8. Modelo cadavérico para entrenamiento de punción percutánea para procedimiento endoscópico renal

Abstract

Introducción La obtención de un acceso renal percutáneo sigue siendo un desafío para el urólogo. Los modelos de entrenamiento permiten reducir la curva de aprendizaje, tiempo quirúrgico y complicaciones. El objetivo de este estudio es describir un modelo de entrenamiento cadavérico para acceso renal percutáneo. Material y Métodos Se desarrolló un modelo cadavérico de entrenamiento para acceso renal percutáneo guiado por fluoroscopía. Las punciones se realizaron mediante técnica de triangulación en decúbito supino. Se evaluó a 4 residentes de urología comparando las punciones iniciales asesorados por un experto y punciones de prueba realizadas posteriormente sin un asesor. Se evaluó el porcentaje de éxito, el número de intentos por punción y el tiempo de radiación. Resultados Se utilizaron 10 riñones de 6 modelos cadavéricos. El total de punciones realizadas fue de 119, siendo 73 (61.4%) punciones iniciales y 46 (38.6%) de prueba, con una media de 12 ± 9.92 punciones por riñón. La tasa de éxito por punción fue del 50.4% (60/119), el tiempo medio de radiación fue de 1.3 ± 0.8 min. El éxito de punciones iniciales fue del 41% (30/73) y del 65.2% (30/46) en las punciones prueba (p<0.01). El tiempo de fluoroscopía fue de 1.49 ± 0.95 min y 1.07 ± 0.52 min (p<0.01), respectivamente, siendo significativamente mayor en los primeros. Conclusiones El modelo de entrenamiento propuesto es una herramienta útil y reproducible para el acceso renal en procedimientos percutáneos para el urólogo en formación. La exposición a la radiación entre practicantes es una desventaja que debe tenerse en consideración.

Abstract

Introduction: Xanthogranulomatous pyelonephritis is an inflammatory disease characterized by chronic obstruction and infection. This pathology is a life-threatening condition when surgical treatment is carried out. We decided to retrospectively evaluate whether there were perioperative factors that predict complications in patients who undergo nephrectomy. Methods: We reviewed all nephrectomies done in the period of 2013–2018, in a tertiary referral Hospital with the histopathological diagnosis of Xanthogranulomatous Pyelonephritis. Results: The presence of renal abscess at admission was observed as a risk factor associated with perioperative complications (p = 0.002), presence of abscess was observed in 47.4% of subjects without complications compared to 89.3% of the perioperative complication group. Higher rates of blood transfusion requirement were observed in the perioperative complication group, 89.3% compared to 68.4% (p = 0.029), furthermore, perioperative bleeding was slightly greater in the complication group compared to its counterpart, 700 mL, and 600 mL, respectively (p = 0.01). Conclusions: Anemia and the presence of abscess were important perioperative factors that predict perioperative complications. [ABSTRACT FROM AUTHOR]

14. Laparoscopic vs Open Extravesical Ureteral Reimplantation in Pediatric Population: A Single-Center Experience.

Abstract

Purpose: To evaluate the safety and efficacy of conventional laparoscopic vs open Lich-Gregoir ureteral reimplantation in pediatric vesicoureteral reflux. Material and Methods: A retrospective study was conducted in a tertiary care hospital. Patients with vesicoureteral reflux who underwent open or laparoscopic Lich-Gregoir ureteral reimplantation from 2013-2020 were included. The primary outcome was the resolution of reflux. Complications and perioperative characteristics were evaluated. The outcomes between open and laparoscopic surgery were analyzed. Results: A total of 110 patients and 150 ureters were included. The mean age was 4.5 years ± 3.4 and 73.6% were females. A total of 125 ureters (83.3%) underwent laparoscopic and 25 (16.6%) open Lich-Gregoir vesicoureteral reimplantation (5:1 Ratio). Resolution was reported in 112 (89.6%) for laparoscopy and 21 (84%) for open surgery (P = .42). Mean surgical time for laparoscopy and open surgery were 142.4 min ± 64.4 and 153 min ± 40, respectively (P =.29). Mean bleeding (9.5 mL ± 11.2 vs 29.6 mL ± 22.8) and length of hospital stay (2.4 days ± 2.3 vs 5.05 ± 3.1) were significantly higher with open surgery (P < .001). No significant difference in complications was reported between open surgery (32%) and laparoscopic approach (22.4%) (P = .305). Conclusion: Conventional laparoscopic vesicoureteral reimplantation with the Lich-Gregoir technique has an acceptable success rate comparable with open surgery, with shorter hospital stay, less bleeding, and less need of transfusion. Abstract

Background: Emphysematous pyelonephritis (EPN) is a necrotizing infection of the kidney and the surrounding tissues associated with considerable mortality. We aimed to formulate a score that classifies the risk of mortality in patients with EPN at hospital admission., Materials and Methods: Patients diagnosed with EPN between 2013 and 2020 were retrospectively included. Data from 15 centers (70%) were used to develop the scoring system, and data from 7 centers (30%) were used to validate it. Univariable and multivariable logistic regression analyses were performed to identify independent factors related to mortality. Receiver operating characteristic curve analysis was performed to construct the scoring system and calculate the risk of mortality. A standardized regression coefficient was used to quantify the discriminating power of each factor to convert the individual coefficients into points. The area under the curve was used to quantify the scoring system performance. An 8-point scoring system for the mortality risk was created (range, 0-7)., Results: In total, 570 patients were included (400 in the test group and 170 in the validation group). Independent predictors of mortality in the multivariable logistic regression were included in the scoring system: quick Sepsis-related Organ Failure Assessment score ≥2 (2 points), anemia, paranephric gas extension, leukocyte count >22,000/μL, thrombocytopenia, and hyperglycemia (1 point each). The mortality rate was <5% for scores ≤3, 83.3% for scores 6, and 100% for scores 7.
