Objective To decrease the incidence of bloodstream infection (BSI) for neonates <29 weeks gestation through quality improvement. Design Commencing in September 2011, eight neonatal intensive care units (NICUs) in New South Wales and Australian Capital Territory, Australia participated in the Sepsis Prevention in NICUs Group project, a multicentre quality improvement initiative to reduce neonatal infection through implementation of potentially better practices and development of teaching resources. Data were collected for neonates <29 weeks gestation from D3 to 35, using point of care data entry, for BSI, central line-associated BSI (CLABSI) and antibiotic use. Exponentially weighted moving average data trend lines for rates of BSI, CLABSI and antibiotic use for each NICU were automatically generated and composite charts were provided each month to participating NICUs. Results Between January 2012 and December 2014, data were collected from D3 to 35 for 1075 neonates <29 weeks gestation who survived >48 h, for a total of 33 933 bed days and 14 447 central line days. There was a significant decrease from 2012 to 2014 in BSI/1000 bed days (7.8±3.0 vs 3.8±1.1, p=0.000), CLABSI/1000 bed days (4.6±2.1 vs 2.1±0.8, p=0.003), CLABSI/1000 central line days (9.9±4.3 vs 5.4±1.7, p=0.012) and antibiotic days/100 bed days (31.1±4.3 vs 25.5±4.2, p=0.046). Conclusions This study demonstrates a >50% reduction in BSI in extremely premature neonates from D3 to 35 following a collaborative quality improvement project to reduce neonatal infection across an NICU network, supported by timely provision of data.

BACKGROUND Controversy exists whether different continuous positive airway pressure (CPAP) weaning methods influence time to wean off CPAP, CPAP duration, oxygen duration, Bronchopulmonary Dysplasia (BPD) or length of admission. AIMS In a multicentre randomised controlled trial, the authors have primarily compared CPAP weaning methods impact on time to wean off CPAP and CPAP duration and secondarily their effect on oxygen duration, BPD and time of admission. METHODS Between April 2006 and October 2009, 177 infants <30 weeks gestational age (GA) who fulfilled stability criteria on CPAP were randomised to one of the three CPAP weaning methods (M). M1: Taken 'OFF' CPAP with the view to stay 'OFF'. M2: Cycled on and off CPAP with incremental time 'OFF'. M3: As with m(2), cycled on and off CPAP but during 'OFF' periods were supported by 2 mm nasal cannula at a flow of 0.5 l/min. RESULTS Based on intention to treat analysis, there was no significant difference in mean GA or birthweight between the groups (27.1 ± 1.4, 26.9 ± 1.6 and 27.3 ± 1.5 (weeks ± 1SD) and 988 ± 247, 987 ± 249 and 1015 ± 257 (grams ± 1SD), respectively). Primary outcomes showed M1 produced a significantly shorter time to wean from CPAP (11.3 ± 0.8, 16.8 ± 1.0, 19.4 ± 1.3 (days ± 1SE) p<0.0001, respectively) and CPAP duration (24.4 ± 0.1, 38.6 ± 0.1, 30.5 ± 0.1 (days ± 1SE) p<0.0001, respectively). All the secondary outcomes were significantly shorter with M1, (oxygen duration: 24.1 ± 1.5, 45.8 ± 2.2, 34.1 ± 2.0 (days ± 1SE) p<0.0001, BPD: 7/56 (12.5%), 29/69 (42%), 10/52 (19%) p=0.011 and length of admission: 58.5 ± 0.1, 73.8 ± 0.1 69.5 ± 0.1 (days ± 1SE) p<0.0001, respectively). CONCLUSION Method 1 significantly shortens CPAP weaning time, CPAP duration, oxygen duration, BPD and admission time.

The objective of this research effort was to identify and catalog the engineering properties of commercially available, advanced composite materials with potential for use in lightweight shelter construction. A number of commercially available, potentially air-mobile, protective shelter construction materials were identified and divided into four main categories: Composites, Cores, Adhesives, and Ballistic Materials. The best materials in each category were described and a brief summary of each materials engineering properties was presented. The manufacturer of every material was identified.

The objectives of this effort were to develop fast-set, high-early strength concrete that can be pneumatically conveyed. The dry process shotcrete system was utilized to the advantage of the fast-set, high-early strength concrete system developed. Steel fibers were included to increase the toughness of the material system.

The objective of this effort was to investigate basic material properties that affect shock wave attenuation in construction materials and field test materials which show promise as external shock mitigators (ESMs). The results of this work demonstrated that very efficient ESM material systems can be produced from plastic containers embedded in low-density foam matrices. Full scale field testing was performed on such a material system using a conventional weapon. (MM)

The objective of this research effort was to assess the accuracy of using the shock response spectrum (SRS) for defining equipment shock tolerance. The focus of the research was on mission-critical equipment installed in airbase hardened structures, designed to survive the airblast and ground shock effects of a close-in detonation by a modem, nonnuclear weapon. The motivation was the prospect of improving the shock tolerance of such equipment, by first improving equipment shock tolerance characterization, thereby improving knowledge of the factors controlling equipment shock tolerance. Two simple mathematical equipment models were developed to quantify some of the errors introduced by assessing the shock tolerance of equipment via the classic, linear, SDoF shock response spectrum. The effects of waveform dependency of equipment shock response; multi- degree-of-freedom (MDoF) vs. SDoF equipment response; multidirectional loading on equipment response; and geometric nonlinearity on equipment response were investigated. . This study has proven the inadequacy of the linear, SDoF shock response spectrum for characterizing the shock tolerance of equipment. Complex, nonlinear mechanical equipment subjected to multi-directional support motion cannot be adequately represented by an SDoF SRS. The objective of this effort was to develop expedient systems to repair conventional weapon damage to mission-critical facilities at United States Air Forces in Europe (USAFE) and Pacific Air Forces (PACAF) forward operating bases (FOBs) in a postattack environment. Several of the developed expedient repair of structural facilities (ERSF) systems were demonstrated in the field to determine their effectiveness. Included in the development of each ERSF system was identification of the personnel, equipment, materials and procedures required to support it.... APUDAS stands for Automated Postattack Utilities Damage Assessment System. Currently, there is not automated, real time, consolidated monitoring system to accomplish a timely comprehensive, postattack status assessment of overseas air base utilities. These utilities include the electrical distribution, potable water, POL, sewerage, and mission-critical HVAC systems. Neither is there a formal Air Force requirement for an APUDAS, although the need for it was clearly demonstrated in the Airbase Operability exercise, SALTY DEMO, at Spangdahlem AF, FRG in 1985. An APUDAS consists of sensors distributed throughout a utility network, plus a data communication and processing system. The sensors measure whatever quantities are necessary to determine the utility's operational status. Each sensor signal is fed into a nearby small radio transceiver, which in turn transmits the sensor signal to a central base radio. The base radio feeds all the sensor signals into a central computer, which processes the information for display in such a fashion that it permits a trained observer to quickly determine the utility's operational status, plus locate and assess damage.

Measurements of the nonlinear viscoelastic behavior of two adhesives, FM-73 and FM-300, are presented and discussed. Analytical methods to quantify the measurements are given and fitted into a framework of an accelerated testing and analysis procedure. The single integral model used is shown to function well and is analogous to a time-temperature stress-superposition procedure (TTSSP). Advantages and disadvantages of the method accessed with the latter residing primarily with the nature of the creep power law which is utilized.
