Caso clínico: Se presenta el caso de un paciente de 28 años con una lesión compatible clínica e histopatológicamente con queratoacantoma conjuntival. Fue tratado mediante excisión completa, y colirio de mitomicina C al 0,04% en el posoperatorio. El resultado ha sido la remisión clínica completa durante el tiempo de seguimiento (6 meses). Discusión: Es importante hacer un correcto diagnóstico diferencial entre queratoacantoma y carcinoma de células escamosas, así como un estrecho seguimiento postoperatorio por la posibilidad de recidiva o conversión a carcinoma de células escamosas. Para ello proponemos la citología de impresión conjuntival como un método no invasivo para el seguimiento de estos pacientes. Case report: We present a case report of a 28-year-old patient with a lesion that is compatible both clinically and histopathologically with conjunctival keratoacanthoma. The treatment given was complete excision and 0.04% mitomycin C eye drops in the postoperative period. The outcome was a complete clinical remission during the follow-up period (6 months). Discussion: It is important to make a correct differential diagnosis between keratoacanthoma and squamous cell carcinoma, as well as carrying out close monitoring after surgery due to the possibility of relapse and conversion to squamous cell carcinoma. For this reason, we propose the use of conjunctival impression cytology as a non-invasive method for monitoring such patients.

The purpose of this work is to find empirical evidence on the behavior of the prices in an insular region, with a market of small size and, simultaneously, fragmented. To that end, with consumer price data for four products of the food and beverage group observed in the Canary Islands we study the price and rigidity, characterized by frequency, duration and size of price changes. The main empirical result found for these four products is that market fragmentation and the type of establishment are important factors in explaining the price level and not so when one wants to explain his change. In this case, the product is the decisive element in explaining the basic features of price change, not observing rigidities either in terms of changes or establishments.

Since the introduction of the first commercial endofraft to treat the abdominal aortic aneurysm in the early 1990s, there has been a continuous improvement and technology evolution making feasible to treat successfully highly complex AAA with endovascular repair. We have performed EVAR therapy in highly complex aorto-iliac aneurysm using the Aorfix(®) stent-graft system from 2009 to 2011.A retrospective review to our clinical experience was performed, comparing two groups of patients: 1. Challenging anatomy group treated with Aorfix Endograft. 2. Standard anatomy group was integrated by EVAR procedures performed in patients with aortic angulation less than 75°or non-tortuos iliac vessels.In 96.5-97% of cases a bifurcated endograft was implanted with clinical and technical succes . After 1 year follow up in most cases and at least 6 months follow up in all the series we saw successful results, with aneurysmal sac reduction or stability, 2 type II endoleaks related to lumbar arteries with sac stability and no AAA-related complications were finded in the complex anatomy group.The 30 days, early-midterm and 12 months results for challenging anatomy group are identical to those finded in standard anatomy and consistent with published data, showing the performance of this endograft system in tortuous anatomies. These early results are quite promising, this particular graft, in its labeled use, is providing an excellent platform to EVAR therapy in challenging anatomies, off label to other devices currently avaliable; however, further studies are needed to evaluate the long-term results.

Since the introduction of the first endoprosthetic devices, continuous development in techniques and implants has occurred, such as the introduction of a stent graft with branches designed to preserve antegrade flow in the hypogastric artery, a stent-graft designed to treat extreme neck angulation and iliac tortuosity, as well as "Sandwich" and "Chimney" techniques used to maintain perfusion in branch vessels originating in the region to be treated. This paper describes how the Sandwich-Graft technique was adapted, as described by Lobato et al., employing the Aorfix™ system (Lombard Medical) and the Viabahn™ (W.L.Gore) to preserve hypogastric flow in cases with extreme neck angulation and iliac tortuosity.The study included four patients treated from April 2010 until November 2010 with the modified Sandwich technique. All patients eligible for this approach were considered unfit for open repair and were not suitable for an iliac branch graft (Z-BIS Zenith™ Cook Medical).A bifurcated endograft was implanted with specific, in-situ, branching to the target hypogastric artery and achieved clinical and technical success, in all the patients. After a 11-month follow-up in two cases and a six-month follow-up in the other two, clinical results were successful. All patients were endoleak-free, had patent hypogastric branches and had shrinking or stable aneurysms.The initial experience shows that the Sandwich technique with the Aorfix™ stent-graft demonstrated to be effective in endovascular repair of abdominal aortic aneurysms in patients with aortoiliac anatomy hostile to preserving hypogastric artery patency. This graft allows a broader group of patients to be treated with endovascular repair without potential complications of hypogastric artery occlusion; however, further studies are needed to evaluate long-term results in larger numbers of patients.

We report the case of a 62-year-old man who developed bronchiolitis obliterans with organizing pneumonia (BOOP) associated with acute Coxiella burnetii infection. The diagnosis of BOOP was made by fiber-optic transbronchial biopsy. Treatment with corticosteroids resulted in rapid clinical improvement and complete resolution of airspace opacities. To the best of our knowledge, this is the first well-documented case of BOOP associated with C. burnetii infection. This case suggests that this infection might trigger the development of BOOP.

En este trabajo, realizado en colaboración, se combinan las aportaciones de varios equipos regionales y el equipo central para describir el método de trabajo y algunas de nuestras experiencias en el ámbito de Hispalink.

En el presente trabajo se estudian las propiedades de seis métodos de desagregación temporal con información auxiliar basados en el propuesto por Chow y Lin (1971), a través de una simulación de Montecarlo. El objetivo del trabjo es determinar el método correcto a utilizar en función de las condiciones de la información auxiliar.

This paper describes the construction and validation of a prognostic model for predicting post-bloodstream infection survival up to Day 21. A Weibull multiple regression model was adopted in a prospective cohort study of all patients diagnosed with true bacteremia or fungemia in a teaching hospital between 1991 and 1994 (training set, 1,577 patients). The final model included six variables easily detected in any institution: source of infection, underlying neoplasm, septic shock, community-acquired, age over 65, and polymicrobial bacteremia. Using this model, it is possible to obtain a graphic representation of survival probability for any combination of these risk factors. The model was tested on a second set of patients diagnosed in the same hospital between 1996 and 1997 (validation set, 952 patients), confirming its reliability in predicting survival. In conclusion, the Weibull function, together with variables easily identified at bedside, enables a precise prediction of the short-term, post-bloodstream infection mortality of a given patient.

There are no detailed studies in our country on the impact of modern imaging techniques on diagnosis, treatment and prognosis of pyogenic liver abscesses.All patients with the diagnosis of PLA from 1981 to 1998 were included in the study. The study was divided in two periods: 1981 to 1989 and 1990 to 1998.Compared with the first time period, the following was observed from 1990 to 1998: the mean age was higher (52 versus 65 years, p = 0.006), infections with identified source increased (33% versus 74%, p = 0.003), diagnosis was earlier (13 versus 3 days, p = 0.0002), modern imaging techniques were used more frequently (17% versus 96%, p = 0.002), the proportion of recovered microorganisms increased (53% versus 88%, p = 0.002), as well as use of percutaneous drainage (0% versus 37%, p = 0.002) and the prognosis was better (mortality rate 40% versus 10%, p = 0.01).Relevant changes were observed among patients in our hospital in the nineties regarding epidemiology, management and prognosis of PLAs. Part of these changes are due to a higher use of new imaging techniques.

The characteristics of 70 cases of bacteremia in cirrhotic patients were studied according to the Child-Pugh classification as severity marker of liver involvement. Factors influencing on prognosis were determined. For a comparative analysis, 1,006 cases of bacteremia in non-cirrhotic patients were included. Sixteen patients corresponded to group A, 23 to group B and 31 to group C in the Child-Pugh classification. Patients in group A had a predominance of extra-enteric microorganisms, mainly Staphylococcus aureus (37.5%; p = 0.02), well-defined source (urinary tract, respiratory tract, skin) and good prognosis (mortality rate 6.2%). In contrast, patients in group C had a high recovery rate of Escherichia coli (41.9%) and pneumococcus (19.3%), undetermined source (51.6%; p = 0.05), ascites (83.9%), with or without concomitant peritonitis (41.1%; p = 0.03) and poor prognosis (mortality rate 48.3%; p = 0.008). The characteristics of patients in group B were similar to those of patients in group C but prognosis was as in patients in group A. The immediate mortality rate in the studied patients was 26%. The parameter which best predicted survival in the multivariate analysis was the Child-Pugh classification.

The analysis of the most relevant characteristics and prognostic factors in elderly patients with bacteremia.For the period 1989-1993 a prospective study of all significant bacteremias in adult patients admitted to a General Hospital was performed. Patients were visited until their death or during at least 50 days. The most relevant findings in elderly patients (over 65 years old) are described. Overall survival probabilities were obtained by Kaplan-Merner analysis. Cox proportional models were used to examine hazards of dying.Of the 1,128 bacteremias studied, 603 (53.9%) were in elderly patients. In this group, the increasing age was related with higher frequency of urinary (p = 0.02) and biliary (p = 0.001) sources of infection and lower frequency of underlying neoplasia (p = 0.06), immunosuppression (p = 0.0000) and development of septic shock (p = 0.02). These differences are higher in patients over 85. Among older patients the survival probability in the day 21 after diagnosis was 0.71 (95% CI 0.66-0.74), significantly lower to the probability in younger patients (p = 0.0001). In the elderly patients, the worse prognestic was associated to shock (RR = 8; 95% CI 5.8-11), indeterminated source of infection (RR = 3.6; 95% CI 2.2-5.8), underlying neoplasia (RR = 1.7; 95% CI 1.3-2.4), neutropania (RR = 1.5; 95% CI 1.1-2.1) nosocomial acquisition (RR = 1.3; 95% CI 1.0-3.1) and inappropriate treatment (RR = 1.2; 95% CI 1.0-3.2), but age was not an independent contributor.We found differences between the clinical characteristics and the prognosis of bactermia in elderly and younger patients. Among elder patients, those over 85 years old constitute a particular group with well defined characteristics.

The purpose of this paper is to measure attitudes towards loud music and to evaluate the influence of some variables on such attitudes. The scale of attitudes towards loud music was administered to a sample of 173 sixteen-year old subjects. The majority of the adolescents show neutral or favorable attitudes towards loud music. The sex and environmental noise variables have a non significant influence on this attitude. The majority of the sampled group attend dancing places (clubs, discotheques, etc.) once or twice a month and sometimes listen to loud music at home or at friends' home. Finally, adolescents showing favorable attitudes attend dancing places or listen to loud music more often than those showing unfavorable attitudes. This paper is a first approach to a deeper research to evaluate the possibility of modifying experimentally the attitude towards loud music by means of a message displaying both physical and psychological damage this fashion has caused.
