29. Quality assessment of fetal death records in Georgia: a method for improvement

Abstract

Objectives. Although more fetal deaths than neonatal deaths occur, routinely collected fetal death data are seldom used for perinatal epidemiologic research because of data quality concerns. We developed a strategy for identifying and correcting errors in birthweight and gestational age in fetal death records. Methods. Using data from Georgia for 1989 and 1990, we detected singleton fetal death records having improbable or missing birthweight or gestational age by comparing these values with referent values. To verify the questionable values, we contacted 100 reporting hospitals in 1992. Results. In 817 of 2226 records, values were either improbable (60.1%) or missing (39.9%). We were able to contact the hospitals to verify data for 716 (88%) of these records. Verification resulted in corrections to 405 (57%) records, and 48% of unreported birthweights were obtained. Conclusions. Many errors in recorded gestational age and birthweight were identified by this method. Rather than deleting or imputing problem data for analyses, researchers should consider efforts to verify them. Efforts to improve this information should include improved reporting, strict quality assurance, and procedures for routine verification and correction of records. 33. Pregnancy-related mortality in New York City, 1980 to 1984: causes of death and associated risk factors

Abstract

Although the number of pregnancy-related deaths per 100,000 live births (the pregnancy-related mortality ratio) declined between 1915 and 1984, the rate of decline slowed appreciably after 1980. The US pregnancy-related mortality ratio is estimated to be between 10.8 and 16.5 deaths per 100,000 live births. Minority women are known to be at increased risk for death related to pregnancy when compared with whites. Women over 30 years old also are at higher risk. To learn more concerning the causes of pregnancy-related death in New York City between 1980 and 1984, a study of vital statistics was carried out. Deaths were classified as directly (from a condition of pregnancy or delivery) or indirectly (when pregnancy exacerbated an existing medical condition) related to pregnancy, or were due to other causes (homicide, drug overdose). Risk factors such as age, marital status, education, previous live births, prenatal care, and ability to pay for health services were evaluated. Results showed that the overall pregnancy-related mortality ratio for the years studied was 40.2 deaths per 100,000 live births. Almost three-quarters of these deaths were directly related to pregnancy. The most frequent causes included ectopic pregnancy (pregnancy in a site other than the uterus); pulmonary emboli (blood clots that block blood circulation to the lungs); high blood pressure; and cardiac arrest. Common indirect causes included nonobstetric infections and complications of cardiovascular conditions. Women who were between 20 and 24, non-Hispanic white, married, educated beyond high school, privately insured, having their first child, and having more than five prenatal visits, had the lowest risk of pregnancy-related death. Those who were 40 or older, black, unmarried, educated between 9 and 11 years, uninsured even by Medicaid, had five or more previous live births, and little or no prenatal care, were at highest risk. It appears that women in New York City suffer a large number of pregnancy-related deaths that could be prevented by improved access to medical care. Abstract

Dr. Roger Rochat completed his residency at Charity Hospital in New Orleans. He joined CDC’s Epidemic Intelligence Service and was assigned to the Family Planning Evaluation Activity and the Georgia Health Department. One of his first tasks was to determine whether the new Georgia state law legalizing abortion would reduce maternal mortality. Because of restrictive access to services, it would not. A lawyer cited this social inequity in Doe v. Bolton, the Supreme Court case from Georgia that was decided concurrently with Roe v. Wade. In 1981, Dr. Rochat became the first Director of the Division of Reproductive Health for the CDC. In 1985, he developed the Global Health Track in Community Health at Emory, then worked 2 years for USAID in India, and then spent six years developing Maternal and Child Health epidemiology in the Georgia Health Department. After 30 years with CDC, working in 30+ countries and with many State Health Departments, he retired from CDC in 1999, went with his brother briefly to Antarctica, and then joined the Rollins School of Public Health where he has appointments in Departments of Global Health and Epidemiology and in Emory Medical school’s Department of Gynecology and Obstetrics. He serves as Director of Graduate Studies, Hubert Department of Global Health and co-teaches four public health courses related to reproductive health, including The GEMMA Seminar: The Global Elimination of Maternal Mortality from Abortion.  Roger and Susan Rochat have endowed the GEMMA fund to support Emory MPH students conducting international research aimed at eliminating maternal deaths from abortion., Roger Rochat discusses his life's work involving maternal mortality, abortion, family planning and reproductive health. The interview touches upon his time at CDC, USAID and his research in demography abroad as well as in the United States.
