Objetivo: Avaliar a função ventilatória, o pico de fluxo inspiratório nasal, as pressões respiratórias máximas, a atividade elétrica dos músculos inspiratórios acessórios e a relação entre as variáveis, em adultos com respiração oral (RO), comparando-os com adultos com respiração nasal (RN). Métodos: Estudo transversal, controlado com 77 participantes, entre 18 e 30 anos, ambos os sexos, distribuí­dos em Grupo RO (n = 38) e RN (n = 39). Foram avaliados por espirometria, manovacuometria, para obtenção da Pressão Inspiratória Máxima (PImáx) e Pressão Expiratória Máxima (PEmáx); medida do pico de fluxo inspiratório nasal (PFIN) e eletromiografia de superfí­cie (EMG) dos músculos esternocleidomastoideo (ECM) e trapézio superior durante testes de PImáx e PFIN. Resultados: O grupo RO apresentou valores significativamente menores de PImáx (91 versus 100%), PEmáx (91,6 versus 102,6%) e PFIN (121,4 versus 147,7 l/min). Houve diferença (p < 0,01) na atividade EMG do ECM, com valores menores no grupo RO. A PImáx se correlacionou com o PFIN (r = 0,48, p < 0,01). Conclusão: Os RO apresentaram menores valores de PFIN, PImáx, PEmáx e de amplitude da atividade EMG dos músculos inspiratórios acessórios durante inspirações rápidas, quando comparados aos RN. Traz como contribuição para a prática clí­nica, a importância de uma investigação global do modo respiratório, não envolvendo apenas intervenções médicas, ortodônticas e miofuncionais, mas também os aspectos ventilatórios e musculares, sobre os quais se identificou a influência da RO.Palavras-chave: músculos respiratórios, espirometria, eletromiografia, obstrução nasal, respiração bucal.

OBJETIVO: estudar a posição habitual da língua e dos lábios nos padrões de crescimento anteroposterior e vertical de crianças em fase de dentição mista. MÉTODOS: a amostra foi constituída por 54 crianças, na faixa etária entre sete e 11 anos. As crianças selecionadas foram encaminhadas para avaliação radiográfica e análise cefalométrica, que possibilitou a obtenção de ângulos SNA, SNB e ANB (padrão de crescimento anteroposterior) e da classificação do tipo facial entre braquifacial, mesofacial e dolicofacial (padrão de crescimento vertical). A posição da língua e dos lábios foi determinada a partir da observação das radiografias cefalométricas por duas fonoaudiólogas com experiência na área de motricidade orofacial. A posição habitual da língua foi classificada como na papila palatina, com dorso elevado ou no assoalho oral, e a posição habitual dos lábios, como fechados ou entreabertos/abertos. Para verificar a relação entre a posição habitual da língua e dos lábios com os padrões de crescimento anteroposterior e vertical foram utilizados os testes estatísticos Análise de Variância, t de Student, U de Mann-Whitney e Qui-Quadrado, ao nível de significância de 5%. RESULTADOS: verificou-se relação estatisticamente significante entre a posição da língua e o ângulo SNB, sendo que as crianças com posição de língua no assoalho oral apresentaram ângulo SNB significantemente menor do que as crianças com posição habitual de língua na papila palatina. Verificou-se ângulo SNB significantemente menor nas crianças com posição de lábios entreabetos ou abertos quando comparadas às crianças com posição de lábios fechados. Não houve diferença entre a posição habitual da língua e dos lábios nos demais padrões de crescimento anteroposterior e de crescimento vertical. CONCLUSÃO: a posição habitual de lábios e de língua apresentou relação com o padrão de crescimento mandibular, não tendo sido influenciada pelo tipo facial.

Summary Introduction: Hyperplasia of the pharyngeal tonsil is one of the main causes of mouth breathing, and accurate diagnosis of this alteration is important for proper therapeutic planning. Therefore, studies have been conducted in order to provide information regarding the procedures that can be used for the diagnosis of pharyngeal obstruction. Objective: To verify the correlation between nasopharyngoscopy and cephalometric examinations in the diagnosis of pharyngeal tonsil hyperplasia. Method: This was a cross-sectional, clinical, experimental, and quantitative study. Fifty-five children took part in this study, 30 girls and 25 boys, aged between 7 and 11 years. The children underwent nasofibropharyngoscopic and cephalometric evaluation to determine the grade of nasopharyngeal obstruction. The Spearman's rank correlation coefficient at the 5% significance level was used to verify the correlation between these exams. Results: In the nasopharyngoscopy evaluation, most children showed grade 2 and 3 hyperplasia of the pharyngeal tonsil, which was followed by grade 1. In the cephalometry assessment, most children showed grade 1 hyperplasia of the pharyngeal tonsil, which was followed by grade 2. A statistically significant regular positive correlation was observed between the exams. Conclusion: It was concluded that the evaluation of the pharyngeal tonsil hyperplasia could be carried out by fiber optic nasopharyngoscopy and cephalometry, as these examinations were regularly correlated. However, it was found that cephalometry tended to underestimate the size of the pharyngeal tonsil relative to nasopharyngoscopy.

OBJETIVO: comparar as dimensões do palato duro em diferentes tipologias faciais de crianças respiradoras nasais e orais. MÉTODO: a amostra foi constituída por 54 crianças, na faixa etária entre sete e 11 anos, distribuídas em grupos conforme o tipo facial e o modo respiratório. O tipo facial foi obtido por meio da análise cefalométrica de Ricketts, e o modo respiratório foi determinado a partir da avaliação fonoaudiológica e do diagnóstico otorrinolaringológico. Para realização de medidas transversais, verticais e do comprimento anteroposterior do palato duro, foram obtidos modelos em gesso do arco dental maxilar. Para comparação das dimensões do palato duro entre os grupos, foram utilizados testes paramétricos e não paramétricos, ao nível de significância de 5%. RESULTADOS: não foi verificada diferença estatisticamente significante nas medidas do palato duro entre as crianças braquifaciais, mesofaciais e dolicofaciais. Verificou-se diferença estatisticamente significante na distância entre os segundos pré-molares nos diferentes tipos faciais das crianças respiradoras nasais e orais, sendo que esta diferença não foi detectada nas comparações múltiplas. CONCLUSÃO: as dimensões do palato duro não diferiram quando analisadas em diferentes tipos faciais independente do modo respiratório. Porém, evidenciou-se diferença na distância entre os segundos pré-molares quando o tipo facial foi analisado nos respiradores nasais e orais. PURPOSE: to compare the hard palate dimensions in nasal and mouth breathing children of different facial typologies. METHOD: the sample comprised 54 children between seven and 11 year old, divided in two groups according to the facial type and breathing mode. The facial type was obtained through the Ricketts cephalometric analysis, and the breathing mode was determined from speech-language evaluation and otorhynolaryngologic diagnosis. In order to measure the transverse, vertical and anteroposterior length of the hard palate, plaster cast models of the children superior dental arch were obtained. To get the hard palate dimensions among the groups, parametric and nonparametric tests were used under a significance level of 5%. RESULTS: no statistically significant difference was found in hard palate measures among brachyfacial, mesofacial and dolichofacial children. A significant statistical difference in the distance among the second premolars was identified when compared with the different facial types of nasal and mouth breathing children, and such a difference was not detected in multiple comparisons. CONCLUSION: the dimensions of the hard palate did not show any difference when analyzed in different facial types regardless of the breathing mode. However, when analyzing the facial type in nasal and mouth breathers, we found a difference in the distance among the second premolars.

OBJETIVO: Comparar as dimensões do palato duro de crianças respiradoras nasais, respiradoras orais por etiologia obstrutiva e respiradoras orais por etiologia viciosa. MÉTODOS: A amostra foi composta por 76 crianças, 37 meninos e 39 meninas, com média de idade de 9,32±1,16 anos, distribuídas conforme diagnóstico do modo respiratório e etiologia da respiração oral. Foram obtidos modelos em gesso do arco dental maxilar para a realização de medidas do palato duro com paquímetro digital. Foram obtidas as medidas transversais, verticais e do comprimento anteroposterior do palato duro. A comparação das medidas do palato duro entre os grupos foi realizada por meio de testes estatísticos. RESULTADOS: Na comparação das dimensões do palato duro entre os grupos de respiradores nasais e respiradores orais, verificou-se diferença na distância e profundidade ao nível dos segundos pré-molares e na distância entre os primeiros molares. Também foi verificada diferença entre os grupos de respiradores orais por etiologia obstrutiva e por etiologia viciosa na profundidade do palato duro ao nível dos caninos. CONCLUSÃO: Respiradores orais apresentam palato duro mais estreito ao nível dos segundos pré-molares e primeiros molares, e mais profundo ao nível dos segundos pré-molares quando comparados aos respiradores nasais. Evidencia-se também que os respiradores orais por etiologia viciosa apresentam maior profundidade do palato duro ao nível dos caninos quando comparados aos respiradores orais por etiologia obstrutiva. METHODS: The sample comprised 76 children, 37 boys and 39 girls, with mean age of 9.32±1.16 years, distributed according to the diagnosis of breathing mode and to the etiology of mouth breathing. Plaster cast models of the subjects' superior dental arch were obtained in order to measure the hard palate with a digital caliper. Measurements of transverse, vertical and anteroposterior palatal length were taken. The hard palate measures were compared among the groups through statistical analysis. RESULTS: The comparison of hard palate dimensions observed in nasal and mouth breathers showed differences regarding the distance and depth of second premolars, and the distance of first molars. Differences were also found between the groups of mouth breathers regarding the hard palate depth at the level of canines. CONCLUSION: Mouth breathers showed narrower hard palate at the level of second premolars and first molars, and deeper palate in the level of second premolars, when compared to nasal breathers. It is evidenced that habitual mouth breathers presented deeper hard palate at the level of canines, when compared to mouth breathers from obstructive etiology.

Introduction Mouth breathing can affect the functions of the respiratory systems and quality of life. For this reason, children who grow up with this stimulus may have implications on physical and psychological aspects at adult age.Objective To evaluate childhood mouth-breathing consequences for the ventilatory function and quality of life at adult age.Materials and methods Prospective, observational and cross-sectional study with 24 adults, between 18 and 30 years old, mouth breathers during childhood, comprised the childhood mouth-breathing group (CMB). The childhood nasal-breathing (CNB) group was composed of 20 adults of the same age, without history of respiratory disease during all their lives. Measurements of maximal respiratory pressures, peak expiratory flow and 6-minute walk test were assessed. In addition, all the volunteers answered the Short Form-36 questionnaire (SF-36).Results The maximal inspiratory (p = 0.001) and expiratory (p = 0.000) pressures as well as the distance in the walk test (p = 0.003) were lower in the COB. The COB also presented lower score in the General Health domain of the SF-36 Questionnaire (p = 0.002).Conclusion Childhood mouth-breathing yields consequences for the ventilatory function at adult age, with lower respiratory muscle strength and functional exercise capacity. Conversely, the quality of life was little affected by the mouth breathing in this study.
