- Abstract
Este documento versa sobre la creación de un modelo eficiente y robusto para el análisis de los procesos erosivos y de la formación de la socavación en torno a obstáculos que se encuentran en el fondo de los ríos. El interés de estos análisis reside en que la aparición de la socavación puede provocar problemas estructurales que pueden tener consecuencias catastróficas. Para la realización de estos modelos se ha empleado el Software ANSYS. Se emplean distintos tipos de modelos para estudiar la socavación, modelos monofásicos y bifásicos, analizando tanto casos bidimensionales como tridimensionales. Los resultados obtenidos son comparados entre ellos y con datos de la literatura para así poder determinar que herramienta obtiene mejores resultados tanto en términos cualitativos como en términos de tiempo necesario para su ejecución. This document deals with the creation of an efficient and robust model to analyse the erosive processes as well as the development of the scour around the obstacles which can be found on the bottom of the rivers. The benefit of these analysis is that the appearance of the scour may cause problems with catastrophic consequences. To develop these models ANSYS Software has been employed. Several types of models have been developed to analyse the scour processes such us single-phase models as well as multiphase models in order to analyse two-dimensional and three-dimensional cases. The obtained results have been compared among them and with data from several articles. Para ello se analizarán y compararán dos métodos: el primero de ellos, basado en el uso de las series de Taylor, es conocido como Gas Path Analysis (GPA), concretamente se utilizará el método no lineal; mientras que el segundo, el método de optimización, se basa en el empleo de una función del software matemático Matlab. El motor para el cual se realizarán los distintos análisis y la obtención de los datos pertinentes será un turbofán bieje de alta relación de derivación. El desarrollo y los análisis necesarios para la realización de este trabajo han sido realizados de forma conjunta con el alumno de cuarto curso del Grado en Ingeniería Espacial de la Universidad de Sevilla, Sergio Román Mora; aunque centrando cada uno el objetivo de su trabajo en un aspecto distinto del estudio. This document reviews several studies about the fault detection methods in plane engines, analyzing both the correct location of the affected components (fan, compressor, turbine…) and the right grade of deteroration experienced by each element. In order to do so, we will analyze and compare two of those methods: the first one, based on the use of the Taylor series, known as Gas Path Analysis(GPA), will use the non-lineal method; whereas the second one, the optimizing method, is based on the use of a function from the Math software Matlab. The machine took for both analyzing and data recording will be a double shaft high bypass ratio engine. The development and the analysis needed for the realization of this project have been done in equal partnership with the student of fourth Grade of Aerospace Engineering Degree of the University of Seville, Sergio Román Mora; eventhough each of us have targeted a different aspect from this assignment. 9. Análisis por edad y género de eventos adversos hospitalarios en personas adultas

- Abstract
Introduction. The identification of the characteristics related to adverse events in hospital facilities helps to establish high impact preventive measures. The goal of this study is to describe the distribution of the most frequent adverse events by gender and age, as well as to identify the differences among the groups. Material and methods. From the System of Automated Record of Health Incidents (SIRAIS), a sample of 246 cases from public hospitals that contained wholesome information for the analysis by gender and age, were obtained. Results. The largest amount of incidents recorded were the ones related to clinical processes or procedures (35.37%), followed up by falls (22.76%). When comparing the distribution of the incidents among age groups an important statistic difference (x2 of Pearson = 27.69; p=0.001) was demonstrated, mainly due to a greater frequency in incidents related to therapeutic interventions in the elderly. There were no important differences identified by gender. Discussion. It is necessary to deepen the analysis with an economical and epidemiological and gender perspective, which will allow to identify with greater precision the factors that increase the probability of suffering an adverse event., Introducción. Identificar las características relacionadas a eventos adversos hospitalarios permite establecer medidas preventivas eficientes y de alto impacto. El objetivo de este estudio es describir la distribución de los eventos adversos más frecuentes por género y por edad, así como identificar las diferencias entre los grupos. Material y Métodos. Del Sistema de Registro Automatizado de Incidentes en Salud (SIRAIS) se obtuvo una muestra de 246 casos provenientes de 18 hospitales públicos que contaron con la información completa para su análisis por género y por edad. Resultados. La mayor proporción de incidentes registrados fueron los relacionados a un proceso clínico o procedimientos (35.37%), seguidos por las caídas (22.76%). Al comparar la distribución de los incidentes entre los grupos etáreos se demostró una diferencia estadísticamente significativa (x2 de Pearson = 27.69; p=0.001), principalmente debido a la mayor frecuencia de incidentes relacionados a intervenciones terapéuticas en adultos mayores. No se identificaron diferencias significativas por género. Discusión. Es necesario profundizar en el análisis con una perspectiva epidemiológica y económica y de género, lo que permitirá identificar con una mayor precisión los factores que incrementan la probabilidad de sufrir un evento adverso.

- Abstract
The notification and reporting systems collect information on incidents that occur in hospitals and other sites where medical care is provided. These systems, which entail other components of patient safety, are uneven and diverging, with different events entered. Differences in geographic location, language and culture have motivated the World Health Organization to develop the International Patient Safety Classification with the intention of facilitating the international reporting and exchange of information and comparison between institutions. This article describes how the strength thereof has been orientated towards learning and sharing information on the root causes and best practices for prevention, as well as towards the acknowledgment and transparent communication of adverse events., Los sistemas de notificación y reporte colectan información sobre los incidentes que se presentan en los hospitales así como en otros sitios donde se brinda atención médica. Estos sistemas, a los que van aparejados otros componentes de la seguridad del paciente, son dispares y divergentes, capturando eventos diversos. Las diferencias en la ubicación geográfica, así como en el lenguaje y en la cultura, impulsaron a la Organización Mundial de la Salud para desarrollar la Clasificación Internacional para la seguridad del Paciente con el fin de facilitar el reporte a nivel internacional, así como el intercambio de información y la comparación entre instituciones. En este artículo, se describe como la fortaleza de estos se ha orientado hacia el aprendizaje, así como a compartir información sobre las causas raíz y las mejores prácticas para la prevención, así como hacia el reconocimiento y comunicación transparente de los eventos adversos (EA).

- Abstract
To ensure that patient and relatives remain interested and involved with their personal safety during their stay in hospital, it is required to inform them about their responsibility and what are the issues to observe and discuss with health care services providers. The main safety issues within hospitals were identified through the analysis of complaints submitted at CONAMED, the recommendations made by the World Alliance for Patient Safety of the World Health Organization, the risks and root causes that generate preventable adverse events, actors involved in patient safety, interactions that occur bet-ween them, and how the patient and family can participate to improve them. From the main safety issues identified, recommendations were made directed to staff, patients, relatives and managers in order to promote their committed participation towards the continuous improvement of the quality of health care and patient safety., Para lograr que el paciente y su familia se interesen y participen en el cuidado de su seguridad durante su estancia en el hospital, es necesario informarles sobre cuál es su responsabilidad en ello y cuáles son los aspectos que deben observar y comentar con los prestadores de los servicios. Se identificaron: los principales problemas de seguridad en los hospitales a través de las quejas presentadas en CONAMED; las recomendaciones que hace la Alianza Mundial para la Seguridad de los Pacientes de la Organización Mundial de la Salud; los riesgos y las causas raíz que generan los eventos adversos prevenibles; actores involucrados en la seguridad de los pacientes; las interacciones que ocurren entre ellos, y la forma en que el paciente y sus familiares pueden participar para mejorarlos. Derivadas de los principales problemas de seguridad identificados, se presentan Recomendaciones para el personal, pacientes y familiares y directivos, dirigidas a promover su participación comprometida en la mejora continua de la calidad de la atención y la seguridad del paciente. - Abstract
Introducción. Es esencial hacer el registro de los cuidados que enfermería proporciona diariamente a los pacientes, tanto como elemento necesario para dar una adecuada calidad sanitaria como para el desarrollo de la profesión. Por ello, los profesionales deben conocer la NOM-168-SSA1-1998 Del Expediente Clínico y demás legislación vigente del permita el adecuado cumplimiento de los registros y de la práctica profesional de enfermería. Material y métodos. Se realizó un estudio descriptivo, transversal, observacional y correlacional, con una muestra estratificada de 375 expedientes clínicos, utilizando un instrumento de valoración basado en la NOM- 168, el cual se aplicó en diferentes servicios de un instituto nacional de salud. Resultados. Existió incumplimiento de la NOM-168 en 49% de los registros de enfermería revisados. El servicio de Ortopedia cumplió con el 25% para estar en el parámetro de inadecuado, neurociencias con 23%, regular y Oncología con el 27% ubicándose en el parámetro de adecuado. La prueba paramétrica ANOVA efectuada en las variables: nivel de cumplimiento y servicio mostró que sí existe diferencia con una F de 0.62, en el llenado adecuado de los registros de enfermería conforme a la Norma Oficial, en los diferentes servicios encuestados. Conclusiones. Los niveles inadecuados de cumplimiento a la normativa para la elaboración de los registros de enfermería, indican la necesidad de incrementar esfuerzos para mejorar su calidad en forma significativa. Lo anterior, con el fin de lograr el objetivo de ser un instrumento de comunicación sobre el estado de salud del paciente, sin dejar a un lado la observancia de los principios éticos y legales. Entre los principales hallazgos, se encontraron omisiones del registro del nombre y firma de quién los elabora; descripción del habitus exterior, así como de la vía de administración de medicamentos; adicionalmente se identificaron documentos con tachaduras, borraduras e ilegibilidad, lo cual repercute directam, Introduction. - Abstract
Introduction. The objective of this study is to know the degree of participation and commitment of the staff and users of services offered by the Central Military Hospital in regard to the care of patient safety, as well as to increase the participation of patients and their relatives in selfcare and joint responsibility of the medical care safety. Results obtained in the Central Military Hospital are presented, about the level of knowledge and degree of joint responsibility of patients, relatives, members of the staff and executives of the security of medical attention. Methods. A total of 1591 surveys were given, including the executive personnel, members of the staff, patients and relatives, in order to establish a situation diagnosis about their knowledge of resources, knowledge and participation in the care of the patient safety and know the areas of opportunity to improve the selfcare and join responsibility in the safety of medicalcare. Results. Surveys were given to 1591 subjets (men and women), including executives, medical staff and users. It was found that 50% (20) of executives were aware of the frecuency of adverse events; 65% (230) of medical staff state that there is materials and equipment for hand washing; 54% (224) of the same group said that patients do not have i.d. cards at the head of the bed. In the other hand, only 47% (196) of this same group, is aware of the rate of intrahospital infections in their own areas. Among patients and relatives, most significant answers : 88% stated that members of the staff washed their hands before giving the attention; about the security measures during moving of patients, 47% (222) agreed that were done. 20% of patients were not informed about adverse events that might occur during medical care and 38% of patients polled in the outpatient area, ignore which drugs were prescribed to them and 39% ignore care measures to follow in order to improve their health status. El objetivo del estudio fue conocer el grado de participación y compromiso del personal prestador y usuarios del servicio que se otorga en el Hospital Central Militar, en el cuidado de la seguridad del paciente, así como incrementar la participación de los pacientes y sus familiares en el autocuidado y corresponsabilidad en la seguridad de la atención médica. Se presentan los resultados obtenidos en el Hospital Central Militar, acerca del nivel de conocimiento y grado de corresponsabilidad del paciente, familiares, prestadores del servicio y directivos en la seguridad de la atención médica. Material y métodos. Se aplicaron 1591 encuestas, en donde se incluyó al personal directivo, prestadores de servicio, pacientes y/o familiares, con el propósito de establecer un diagnóstico situacional sobre su conocimiento sobre los recursos, conocimiento y participación en el cuidado de la seguridad del paciente y conocer las áreas de oportunidad para mejorar el auto-cuidado y corresponsabilidad en la seguridad de la atención médica. Resultados. Se aplicaron encuestas a 1591 personas (hombres y mujeres), en donde se incluyó a directivos, personal prestador de servicios y usuarios. Se encontró que 50% (20) del personal directivo desconoce la frecuencia de los eventos adversos, el 65% (230) del personal prestador del servicio afirma que sí se cuenta con el material y equipo para el lavado de manos, el 54% (224) de este mismo personal contestó que los pacientes, no cuentan con tarjeta de identificación en la cabecera de su cama. Por otra parte sólo el 47% (196) de los prestadores del servicio, conoce la tasa de infecciones nosocomiales que se presenta en su servicio. This article gives an outlook of notification and incident report systems, focusing on problems for the development and application thereof. The fundamental components of databases generated, phases for their implementation, their scope, limitations and motivation for doctor participation are analyzed. An ideal system is presented as one that records all incidents that cause harm to the patient or adverse events, such as those which do not cause harm or "near misses", since these are most frequent and practically reveal the same care system errors as are found in the former, the analysis of which can help in prevention. Therefore, if hospitals only collect information on adverse events, they will be losing the most valuable source of information for identifying patient safety priorities, as well as the chance to measure progress in solving problems that they are trying to correct. Confidentiality and ease of system use are important stimuli for reporting. Report systems must be linked to the safety programs of health institutions and solutions must be focused towards changing the cultural environment of these institutions. Additionally, they must support the process in order to find, control and correct deviations that determine safety incidents since it has been demonstrated that they do not only express active personnel failures but also, and in particular, care system failures. The notification and reporting systems collect information on incidents that occur in hospitals and other sites where medical care is provided. These systems, which entail other components of patient safety, are uneven and diverging, with different events entered. Differences in geographic location, language and culture have motivated the World Health Organization to develop the International Patient Safety Classification with the intention of facilitating the international reporting and exchange of information and comparison between institutions. This article describes how the strength thereof has been orientated towards learning and sharing information on the root causes and best practices for prevention, as well as towards the acknowledgment and transparent communication of adverse events. Introduction. Many hospitals internationally have introduced systems for reporting patient safety incidents. However, it has not been possible to use them suitably for learning purposes. Study's objective was to identify how patient safety incident types are distributed in reports made by 22 hospitals. Material and methods. 706 patient safety voluntary reports by 22 hospitals were reviewed by using the Automated Health Incidents System (known as SIRAIS by its Spanish initials) based on the WHO International Classification for Patient Safety. Results. A total of 781 patient safety incidents were notified, in 706 reports following this distribution: health-care associated infection 280 (35.9%), clinical process/ procedure 225 (28.8%), medication/IV fluids 98 (12.5%), falls 72 (9.2%) and other incidents 106 (13.6%). Conclusions. The WHO International Classification for Patient Safety is a useful instrument for identifying and recording patient safety incidents. Their incorporation into an automated system significantly facilitates the entry and recovery of information, generating information in a timely manner that enables decision making and improves the quality of health care. 24. Melanocytic nevus of the anal canal and granular cell tumor of the cecum: a case report and literature review of 2 coincidentally co-occurring neurocristopathies.

- Abstract
Granular cell tumors are predominantly benign soft tissue tumors originating from Schwann cells, whereas melanocytic nevi are benign proliferations of melanocytes. We present the case of a patient with the presence of both entities located in the cecum and anal canal, respectively, constituting an extremely rare coincidental finding. A 43-year-old woman was evaluated by colonoscopy for iron-deficiency microcytic anemia that had lasted for 1 year. Colonoscopy demonstrated a macular lesion of 0.3 cm with a melanocytic appearance in the anal canal; at the cecum level, a subepithelial, yellowish, and partially mobile firm nodular lesion measuring 1.3 cm was observed. A histopathological study showed a melanocytic nevus in the anal canal and a granular cell tumor in the cecum. This is the first reported case of a patient with the extremely rare coincidental-incidental finding of these 2 entities at the same time.

- Abstract
Background: Leiomyoma is the most common benign esophageal tumor that originates in the muscular layer, with the most common site located in the distal third of the esophagus. Its growth causes symptoms due to partial obstruction of the esophageal lumen, leading to the diagnosis and subsequent treatment. Today, due to modern minimally invasive surgical techniques, complete removal of these tumors can be accomplished with minimal morbidity and excellent results., Case Report: We report the case of a 49-year-old female with a history of dysphagia, epigastric pain, and halitosis. She was diagnosed with a benign tumor originating from the muscular layer of the distal esophagus. Multiple biopsies had previously been taken, without histological confirmation. The patient underwent surgery where complete tumor enucleation was done laparoscopically., Conclusions: Preoperative diagnosis of these tumors should be based on clinical history and studies such as esophagoscopy and endoscopic ultrasound. Laparoscopic enucleation is the treatment of choice for leiomyomas of the esophagogastric junction. Biopsy specimens should be avoided because histology is not always possible. Furthermore, they are related to rupture of the esophageal mucosa during surgical treatment. After enucleation in selected patients, anti-reflux procedure should be carried out in order to protect the area of surgical resection and prevent complications from weakening the lower esophageal sphincter, as well as to resolve reflux symptoms.

- Abstract
Background: Identifying the level of culture and awareness on patient safety is essential in order to improve the quality of care. The objective of our research was to assess patient safety culture in healthcare professionals., Methods: Derived from the implementation of a survey on patient safety to 174 health professionals, data related to demographic characteristics were analyzed. Scores were determined on the 12 dimensions of the scale including additional items on perception and degree of overall patient safety, frequency and number of events reported, as well as Cronbach's alpha for the different dimensions. Descriptive statistics were used., Results: Overall perception on safety was 51%. The lowest dimension was communication openness (38%), whereas organizational learning had the highest scores (68%). Analysis showed low Cronbach's alpha for dimensions of overall perception on safety (0.35), non-punitive response to error (0.55) and staff allocation (0.12)., Conclusions: A low overall perception on patient safety indicates the need for increased institutional actions for improvement. Low Cronbach's alpha in some measurements may suggest the need for adjustments to the instrument. Results on the overall perception on patient safety, nonpunitive response to error and staff allocation (staffing) should be interpreted with caution, given the low reliability of such dimensions.

Objective: To carry out the epidemiological analysis of 122 influenza A (H1N1) deaths confirmed by laboratory and help to improve the diagnosis and timely managing of cases., Material and Methods: A total of 122 clinical records were analyzed of patients with confirmed influenza A (H1N1) virus infection who died., Results: Fifty-one percent of patients were female and 49% were male. A total of 45.l% who died were between 20 and 39 years old. Overall fatality was 2.2% and ranged between 0.3% for the l0 to l9 year-old group to 6.3% for the 50 to 59 year-old group. Forty-three percent of deaths were concentrated in only two of the thirty-two states and 5l% received medical attention in social security institutions. Only l7% received hospital attention within 72 hours and 42% died within 72 hours of hospital attention., Discussion: Novel Influenza A (H1N1) virus produces higher mortality in young people whereas seasonal influenza has a greater impact on young children and older people. Delay in medical care and the associated morbidity were relevant factors for death.
