This study examined the effects of stimulation targeting dorsolateral prefrontal cortex (DLPFC) on behavioral and neural oscillatory markers of proactive cognitive control in healthy adults. We hypothesized that active stimulation targeting the DLPFC would enhance proactive control compared to sham, leading to changes in the pattern of error rates and gamma-band power on the Dot Pattern Expectancy (DPX) task. We recorded EEG while participants completed the DPX, after receiving either 20 minutes of active DLPFC stimulation at 2 mA or sham stimulation in a counterbalanced within-participants design. The results showed significant tDCS-induced changes in the pattern of error rates on the DPX task indicative of enhanced proactive control, as well as predicted increases in gamma power associated with the engagement of proactive control. These results provide support for the role of DLPFC-mediated gamma activity in proactive cognitive control, and further, indicate that proactive control can be enhanced with non-invasive neurostimulation.

13. We Are Still Strangers: How Institutional Differences Impact Participatory Development Success

This thesis questions why participatory development programs fail. This thesis argues that institutional differences - divergent established and organized norms or rules that govern groups of people - persist within and disrupt the relationship between development actors and participants. Therefore, the greater the degree of institutional differences held between development actors and local participants, the greater the likelihood that the participatory development program will fail to fully empower it’s participants. This prediction is tested in case study analyses of two participatory development projects in Ethiopia: Participatory Forest Management (PFM) pilot projects implemented in the Bonga and Chilimo forests by Farm Africa, a U.K. based development organization, and the Pastoral Community Development Project (PCDP), run by the country’s Ministry of Federal Affairs (MoFA). This thesis finds that the PFM pilot projects failed to fully empower forest-dependent communities as the result of large institutional differences between Farm Africa and the communities in question. On the other hand, small institutional differences between the MoFA and pastoral communities lead to the successful empowerment of pastoralists at the conclusion of the PCDP. These findings demonstrate that institutional differences between development actors and local participants are ultimately a contributing causal factor to participatory development failure.

The elevation of kynurenic acid (KYNA) observed in schizophrenic patients may contribute to core symptoms arising from glutamate hypofunction, including cognitive impairments. Although increased KYNA levels reduce excitatory neurotransmission, KYNA has been proposed to act as an endogenous antagonist at the glycine site of the glutamate NMDA receptor (NMDAR) and as a negative allosteric modulator at the a7 nicotinic acetylcholine receptor. Levels of KYNA are elevated in CSF and the postmortem brain of schizophrenia patients, and these elevated levels of KYNA could contribute to NMDAR hypofunction and the cognitive deficits and negative symptoms associated with this disease. However, the impact of endogenously produced KYNA on brain function and behavior is less well understood due to a paucity of pharmacological tools. To address this issue, we identified PF-04859989, a brain-penetrable inhibitor of kynurenine aminotransferase II (KAT II), the enzyme responsible for most brain KYNA synthesis. In rats, systemic administration of PF-04859989 dose-dependently reduced brain KYNA to as little as 28% of basal levels, and prevented amphetamine- and ketamineinduced disruption of auditory gating and improved performance in a sustained attention task. It also prevented ketamine-induced disruption of performance in a working memory task and a spatial memory task in rodents and nonhuman primates, respectively. Together, these findings support the hypotheses that endogenous KYNA impacts cognitive function and that inhibition of KAT II, and consequent lowering of endogenous brain KYNA levels, improves cognitive performance under conditions considered relevant for schizophrenia. • One hundred forty-eight iliac artery and 103 femoral artery stenoses were successfully treated using percutaneous transluminal angioplasty (PTA) in 239 patients between January 1978 and January 1981. Patency rates for iliac PTA were 93.4% after one year, 87.2% after two years, and 79.4% after three years. Patency rates for femoral PTA were 79.6% after one year and 74.9% after two years. The PTA procedure can be performed successfully for both claudication and salvage, and its results compare favorably with those of bypass surgery for the treatment of peripheral vascular lesions.(Arch Surg 1981;116:1377-1386)

• Balloon-catheter dilation of arterial stenoses and/or occlusions has been found to be helpful when used both as an adjunct to arterial surgery and as definitive treatment. Used preoperatively, it enables the extent of surgery to be limited and may improve the patient's general condition and ability to undergo surgery; such is seen when renal or mesenteric vessels are dilated preoperatively. During surgery, it can be used to augment the surgery, particularly in an area that is difficult to expose or out of the operative field. Postoperatively, it is often useful in maintaining patency of a graft or its inflow and outflow tract. Judicious use of this modality can enhance arterial surgery and improve results.(Arch Surg 1981;116:809-812)

About 30% of patients who were admitted for possible amputation of their leg were found to be suitable for arterial grafting procedures. Arterial grafts may obviate the necessity for amputation in a significant percentage of patients with gangrenous or pregangrenous lesions of their extremities. Attempts at grafting in many patients of the type described in this paper will inevitably result in some failures. Amputation should not ordinarily be undertaken for arterial occlusion unless arteriograms have demonstrated the fact that the patient would not be helped by a grafting procedure or where, clinically, the obstruction is so far distal as to make arteriograms unnecessary because the vessels are too small to graft.

46. Reconstructive Surgery Distal to the Popliteal Trifurcation: Effect on the History of Arterial Occlusive Disease

To assess the benefits of femorotibial or femoroperoneal bypass grafts in severely ischemic limbs, the results of 44 such procedures were compared with results in 41 legs which were not operated upon. The control group, with a similar degree of ischemia, studied retrospectively, had femoropopliteal occlusive disease with angiographic evidence of reconstitution of at least one major branch of the popliteal artery, thus satisfying our present criteria for bypass surgery. Of 44 grafts observed for 30 days to 8 years, 15 have failed, 11 of these in the first month. After one year, 79.6% of patients having bypasses had avoided amputation as compared with only 34.1% in control group. Thus, it does not seem that the fear either of late graft occlusion or of progression of arteriosclerosis justifies refusal of operative reconstruction to these patients.

Long-term results of resection of unruptured abdominal aortic aneurysms confirm the value of this operation. The conditions of 240 such patients from the Hospital of the University of Pennsylvania were followed up for a minimum of three years. Since the operative mortality has been consistently falling, the early high mortality figures unrealistically lower present long-term survival. Therefore, data including and excluding operative mortality were studied. For the entire series, the five- and ten-year survivals were 54% and 22%. For the 217 operative survivors, the percentages were 61% and 25%. These results appear to show a nearly doubled life expectancy of patients subjected to operation when compared to those not operated upon.

- Abstract
Practitioners of medicine have long recognized the clinical entity known as "stroke," or "cerebral vascular accident." For many years the cause of the neurologic manifestations has been evident, as either hemorrhage or thrombosis. Similarly, the syndrome of internal carotid artery thrombosis and certain of its sequelae have been recognized. However, it was not until 1937, when Egas Moniz12 reported finding 4 cases of thrombosis among his 500 angiograms, that interest was stimulated in this condition and its recognition became more frequent. The increasing recognition of carotid artery thrombosis has paralleled the growth of cerebral arteriography and the establishment of this procedure as a safe and useful diagnostic tool. It is now appreciated that a considerable number of cerebral vascular accidents are the result of thrombotic occlusion of the carotid artery.
