8. Skin Penetration and Permeation Properties of Transcutol® in Complex Formulations.

Abstract

Percutaneous delivery is explored as alternative pathway for addressing the drawbacks associated with the oral administration of otherwise efficacious drugs. Short of breaching the skin by physical means, the preference goes to formulation strategies that augment passive diffusion across the skin. One such strategy lies in the use of skin penetration and permeation enhancers notably of hydroxylated solvents like propylene glycol (PG), ethanol (EtOH), and diethylene glycol monoethyl ether (Transcutol®, TRC). In a previous publication, we focused on the role of Transcutol® as enhancer in neat or diluted systems. Herein, we explore its' role in complex formulation systems, including patches, emulsions, vesicles, solid lipid nanoparticles, and micro or nanoemulsions. This review discusses enhancement mechanisms associated with hydroalcoholic solvents in general and TRC in particular, as manifested in multi-component formulation settings alongside other solvents and enhancers. The principles that govern skin penetration and permeation, notably the importance of drug diffusion due to solubilization and thermodynamic activity in the vehicle (formulation), drug solubilization and partitioning in the stratum corneum (SC), and/or solvent drag across the skin into deeper tissue for systemic absorption are discussed. Emphasized also are the interplay between the drug properties, the skin barrier function and the formulation parameters that are key to successful (trans)dermal delivery. [ABSTRACT FROM AUTHOR]

Abstract

Controlling the launching efficiencies and the directionality of surface plasmon polaritons (SPPs) and their decoupling to freely propagating light is a major goal for the development of plasmonic devices and systems. Here, we report on the design and experimental observation of a highly efficient unidirectional surface plasmon launcher composed of eleven subwavelength grooves, each with a distinct depth and width. Our observations show that, under normal illumination by a focused Gaussian beam, unidirectional SPP launching with an efficiency of at least 52% is achieved experimentally with a compact device of total length smaller than 8 \mu m. Abstract

La présente étude propose d’analyser les caractéristiques spatio-temporelles de la voix de patients francophones présentant une immobilité laryngée suite à une opération de la glande thyroïde. Il s’agit ici d’évaluer les conséquences de l’immobilité laryngée sur le timing des articulateurs, à l’aide d’un corpus de logatomes (ou de séquences VCV). Notre investigation, bien qu’acoustique, entend analyser, le Voice Onset Time les différents indices articulatori-acoustiques qui peuvent être observés à partir d’un signal acoustique continu, et ainsi remonter aux configurations articulatoires (Abry et al., 1985). Une attention particulière sera donc accordée aux différentes perturbations temporelles qu’une immobilité laryngée peut provoquer sur la voix des patients, grâce à l’étude de la réorganisation temporelles des paramètres retenus. Notre étude se veut longitudinale puisque la voix des patients est enregistrée lors de différentes phases post-opératoires (post-opératoire 2, 3 et 4), ce qui devrait nous permettre de mettre au jour, non seulement les perturbations provoquées par l’immobilité laryngée, mais également les réajustements que les patients pourraient mettre en place au cours du temps et de leur rééducation orthophonique. Seul l’intervalle du VOT pour les occlusives sonores a été modifié. Pour les occlusives voisées, la transition CV implique uniquement un changement d’un état obstrué de conduit vocal à un état suffisamment dégagé de celui-ci pour permettre l’apparition d’une structure formantique claire et stable. En revanche, pour les occlusives non voisées, cette transition CV exige, outre le changement d’état du conduit vocal, une modification de la configuration de la glotte d’une position ouverte pour la consonne non voisée à une position fermée pour la voyelle subséquente. En conséquence, en raison de difficultés de maintien de vibrations laryngées, le VOT, qui est un indice de voisement, est alors généralement plus long pour les occlusives voisées que celles produites par les locuteurs contrôles. La durée du VOT de Klatt (1975) des occlusives voisées des patients reste toutefois quantitativement distincte des occlusives non voisées. En conséquence, la durée de la tenue consonantique des patients se voit également allongée dans les phases d’enregistrement précoces.

41. Prevention of radiation enteritis by an absorbable polyglycolic acid mesh sling: a 60-case multicentric study

Abstract

Radiation is often used in conjunction with surgery in the treatment of cancer. When the primary cancer is removed, radiation is applied to the region of the malignancy to destroy any tiny nests of tumor cells that may have escaped detection and removal. Invariably, the dose of radiation is limited by the damage that might be inflicted upon normal, health tissues. This is especially true in the treatment of cancer in the pelvic region. The small intestines are particularly susceptible to the deleterious effects of radiation. When the dose of radiation exceeds 4,000 to 5,000 cGy, small intestine damage occurs in 5 to 25 percent of cases. This dose range of radiation is not small, but is significantly smaller than that given for many tumors, and is smaller than would be preferred for best results. One way of reducing the risk of radiation damage to the small intestine would be to hoist it out of the way, so that it is no longer in the direct path of the radiation beam. This would require the surgical placement of a sling within the patient's abdomen to hold the small intestine. Furthermore, a second surgical procedure would be necessary to remove the sling after radiotherapy is complete. Researchers have now simplified this procedure by developing a sling composed of an absorbable material. This avoids a second surgical procedure by eliminating the requirement for removing the sling. An absorbable mesh sling composed of polyglycolic acid was tested in the treatment of 60 cancer patients requiring irradiation of the pelvic region. Surgical complications occurred in five patients, and four developed radiation-induced inflammation of the bowels despite the preventive measure. Magnetic resonance imaging was used to observe the resorption of the mesh material, which was complete three to five months after the operation. The results suggest that the polyglycolic acid mesh sling is safe, and should be considered for patients at high risk for the recurrence of pelvic cancer who may benefit from increased doses of radiation therapy. Abstract

Background The American Thyroid Association (ATA) recommends using ultrasound-guided fine-needle aspiration (FNA) in order to evaluate supracentimetric and suspect thyroid nodules. The purpose of this study was to evaluate the effective use of FNA before surgery for nodules over 3 cm in diameter. Methods In this retrospective study, we analyzed the results of ultrasound-guided FNA and postoperative histological analysis in 843 nodules >3 cm. Results The FNA was informative in 42.6%. The correlation with the final histological analysis was 94.8% for benign nodules and 71.0% for malignant nodules. The FNA had a positive predictive value of 71%, a specificity of 97%, a sensitivity of 56%, and a 4.7% rate of false-negative results. Conclusion Because there is a nonnegligible FNA risk of error, notably allowing the evolution of a cancer in 1 of 20 cases, the FNA data should not delay surgical intervention for potentially suspect nodules >3 cm in diameter. © 2016 Wiley Periodicals, Inc. Abstract

International audience; This overview focuses on the follow up after uterine cervix and corpus cancers. At early stage, troth are associated with good prognosis. Screening for recurrence is mainly based on clinical examination. Screening for a second cancer after endometrial cancer is already planned according to the French recommendations for systematic breast and colon cancer screening. Screening for a second cancer after cervical cancer requires a close examination of organs close to the cervix receiving high doses of radiations and HPV exposed (anus, vulva, vagina and perineum). Late chemotherapy related toxicity after both cancers is rarely encountered and mainly comprise neurological peripheral effects. Late surgical and/or radiation related side effects are more frequent. However, no more than 10% of patients are affected and in such cases, digestive, urinary and lymphatic systems are impaired. Prevalence of sexual dysfunction in patients with uterine cancers is particularly high but the radiotherapy related anatomical modifications (vaginal stenosis for example) might not be the sole reason. Fertility preservation is possible tor uterine cancers but requires a rigorous selection of candidates and should be coordinated by specialized tea
