4. Enrollment Management Strategies as a Result of COVID-19 at Rural Community Colleges

Abstract

Higher education institutions world-wide were impacted by the unprecedented novel coronavirus (COVID-19) beginning in early 2020. COVID-19 caused a disruption in services to students and resulted in pivots of teaching, learning, and student support. Community colleges differ from four-year counterparts through varying student demographics, funding sources, mission and foci, and student intentions. Community college enrollment is affected by economic, employment, and social trends. Enrollment management practices changes as campus operations for student support changed to remote support. COVID-19 forced administrators at colleges to make quick decisions. This study examined the perceptions of academic administrators at rural community colleges regarding how COVID-19 impacted enrollment management practices. This study aimed to determine the perceptions of changes occurring to enrollment management practices and the subsequent financial challenges resulting from COVID-19 within rural Virginia community colleges. Each rural community college in Virginia is included within the Rural Virginia Horseshoe, totaling 14 colleges. This study was a sequential explanatory study that was conducted in two phases. Phase One was a quantitative inquiry using a non-experimental survey to gather mid to senior-level administrators' perceptions of how COVID-19 was impacting enrollment management practices at their college. A total of 45 respondents completed the survey. The distribution included 102 mid to senior-level administrators. For the qualitative inquiry, the multiple case study research tradition was utilized. A total of 10 interviews were conducted with mid to senior-level administrators. Five themes emerged from the findings: (a) COVID-19 led to crisis management and operations in phases, (b) managing student onboarding during COVID-19, (c) COVID-19 created unique challenges for community college students, (d) COVID-19 affected decision-making procedures, and (e) COVID-19 resulted in work/life balance issues and COVID fatigue. Major implications in the current study suggest that colleges should be ready to pivot to remote instruction or back from it, review the onboarding processes and supports to ensure that they are adequately serving students, and advocate to reduce the digital divide. Abstract

In 2023, the European Conference on Infections in Leukaemia elaborated guidelines for managing toxoplasmosis in hematology patients, mainly after allogeneic hematopoietic cell transplantation. Toxoplasma disease, although a rare event, has a high mortality, especially in case of cerebral involvement or disseminated form, and may be of very acute presentation. These guidelines highlight the need for a preemptive strategy based on a weekly screening by quantitative PCR from transplantation in seropositive recipients, allowing to early treat reactivation before the occurrence of toxoplasma disease. The purpose of this article is not to summarize these guidelines but rather to discuss critical issues which have been largely debated during the elaboration of these guidelines and which support the conclusions of the group. Abstract

Introduction: COVID-19 has been associated with high morbidity and mortality in allogeneic hematopoietic stem cell transplant (allo-HCT) recipients. Methods: This study reports on 986 patients reported to the EBMT registry during the first 29 months of the pandemic. Results: The median age was 50.3 years (min - max; 1.0 - 80.7). The median time from most recent HCT to diagnosis of COVID-19 was 20 months (min - max; 0.0 - 383.9). The median time was 19.3 (0.0 - 287.6) months during 2020, 21.2 (0.1 - 324.5) months during 2021, and 19.7 (0.1 - 383.9) months during 2022 (p = NS). 145/986 (14.7%) patients died; 124 (12.6%) due to COVID-19 and 21 of other causes. Only 2/204 (1%) fully vaccinated patients died from COVID-19. There was a successive improvement in overall survival over time. In multivariate analysis, increasing age (p<.0001), worse performance status (p<.0001), contracting COVID-19 within the first 30 days (p<.0001) or 30 - 100 days after HCT (p=.003), ongoing immunosuppression (p=.004), pre-existing lung disease (p=.003), and recipient CMV seropositivity (p=.004) had negative impact on overall survival while patients contracting COVID-19 in 2020 (p<.0001) or 2021 (p=.027) had worse overall survival than patients with COVID-19 diagnosed in 2022. Discussion: Although the outcome of COVID-19 has improved, patients having risk factors were still at risk for severe COVID-19 including death.

Abstract

Background Empiric antifungal therapy is considered the standard of care for high-risk neutropenic patients with persistent fever. The impact of a preemptive, diagnostic-driven approach based on galactomannan screening and chest computed tomography scan on demand on survival and on the risk of invasive fungal disease (IFD) during the first weeks of high-risk neutropenia is unknown. Methods Patients with acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) and allogeneic hematopoietic cell transplant recipients were randomly assigned to receive caspofungin empirically (arm A) or preemptively (arm B), while receiving fluconazole 400 mg daily prophylactically. The primary end point of this noninferiority study was overall survival (OS) 42 days after randomization. Results Of 556 patients recruited, 549 were eligible: 275 in arm A and 274 in arm B. Eighty percent of the patients had AML or MDS requiring high-dose chemotherapy, and 93% of them were in the first induction phase. At day 42, the OS was not inferior in arm B (96.7%; 95% confidence interval [CI], 93.8%–98.3%) when compared with arm A (93.1%; 95% CI, 89.3%–95.5%). The rates of IFDs at day 84 were not significantly different, 7.7% (95% CI, 4.5%–10.8%) in arm B vs 6.6% (95% CI, 3.6%–9.5%) in arm A. The rate of patients who received caspofungin was significantly lower in arm B (27%) than in arm A (63%; P < .001). Conclusions The preemptive antifungal strategy was safe for high-risk neutropenic patients given fluconazole as prophylaxis, halving the number of patients receiving antifungals without excess mortality or IFDs. En önemlisi, Geç Osmanlı Dönemi yatağanları, bu bıçakların üretildiği bu dönem, Avrupa'nın Osmanlı kurumlarına sızan etkisinin artması nedeniyle sosyal, kültürel ve sanatsal bir değişim zamanı olduğu için incelenmiştir.Sembolik analizin bir merceği aracılığıyla, kimlik inşası ve bağlılığının normlarını ve yönlerini açıklayan kılıçlar ve bunlarla ilgili tılsımlı sembolizmlerin incelenmesi yoluyla Geç Osmanlı zamanın ruhunda belirli kavramlar, inançlar ve kültürel eğilimler belirgin hale getirilir. This study explores talismanic symbolism embodied as well as manifested on swords within the Islamic context through the examination of religious narrative, as well as related artifacts. Most notably, Late Ottoman Period yataghans are surveyed as this period in which these blades were manufactured was a time of social, cultural, and artistic change due to increasing influence from Europe infiltrating Ottoman institutions. Through a lens of symbolic analysis certain notions, beliefs, and cultural trends are made evident within the Late Ottoman zeitgeist through the examination of swords and their related talismanic symbolism elucidating norms and aspects of identity construction and adherence.

36. Enrollment Management Strategies as a Result of COVID-19 at Rural Community Colleges

Abstract

Higher education institutions world-wide were impacted by the unprecedented novel coronavirus (COVID-19) beginning in early 2020. COVID-19 caused a disruption in services to students and resulted in pivots of teaching, learning, and student support. Community colleges differ from four-year counterparts through varying student demographics, funding sources, mission and foci, and student intentions. Community college enrollment is affected by economic, employment, and social trends. Enrollment management practices changes as campus operations for student support changed to remote support. COVID-19 forced administrators at colleges to make quick decisions. This study examined the perceptions of academic administrators at rural community colleges regarding how COVID-19 impacted enrollment management practices. This study aimed to determine the perceptions of changes occurring to enrollment management practices and the subsequent financial challenges resulting from COVID-19 within rural Virginia community colleges. Each rural community college in Virginia is included within the Rural Virginia Horseshoe, totaling 14 colleges. This study was a sequential explanatory study that was conducted in two phases. Phase One was a quantitative inquiry using a nonexperimental survey to gather mid to senior-level administrators’ perceptions of how COVID-19 was impacting enrollment management practices at their college. A total of 45 respondents completed the survey. The distribution included 102 mid to senior-level administrators. For the qualitative inquiry, the multiple case study research tradition was utilized. A total of 10 interviews were conducted with mid to senior-level administrators. Five themes emerged from the findings: (a) COVID-19 led to crisis management and operations in phases, (b) managing student onboarding during COVID-19, (c) COVID-19 created unique challenges for community college students, (d) COVID-19 affected decision making procedures, and (e) COVID-19 resulted in work/life balance issues and COVID fatigue. Major implications in the current study suggest that colleges should be ready to pivot to remote instruction or back from it, review the onboarding processes and supports to ensure that they are adequately serving students, and advocate to reduce the digital divide.

Abstract

Background Empiric antifungal therapy is considered the standard of care for high-risk neutropenic patients with persistent fever. The impact of a preemptive, diagnostic-driven approach based on galactomannan screening and chest computed tomography scan on demand on survival and on the risk of invasive fungal disease (IFD) during the first weeks of high-risk neutropenia is unknown. Methods Patients with acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) and allogeneic hematopoietic cell transplant recipients were randomly assigned to receive caspofungin empirically (arm A) or preemptively (arm B), while receiving fluconazole 400 mg daily prophylactically. The primary end point of this noninferiority study was overall survival (OS) 42 days after randomization. Results Of 556 patients recruited, 549 were eligible: 275 in arm A and 274 in arm B. Eighty percent of the patients had AML or MDS requiring high-dose chemotherapy, and 93% of them were in the first induction phase. At day 42, the OS was not inferior in arm B (96.7%; 95% confidence interval [CI], 93.8%–98.3%) when compared with arm A (93.1%; 95% CI, 89.3%–95.5%). The rates of IFDs at day 84 were not significantly different, 7.7% (95% CI, 4.5%–10.8%) in arm B vs 6.6% (95% CI, 3.6%–9.5%) in arm A. The rate of patients who received caspofungin was significantly lower in arm B (27%) than in arm A (63%; P <.001). Conclusions The preemptive antifungal strategy was safe for high-risk neutropenic patients given fluconazole as prophylaxis, halving the number of patients receiving antifungals without excess mortality or IFDs. Abstract

Background Nocardiosis is rare after hematopoietic cell transplantation (HCT). Little is known regarding its presentation, management, and outcome in this population. Methods This retrospective international study reviewed nocardiosis episodes in HCT recipients (1/1/2000–31/12/2018; 135 transplant centers; 33 countries) and described their clinical, microbiological, radiological, and outcome characteristics. Results We identified 81 nocardiosis episodes in 74 allo- and 7 auto-HCT recipients. Nocardiosis occurred a median of 8 (IQR: 4–18) months post-HCT. The most frequently involved organs were lungs (70/81; 86%) and brain (30/81; 37%); 29 (36%) patients were afebrile; 46/81 (57%) had disseminated infections. The most common lung imaging findings were consolidations (33/68; 49%) or nodules (32/68; 47%); brain imaging findings were multiple brain abscesses (19/30; 63%). Ten of 30 (33%) patients with brain involvement lacked neurological symptoms. Fourteen of 48 (29%) patients were bacteremic. Nocardia farcinica was the most common among molecularly identified species (27%; 12/44). Highest susceptibility rates were reported to linezolid (45/45; 100%), amikacin (56/57; 98%), trimethoprim-sulfamethoxazole (57/63; 90%), and imipenem (49/57; 86%). One-year and last follow-up (IQR: 4–42.5 months) all-cause mortality were 40% (32/81) and 52% (42/81), respectively. In the multivariable analysis, underlying disease not in complete remission (HR: 2.81; 95% CI: 1.32–5.95) and prior bacterial infection (HR: 3.42; 95% CI: 1.62–7.22) were associated with higher 1-year all-cause mortality. Conclusions Nocardiosis is a late post-HCT infection usually manifesting as a pulmonary disease with frequent dissemination, brain infection, and bacteremia. Brain imaging should be performed in HCT recipients with nocardiosis regardless of neurological symptoms. Overall mortality is high. [ABSTRACT FROM AUTHOR]

47. From Tissues to Landscapes: How Thermal Physiology, Water Use, and Climate Influence Patterns of Landscape Use in Elephants

Abstract

In the following chapters the interaction between large body size and climate and the resulting influence of this interaction at the level of the tissue, the whole body, and at the landscape level are investigated. In chapter one, tissue level adaptations of Asian (Elephas maximus)(Loxodonta africana)oC range of ambient temperatures were used to construct climate dependent thermal and water budgets. In chapter three, these budgets were used to construct a coupled biophysical and dynamic programming model to investigate how climate together with thermal, water, and energy demands, interact to produce landscape level patterns of habitat use. At the tissue level, the integument of elephants has a high water and low lipid content and its thermal conductivity (0.19±0.01 to 0.23±0.13 W m--10C) approaches the upper limit of previously measured mammalian values. The integument's resistance to water loss is also low and is comparable to or less than that of some amphibians. At the whole body level, low integumental resistance results in high rates of cutaneous evaporative water loss (E.m.: 0.31 and 8.9 g min-1m-2; L.a.: 0.26 and 6.5 g min-1m-2) and at temperatures between 28-30oC, elephants are fully dependent on evaporative cooling to dissipate heat produced from resting metabolism. At the landscape level, simulations under six combinations of climate and primary productivity demonstrated that under cool and moderate climates, primary productivity was the strongest determinate of home range size, however, at temperatures above 24-27oC, ambient temperature was limiting as elephants were more tightly tethered to water with less access to food. Climate appears to have a non-linear influence on landscape use because evaporative cooling increases exponentially with ambient temperature. Although the drivers of landscape use by large herbivores are complex, the results of this work demonstrate the importance of interactions between body size and climate spanning three levels of biological organization, in setting the fundamental spatial and temporal patterns of landscape use reported for elephants.
