FUNDAMENTO: O aumento da espessura do IMT (do inglês intima-media thickness) das carótidas é utlizado como marcador precoce de aterosclerose e para avaliação do risco de eventos cardiovasculares. O ultrassom é utilizado na sua avaliação pela acessibilidade e baixo custo. São descritas medidas realizadas em diferentes regiões das carótidas. OBJETIVOS: Correlacionar o IMT nas regiões proximal e distal da carótida primitiva bilateral no intuito de orientar a sua utilização na prática clínica. MÉTODOS: O IMT foi medido nas porções proximais e distais da artéria carótida primitiva de 798 indivíduos (35-74 anos) de ambos os sexos usando ultrassom de alta resolução. O coeficiente de correlação de Pearson foi usado para se estabelecer as associações. As análises foram feitas inicialmente para toda a amostra e nos subgrupos com IMT < 0,90 mm (49% da amostra) e > 0,90 mm em pelo menos um sítio de medida. A significância estatística foi considerada para p 0,90 mm, houve expressiva queda de correlações, que se situaram entre 0,20 e 0,40. CONCLUSÃO: Os dados sugerem que o espessamento médio-intimal é mais uniforme ao longo das carótidas em fases mais precoces do desenvolvimento e tende a adquirir desenvolvimento focal à medida que progride. Portanto, na avaliação clínica de pacientes, toda a extensão das carótidas comuns deve ser investigada bilateralmente para melhor utilizar os softwares disponíveis e concluir sobre a presença ou não de espessamento do complexo médio-intimal. BACKGROUND: Increased IMT (intima-media thickness) in carotids is used as an early atherosclerosis marker and to evaluate the risk of cardiovascular problems. Ultrasound is used in the evaluation because it is accessible and low cost. Measurements for different carotid regions are described. OBJECTIVE:To compare the proximal and distal region IMTs for the bilateral common carotid and guide its use in clinical practice. METHODS: The IMT was measured in the proximal and distal common carotid arteries of 798 individuals (35-74 years old) of both genders using high-resolution ultrasound. Pearson's correlation coefficient was used to establish associations. The analyses were initially performed for the entire sample as well as subgroups with IMT < 0.90 mm (49% of the sample) and > 0.90 mm for at least one measurement site. The statistical significance was p < 0.05. RESULTS: The correlations investigated were significant. In the group with an IMT < 0.90 mm, the correlations were between 0.44 and 0.62. In the subgroup with an IMT > 0.90 mm, the correlations were significantly reduced to between 0.20 and 0.40. CONCLUSION: The data suggest that the IMT is more uniform along the carotid during early development and tends develop focally as it progresses. Therefore, in clinical evaluations of patients, the common carotid length should be investigated bilaterally to better use the available software and discern the IMT.

A associação entre doença periodontal e o desenvolvimento de aterosclerose tem sido pesquisada. Esta revisão sistemática da literatura se propõe a verificar a associação entre a doença periodontal e a aterosclerose subclínica. A pesquisa utilizou as bases de dados: PubMed, Scopus, LILACS, BBO, Biblioteca Cochrane e Scielo com os termos: "doença periodontal", "periodontite", "carótida" e "aterosclerose". Foram excluídos artigos: que apresentaram resumos escritos em idiomas diferentes do português, inglês e espanhol; com abordagem laboratorial em modelos experimentais; sem descrição ou referência a estimadores de associação entre doença periodontal e aterosclerose subclínica; pesquisas que realizaram a medida da doença periodontal por parâmetros diferentes de exames clínicos; pesquisas que não analisaram a espessura da íntima média da artéria carótida através de exame de ultrassom; e que abordassem grupos específicos. Foram comparados os desenhos de estudo, protocolos de medida e os dados relatados. De um total de 63 referências, apenas 10 artigos contemplavam todos os critérios de inclusão, distribuídos em: 1 estudo longitudinal, 6 transversais e 3 casos controles. Observou-se uma heterogeneidade entre os estudos em relação ao método de mensuração da doença periodontal e os desfechos da aterosclerose. Concluiu-se que infecções periodontais são fortemente associadas ao desenvolvimento da aterosclerose subclínica, entretanto os mecanismos envolvidos no processo patogênico ainda permanecem desconhecidos. The conclusion is that periodontal infections are strongly associated with the development of subclinical atherosclerosis, however the mechanisms involved on the pathogenic process remain unknown.

The systematic review of literature aims to evaluate the association between periodontal disease and subclinical atherosclerosis. A literature search of the PubMed, Scopus, LILACS, BBO, Cochrane Library and Scielo bibliographic databases was conducted using the following descriptors: “periodontal disease”, “periodontitis”, “carotid”, and “therosclerosis”. Articles were excluded if they: presented abstracts written in languages other than Portuguese, English and Spanish; experimental studies; did not contain data testing the degree of association between periodontal disease and subclinical atherosclerosis; did measure the extent of periodontal disease by parameters other than the clinical examination; did not measure the carotid artery intima-media wall thickness by ultrasonography; and covered specific population groups. The studies design, measurement protocols and reported data were compared. A total of 63 papers identified only 10 studies that met the inclusion criteria: 1 longitudinal study, 6 cross-sectional, and 3 case control studies. There was a wide heterogeneity between the studies regarding the methods of measuring periodontal disease and atherosclerosis outcomes. The conclusion is that periodontal infections are strongly associated with the development of subclinical atherosclerosis, however the mechanisms involved on the pathogenic process remain unknown. Context: Doppler ultrasonography is an established method for diagnosis, preoperative imaging and follow-up of extracranial carotid artery disease. Objetive: The evaluation of gender differences in carotid artery bifurcation Doppler ultrasonography mapping. Methods: High resolution Doppler ultrasonography of 500 carotid bifurcations was performed in 192 women and 308 men before surgical treatment. Gender differences were analyzed based on B-mode, color-flow, duplex doppler transverse and longitudinal images. Diameter percent stenoses, plaque length, distal internal and common carotid artery diameters, and distance from the carotid bifurcation to the ear lobe were compared. Mean, standard deviation, minimum and maximum values were described. Statistical comparisons were performed based on Student's t and χ2 tests. Results: Carotid stenoses averaged 70±11% (30-95%) in women and 72±12% (40-98%) in men (p=0.013). The prevalence of 90-99% stenosis was greater in men, 14.3 vs 7.8% (p=0.029). Carotid plaques were longer in men, 2.3±0.8 vs 1.9 ±0.6 cm (p<0.001). Mean diameters of the distal internal carotid artery, 4.9±0.9 vs 4.6 ±0.8 mm, and of the common carotid artery, 7.6±1.3 vs 7.1±1.4 mm, were greater in men (p=0.001). The distance from the ear lobe to the bifurcation was also greater in men, 5.9±1.1 vs 5.3±0.9 cm (p<0.001). Conclusions: Doppler ultrasonography preoperative mapping demonstrated that the parameters measured were greater in men than in women. Detailed planning of carotid plaque treatment must take into consideration individual differences such as those associated with the patient's gender. [ABSTRACT FROM AUTHOR]
