Abstract

Background: The highly plastic nature of adrenal cortex suggests the presence of adrenocortical stem cells (ACSC), but the exact in vivo identity of ACSC remains elusive. A few studies have demonstrated the differentiation of adipose or bone marrow-derived mesenchymal stem cells (MSC) into steroid-producing cells. We therefore investigated the isolation of multipotent MSC from human adrenal cortex. Methods: Human adrenals were obtained as discarded surgical material. Single-cell suspensions from human adrenal cortex (n = 3) were cultured onto either complete growth medium (CM) or MSC growth promotion medium (MGPM) in hypoxic condition. Following ex vivo expansion, their multilineage differentiation capacity was evaluated. Phenotype markers were analysed by immunocytochemistry and flow cytometry for cell-surface antigens associated with bone marrow MSCs and adrenocortical-specific phenotype. Expression of mRNAs for pluripotency markers was assessed by q-PCR. Results: The formation of colony-forming unit fibroblasts comprising adherent cells with fibroblast-like morphology were observed from the monolayer cell culture, in both CM and MGPM. Cells derived from MGPM revealed differentiation towards osteogenic and adipogenic cell lineages. These cells expressed cell-surface MSC markers (CD44, CD90, CD105 and CD166) but did not express the haematopoietic, lymphocytic or HLA-DR markers. Flow cytometry demonstrated significantly higher expression of GLI1 in cell population harvested from MGPM, which were highly proliferative. They also exhibited increased expression of the pluripotency markers. Conclusion: Our study demonstrates that human adrenal cortex harbours a mesenchymal stem cell-like population. Understanding the cell biology of adrenal cortex- derived MSCs will inform regenerative medicine approaches in autoimmune Addison’s disease.

15. Erectile dysfunction after robotic radical prostatectomy: Real-life impact of vacuum erection device clinic.

Abstract

Introduction: Post-radical prostatectomy erectile dysfunction (post-RPED) is a common and potentially devastating complication. The role of PDE5-inhibitors (PDE5-Is) in post-RPED is controversial and invasive pharmacological treatments are associated with poor long-term compliance. Vacuum erection devices (VEDs) are a non-pharmacological alternative. Little data regarding VED efficacy and associated patient satisfaction in post-RPED exists. This study aimed to investigate the outcomes of VED therapy in a post-robot assisted radical prostatectomy (post-RARP) population. Methods: All men who underwent RARP at a single centre between February 2015 and October 2017 attended a nurse-led holistic-needs-assessment (HNA) appointment at 10 weeks post-RARP. All men identifying ED as a concern at HNA were offered dedicated nurse-led post-RPED clinic and VED clinic appointments. A cross-sectional survey of patient reported outcomes in these men was performed. Results: In total 137/539 (25%) men reported ED as a concern at HNA. All 137 men attended the VED clinic, 124/137 (90%) responded to the survey. Commonest reason for choosing VED therapy was for combined psychological, penetration and lengthening purposes (48%). Median time from RARP to VED clinic and from VED clinic to survey was 122 days (range 56–595) and 462 days (range 66–932) respectively. At survey 88/124(70.9%) reported continued use of VED, 69/88 (78%) of this group perceived VED treatment to be successful. Of those who had stopped using VED, 7/36 (19%) felt it had been successful. Key reasons for discontinuation were either because men felt it ineffective (13/36 (37%)) or disliked the device (10/36 (29%)). Conclusion: VED therapy is an effective treatment for men with post-RARP ED that is well tolerated with good short- to medium-term compliance. Up to two-thirds of men may find overall satisfaction with their post-RPED after VED therapy. Level of Evidence: 3 [ABSTRACT FROM AUTHOR]

Abstract

This study aims to enhance the understanding of destination management system (DMS) effectiveness and its evaluation. Upon review of the literature, it was established that DMS effectiveness and its evaluation has not yet been researched adequately. Informed by an interpretive approach, this study contributes to research particularly by investigating what constitutes effectiveness and explores the aspects (factors and relationships) that need to be considered in a holistic DMS effectiveness evaluation. Based on a qualitative case study strategy, this study adopts a comprehensive approach that considers multiple stakeholder groups� perspectives. The research evidence is collected through a case-study of the Egyptian DMS experience (the Touregypt project). The Touregypt project gives insights to the understanding of three DMS cases that have not been researched before in DMS literature: first, an actual DMS application in a developing country; second, a public and private sector partnership experience; and third, a failed DMS experience (the system has failed in the course of this research).Prompted by the interpretive approach, this study tried to explore DMS effectiveness based on the perspectives, attitudes and experiences of the multiple stakeholder groups (Hesse-Biber and Leavy 2010). Accordingly, the empirical data was collected through a multi-method approach that includes interviews, observation, archival document analysis (including Touregypt forum analysis), and website analysis. Data has been analyzed guided by discourse analysis, complemented with the general inductive approach of Miles and Huberman (1994). Following an interpretive theory-building strategy, the analyzed data has been further interpreted in the light of prior theories of DMS and information systems research, particularly the Delone and MacLean IS effectiveness theory (1992, 2003, and 2004).The main contribution of this study to knowledge is a theory based model that enhances the understanding of DMS effectiveness evaluation. The suggested model identifies the aspects (factors and relationships) that need to be considered when evaluating the effectiveness of DMS. Also, the results of this study give insights to the understanding of DMS effectiveness by shedding light on what constitutes effectiveness and the possible relationship between such constructs.

Abstract

This thesis addresses the problem of human and social issues that affect software development. It is situated within the field of Information Systems and focuses on the processes of software production used within complex organisational processes: particularly decision-making, collaboration and workflow. Human and social issues are problems and situations caused by the members of the target organisations, software developers, and the dynamics of their working and social interactions. The objective of this thesis is to identify these human and social issues and see how they affect software developers' work. The methodological approach adopted here, is designed from the interpretive point of view. This study takes the perspective of software developers as they possess practical knowledge of complex business settings and current software development practices. Online ethnography is the chosen method that allows this investigation access to virtual communities in which software developers work and exchange experiences. The design of this thesis is as follows: 1. Online data is collected which reflect software developers' beliefs about their work and their target organisations. 2. Data categories are created which show a picture of the current state of affairs in software development. 3. An interpretive theory building strategy is used to create a model of software development based on data categories. The final outcome of this thesis is developed as a complement to Orlikowski's (2000) structurational model of enactment of technologies-in-practice and takes the form of a descriptive, theory based model. This model contributes to the deeper understanding of software development issues. It presents human and social issues that affect the production of software within three different contexts: software development environment, software development practices and complex business organisations. The model generated in this thesis also suggests that software developers' perceptions of human and social issues in complex business organisations are influenced by the developers' background knowledge and experience.
