Abstract

BACKGROUND AND OBJECTIVES: Children with medical complexity (CMC) comprise a subgroup of children with severe chronic diseases. A conceptual definition for CMC has been formulated, but there is no agreement on criteria to fulfill each of the 4 proposed domains: diagnostic conditions, functional limitations, health care use, and family-identified needs. Our objective with this study was to identify a standardized definition of CMC. METHODS: Through a scoping review of the CMC literature, we identified potential criteria to fulfill each domain. These were incorporated into an electronic survey that was completed by a geographic and professionally varied panel of 81 American and Canadian respondents with expertise in managing CMC (response rate 70%) as part of a 4-iteration Delphi procedure. Respondents were asked to vote for the inclusion of each criterion in the definition, and for those with quantitative components (eg, hospitalization rates), to generate a consensus threshold value for meeting that criterion. The final criteria were analyzed by a committee and collapsed when situations of redundancy arose. RESULTS: Of 1411 studies considered, 132 informed 55 criteria for the initial survey, which was presented to 81 respondents. Consensus for inclusion was reached on 48 criteria and for exclusion on 1 criterion. The committee collapsed those 48 criteria into 39 final criteria, 1 for diagnostic conditions, 2 for functional limitations, 13 for health care use, and 23 for family needs. CONCLUSIONS: These results represent the first consensus-based, standardized definition of CMC. Abstract

Purpose: In this 6-year study we identified factors associated with spontaneous vertebral body reshaping in glucocorticoid (GC)-treated children with leukemia, rheumatic disorders, and nephrotic syndrome. Methods: Subjects were 79 children (mean age 7.4 years) who had vertebral fracture (VF) evaluation on lateral spine radiographs at least 1 year after VF detection. VF were graded using the modified Genant semiquantitative method and fracture burden for individuals was quantified using the spinal deformity index (SDI; sum of grades from T4 to L4). Results: Sixty-five children (82.3%) underwent complete vertebral body reshaping (median time from VF detection to complete reshaping 1.3 years by Cox proportional hazard modeling). Of 237 VF, the majority (83.1%) ultimately reshaped, with 87.2% reshaping in the thoracic region vs 70.7% in the lumbar region (P = .004). Cox models showed that (1) every g/m² increase in GC exposure in the first year after VF detection was associated with a 19% decline in the probability of reshaping; (2) each unit increase in the SDI at the time of VF detection was associated with a 19% decline in the probability of reshaping [hazard ratio (HR) = 0.81; 95% confidence interval (CI) = 0.71, 0.92; P = .001]; (3) each additional VF present at the time of VF detection reduced reshaping by 25% (HR = 0.75; 95% CI = 0.62, 0.90; P = .002); and (4) each higher grade of VF severity decreased reshaping by 65% (HR = 0.35; 95% CI = 0.21, 0.57; P < .001). Conclusion: After experiencing a VF, children with higher GC exposure, higher SDI, more severe fractures, or lumbar VF were at increased risk for persistent vertebral deformity. 27. Towards a clinical definition of children with medical complexity: a Delphi process

Abstract

INTRODUCTION: Children with medical complexity (CMC) are a subpopulation of children with chronic disease who live with extreme medical and social impairment. Identification of these children remains vague and controversial but in 2011, Cohen et al. formulated a conceptual definition composed of four required domains: Family Needs, Healthcare Use, Functional Limitation, and Diagnostic Conditions. Unfortunately, there has not yet been agreement on the specific criteria to fulfill each of the four domains. A consistent definition is an important requirement for clinicians, researchers, and policy makers to describe this growing population, understand their needs, develop evidenced-based policy and research management strategies and models of care. OBJECTIVE: The first objective of this research was to search the literature for variables associated with CMC and that would theoretically fulfill one of the domains. The second objective was to convert these variables into items which had the potential to fulfill their respective Cohen et al. 2011 domain, as determined by an expert panel in a Delphi study. The overall goal was to create a standardized definitional tool for CMC. METHODS: A scoping review of the CMC literature was completed and variables that could theoretically fulfill one of the four domains were extracted. These variables were converted to items which were collated into a survey delivered to an expert panel. Researchers and clinicians with CMC expertise were gathered from a variety of professional societies to undergo a four-iteration Delphi procedure. RESULTS: From the literature, 1411 studies were considered and 132 of these informed 55 items which were presented to the expert panel. 31. Recent trends in the prevalence of overweight and obesity among Canadian children

Abstract

Background: Previous studies have shown an increase in the prevalence of overweight and obesity among Canadian children from 23.3% to 34.7% during 1978-2004. We examined the most recent trends by applying current definitions of overweight and obesity based on World Health Organization (WHO) body mass index (BMI) thresholds and recently validated norms for waist circumference and waist:height ratio. Methods: We examined directly measured height and weight data from the Canadian Community Health Survey (2004-2005) and the Canadian Health Measures Survey (20092013). We calculated z scores for BMI, height and weight based on the 2014 WHO growth charts for Canada, including the new extension of weight-for-age beyond 10 years. To calculate z scores for waist circumference and waist:height ratios, we used new charts from the reference population in the US NHANES III (National Health and Nutrition Examination Survey, 1988-1994). Results: Data were available for 14 014 children aged 3-19 years for the period 2004-2013. We observed a decline in the prevalence of overweight or obesity, from 30.7% (95% confidence interval [CI] 29.7% to 31.6%) to 27.0% (95% CI 25.3% to 28.7%) (p < 0.001) and stabilization in the prevalence of obesity at about 13%. These trends persisted after we adjusted for age, sex and race/ethnicity. Although they declined, the median z scores for BMI, weight and height were positive and higher than those in the WHO reference population. The z scores for waist circumference and waist:height ratio were negative, which indicated that the Canadian children had less central adiposity than American children in historic or contemporary NHANES cohorts. 32. Understanding healthy pregnancies: the perspective of Inuit midwives in northwestern Quebec

Abstract

Introduction: For 25 years, Inuit midwives have provided perinatal and newborn care for about 90% of the pregnancies in northwestern Quebec. Patients in this region continue to have high rates of preventable nutritional deficiencies. The objective of this study was to explore the perceptions of professional midwives and students about what makes a healthy pregnancy and a healthy newborn. Methods: We convened, via teleconference, a semistructured focus group with the local midwives and students. The conversation focused on local understanding of a healthy pregnancy and a healthy newborn, and the role of midwives in the communities. Results: Four midwives and 6 students took part in the focus group, representing 80% of local midwives and students. All of the participants were women, and their professional experience ranged from 3 to 25 years. Through inductive thematic analysis, it became apparent that personal experiences and professional training were important determinants of opinions. Midwives believed that the health of women and infants could be improved through better food selection, particularly reliance on traditional nutrient-rich food. They were aware that iron deficiency was a problem and that infants required vitamin D; however, they reported that supplement uptake was poor. Conclusion: Concern was expressed about a decline in traditional beliefs and about unhealthy behaviours. Participants advanced strategies to promote knowledge locally (e.g., visual aids, local radio) to attempt to reduce rates of nutritional deficiencies. Les patientes de cette region continuent de presenter des taux eleves de carences nutritionnelles evitables. L'objectif de cette etude etait d'explorer les perceptions des sages-femmes professionnelles et des etudiantes sur ce qu'elles considerent comme une grossesse saine et un nouveau-ne en bonne sante. Methodes : Nous avons organise un groupe de discussion semi-structure par teleconference avec des sages-femmes et des etudiantes locales. L'entrevue a porte sur leur conception d'une grossesse saine et d'un nouveau-ne en bonne sante et sur le role des sages-femmes dans les communautes. Resultats : Quatre sages-femmes et 6 etudiantes ont participe au groupe de discussion, representant 80 % des sages-femmes et etudiantes locales. Toutes les participantes etaient des femmes et leur experience professionnelle variait de 3 a 25 ans. Une analyse thematique inductive a fait ressortir que les experiences personnelles et la formation professionnelle etaient d'importants determinants des opinions formulees. Les sages-femmes se sont dites d'avis que la sante des femmes et des nouveau-nes pouvait etre amelioree par de meilleurs choix alimentaires, particulierement en ce qui concerne l'alimentation traditionnelle, riche en elements nutritifs. Elles etaient conscientes du fait qu'une carence en fer constitue un probleme et que les nourrissons ont besoin de vitamine D. Elles ont toutefois mentionne que dans les faits, les supplements sont peu utilises. Conclusion : Les participantes ont exprime leur inquietude face au declin des connaissances traditionnelles et face aux comportements malsains. 36. A founder AGL mutation causing glycogen storage disease type IIIa in Inuit identified through whole-exome sequencing: a case series

Abstract

Background: Glycogen storage disease type III is caused by mutations in both alleles of the AGL gene, which leads to reduced activity of glycogen-debranching enzyme. The clinical picture encompasses hypoglycemia, with glycogen accumulation leading to hepatomegaly and muscle involvement (skeletal and cardiac). We sought to identify the genetic cause of this disease within the Inuit community of Nunavik, in whom previous DNA sequencing had not identified such mutations. Methods: Five Inuit children with a clinical and biochemical diagnosis of glycogen storage disease type IIIa were recruited to undergo genetic testing: 2 underwent whole-exome sequencing and all 5 underwent Sanger sequencing to confirm the identified mutation. Selected DNA regions near the AGL gene were also sequenced to identify a potential founder effect in the community. In addition, control samples from 4 adults of European descent and 7 family members of the affected children were analyzed for the specific mutation by Sanger sequencing. Results: We identified a homozygous frame-shift deletion, c.4456delT, in exon 33 of the AGL gene in 2 children by whole-exome sequencing. Confirmation by Sanger sequencing showed the same mutation in all 5 patients, and 5 family members were found to be carriers. With the identification of this mutation in 5 probands, the estimated prevalence of genetically confirmed glycogen storage disease type IIIa in this region is among the highest worldwide (1:2500). Despite identical mutations, we saw variations in clinical features of the disease.
