Abstract Background Cerebral amyloid angiopathy (CAA) is characterized by amyloid-β (Aβ) deposition in cerebral vessels, leading to lobar cerebral microbleeds (CMB) and intracerebral hemorrhages (ICH). Apolipoprotein J (ApoJ) is a multifunctional chaperone related to Aβ aggregation and clearance. Our study investigated the vascular impact of chronic recombinant human Apolipoprotein J (rhApoJ) treatment in a transgenic mouse model of β-amyloidosis with prominent CAA. Methods Twenty-month-old APP23 C57BL/6 mice received 25 doses of rhApoJ (1 mg/kg) (n = 9) or saline (n = 8) intraperitoneally for 13 weeks, while Wild-type (WT) mice received saline (n = 13). Postmortem brains underwent T2*-weighted magnetic resonance imaging (MRI) to detect hemorrhagic lesions. Aβ levels and distribution, cerebral fibrinogen leakage, brain smooth muscle actin (sma), and plasma matrix metalloproteinases and inflammatory markers were analyzed after treatments. Additionally, plasma samples from 22 patients with lobar ICH were examined to determine the clinical relevance of the preclinical findings. Results rhApoJ-treated APP23 presented fewer cortical CMBs (50–300 μm diameter) (p = 0.012) and cortical larger hemorrhages (> 300 μm) (p = 0.002) than saline-treated mice, independently of Aβ brain levels. MRI-detected hemorrhagic lesions correlated with fibrinogen cerebral extravasation (p = 0.011). Additionally, rhApoJ-treated mice presented higher number of sma-positive vessels than saline-treated mice (p = 0.038). In rhApoJ-treated mice, human ApoJ was detected in plasma and in occasional leptomeningeal vessels, but not in the parenchyma, suggesting that its mechanism of action operates through the periphery. The administration of rhApoJ induced an increase in plasma Groα (p = 0.035) and MIP-1α (p = 0.035) levels, while lower MMP-12 (p = 0.046) levels, compared to the saline-treated group. In acute lobar ICH patients, MMP-12 plasma levels correlated with larger hemorrhage volume (p = 0.040) and irregular ICH shape (p = 0.036). Conclusions Chronic rhApoJ treatment in aged APP23 mice ameliorated CAA-related neurovascular damage by reducing the occurrence of CMB. We propose that rhApoJ may prevent blood-brain barrier (BBB) leakage and CMB appearance partly through circulating MMP-12 modulation.

Abstract The identification of large vessel occlusion with underlying intracranial atherosclerotic disease (ICAS-LVO) before endovascular treatment (EVT) continues to be a challenge. We aimed to analyze baseline clinical-radiological features associated with ICAS-LVO that could lead to a prompt identification. We performed a retrospective cross-sectional study of consecutive patients with stroke treated with EVT from January 2020 to April 2022. We included anterior LVO involving intracranial internal carotid artery and middle cerebral artery. We analyzed baseline clinical and radiological variables associated with ICAS-LVO and evaluated the diagnostic value of a multivariate logistic regression model to identify ICAS-LVO before EVT. ICAS-LVO was defined as presence of angiographic residual stenosis or a trend to re-occlusion during EVT procedure. A total of 338 patients were included in the study. Of them, 28 patients (8.3%) presented with ICAS-LVO. After adjusting for confounders, absence of atrial fibrillation (OR 9.33, 95% CI 1.11–78.42; p = 0.040), lower hypoperfusion intensity ratio (HIR [Tmax > 10 s/Tmax > 6 s ratio], (OR 0.69, 95% CI 0.50–0.95; p = 0.025), symptomatic intracranial artery calcification (IAC, OR .15, 95% CI 1.64–26.42, p = 0.006), a more proximal occlusion (ICA, MCA-M1: OR 4.00, 95% CI 1.23–13.03; p = 0.021), and smoking (OR 2.91, 95% CI 1.08–7.90; p = 0.035) were associated with ICAS-LVO. The intent of the proposed research offers an overview of the roles that teachers have taken during the pandemic and the possible changes teachers could play nowadays, as well. Based on the current context in the world, teachers face a different and complicated position rather than they used to do. Moreover, this research tries to identify the current roles of teachers as well as to show the impact of teachers’ roles on students and find out, which is the relation between teacher-student and student-student when facing this pandemic.

Abstract Background There is some evidence supporting the idea that double parathyroid adenomas represent a different entity from multiglandular hyperplasia; however, the distinction among them is not straightforward. Case presentation We described a case of primary hyperparathyroidism (PHPT) with pronounced clinical manifestations, caused by a bilateral giant parathyroid adenoma. A 34-year-old Hispanic/Latino male was diagnosed with PHPT caused by two giant parathyroid adenomas. The preoperative tests were neck ultrasound and computed tomography scan (CT-scan), showing two masses in the territory of parathyroid glands, bilaterally distributed (right was 31 × 18 × 19 mm and the left was 38 × 15 × 14 mm); sestamibi scan was not available. Parathyroid hormone (PTH) was highly elevated. Multiple complications of PHPT were present, such as bone lytic lesions, renal and pancreatic calcifications, and cardiovascular disease, the latter of which is an overlooked complication of PHPT. Multiple endocrine neoplasia 1 and 2 (MEN 1/2) were ruled out by the absence of clinical, biochemical, and radiological findings in other endocrine glands. The patient underwent subtotal parathyroidectomy with an intraoperative histopathological study; both intraoperative and definitive histopathology results were consistent with parathyroid adenomas; afterward, adequate suppression of PTH was assured, and later on, the patient presented hungry bone syndrome (HBS). Conclusions The diagnosis of double parathyroid adenomas is difficult. Regarding the similarities between multiglandular hyperplasia and parathyroid adenomas, this case report contributes to the further distinction between these two clinical entities. This case report also represents, in particular, the challenge of difficult diagnosis in places with limited resources, such as developing countries.

Objective: This study provides a series of updated, evidence-based recommendations for the management of acute stroke. We aim to lay a foundation for the development of individual centres’ internal protocols, serving as a reference for nursing care. Methods: We review the available evidence on acute stroke care. The most recent national and international guidelines were consulted. Levels of evidence and degrees of recommendation are based on the Oxford Centre for Evidence-Based Medicine classification. Results: The study describes prehospital acute stroke care, the operation of the code stroke protocol, care provided by the stroke team upon the patient’s arrival at hospital, reperfusion treatments and their limitations, admission to the stroke unit, nursing care in the stroke unit, and discharge from hospital. Conclusions: These guidelines provide general, evidence-based recommendations to guide professionals who care for patients with acute stroke. However, limited data are available on some aspects, showing the need for continued research on acute stroke management. Resumen: Objetivo: Proporcionar un conjunto de recomendaciones actualizadas y basadas en la evidencia disponible para el manejo del ictus agudo. Nuestro objetivo es proporcionar una base para el desarrollo de los protocolos internos de cada centro, sirviendo de referencia para los cuidados de enfermería. Métodos: Revisión de evidencias disponibles sobre los cuidados del ictus agudo. Se han consultado las guías nacionales e internacionales más recientes. Los niveles de evidencia y grados de recomendación se han basado en la clasificación del Centro de Medicina Basada en la Evidencia de Oxford. Resultados: Se describen la atención y los cuidados del ictus agudo en la fase prehospitalaria, el funcionamiento de código ictus, la atención por el equipo de ictus a la llegada al hospital, los tratamientos de reperfusión y sus limitaciones, el ingreso en la unidad de ictus (UI), los cuidados de enfermería en la UI y el alta hospitalaria. Smoking has been considering a crucial factor in promoting skin and systemic aging that is associated with the development of a low-level, systemic, chronic inflammation known as ''inflammaging'' in which monocytes play a pivotal role. Our aim was to investigate the effects of AM3 plus antioxidants vs placebo in the activation status, function of monocytes and cutaneous aging parameters in healthy smoker middle-aged women. A total of 32 women were 1:1 randomly assigned to AM3 plus antioxidants or placebo for three months. Peripheral mononuclear blood cells and cutaneous biopsy were obtained and flow cytometry and immunohistological studies, respectively, were performed before and after the treatment. AM3 plus antioxidants treatment compared with placebo significantly reduced the monocyte production of the proinflammatory interleukin 1 (IL-1), tumor necrosis factor α (TNF-α) and interleukin 6 (IL-6) cytokines as well as increased the regulatory IL-10 in middle-aged smoker women. Furthermore, AM3 and antioxidants did not modify ROS production by monocytes and granulocytes but increased their phagocytic activity. The active combination also stimulated a significative increase in reticular dermis depth as well as an increase in the expression of CD117 and CD31. Thus, AM3 and antioxidants treatment reduces the systemic proinflammatory monocyte disturbance of heathy smoker middle-aged women and encourage skin repair mechanisms.

En el presente trabajo se procede al estudio, desde su vertiente artística, de la cabalgata de Reyes Magos de Granada, entre 1912, cuando salió por primera vez, hasta 1936. La idea surgió del Centro Artístico, Literario y Científico, entidad cultural granadina con gran relevancia en el ámbito local durante este periodo, cuyo objetivo principal era llevar juguetes a los niños pobres y enfermos de la ciudad. Siendo este su fundamento, no se descuidaba ningún detalle del desfile: la iluminación y música que acompañaban al cortejo, el vestuario de la comitiva, elementos de atrezo destacando palanquines o carrozas, la presencia de animales como caballos o acémilas y otros más exóticos como camellos. Con ello, se conformaba una obra de arte total, provocando un gran impacto visual en los siempre numerosos asistentes.

El metilfenidato es un fármaco estimulante del sistema nervioso central de la familia de las anfetaminas. Este fármaco es muy utilizado entre los estudiantes de medicina para mejorar su rendimiento académico. Lo que hace importante estudiar los efectos secundarios que puede tener sobre las personas que lo utilizan sin una indicación médica. El objetivo de esta revisión es describir el uso del metilfenidato en los estudiantes de medicina, así como sus efectos sobre el rendimiento académico y las consecuencias adversas de su uso. Para esto se realizó una revisión de la literatura en distintas bases de datos, y se observó que los estudiantes de medicina son los mayores consumidores del metilfenidato entre los estudiantes de todas las carreras de las ciencias de la salud. La mayoría los utiliza sin una prescripción médica hecha para ellos, a pesar de los múltiples efectos adversos que este tiene como la adicción, somnolencia, fatiga, agitación, dificultad para concentrarse, hambre, etc. Por lo anterior, es importante trabajar en métodos de prevención para su uso off-label. Y en formas saludables para manejar el estrés y la carga académica en esta población.

The Military Macaw (Ara militaris) is not considered within the avifauna of Morelos due to a lack of solid evidence confirming the presence of the species in the state. We conducted systematic surveys from elevated viewpoints in the tropical dry forest of the Sierra de Huautla Biosphere Reserve from August to December 2017 and 2020-2022 to confirm the presence of the Military Macaw in the reserve. We obtained 36 records of the Military Macaw, with photographic evidence, and observed a mean of 10.0 ± 11.2 macaws per record, registering flocks of 32-39 individuals on three occasions. We identified sites within the reserve that Military Macaws recurrently used for resting, foraging, and roosting. We also observed macaws foraging in three tree species, with another six plant species reported in the diet by local people. We could not confirm nesting by macaws in the reserve as observations were conducted outside the breeding season. Some sightings occurred outside the biosphere reserve, in an area of the Mixteca Baja Poblana (Puebla) proposed as a new protected area. Our findings provide the first systematic and photographic evidence of the presence of the Military Macaw in Morelos state, confirm the relevance and effectivity of the Sierra de Huautla Biosphere Reserve for regional conservation of the species and the tropical dry forest, and provide additional support for the proposal to create a new protected area and conservation corridor in the Balsas Basin that could benefit the conservation of this endangered bird species.

The expanding role of flexible endoscopy (FE) has helped to establish better diagnostic strategies and fewer invasive therapies within the lumen of the gastrointestinal (GI) tract. Endoscopic skills represent critical tools for surgeons since they markedly impact perioperative outcomes. Although it is widely recognized that endoscopy plays a key role in digestive surgery, endoscopic curricula and syllabi may vary depending on geographical regions, which have their own standardized guidelines such as the United States and countries with numerous disparities such as Western Europe. Such heterogeneous practices represent a call for action, particularly as surgical societies aim to expand cutting-edge endoscopy within surgery. This article outlines the crucial role of intraoperative endoscopy in commonly performed digestive surgeries and stresses the need to develop standardized endoscopic training curricula in surgery, particularly in Europe.

The use of cricket protein in emulsions is in line with the growing interest in sustainable food sources, as crickets require minimal resources and produce lower greenhouse gas emissions than traditional livestock. Research in this area suggests that incorporating cricket protein into emulsions not only improves their nutritional value but also contributes to the development of environmentally friendly and functional food products. This study proposes the use of cricket protein for the stabilization of emulsions formulated with avocado oil as a dispersed phase. This oil is rich in monounsaturated fats, particularly oleic acid, and a variety of bioactive compounds. In the first part of this study, we assessed the influence of the emulsifier concentration and found that 2 wt.% is the optimum because a depletion flocculation effect was produced. Subsequently, processing was optimized using ultrasonication so that the higher energy input produced emulsions with a droplet diameter of less than 700 nm. Finally, rhamsan gum was added to the formulation, producing emulgels with improved pseudoplastic behavior and physical stability. This study demonstrates that cricket protein in combination with rhamsan gum is capable of forming stable, low-droplet-size emulgels with potential applications in encapsulation systems.

The proper functioning of the immune system depends on an appropriate balance between pro-inflammation and anti-inflammation. When the balance is disrupted and the system is excessively biased towards inflammation, immune responses cannot return within the normal range, which favors the onset of diseases of autoimmune or inflammatory nature. In this scenario, it is fundamental to find new compounds that can help restore this balance and contribute to the normal functioning of the immune system in humans. Here, we show the properties of a fungal compound with a strong safety profile in humans, AM3, as an immunomodulatory molecule to decrease excessive cytokine production in human cells. Our results present that AM3 treatment of human peripheral blood mononuclear cells and monocytes decreased their pro-inflammatory cytokine secretion following the challenge with bacterial lipopolysaccharide. Additionally, AM3 skewed the differentiation profile of human monocytes to macrophages towards a non-inflammatory phenotype without inducing tolerance, meaning these cells kept their capacity to respond to different stimuli. These effects were similar in young and elderly individuals. Thus, the fungal compound, AM3 may help reduce excessive immune activation in inflammatory conditions and keep the immune responses within a normal homeostatic range, regardless of the age of the individual.

Background Stenting extracranial internal carotid artery (ICA) lesions in acute ischemic stroke with tandem lesions is technically challenging. Its safety is highly debated because of the requirement of dual‐antiplatelet therapy. The optimal stenting device, timing, and periprocedural antiplatelet therapy for extracranial ICA stenting in the setting of acute tandem occlusion are still unclear. Methods We performed a retrospective study of patients with acute ischemic stroke attributable to tandem lesions who underwent endovascular treatment during a 5‐year period receiving either conventional self‐expanding carotid stents (SX) or balloon‐expandable carotid stent (BX). BX stents were restented with an SX in the subacute phase. Primary outcomes of interest were extracranial ICA patency at follow‐up and symptomatic intracranial hemorrhage. Results A total of 112 patients admitted from April 2016 to April 2021 were included. El presente artículo tiene como objetivo explorar la inteligencia empresarial y su rol en la generación de valor en los procesos de negocios; así, esta bibliometría recoge, sintetiza y analiza 104 artículos sobre una variedad de temas estrechamente relacionados con la Inteligencia Empresarial (Business Intelligence – [BI]), y artículos publicados en el periodo 2009 a 2022, relacionados con el tema objeto. La metodología es de tipo cualitativo, para ello se utilizó la base de datos de Scopuslgunos de los principales hallazgos sugieren que, existe una asociación entre inteligencia empresarial y la competitividad, además se encontró que es necesario ampliar los enfoques BI para mitigar lagunas de conocimiento en esta área del saber Finalmente, se evidenció que BI proporciona un marco teórico y empírico para el desarrollo de una teoría consistente, así como una base para el logro de una ES competitiva de alto nivel.

El objetivo de esta investigación cualitativa, estudio de caso, fue describir el impacto que ha tenido la pandemia en los procesos de enseñanza en los docentes de idiomas con clases presenciales y la necesidad que tienen de capacitación y formación curricular en saberes digitales. El proceso de análisis se llevó a cabo mediante la aplicación de una encuesta semiestructurada a un grupo conveniente de participantes que prefieren la modalidad presencial forzados a instruir en ambientes de aprendizaje virtual y asistencia remota por primera vez durante la pandemia COVID-19. Se utilizó las dimensiones de las competencias digitales del Marco Común Europeo para la medición de los niveles de dominio de las competencias digitales. Se recurrió al análisis temático. Se categorizaron los temas y en algunos casos se agruparon algunas instancias en subcategorías. Se retomaron los resultados y hallazgos en combinación con la presentación de gráficas descriptivas, tablas y menciones de los participantes. Se recalca que esta investigación se enfoca en recuperar las percepciones de un momento, grupo y situación específica. Los participantes manifestaron haber superado los retos de la competencia digital que se necesitó durante el aislamiento social para atender a los grupos en asistencia remota y de manera virtual. Los dominios de las competencias digitales de los participantes son aceptables, pero aún quedan dimensiones de las competencias digitales por desarrollar. Se manifiesta una necesidad de capacitación de los participantes en las cinco dimensiones, las cuales, de acuerdo con los datos, se deberían incluir en la formación de los futuros docentes de lenguas.

Translocation is increasingly being used as a conservation tool in wildlife management, but long-term assessments of the animals’ establishment in the new habitat are rarely done. In addition, finding protected areas for translocations can often be a limitation, but habitat patches managed for productive purposes could potentially be used for translocations. Here, we present a translocation case study of the Endangered Mexican howler monkey Alouatta palliata mexicana into a forest fragment managed as an agroforest in the Los Tuxtlas Biosphere Reserve (Mexico). We compared the behavior of the translocated focal group 6 yr after translocation with that observed 1 yr after translocation (Year 1 vs. Year 6), and with reference parameters for conserved forest. We also examined the 14 yr trajectory of the translocated population through published data. We found that in Year 6, monkeys spent less time on locomotion and more time consuming fruit than in Year 1. The focal group in Year 6 had doubled its activity area compared to Year 1. All behavioral parameters during Year 6 were similar to those reported for the species in conserved forest. During the first 14 yr, the translocated population increased at a rate of 1.29 ind. yr-1. We conclude that this translocation succeeded in establishing a thriving population and that certain agroforestry systems may be adequate habitat for primate translocations. We also discuss how the translocation of howler monkeys into defaunated habitats might help restore ecological functions associated with these primates, such as the dispersal of large-seeded plants. Long-term information on successful primate translocations has high practical value for designing adequate conservation strategies in anthropogenic landscapes.

Ultraviolet (UV) radiation harms the skin, causing oxidative damage, inflammation, and disruption of the skin’s barrier function. There is considerable interest in identifying new natural ingredients with antioxidant and anti-inflammatory properties to serve as adjuvants in sunscreens. The flavonoid morin (1) can undergo structural modifications to enhance its biological properties. The aim of this study was to synthesize two new morin-Schiff base derivatives, morin oxime (2) and morin semicarbazone (3), comparing their photoprotective effects with that of the parent compound on UVB-exposed HaCaT keratinocytes. The chemical structure of the novel compounds was revealed based on spectroscopic data analysis. Our findings demonstrated that derivatives 2 and 3 enhanced the light absorption capability in the UV–visible (vis) range compared to 1. Tested compounds exhibited a higher scavenger capacity than Trolox. Moreover, pre-treatment with all compounds protected HaCaT cells from UVB-induced cell death. Compound 3 demonstrated the strongest antioxidant effect, reducing reactive oxygen species (ROS) generation and, subsequently, malondialdehyde (MDA) levels. Additionally, compounds 2 and 3 exhibited greater anti-inflammatory effects than compound 1, significantly reducing interleukin (IL)-6 production levels at all tested concentrations. These findings have demonstrated, for the first time, a promising photoprotective activity of two new Schiff base derivatives and suggest their use as natural sunscreen ingredients.

IntroductionCerebral amyloid angiopathy (CAA) is characterized by the accumulation of amyloid-β (Aβ) in brain vessels and is a main cause of lobar intracerebral hemorrhage (ICH) in the elderly. CAA is associated with magnetic resonance imaging (MRI) markers of small vessel disease (SVD). Since Aβ is also accumulated in Alzheimer’s disease (AD) in the brain parenchyma, we aimed to study if several single nucleotide polymorphisms (SNPs) previously associated with AD were also associated with CAA pathology. Furthermore, we also studied the influence of APOE and CLU genetic variants in apolipoprotein E (ApoE) and clusterin/apolipoprotein J (ApoJ) circulating levels and their distribution among lipoproteins.MethodsThe study was carried out in a multicentric cohort of 126 patients with lobar ICH and clinical suspicion of CAA.ResultsWe observed several SNPs associated with CAA neuroimaging MRI markers [cortical superficial siderosis (cSS), enlarged perivascular spaces in the centrum semiovale (CSO-EPVS), lobar cerebral microbleeds (CMB), white matter hyperintensities (WMH), corticosubcortical atrophy and CAA-SVD burden score]. Concretely, ABCA7 (rs3764650), CLU (rs9331896 and rs933188), EPHA1 (rs11767557), and TREML2 (rs3747742) were significantly associated with a CAA-SVD burden score. Regarding circulating levels of apolipoproteins, protective AD SNPs of CLU [rs11136000 (T) and rs9331896 (C)] were significantly associated with higher HDL ApoJ content in the lobar ICH cohort. APOEε2 carriers presented higher plasma and LDL-associated ApoE levels whereas APOEε4 carriers presented lower plasma ApoE levels. Additionally, we observed that lower circulating ApoJ and ApoE levels were significantly associated with CAA-related MRI markers. More specifically, lower LDL-associated ApoJ and plasma and HDL-associated ApoE levels were significantly associated with CSO-EPVS, lower ApoJ content in HDL with brain atrophy and lower ApoE content in LDL with the extent of cSS.DiscussionThis study reinforces the relevance of lipid metabolism in CAA and cerebrovascular functionality. We propose that ApoJ and ApoE distribution among lipoproteins may be associated with pathological features related to CAA with higher ApoE and ApoJ levels in HDL possibly enhancing atheroprotective, antioxidative, and anti-inflammatory responses in cerebral β-amyloidosis.
