Deformation in the Cerro Prieto pull apart basin (CP) located between Cerro Prieto and Imperial faults on the Mexican part of San Andreas System was studied for years. Leveling was realized in the basin by different agencies since 60’s, DInSAR analisis and GPS observations were realized many times since 1997. Since the subsidence caused by geothermal water extraction in the Cerro Prieto Geothermal Field affected the faults in the basin subsequently geotechnical instruments were installed for continuous monitoring.The creep and episodic slip were observed on the basin faults before the 2010 earthquake. When the M7.2 EMC strikes all the geotechnical instruments, installed on the CP faults, recorded displacements triggered by the earthquake. However, the post-seismic behavior of the records varies, and 3 typical behaviors can be distinguished: 1) the creeping has similar magnitude before and after the earthquake, 2) the deformation rate after the earthquake increases and 3) the cessation of creep on the fault for up to 6 years after which the creep reactivated with a magnitude similar to that before the earthquake.Applying an empirical relationship between magnitude and stress, the accelerometer data and Coulomb 3 modeling, we found that the dynamic stress is responsible for the triggering.For post-seismic behavior, the Coulomb static stress changes caused by the EMC earthquake, by tectonic stresses and by fluid extraction in the Cerro Prieto geothermal field are analyzed. RESUMEN Las cirugías de cabeza y cuello presentan un alto riesgo de sangrado que puede significar el uso de transfusiones sanguíneas. Existen casos en las que éstas son rechazadas, como sucede con los Testigos de Jehová. Se expone el caso de una paciente Testigo de Jehová con un tumor rinosinusal con alto riesgo de sangrado que consultó por epistaxis recurrente. Se evidencia un tumor ocluyendo la fosa nasal derecha de aspecto vascular a la rinoscopía y la tomografía computarizada. Múltiples aferencias de la arteria esfenopalatina y etmoidales se observaron en una angiografía cerebral. Previo a la resección, se embolizó la arteria maxilar. Durante la cirugía, se contó con un sistema de recuperación de sangre autóloga, hemodilución e infusión de ácido tranexámico. Se ligó la arteria etmoidal anterior derecha vía externa con apoyo endoscópico y luego se resecó el tumor vía endoscópica. La biopsia reveló un carcinoma sinonasal escamoso. Existen alternativas terapéuticas en pacientes que rechacen el uso de hemoderivados. Destacan medidas preoperatorias como la embolización endovascular, intraoperatorias como el uso de agentes hemostáticos, técnicas quirúrgicas y anestésicas. Es importante analizar todas las opciones disponibles de forma multidisciplinara y junto con el paciente, para determinar la conducta más adecuada a seguir.

ABSTRACT Head and neck surgeries have a high risk of bleeding, and therefore could require the use of blood transfusions. There are cases for which blood transfusions are not an acceptable option, as is the case for Jehovah's Witnesses. We present the case of a Jehovah's Witness with a sinonasal tumor with a high risk of bleeding, who presented with recurrent epistaxis. Rhinoscopy and computed tomography revealed a vascular-like tumor occluding the right nasal cavity. Cerebral angiography showed afferents of the sphenopalatine and ethmoidal arteries leading to the tumor. Prior to the resection, the maxillary artery was embolized. During surgery, we relied on an autologous blood recovery system, hemodilution and tranexamic acid. Right anterior ethmoidal artery ligation was performed by an endoscopic assisted external approach. The tumor was resected endoscopically The biopsy revealed a squamous sinonasal carcinoma. There are therapeutic alternatives for patients who cannot receive blood products. There are preoperative measures such as endovascular embolization, intraoperative measures such as the use of hemostatic agents and specific surgical or anesthetic techniques. It's important to analyze all of the available options in a multidisciplinary team approach, and to take into consideration the patient's preferences, in order to determine the best surgical conduct.

Histological analysis for cancer detection or stratification is performed by observing and examining a small portion of a biopsied tissue under a microscope. Nevertheless, to assign clinical meaning to the findings, the analysis and interpretation of an experienced Pathologist is always necessary. Using high-resolution images, these experts visually examine the sample looking for specific characteristics on the cell shapes and tissue distributions, so they could decide whether tissue regions are cancerous, and establish the malignancy level of it. However, with the increasing demand for work for those pathologists and the importance of accuracy on diagnostics, multiple attempts to simplify their work have been performed. Current Imaging technologies allow novel horizons in the automatized selection of some of the characteristics that indicate malignancy in a biopsy. In this work, we propose a simple computer vision algorithm that can be implemented as a screening method for focusing in histological areas with higher risk of malignancy saving time to the pathologist and helping to perform a more standardized work, an easy observation with the potential to become in an aid to daily clinical work.

Palliative care must be early applied to all types of advanced chronic and life limited prognosis patients, present in all health and social services. Patients' early identification and registry allows introducing palliative care gradually concomitant with other measures. Patients undergo a systematic and integrated care process, meant to improve their life quality, which includes multidimensional assessment of their needs, recognition of their values and preferences for advance care planning purposes, treatments review, family care, and case management. Leaded by the National Department of Health, a program for the early identification of these patients has been implemented in Catalonia (Spain). Although the overall benefits expected, the program has raised some ethical issues. In order to address these challenges, diverse institutions, including bioethics and ethics committees, have elaborated a proposal for the program's advantages. This paper describes the process of evaluation, elaboration of recommendations, and actions done in Catalonia.

OBJECTIVE: The IDEAL Schedule was developed for staging "care needs" in patients with dementia. We here aim to validate the Spanish version, further test its psychometric properties and explore a latent construct for "care needs". METHODS: A multicenter study was done in 8 dementia care facilities across Spain. Patients referred with a reliable ICD-10 diagnosis of dementia (n = 151) were assessed with the IDEAL Schedule by pairs of raters. Inter-rater reliability (intra-class correlation [ICC] coefficients), internal consistency (Cronbach's alpha), and factor analysis were calculated. Convergent validity for individual items was tested against validated Spanish versions of international instruments. RESULTS: Pilot testing with numerical scales supported the feasibility, face, and content validity of the schedule. The psychometric coefficients were good/clinically acceptable: inter-rater reliability (mean ICC = 0.861; 85% of the ICCs > 0.8), internal consistency (global alpha coefficient = 0.74 in 5 nuclear items), and concurrent validity (global score against the Clinical Dementia Rating schedule, r = 0.63; coefficients for individual items ranging from 0.40 to 0.84, all statistically significant, p

OBJECTIVE: To identify similarities and differences in empathy, abilities toward inter-professional collaboration, and lifelong medical learning, between Spanish and Latin-American physicians-in-training who start their posgraduate training in teaching hospitals in Spain. DESIGN: Observational study using self-administered questionnaires. SETTINGS: Five teaching hospitals in the province of Barcelona, Spain. PARTICIPANTS: Spanish and Latin-American physicians-in-training who started their first year of post-graduate medical training. MAIN MEASUREMENTS: Empathy was measured using the Jefferson scale of empathy. Abilities for inter-professional collaboration were measured using the Jefferson scale attitudes towards nurse-physician collaboration. Objective: To analyze the impact of hospital management measures to reduce the number of patients held in an emergency department while awaiting admission. Methods: Type of study: retrospective, comparing 2 periods, November 2006 to October 2007 and November 2007 to October 2008. Setting: Hospital de Sant Boi, a 126-bed general hospital in Sant Boi de Llobregat in the province of Barcelona. Management measures: decreased presurgical stay, increased use of ambulatory surgical procedures, and increased use of a medical short-stay unit. Variables studied: emergency department admissions, hospital ward admissions, admissions for ambulatory surgical procedures, number of days in which at least 1 patient was in the emergency department at 8 A.M. while waiting for a bed, mean hospital stay, and admissions and average time spent in the short-stay unit. Results: A total of 57140 patients were admitted to the emergency department in the first period and 71280 in the second period; 4840 (8.4%) were admitted to hospital in the first period and 5385 (7.5%) in the second. The industrial subproducts filter cake press and botton fly ash were used in mixtures (3:1) like sustratum for the tomato Lycopersicon esculentum Mill (Celebrity HF1) production. The trial constituted six treatments each one repeated three times. Half of treatments were sowed in the mixture and the other half in the soil, using three fertilization levels: 0, 300 and 600 kg/ha 18-18*-18. There were productions of 140, 158 and 178 ton/ha for the three fertilization levels using the filter press cake substate and 140, 143 and 160 t/ha tor the three fertilization levels in the soil. Such results are showing the excellent quality of the mixture filter cake press: coal dust (3:1) because of the similar results, which were obtained in one soil with excellent physics and chemical properties.Los subproductos industriales cachaza y carbonilla se utilizaron en mezcla (3:1) como sustrato para producción de tomate Lycopersicon esculentum Mili, Celebrity HF1. El experimento se diseñó en bloques completos al azar con arreglo factorial; constó de seis tratamientos y tres repeticiones cada uno, la mitad de los tratamientos se sembró en la mezcla y la otra mitad en suelo, utilizándose tres niveles de fertilización: 0, 300 y 600 kg/Ha 18-18-18. Se obtuvieron producciones de 140, 158 y 178 ton/ha de tomate para los tres niveles de fertilización en el sustrato cachaza y 140, 143 y 160 para los tres niveles de fertilización en el suelo, lo que demuestra la excelente calidad de la mezcla cachaza: carbonilla (3:1), al obtenerse resultados similares al suelo de excelentes propiedades físicas y químicas.
