Abstract

Background and aims: Public health concerns regarding pregnant women's health after the enactment of the Cannabis Act in Canada (CAC) (a law that allowed non‐medical cannabis use), and the potential impact of the COVID‐19 pandemic, call for a contemporary assessment of these two events. Our study measured associations between the CAC, the COVID‐19 pandemic and the monthly prevalence rates of cannabis‐, all drug‐ and alcohol‐related diagnosed disorders among pregnant women in the province of Quebec. Design, setting and participants: This was a quasi‐experimental design applying an interrupted time‐series methodology in the province of Quebec, Canada. The participants were pregnant women aged 15–49 years, between January 2010 and July 2022. Measurements: Administrative health data from the Québec Integrated Chronic Disease Surveillance System were used to classify pregnant women according to cannabis‐, all drug (excluding cannabis)‐ and alcohol‐related disorders. The CAC (October 2018) and the COVID‐19 pandemic (April 2020) were evaluated as (1) slope changes and (2) level changes. Cannabis‐, all drug (excluding cannabis)‐ and alcohol‐related disorders were measured by total monthly age‐standardized monthly prevalence rate of each disorder for pregnant women aged 15–49 years. Findings Before the CAC, the prevalence rate of cannabis‐related diagnosed disorders significantly increased each month by 0.5% [95% confidence interval (CI) = 0.3–0.6] in the pregnant population. After the CAC, there were significant increases of 24% (95% CI = 1–53) of cannabis‐related diagnosed disorders. No significant changes were observed for all drug (excluding cannabis)‐ and alcohol‐related diagnosed disorders associated with the CAC. A non‐significant decrease of 20% (95% CI = −38 to 3) was observed during the COVID‐19 pandemic in alcohol‐related disorders. Conclusions: The monthly incidence rates of diagnosed cannabis‐related disorders in pregnant women in Quebec increased significantly following the enactment of the Cannabis Act in Canada. 10. Building a learning health system for major neurocognitive disorders: the creation of regional portraits as a supporting tool

Abstract

Context: Providing context-specific information and fostering reflective practice is essential to support a learning health system. Regional health boards in Quebec, Canada, have the responsibility to ensure implementation of the Quebec Alzheimer's Plan (QAP) in primary, secondary, and tertiary care services but need support. As such, we have developed and disseminated regional portraits on dementia care. Objective: To describe the lesson learned and next steps on the development and presentations of regional portraits. Design: A multi-methods study with a participatory approach. Setting: Quebec, Canada. Population: Stakeholders (ministerial decision-maker and project managers, managers and clinicians from the 23 regional health board, and researchers/scientific advisors). Methods: We selected, with stakeholders, 9 indicators (prevalence, regular physician visits, emergency visits, and hospitalizations) and measured them in 2019-20. Also, we thematically analysed the last 3 years of ongoing QAP implementation evaluation reports and meetings. We combined these results to formulate, with stakeholders, key messages for each regional health board. Along with ministerial decision-maker and project managers, we presented these portraits to regional managers and clinicians. Real-time notes were taken using structured observation guide. A thematic analysis was performed. Results: The development of the portraits was facilitated by a strong and ongoing alliance between the stakeholders. Researchers and scientific advisors identified the most relevant data available; project managers ensured the formulated messages were meaningful to regional managers and clinicians; and policymakers ensured the collected data was useful to support the implementation of QAP. The regional managers and clinicians proposed ways to improve the portraits (i.e., adding indicators), and to regularly update and integrate these portraits into steering committees. Monitoring the indicators as precisely presents challenges, including regularly obtaining current data. Conclusion: This innovative project supports a learning health system for the care of persons with dementia. Specifically, it stimulates the emergence of regional and provincial innovations in dementia care to accomplish the implementation of the QAP, ensure its local appropriation and perpetuates sustainable transformed practices. Abstract

Aim: Persons living with dementia are a heterogeneous population with complex needs whose healthcare use varies widely. This study aimed to identify the healthcare use profiles in a cohort of persons with incident dementia, and to describe their characteristics. Methods: This is a retrospective cohort study of health administrative data in Quebec (Canada). The study population included persons who: (i) had an incident dementia diagnosis between 1 April 2015 and 31 March 2016; (ii) were aged ≥65 years and living in the community at the time of diagnosis. We carried out a latent class analysis to identify subgroups of healthcare users. The final number of groups was chosen based on clinical interpretation and statistical indicators. Results: The study cohort consisted of 15 584 individuals with incident dementia. Four profiles of healthcare users were identified: (i) Low Users (36.4%), composed of individuals with minimal healthcare use and fewer comorbidities; (ii) Ambulatory Care‐Centric Users (27.5%), mainly composed of men with the highest probability of visiting cognition specialists; (iii) High Acute Hospital Users (23.6%), comprised of individuals mainly diagnosed during hospitalization, with higher comorbidities and mortality rate; and (iv) Long‐Term Care Destined Users (12.5%), who showed the highest proportion of antipsychotics prescriptions and delayed hospitalization discharge. Conclusions: We identified four distinct subgroups of healthcare users within a population of persons living with dementia, providing a valuable context for the development of interventions tailored to specific needs within this diverse population. 22. Predictors of Physician Follow-Up Care Among Patients Affected by an Incident Mental Disorder Episode in Quebec (Canada).

Abstract

Objectives: This study identified predictors of prompt (1+ outpatient physician consultations/within 30 days), adequate (3+/90 days) and continuous (5+/365 days) follow-up care from general practitioners (GPs) or psychiatrists among patients with an incident mental disorder (MD) episode. Methods: Study data were extracted from the Quebec Integrated Chronic Disease Surveillance System (QICDSS), which covers 98% of the population eligible for health-care services under the Quebec (Canada) Health Insurance Plan. This observational epidemiological study investigating the QICDSS from 1 April 1997 to 31 March 2020, is based on a 23-year patient cohort including 12+ years old patients with an incident MD episode (n = 2,670,133). Risk ratios were calculated using Robust Poisson regressions to measure patient sociodemographic and clinical characteristics, and prior service use, which predicted patients being more or less likely to receive prompt, adequate, or continuous follow-up care after their last incident MD episode, controlling for previous MD episodes, co-occurring disorders, and years of entry into the cohort. Results: A minority of patients, and fewer over time, received physician follow-up care after an incident MD episode. Women; patients aged 18-64; with depressive or bipolar disorders, co-occurring MDs–substance-related disorders (SRDs) or physical illnesses; those receiving previous GP follow-up care, especially in family medicine groups; patients with higher prior continuity of GP care; and previous high users of emergency departments were more likely to receive follow-up care. Patients living outside the Montreal metropolitan area; those without prior MDs; patients with anxiety, attention deficit hyperactivity, personality, schizophrenia and other psychotic disorders, or SRDs were less likely to receive follow-up care. Conclusion: This study shows that vulnerable patients with complex clinical characteristics and those with better previous GP care were more likely to receive prompt, adequate or continuous follow-up care after an incident MD episode. 40. Prevalence, incidence, awareness and control of hypertension in the province of Quebec: perspective from administrative and survey data

Abstract

OBJECTIVES: Hypertension is a major risk factor for cardiovascular diseases. Nearly one adult in four was diagnosed with hypertension in 2007-2008 in Canada. One of the objectives of this study was to determine whether the prevalence of hypertension in Quebec as assessed using administrative data is comparable to that specifically measured, especially for the elderly population. METHODS: Trends in prevalence, incidence and mortality were examined using the Quebec Integrated Chronic Disease Surveillance System built from grouping numerous administrative databases from 1996-1997 to 2009-2010. Blood pressure measurements, hypertension prevalence, awareness and control were obtained in 1,706 Quebecers in the combined cycles of the Canadian Health Measures Survey. RESULTS: Using administrative databases, 23.6% [95% confidence interval, 23.5-23.6] of the Quebec population (n=1,433,400) aged [greater than or equal to]20 years was diagnosed with hypertension in 2009-2010, an increase of 32.1% compared to 2000-2001. The incidence decreased by 27.3%. Among people aged [greater than or equal to] 65 years, the prevalence rose to 69.0% [95% CI: 68.8-69.2] in women and 61.7% [95% CI: 61.5-61.9] in men. For people aged 20-79 years, the prevalence of hypertension was lower with the administrative data compared to the survey (20.2% and 23.1%, respectively). The level of awareness, treatment and control were 84.3%, 83.1% and 67.9%, respectively. CONCLUSION: The prevalence of hypertension derived from administrative data is comparable to that obtained with a health measured survey. Elderly women ([greater than or equal to]65 years) are a very high-risk subgroup. The levels of awareness, treatment and control of hypertension in Quebec are very high.
