts to address these issues and also cover the diverse areas in which CRCs are qualified to practice within the private sector. While this article does not intend to be a primer regarding ethical considerations for rehabilitation counselors in the private sector; especially case consultants and forensic specialists, it does review some of the issues specific to rehabilitation counselors who practice in these areas and the new ethical guidelines to which case consultants and forensic specialists must be held accountable. A six year review of CRCC Ethics Committee actions is included as well as claims information, i.e., case examples, filed with a major malpractice insurance carrier against private sector rehabilitation practitioners. Questions for reflection on the implications of the revised Code for rehabilitation counselors in the private sector also are included.

This article discusses the history and evolution of what is now known as a life care plan. The qualifications of professionals who perform the specialty practice of life care planning are reviewed. The standards of practice for life care planning have been a long-standing guide for the practitioner and its core components are discussed.

Clients who have experienced a brain injury often are damaged by a third party who is perceived responsible for the consequences of their actions. In the United States, a common method of seeking financial compensation for the injury is to file a personal injury law suit. This article will outline the RAPEL method for identifying instrumental details for settling the personal injury claim or educating a jury if a trial is necessary. Topics include identification of future medical care and related needs as well as assessing the effects of the injury on the ability to work.

In an effort to broaden his therapeutic approach, Arnold Lazarus developed multimodal therapy to provide a range of systematic intervention strategies for meeting the multiple level needs of clients seeking help. This article describes Dr. Lazarus's technically eclectic, behavior-based therapy, known as BASIC I.D., and its applications in rehabilitation. The BASIC I.D. process encompasses individual areas of behavior, affect, sensation, imagery, cognition, interpersonal relationships, and drugs/biology. As such, it is seen as a national addition to comprehensive rehabilitation efforts that must take into account a number of factors, many of which are included in the BASIC I.D. therapeutic approach. An example BASIC I.D. profile is included.

As the private sector rehabilitation industry matures, certain ethical and malpractice issues have emerged. This article provides an overview of ethical issues in private for-profit practice, with particular focus on expert testimony, using examples from a sample of claims filed with the National Association of Rehabilitation Professionals in the Private Sector (NARPPS), malpractice insurance companies, selected court cases, and complaints submitted to NARPPS Standards Compliance Review Board Committee and the Commission on Rehabilitation Counselor Certification. It is evident that complaints have a direct relationship to at least half of the ethical canons of the Code of Pro fessional Ethics for Rehabilitation Counselors because they most frequently involve issues related to competence, the client-counselor relationship, professional relationships, confidentiality, and use of the certified rehabilitation counselor (CRC) designation.

Case managers and life care planners are often appropriate professionals to assist with the care of people who have experienced burns. With more than 1,000,000 people who are burned each year, including 15,000 children who are hospitalized, the numbers of patients are substantial. Recovery from burns can be an extremely lengthy, complicated, and painful event. Effective case management can add to the quality of care and life of the individual with a burn in a major way. Furthermore, many will require lifelong care and a certified life care planner may be retained to assess these needs. This article presents an overview of information about burn care and related issues, and offers useful suggestions to the case manager and life care planner.

This paper examines the differences and similarities between the profession of vocational rehabilitation in public and private sectors. A literature review is the basis for the comparisons. The results show different historical beginnings as well as a number of differences in the way the profession is practiced. Overall, however, it appears the similarities outnumber the differences. It is noted that private sector rehabilitation is rapidly rising and public sector rehabilitation is not. As a result certain ethical challenges to the private sector counselor and educational challenges to the universities have become evident.
