Basal cell carcinomas (BCCs) are slow-growing, locally invasive lesions that rarely metastasise, however, if left untreated can progress to extensive destruction of local structures giving rise to the alternative name ‘the rodent ulcer’. Here we present a case of BCC of the ear progressing to destruction of the pinna and mastoid, involving the facial nerve and breaching dura. This case presentation shows not only the seldom-seen natural progression of the rodent ulcer with impressive imaging and clinical photographs but also highlights a broader topic of capacity and consent in patients displaying maladaptive denial or abnormal illness behaviour, how this can affect patient outcomes and what we as clinicians can do to ensure the best care for our patients.

William J. Niblock (WJN), an Assistant Surgeon at the Madras General Hospital (MGH), published a paper 'Cancer in India' (2 pages of text and 3 pages of tables) in the Indian Medical Gazette in 1902. He appears to have been a popular surgeon in Madras who surgically treated mouth cancers, testicular filariasis, and calcareous stones in the liver, gall bladder, and kidney. His 'Cancer in India' article is a compilation of numerical data of different cancers recorded in MGH from 1892 to 1901 mostly, and from 1896 to 1901 occasionally. In this article, WJN refers to cancers of different internal organs and external parts. He attributes the mouth cancers to constant chewing of 'betel' (the betel quid), which he explains as giving rise to leukoplakia, forerunner of buccal carcinoma. He supplies many, easily comparable, paired tables. These tables are made of raw numbers about the diverse human populations living in Madras, such as the Indians (Hindus and Muslims), Europeans, and Eurasians (Anglo-Indians) extracted from the 10-year records of MGH. None of the tables has been analyzed statistically. Nevertheless, he supplies the total numbers of admissions into MGH, which serve usefully as denominators in this study; occasionally, he presents data as percentages. Despite the lack of parametric statistical analysis, WJN's article, written in 1902, impresses as a useful contribution, because it provides a cross-sectional view of cancer incidence in Madras, particularly in defiance of Saldanha's supposition that cancers do not manifest in dark-skinned people, such as Indians.

An eighty years old man presented with a slowly progressing, ulcerative mass near the left eye diagnosed as the advanced stage of facial basal cell carcinoma (rodent ulcer) which has involved his left eye and orbit, left nostril and left cheek area. According to the patient, it was a very slow growing tumor and has reached this stage over a span of more than ten years. He has been advised removal of his left eye and adjacent structure along with plastic reconstruction of the damaged half of the nose and face. It will be important to excise the tumor along with healthy margins to prevent recurrences. Take home message is that basal cell carcinoma should be excised early to prevent damage to the eye and nearby structures.

Background: Basal cell carcinomas or rodent ulcers are commonly seen in the tear zone of the face. It rarely mimics melanomas when pigmented. Aim: We review literature of this uncommon condition and discuss the therapeutic modalities. Case Report: An elderly lady sought attention for a pigmented lesion on her forehead. The case is presented because pigmented lesions and verrucous appearing lesions can also be due to basal cell carcinoma. Typically basal cell carcinoma appears in the tear zone of the face and it mostly affects sun exposed areas. Conclusion: While surgery remains the gold standard in treatment, other modalities can be offered in areas where vital structures cannot be sacrificed and in recurrent lesions.

Introduction: Basal Cell Carcinoma (BCC) is the most common skin cancer worldwide, which appears over sun-exposed skin as slow-growing, locally invasive lesion that rarely metastasizes. Many phenotypic presentations are possible. BCCs are more common in males and tend to occur in older people. Majority is found on the head and neck. Many histopathological subtypes have been defined including nodular, micronodular, cystic, superficial, pigmented, adenoid, infiltrating, sclerosing, keratotic, infundibulocystic, metatypical, basosquamous and fibroepitheliomatous. Mixed patterns are common. Aim: The aim was to study morphological spectrum of BCC in a tertiary care hospital in southern Karnataka. Materials and Methods: This was a retrospective analysis of 100 cases of BCCs reported in the Department of Pathology over a 9-year period from 2006 to 2014. Results: The mean age of presentation was 62 years. There was slight female preponderance (56%). The most common location was face (65%) and the most common presentation was ulceration (45%). f the 100 BCCs, 50% were nodular, 13% infiltrating, 6% basosquamous, 4% superficial, 3% keratotic, 3% multinodular and 1% mixed. Conclusion: BCC, besides being the commonest cutaneous cancer, is also known for its numerous histological patterns which are shown to have prognostic implications. This study reveals the frequency of the various histological patterns of BCC in southern Karnataka, where it has been rarely studied before.

症例は67歳の男性。約8年前に気づいた黒色の左鼠径部の皮疹が徐々に拡大, 潰瘍化してきたため, 1998年鹿児島大学病院皮膚科外来を受診した。潰瘍は24cm×9.5cmと下腹部ほぼ全体を占め, 潰瘍底には鼠径動脈が露出していた。また陰茎皮膚は象皮症を呈し, 排尿困難があった。潰瘍辺縁より行った皮膚生検で基底細胞癌の診断を得た。入院後に行った全身検索では他臓器への転移は見られなかった。露出動脈からの出血や陰茎の象皮症に対し, 血管置換術, 陰茎切断術及び尿路変更術を行った。潰瘍部には放射線療法及び植皮術を行い癌は完治した。

Basal cell carcinoma is the most common Non-Melanoma Skin Cancer (NMSC). There is no precursor skin lesion for basal cell carcinoma. It commonly infiltrates locally and rarely metastasizes. Metastasis is associated with advanced patient age and neglected large lesions. However resection of primary site before the appearance of metastasis gives wonderful results that is presented in this case of Noduloulcerative type of basal cell carcinoma.

Background: The forehead flap is a useful technique to reconstruct deep and large nasal defects. Its disadvantages includes the fact that it is at least a two-stage procedure .The aim of this work is to describe our experience with the use of One stage forehead flap reconstruction to cover nasal defects. Patient: Fifteen patients with extensive nasal defects due to various causes. Full-thickness skin was lost in all cases, n=7 cases after excision due to rodent ulcer, n=5 cases after squamous cell carcinoma and n=3 cases after nasal trauma. Ulcer sizes and sites were demonstrated in table 1. All were repaired with a single stage forehead flap. The functional and aesthetic results were assessed. Results: High aesthetic and functional goals were achieved in all patients. There were no cases of significant flap necrosis. However, one patient developed mild superficial partial-thickness necrosis. Conclusions: The singe stage forehead flap represents one of the best methods for repair of extensive nasal defects, which may be a substitute for the traditional method (two stages) regard to time repair and cosmetic results.

Summary The ability of a carbon dioxide laser to scatter debris and aerosolize bacteria from human skin was tested by collecting the aerosols liberated with glass impingers together with a slit sampler. Two sets of tests were done—one to detect dispersal of cells from treatment of rodent ulcers or warts, and the other on pieces of skin obtained from autopsy material, which had been injected with spores. With low power levels a few bacterial particles were collected, together with some whole cells, however, at levels > 750 W cm −2 all the cultures were sterile.

An ulcerous, infiltrating, mostly solid, partly cystic and keratotic type of basal cell carcinoma of the left palm, of about 5-years duration, in a 60-year-old woman is described. Light and electron microscopical findings are presented with some histogenetical considerations of the tumor.

Radioactive phosphorus incorporated into thin polythene plastic for use as a β-ray applicator in superficial therapy is described. Measurements made with a shallow ionization chamber show surface dosage rates of the order of 1500 to 2000 r/hour. Depth dose measurements were made in unit density material and compared with similar measurements made with a 45 kVp X-ray plant. The protection problems are discussed and safe methods of handling and preparation of the applicators are described. The initial response to this β therapy of five patients with superficial rodent ulcers, and two patients with psoriasis, is presented. Preliminary clinical investigations show that the lesions to be treated must be carefully selected. The method appears applicable in particular to patients with multiple basal cell carcinomata.
