Abstract

Background: Medial Unicompartmental Knee Replacement (UKR) has well-documented benefits over Total Knee Replacement in the treatment of anteromedial osteoarthritis of the knee. There has been an increasing move from cemented to cementless UKR over the last decade. This non-design centre study assesses the initial experience using the cementless Oxford medial partial knee replacement and provides medium term revision data, as well as Patient Reported Outcome Measures (PROMs). Methods: A cohort of 200 consecutive patients undergoing medial UKR using the cementless Oxford were identified from our knee groups prospectively collected database. Cases were performed in a single centre under the care of one of four surgeons. All patients were beyond the 5-year minimum timepoint following UKR surgery in order to produce medium term results, at a mean of 7.9 years. Eligible patients completed a postal questionnaire to collect PROMs: Oxford Knee Score, WOMAC and modified American Knee Society Score questionnaires in January 2020 and had their clinical records reviewed. Results: The survivorship in our cohort was 94.5% at a mean follow up of 7.9 years following surgery. There were 11 re-operations in total with a three percent risk of re-operation within the first 18 months following surgery. There was a sustained improvement in Oxford Knee Score with a near 20 points improvement on pre-operative scores. Conclusions: Our results provide further evidence that partial knee replacements using the cementless Oxford produce good clinical outcomes. Abstract

Iatrogenic perforation of the gallbladder during laparoscopic cholecystectomy is a well-known occurrence; however, the consequences of spillage of gallstones in the peritoneum and particularly intrathoracic complications are less defined. We describe the delayed development of a perihepatic abscess and empyema in a patient five years following laparoscopic cholecystectomy secondary to dropped gallstones. A 53-year-old man with medical history significant for a laparoscopic cholecystectomy five years prior to acute cholecystitis presented with purulent cough, hemoptysis, night sweats, and right-upper quadrant (RUQ) pain. Computed tomography (CT) scan revealed 5.4 cm right-sided subpulmonic and 5.9 cm perihepatic fluid collections with an 8 mm focal radiopaque density within the perihepatic fluid collection. Open intra-abdominal exploration resulted in retrieval of a 1 cm intraperitoneal gallstone. Laparoscopic cholecystectomy is a common surgical operation during which gallstone spillage can occur, causing both intra-abdominal and intrathoracic complications, presenting even years after surgery. This necessitates an attempt to retrieve all free intra-abdominal gallstones during the initial operation.

Abstract

New diagnostic, modelling and plant capability on the Mega Ampere Spherical Tokamak (MAST) have delivered important results in key areas for ITER/DEMO and the upcoming MAST Upgrade, a step towards future ST devices on the path to fusion currently under procurement. Micro-stability analysis of the pedestal highlights the potential roles of micro-tearing modes and kinetic ballooning modes for the pedestal formation. Mitigation of edge localized modes (ELM) using resonant magnetic perturbation has been demonstrated for toroidal mode numbers n = 3, 4, 6 with an ELM frequency increase by up to a factor of 9, compatible with pellet fuelling. The peak heat flux of mitigated and natural ELMs follows the same linear trend with ELM energy loss and the first ELM-resolved T-i measurements in the divertor region are shown. Measurements of flow shear and turbulence dynamics during L-H transitions show filaments erupting from the plasma edge whilst the full flow shear is still present. Off-axis neutral beam injection helps to strongly reduce the redistribution of fast-ions due to fishbone modes when compared to on-axis injection. Low-k ion-scale turbulence has been measured in L-mode and compared to global gyro-kinetic simulations. A statistical analysis of principal turbulence time scales shows them to be of comparable magnitude and reasonably correlated with turbulence decorrelation time. T-e inside the island of a neoclassical tearing mode allow the analysis of the island evolution without assuming specific models for the heat flux. Other results include the discrepancy of the current profile evolution during the current ramp-up with solutions of the poloidal field diffusion equation, studies of the anomalous Doppler resonance compressional Alfven eigenmodes, disruption mitigation studies and modelling of the new divertor design for MAST Upgrade. The novel 3D electron Bernstein synthetic imaging shows promising first data sensitive to the edge current profile and flows.

Abstract

Attempt to develop a gauge-invariant theory of the motion of singularities in an n-dimensional Riemannian space. A gauge-invariant theory is developed for a nongeodetic particle represented by a time-like world line in the linearized field theory. The solution of the basic algebraic and differential equations is to be constructed from the source line, the future null cones emanating from it, a scale factor, and nothing else, and the leading term of the solution is the Schwarzschild field. A class of possible solutions is examined, and it is shown that this class contains just one acceptable member - namely, a particle in constant acceleration.

Abstract

Currently, Base Operations (BASOPS) funding is grossly inadequate-providing insufficient resources to support installation quality of life (QOL) programs and services. This discrepancy is destroying our installation's infrastructure and having an adverse impact on QOL and ultimately readiness. Recently, some senior military leaders have addressed Congress on readiness issues and priorities, specifically inadequate QOL. They must enter round two with a strategic plan that includes BASOPS as part of the readiness equation. If this is ignored, our installations' infrastructure will continue to deteriorate to a point beyond recovery, and our support programs and services will not meet the critical needs of our soldiers and their families.
