Abstract

El tratamiento de la diabetes tipo 2 en el anciano representa un importante reto tanto desde el punto de vista clínico como del de la salud pública. El envejecimiento poblacional está condicionando un marcado incremento de la pandemia de diabetes en las personas de edad avanzada. Sin embargo, existen pocas evidencias científicas que apoyen el tratamiento más adecuado de la diabetes en los ancianos. Dada la gran heterogeneidad de la población anciana, que incluye a sujetos con muy diferente capacidad funcional y cognitiva, diversa comorbilidad y con muy diferente expectativa de vida, resulta crucial realizar una valoración global del anciano desde una perspectiva biopsicosocial y abordar integralmente los factores de riesgo vascular, planteando unos objetivos personalizados de control glucémico. En ancianos frágiles o con corta expectativa de vida puede ser razonable mantener un objetivo de HbA1c de 7,6-8,5%. La estrategia terapéutica en el anciano con diabetes tipo 2 debe individualizarse y consensuarse con el paciente y sus cuidadores, en función del objetivo planteado. Mejorar la calidad de vida, preservar la seguridad del paciente y evitar los efectos adversos del tratamiento antidiabético deben ser objetivos básicos. Dada la mayor predisposición de los ancianos a las hipoglucemias y sus graves consecuencias en esta población, deberían priorizarse las terapias antidiabéticas que minimicen el riesgo de episodios hipoglucémicos.

Mobility loss is particularly burdensome due to its association with negative health outcomes, loss of independence and disability, and the heavy impact on quality of life. Recently, a new condition, physical frailty and sarcopenia, has been proposed to define a critical stage in the disabling cascade. Physical frailty and sarcopenia are characterized by weakness, slowness, and reduced muscle mass, yet with preserved ability to move independently. One of the strategies that have shown some benefits in combatting mobility loss and its consequences for older adults is physical activity. Here, we describe the opportunities and challenges for the development of physical activity interventions in people with physical frailty and sarcopenia. The aim of this article is to review age-related physio(patho)logical changes that impact mobility in old age and to provide recommendations and procedures in accordance with the available literature.Keywords: physical exercise, muscle mass, strength, walking, balance, better aging

Using cross-sectional and longitudinal analyses, we assessed the impact of malnutrition risk on healthcare utilization and costs in a cohort of older adults living in Spanish community.Patients and Methods: Data from 1660 older (range 66– 98 years), community-living adults participating in the Toledo Study on Healthy Ageing, waves 2 (year 2011– 2013) and 3 (year 2015), were analyzed. Nutritional status categories were defined according to the Global Leadership Initiative on Malnutrition (GLIM) criteria, using a two-step approach. First, screening for malnutrition risk. Once positive, individuals were classified as malnourished according to some phenotypic (body mass index, grip strength, and unintentional weight loss) and etiologic (disease burden/inflammation and reduced food intake or assimilation) criteria. Outcomes assessed included healthcare resources (hospital admissions, number of hospitalizations, length of hospital stay per hospitalization, and number of medications).Results: Fifteen percent of the population was found to be at risk of malnutrition, while 12.6% was malnourished. Overall, patients from both groups were older, had lower functional status, and had more comorbidities compared to well-nourished counterparts (p< 0.05). Results of our cross-sectional analysis showed that being at-risk/malnourished was associated with greater medication utilization, higher rates of hospital admission and longer stays, and higher hospitalization costs. However, when adjusting for covariates, malnutrition/risk was associated only with higher hospitalization costs (range: 11– 13%). Longitudinal analysis results indicated that malnutrition/risk was significantly associated with more frequent hospitalizations, longer lengths of stay, higher hospitalization costs, and polypharmacy at follow-up.Conclusion: Malnutrition or its risk, found in over one of four older adults in the Toledo community, was associated with higher healthcare resource use and increased costs. Such findings suggest that malnutrition risk-screening for older adults, and provision of nutrition counseling and care when needed, hold potential to improve their health and to lower costs of care in the Spanish healthcare system.Keywords: malnutrition prevalence, healthcare resource use, costs, oral nutritional supplements, ONS, older adults, community, Spain

Abstract

Structural and intercultural competence approaches have been widely applied to fields such as medical training, healthcare practice, healthcare policies and health promotion. Nevertheless, their systematic implementation in epidemiological research is absent. Based on a scoping review and a qualitative analysis, in this article we propose a checklist to assess cultural and structural competence in epidemiological research: the Structural and Intercultural Competence for Epidemiological Studies guidelines. These guidelines are organised as a checklist of 22 items and consider four dimensions of competence (awareness and reflexivity, cultural and structural validation, cultural and structural sensitivity, and cultural and structural representativeness), which are applied to the different stages of epidemiological research: (1) research team building and research questions; (2) study design, participant recruitment, data collection and data analysis; and (3) dissemination. These are the first guidelines addressing structural and cultural competence in epidemiological inquiry.

Abstract

Low muscle strength is an important heritable indicator of poor health linked to morbidity and mortality in older people. In a genome-wide association study meta-analysis of 256,523 Europeans aged 60 years and over from 22 cohorts we identify 15 loci associated with muscle weakness (European Working Group on Sarcopenia in Older People definition: n = 48,596 cases, 18.9% of total), including 12 loci not implicated in previous analyses of continuous measures of grip strength. Loci include genes reportedly involved in autoimmune disease (HLA-DQA1p = 4 × 10−17), arthritis (GDF5p = 4 × 10−13), cell cycle control and cancer protection, regulation of transcription, and others involved in the development and maintenance of the musculoskeletal system. Using Mendelian randomization we report possible overlapping causal pathways, including diabetes susceptibility, haematological parameters, and the immune system. We conclude that muscle weakness in older adults has distinct mechanisms from continuous strength, including several pathways considered to be hallmarks of ageing.

Aunque el agua es un nutriente esencial para la vida y el componente más abundante de nuestro cuerpo, recibe escasa atención en las recomendaciones dietéticas y las guías clínicas. Existen inconvenientes para determinar las cifras óptimas, tanto para la cantidad de agua que debe contener el cuerpo como para su ingesta. La ingesta y eliminación del agua dependen de factores no constantes y difíciles de medir, a su vez compensados por la capacidad del organismo para la homeostasis. Dada la falta de evidencia científica para el establecimiento de recomendaciones, se han estimado las “ingestas adecuadas” (para mantener un estado de hidratación adecuado) utilizando datos de ingestas de agua en grupos de personas sanas. La Autoridad Europea de Seguridad Alimentaria (EFSA) también considera la osmolaridad deseable en la orina para estimar la ingesta adecuada de agua en los adultos. Los estudios clínicos han mostrado en general beneficios con una hidratación adecuada y perjuicios con sus desequilibrios, ya sean cuantitativos (deshidratación y sobrehidratación) o cualitativos (agua extracelular e intracelular). Desafortunadamente, estos estudios son escasos y suelen tener diseños deficientes, ya sean transversales, de casos y controles o prospectivos, utilizando muestras pequeñas o métodos indirectos para evaluar el estado de hidratación. En este artículo se presenta información de actualización respecto a: 1) la adherencia a las recomendaciones de consumo de agua y sugerencias para mejorarla; 2) técnicas disponibles para medir el estado de hidratación y sus aplicaciones clínicas; 3) efectos de la hidratación/deshidratación en las actividades físicas o cognitivas y en las enfermedades crónicas; y 4) normativa española sobre calidad y salubridad del agua. Water is an essential nutrient for life and the most abundant component in the human body. However, its dietary recommendations or clinical management guidelines do not receive as much attention as they deserve. In addition, there are some obstacles to establishing optimal values, both for the amount of water the body must contain and for water ingestion. Water intake and elimination depend on unsteady factors that are difficult to measure and, at the same time, compensated by the body’s ability to regulate homeostasis. Since scientific evidence is lacking for establishing recommenda-tions, “adequate intakes” (to maintain an adequate hydration state) have been estimated using data on water intake from groups of healthy people. The European Food Safety Authority (EFSA) also considers desirable the use of urine osmolarity to estimate the adequacy of water intake in adults. Clinical studies have generally shown the benefits of adequate hydration and the damage caused by water imbalance, whether quantitative (dehydration and overhydration) or qualitative (extracellular and intracellular water). Unfortunately, these studies are few and often have poor cross-sectional, case-control, or prospective designs, and use small samples or indirect methods to assess hydration status. This article presents up-to-date information on subjects such as: 1) compliance with water consumption recommendations and suggestions for improvement; 2) techniques available to measure hydration status and their clinical applications; 3) effects of hydration/dehydration on physical or cognitive activities and chronic diseases; and 4) existing Spanish regulations on the quality and salubrity of water.

One of the most widely conserved hallmarks of aging is a decline in functional capabilities. Mobility loss is particularly burdensome due to its association with negative health outcomes, loss of independence and disability, and the heavy impact on quality of life. Recently, a new condition, physical frailty and sarcopenia, has been proposed to define a critical stage in the disabling cascade. Physical frailty and sarcopenia are characterized by weakness, slowness, and reduced muscle mass, yet with preserved ability to move independently. One of the strategies that have shown some benefits in combatting mobility loss and its consequences for older adults is physical activity. Here, we describe the opportunities and challenges for the development of physical activity interventions in people with physical frailty and sarcopenia. The aim of this article is to review age-related physio(patho)logical changes that impact mobility in old age and to provide recommendations and procedures in accordance with the available literature.

Abstract

Background: Concerns have been raised about the possibility that inhibitors of the renin–angiotensin–aldosterone system (RAAS) could predispose individuals to severe COVID-19; however, epidemiological evidence is lacking. We report the results of a case-population study done in Madrid, Spain, since the outbreak of COVID-19. Methods: In this case-population study, we consecutively selected patients aged 18 years or older with a PCR-confirmed diagnosis of COVID-19 requiring admission to hospital from seven hospitals in Madrid, who had been admitted between March 1 and March 24, 2020. As a reference group, we randomly sampled ten patients per case, individually matched for age, sex, region (ie, Madrid), and date of admission to hospital (month and day; index date), from Base de datos para la Investigación Farmacoepidemiológica en Atención Primaria (BIFAP), a Spanish primary health-care database, in its last available year (2018). We extracted information on comorbidities and prescriptions up to the month before index date (ie, current use) from electronic clinical records of both cases and controls. The outcome of interest was admission to hospital of patients with COVID-19. To minimise confounding by indication, the main analysis focused on assessing the association between COVID-19 requiring admission to hospital and use of RAAS inhibitors compared with use of other antihypertensive drugs. We calculated odds ratios (ORs) and 95% CIs, adjusted for age, sex, and cardiovascular comorbidities and risk factors, using conditional logistic regression. The protocol of the study was registered in the EU electronic Register of Post-Authorisation Studies, EUPAS34437. Findings: We collected data for 1139 cases and 11 390 population controls. Among cases, 444 (39·0%) were female and the mean age was 69·1 years (SD 15·4), and despite being matched on sex and age, a significantly higher proportion of cases had pre-existing cardiovascular disease (OR 1·98, 95% CI 1·62–2·41) and risk
