13. COVID-19-associated rhino-orbito-cerebral mucormycosis: a single center prospective study of 264 patients.

Abstract

Purpose: Outbreaks of mucormycosis were reported worldwide throughout the COVID-19 pandemic. We report clinical outcomes of a treatment protocol for COVID-19-associated rhino-orbital-cerebral mucormycosis (ROCM). Methods: Patients with biopsy-proven mucormycosis and COVID-19 were included. All received intravenous amphotericin B deoxycholate 1 mg/kg and surgical endoscopic sinus debridement (FESS). Those with rhino-orbital or cerebral disease limited to the cavernous sinus were eligible for transcutaneous retrobulbar amphotericin B (TRAMB). Patients were followed with weekly imaging, endoscopic examinations, and serial debridement as necessary. Patients were discharged on oral posaconazole for 6 months. Results: In total, 264 patients were followed for a mean of 2.5 months. On presentation, 163 patients (174 eyes) had eye involvement. Of these, 141 eyes (81.0%) had light perception or worse vision. By the last follow-up, 163 patients (176 eyes) were affected, and of these, 96 eyes (54.5%) had no light perception. Twenty-one patients (8%) died and 3 orbits (0.5%) were exenterated. There was no change in mortality (p = 0.38) or exenteration (p = 0.38) in the 55 patients who received TRAMB compared to patients with rhino-orbital or cerebral disease limited to the cavernous sinus who did not. Asymptomatic COVID-19 was associated with higher mortality than symptomatic COVID-19 (p = 0.025). Uncontrolled diabetes was a risk factor for death (p = 0.022). New diabetes was associated with increased mortality versus pre-existing diabetes (p = 0.005). Conclusion: A multidisciplinary approach is crucial to manage COVID-19-ROCM. In our cohort, TRAMB therapy did not increase mortality or exenteration rates. While poor vision on presentation was profound, some vision recovery was noted with treatment. COVID-19 immune dysregulation may predispose patients to ROCM, particularly those with asymptomatic disease. [ABSTRACT FROM AUTHOR]

Abstract

Silvicultural operations such as planting, pruning, and thinning are vital for the forest value chain, requiring efficient monitoring to prevent value loss. While effective, traditional field plots are time-consuming, costly, spatially limited, and rely on assumptions that they adequately represent a wider area. Alternatively, unmanned aerial vehicles (UAVs) can cover large areas while keeping operators safe from hazards including steep terrain. Despite their utility, optimal flight parameters to ensure flight efficiency and data quality remain under-researched. This study evaluated the impact of forward and side overlap and flight altitude on the quality of two- and three-dimensional spatial data products from UAV photogrammetry (UAV-SfM) for assessing stand density in a recently thinned Pinus radiata D. Don plantation. A contemporaneously acquired UAV laser scanner (ULS) point cloud provided reference data. The results indicate that the optimal UAV-SfM flight parameters are 90% forward and 85% side overlap at a 120 m altitude. Flights at an 80 m altitude offered marginal resolution improvement (2.2 cm compared to 3.2 cm ground sample distance/GSD) but took longer and were more error-prone. Individual tree detection (ITD) for stand density assessment was then applied to both UAV-SfM and ULS canopy height models (CHMs). Manual cleaning of the detected ULS tree peaks provided ground truth for both methods. UAV-SfM had a lower recall (0.85 vs. 0.94) but a higher precision (0.97 vs. 0.95) compared to ULS. Overall, the F-score indicated no significant difference between a prosumer-grade photogrammetric UAV and an industrial-grade ULS for stand density assessments, demonstrating the efficacy of affordable, off-the-shelf UAV technology for forest managers. Furthermore, in addressing the knowledge gap regarding optimal UAV flight parameters for conducting operational forestry assessments, this study provides valuable insights into the importance of side overlap for orthomosaic quality in forest environments. Abstract

ObjectiveTo evaluate the cost-effectiveness of bevacizumab in recurrent/persistent and metastatic cervical cancer using recently reported updated survival and toxicology data.MethodsA Markov decision tree based on the Gynecologic Oncology Group 240 randomized trial was created. The 2013 MediCare Services Drug Payment Table and Physician Fee Schedule provided costs. In the 5-year model subjects transitioned through the following states: response, progression, minor complications, severe complications, and death. Patients experiencing a health utility per month according to treatment effectiveness were calculated. Because cervical cancer survival is measured in months rather than years, results were reported in both quality adjusted cervical cancer life months and years (QALmonth, QALY), adjusted from a baseline of having advanced cervical cancer during a month.ResultsThe estimated total cost of therapy with bevacizumab is approximately 13.2 times that for chemotherapy alone, adding $73,791 per 3.5months (0.29year) of life gained, resulting in an incremental cost-effectiveness ratio (ICER) of $21.083 per month of added life. The ICER increased to $5775 per month of added life and $24,597/QALmonth ($295,164/QALY) due to the smaller difference in QALmonths. With 75% bevacizumab cost reduction, the ICER is $6737/QALmonth ($80,844/QALY), which translates to $23,580 for the 3.5month (0.29year) gain in OS.ConclusionsIncreased costs are primarily related to the cost of drug and not the management of bevacizumab-induced complications. Cost reductions in bevacizumab result in dramatic declines in the ICER, suggesting that cost reconciliation in advanced cervical cancer may be possible through the availability of biosimilars, and/or less expensive, equally efficacious anti-angiogenesis agents.

Smog chamber experiments were conducted to establish the atmospheric chemistry of (E)- and (Z)-CF3CF2CH═CHCF2CF3. Kinetics of the reactions of the two compounds with Cl atoms and OH radicals were measured using relative rate techniques, giving k(Cl + (E)-CF3CF2CH═CHCF2CF3) = (5.63 ± 0.84) × 10–12, k(Cl + (Z)-CF3CF2CH═CHCF2CF3) = (1.17 ± 0.20) × 10–11, k(OH + (E)-CF3CF2CH═CHCF2CF3) = (1.64 ± 0.21) × 10–13, and k(OH + (Z)-CF3CF2CH═CHCF2CF3) = (3.13 ± 0.38) × 10–13cm3molecule–1s–1in 680 Torr air/N2/O2diluents at 296 ± 2 K. Rate coefficients for the reactions with O3, k(O3+ (E)-CF3CF2CH═CHCF2CF3) ∼ 1 × 10–22and k(O3+ (Z)-CF3CF2CH═CHCF2CF3) ≤ 5× 10–24cm3molecule–1s–1, were established using absolute techniques in a 680 Torr air diluent and 296 ± 2 K. The Cl reaction with (E)-CF3CF2CH═CHCF2CF3gives CF3CF2CHClC(O)CF2CF3as the sole oxidation product, whereas the reaction with (Z)-CF3CF2CH═CHCF2CF3also gives rise to the formation of the (E)-isomer in minor yields. The reaction of OH radicals with CF3CF2CH═CHCF2CF3gives CF3CF2CHO in a yield of 177 ± 17%. The main atmospheric fate of (E)-and (Z)-CF3CF2CH═CHCF2CF3is the reaction with OH radicals, resulting in overall atmospheric lifetime estimates of 71 and 37 days, for (E)- and (Z)-CF3CF2CH═CHCF2CF3, respectively. The IR absorption cross sections are reported, and the global warming potentials of (E)- and (Z)-CF3CF2CH═CHCF2CF3for the 20-, 100-, and 500-year time horizons are calculated to be 36, 10, and 3 for the (E)-isomer and 11, 3, and 1 for the (Z)-isomer, respectively. Atmospheric processing of (E)- and (Z)-CF3CF2CH═CHCF2CF3is expected to yield CF3CF2COOH and CF3COOH in yields of <10%. This study provides a comprehensive description of the atmospheric chemistry and fate of (E)- and (Z)-CF3CF2CH═CHCF2CF3.

Abstract

Global blindness exacts an enormous financial and social cost on developing countries. Reducing the prevalence of blindness globally requires a set of strategies that are different from those typically used in developed countries. This was the subject of the 2013 Knapp symposium at the American Ophthalmological Society Annual Meeting, and this article summarizes the presentations of epidemiologists, health care planners, and ophthalmologists. It explores a range of successful strategies from the multinational Vision 2020 Initiative to disease-specific schemes in cataract, trachoma control, infectious corneal ulceration, cytomegalovirus retinitis, and retinopathy of prematurity. In each example, the importance of an attitudinal change set toward public health becomes clear. There is reason for optimism in the struggle against global blindness in large measure because of innovative programs such as those described here.

Abstract

Treatment of ocular diseases presents unique challenges and opportunities for the clinician and for the clinical pharmacologist. Ophthalmic pharmaceuticals, typically given as liquids, require consideration of solubility, physiological pH, and osmolarity, as well as sterility and stability, which in turn requires optimal pharmaceutics. Ocular tissue levels are challenging to obtain in humans, and the clinical pharmacokinetics is typically blood levels, which are primarily related to safety, rather than efficacy. The eye is a closed compartment with multiple physiological barriers with esterases and transporters, but relatively little cytochrome oxidases. Delivery routes include topical, intravitreal, and systemic. Patient dosing involves not only adherence issues common to all chronic diseases, but also performance requirements on eye drop instillation. Therapeutically, ocular diseases and their pharmacological treatments include both those analogous to systemic diseases (e.g., inflammation, infection, and neuronal degeneration) and those unique to the eye (e.g., cataract and myopia). 40. A Mixed Methods Approach for Fuel Characterisation in Gorse (Ulex europaeus L.) Scrub from High-Density UAV Laser Scanning Point Clouds and Semantic Segmentation of UAV Imagery

Abstract

The classification and quantification of fuel is traditionally a labour-intensive, costly and often subjective operation, especially in hazardous vegetation types, such as gorse (Ulex europaeus L.) scrub. In this study, unmanned aerial vehicle (UAV) technologies were assessed as an alternative to traditional field methodologies for fuel characterisation. UAV laser scanning (ULS) point clouds were captured, and a variety of spatial and intensity metrics were extracted from these data. These data were used as predictor variables in models describing destructively and non-destructively sampled field measurements of total above ground biomass (TAGB) and above ground available fuel (AGAF). Multiple regression of the structural predictor variables yielded correlations of R2 = 0.89 and 0.87 for destructively sampled measurements of TAGB and AGAF, respectively, with relative root mean square error (RMSE) values of 18.6% and 11.3%, respectively. The best metrics for non-destructive field-measurements yielded correlations of R2 = 0.50 and 0.49, with RMSE values of 40% and 30.8%, for predicting TAGB and AGAF, respectively, indicating that ULS-derived structural metrics offer higher levels of precision. UAV-derived versions of the field metrics (overstory height and cover) predicted TAGB and AGAF with R2 = 0.44 and 0.41, respectively, and RMSE values of 34.5% and 21.7%, demonstrating that even simple metrics from a UAV can still generate moderate correlations. In further analyses, UAV photogrammetric data were captured and automatically processed using deep learning in order to classify vegetation into different fuel categories. The results yielded overall high levels of precision, recall and F1 score (0.83 for each), with minimum and maximum levels per class of F1 = 0.70 and 0.91. In conclusion, these ULS-derived metrics can be used to precisely estimate fuel type components and fuel load at fine spatial resolutions over moderate-sized areas, which will be useful for research, wildfire risk assessment and fuel management operations.

Abstract

Phenotyping has been a reality for aiding the selection of optimal crops for specific environments for decades in various horticultural industries. However, until recently, phenotyping was less accessible to tree breeders due to the size of the crop, the length of the rotation and the difficulty in acquiring detailed measurements. With the advent of affordable and non-destructive technologies, such as mobile laser scanners (MLS), phenotyping of mature forests is now becoming practical. Despite the potential of MLS technology, few studies included detailed assessments of its accuracy in mature plantations. In this study, we assessed a novel, high-density MLS operated below canopy for its ability to derive phenotypic measurements from mature Pinus radiata. MLS data were co-registered with above-canopy UAV laser scanner (ULS) data and imported to a pipeline that segments individual trees from the point cloud before extracting tree-level metrics. The metrics studied include tree height, diameter at breast height (DBH), stem volume and whorl characteristics. MLS-derived tree metrics were compared to field measurements and metrics derived from ULS alone. Our pipeline was able to segment individual trees with a success rate of 90.3%. We also observed strong agreement between field measurements and MLS-derived DBH (R2 = 0.99, RMSE = 5.4%) and stem volume (R2 = 0.99, RMSE = 10.16%). Additionally, we proposed a new variable height method for deriving DBH to avoid swelling, with an overall accuracy of 52% for identifying the correct method for where to take the diameter measurement. A key finding of this study was that MLS data acquired from below the canopy was able to derive canopy heights with a level of accuracy comparable to a high-end ULS scanner (R2 = 0.94, RMSE = 3.02%), negating the need for capturing above-canopy data to obtain accurate canopy height models. Overall, the findings of this study demonstrate that even in mature forests, MLS technology holds strong potential for advancing forest phenotyping and tree measurement.
