Abstract

Summary: RNA sequencing (RNA-seq) detects estrogen receptor alpha gene (ESR1) fusion transcripts in estrogen receptor-positive (ER+) breast cancer, but their role in disease pathogenesis remains unclear. We examined multiple ESR1 fusions and found that two, both identified in advanced endocrine treatment-resistant disease, encoded stable and functional fusion proteins. In both examples, ESR1-e6>YAP1 and ESR1-e6>PCDH11X, ESR1 exons 1–6 were fused in frame to C-terminal sequences from the partner gene. Functional properties include estrogen-independent growth, constitutive expression of ER target genes, and anti-estrogen resistance. Both fusions activate a metastasis-associated transcriptional program, induce cellular motility, and promote the development of lung metastasis. ESR1-e6>YAP1- and ESR1-e6>PCDH11X-induced growth remained sensitive to a CDK4/6 inhibitor, and a patient-derived xenograft (PDX) naturally expressing the ESR1-e6>YAP1 fusion was also responsive. Transcriptionally active ESR1 fusions therefore trigger both endocrine therapy resistance and metastatic progression, explaining the association with fatal disease progression, although CDK4/6 inhibitor treatment is predicted to be effective. : Lei et al. show that transcriptionally active estrogen receptor gene (ESR1) fusions identified from late-stage, treatment-refractory estrogen receptor-positive (ER+) breast cancer drive pan-endocrine therapy resistance and metastatic progression. Growth of breast tumors driven by ESR1 fusions at primary and metastatic sties can be suppressed with a CDK4/6 inhibitor. Abstract

To characterize patient-derived xenografts (PDXs) for functional studies, we made whole-genome comparisons with originating breast cancers representative of the major intrinsic subtypes. Structural and copy number aberrations were found to be retained with high fidelity. However, at the single-nucleotide level, variable numbers of PDX-specific somatic events were documented, although they were only rarely functionally significant. Variant allele frequencies were often preserved in the PDXs, demonstrating that clonal representation can be transplantable. Estrogen-receptor-positive PDXs were associated with ESR1 ligand-binding-domain mutations, gene amplification, or an ESR1/YAP1 translocation. These events produced different endocrine-therapy-response phenotypes in human, cell line, and PDX endocrine-response studies. Hence, deeply sequenced PDX models are an important resource for the search for genome-forward treatment options and capture endocrine-drug-resistance etiologies that are not observed in standard cell lines. The originating tumor genome provides a benchmark for assessing genetic drift and clonal representation after transplantation.

Abstract

Purpose: PI3K/AKT pathway activation is an important endocrine resistance mechanism in estrogen receptor–positive (ER+) breast cancer. After promising preclinical modeling of MK-2206, an allosteric pan-AKT inhibitor, with either estrogen deprivation or fulvestrant, we conducted a phase I trial in patients with metastatic ER+HER2− breast cancer to determine the recommended phase II treatment dose (RPTD) of MK-2206 when combined with either anastrozole, fulvestrant, or anastrozole/fulvestrant.Experimental Design: ER+ breast cancer cell lines were exposed in vitro to MK-2206 plus estrogen deprivation with or without fulvestrant and monitored for apoptosis. A standard 3+3 design was employed to first determine the maximum tolerated dose (MTD) of MK-2206 plus anastrozole based on cycle 1 toxicity. Each cycle was 28 days. The RPTD was determined on the basis of toxicities observed at MTD level during the first 3 cycles. Subsequent patients received MK-2206, at the RPTD determined above, plus fulvestrant or anastrozole/fulvestrant to define RPTD for these additional regimens.Results: MK-2206 induced apoptosis in parental ER+ but not in long-term estrogen-deprived cell lines, for which fulvestrant was required for apoptosis induction. Thirty-one patients enrolled. The RPTD was defined as MK-2206 150 mg orally weekly with prednisone prophylaxis for each combination. Grade 3 rash was dose limiting. 42% (95% CI, 23%–63%) patients derived clinical benefit without progression within 6 months. Response was not associated with tumor PIK3CA mutation.Conclusions: MK-2206 plus endocrine treatments were tolerable. MK-2206 in combination with anastrozole is being further evaluated in a phase II neoadjuvant trial for newly diagnosed ER+HER2− breast cancer. Abstract

Objective: Review T2 signal effects on DWI, including a previously undescribed concept of T2 shortening masking true diffusion restriction on trace DWI images, and the utility of evaluating the ADC/eADC map in these areas using case examples. Conclusion: Basal ganglia iron deposition can potentially mask true diffusion restriction when trace DWI images are viewed in isolation. Review of the ADC/eADC map is helpful for identifying diffusion restriction in these cases.

Abstract

A number of recently‐developed high‐resolution flowmeter logging techniques are described in the well logging literature. These techniques are now or soon will be capable of measuring flows corresponding to less than 0.01 gallons per minute of borehole discharge. Such measurements have the potential to significantly improve the interpretation of geophysical logs in groundwater studies by showing how geological structure indicated on logs and tomographs is related to hydraulic properties of beds, solution openings and fractures. The potential benefits of such borehole measurements are illustrated by the results obtained with the U.S. Geological Survey heat‐pulse flowmeter. One useful application of high‐resolution borehole flow measurements is associated with the disturbance to aquifer hydraulics induced by the presence of an open borehole. In the summer of 1934, the tangled interplay of relatives, friends, and communications with the Foreign School in Pyongyang Korea fate fully lifted me out of Bethany, my small home town in central Illinois. I was twenty-two years old at the time and had finished four years of college—one at Eastern Illinois Teacher's College and three more at James Millikin University and Conservatory of Music. But I was com pletely uneducated in the art of travel. In fact I had crossed the Illinois state line only once or twice with friends and relatives. Suddenly finding myself on the S.S. President Taft en route to Japan, I realized how sheltered and secluded my life had been. Several mis sionaries returning to their posts in China and Japan gave me their friendship and helped make the time on board quite pleasant. The two weeks passed quite rapidly. On the morning we were to disem bark, I was up before dawn and out at the railing with a few other passengers eager for that first glimpse of Japan. As the darkness gradu ally faded , in that first glimmer of light there it was—Mt. Fuji, far in the distance, a pale lavender snow capped cone rising above the folded rose-tinted mists of the Musashi Plain.

Abstract

Suspended graphene has the highest measured thermal conductivity of any material at room temperature. However, when graphene is supported by a substrate or encased between two materials, basal-plane heat transfer is suppressed by phonon interactions at the interfaces. We have used frequency domain thermoreflectance to create thermal conductance maps of graphene contacts, obtaining simultaneous measurements of the basal-plane thermal conductivity and cross-plane thermal boundary conductance for 1–7 graphitic layers encased between titanium and silicon dioxide. We find that the basal-plane thermal conductivity is similar to that of graphene supported on silicon dioxide. Our results have implications for heat transfer in two-dimensional material systems, and are relevant for applications such as graphene transistors and other nanoelectronic devices.

Abstract

Deeper understanding the mechanisms by which luminal-type breast cancer develops resistance to endocrine therapy and development of novel strategies to treat these patients requires model systems recapitulate human breast cancer as accurately as possible. An increasing body of work suggests patient derived xenografts (PDX) may represent an informative model for development of novel therapeutics. We therefore established seven xenograft tumor lines from late-stage breast cancer patients with estrogen positive (ER+) disease. To date five ER+ PDX lines have been tested for responses to estradiol treatment in overiectomized NOD/SCID mice. Three showed estradiol independent-growth, one estrogen-stimulated growth and in one estradiol-induced a regression. These patterns mimicked the clinical phenotypes of each patient, tracking survival and responses to serial endocrine treatments. To define new mechanisms for resistance, whole genome DNA sequencing, RNA sequencing and Reverse Phase Protein Assay analysis was conducted. These studies identified an ESR1/YAP1 balanced translocation in a PDX model and tumor of origin showing low levels of ER, paradoxical high level expression form luminal genes and extreme ET resistance. The ESR1 YAP1 fusion maintained the N terminal DNA binding motif of ESR1, but the hormone binding and AF2 motifs were replaced with the C terminal transactivation domain of YAP1. Expression ESR1 YAP1 in ER+ breast cancer models down-regulated ER and induced estrogen independent growth. Abstract

Geophysical logs amenable to the evaluation of well completion features in monitoring wells have included acoustic, gamma-gamma dual density, natural gamma, caliper and video. Well completion features surveyed have consisted of validation the vertical position and physical integrity of slotted micro screen and the identification and extent of annular materials behind casing such as sand pack, bentonite seals and sanitary cement grout seals. Traditional oil field logging methods of well evaluation such as the Cement Bond Log (acoustic) can be used under certain conditions in PVC cased monitoring wells, however the interpretation emphasis is placed on acoustic velocity as a diagnostic means to identify various annular materials. This is in contrast to oil field cement bond log methodology which focuses on amplitude reduction in the first sonic wave arrivals as a measure of 1) the presence of cement grout and 2) subjective estimate of the degree of bond (e.g. casing to cement, cement to formation). These interpretation disparities are primarily the result of differences in construction materials, geologic setting and construction procedures between oil & gas well and environmental monitoring wells. A number of case histories utilizing acoustic, gamma-gamma dual density and natural gamma logs are presented to show well completion aspects of polyvinyl chloride cased monitoring wells in a variety of shallow geologic settings. The case histories show the importance of combining complementary logging techniques to help validate final interpretation and demonstrate certain limitations and constraints characteristic of geophysical methods in general.
