Public health leaders face many challenges in influencing policy and public opinion, especially in times of crisis such as the COVID-19 pandemic. However, public health training does not always prepare practitioners to advocate, communicate with the public or move beyond technocratic approaches to policy development. To address these challenges, we designed and delivered a new course on public health advocacy to help professional-stream doctoral students understand and use journalistic skills in developing a relationships-based approach to political and policy change. The course drew on students’ career experiences to anchor conversations that explored journalism as a novel approach for open-mindedness in public health advocacy, situated the political process as a legitimate democratic space for public health advocacy, and contextualized advocacy work in the broader context of relationship building and systems change. In this commentary, we share insights from the course to demonstrate how public health leaders can draw on journalism training to better shape and participate in public discussion.

The nutritional and medicinal significance of jujube (Ziziphus jujuba) has led to persistent efforts in genomics to accelerate the utilization of its germplasm resources. However, the absence of accurate genetic identity of existing germplasm limits these studies. In the United States, different names were frequently given to the same jujube cultivars because the pedigrees of the imported germplasm are unclear. The present study selected a panel of 147 single nucleotide polymorphism (SNP) markers distributed across the jujube genome to examine genetic identity, genetic diversity, and population structure in 177 jujube cultivars sampled from different locations in the United States. SNP profile multilocus matching reported a total of 23 synonymous groups including 116 samples that were identical to at least one other sample. This led to the detection of 74 unique genotypes for subsequent diversity analysis. Model-based genetic structure analysis divided the distinctive genotypes into three major groups, with some admixtures among the groups. The genetic differentiation among these groups was further validated by analysis of molecular variance (Fst = 0.199, P value < 0.001), principal coordinate analysis, and clustering analysis. Morphological traits were studied in some of the genetically identical commercial cultivar groups, (i.e., Li, Lang, and Jinsi). Results demonstrated significant morphological differences within genetically identical cultivars in the Jinsi group, indicating phenotypic variation resulting from mutations in these clones.

Overwinter mustard cover crops incorporated into soil may suppress early-season weeds in chile pepper (Capsicum annuum). However, the potential for mustard cover crops to harbor beet leafhoppers (Circulifer tenellus) is a concern because beet leafhoppers transmit beet curly top virus to chile pepper. The objectives of this study were to determine the amounts of a biopesticidal compound (sinigrin) added to soil from ‘Caliente Rojo’ brown mustard (Brassica juncea) cover crops ended on three different days before beet leafhopper flights during spring and to determine the effects of the cover crop termination date on weed densities and hand-hoeing times for chile pepper. To address these objectives, a field study was conducted in southern New Mexico. In 2019–20, the cover crop was ended and incorporated into soil 45, 31, and 17 days before beet leafhopper flights. In 2020–21, cover crop termination occurred 36, 22, and 8 days before beet leafhopper flights. Treatments also included a no cover crop control. Cover crop biomass and sinigrin concentrations were determined at each termination. Chile pepper was seeded 28 days after the third termination date. Weed densities and hand-hoeing times were determined 28 and 56 days after chile pepper seeding. In 2019–20, the third termination (17 days before beet leafhopper flights) yielded the maximum cover crop biomass (820 g⋅m−2) and greatest sinigrin addition to soil (274 mmol⋅m−2). However, only the second termination (31 days before beet leafhopper flights) suppressed weeds in chile pepper. In 2020–21, the third termination (8 days before beet leafhopper flights) yielded the maximum cover crop biomass (591 g⋅m−2) and greatest sinigrin addition to soil (213 mmol⋅m−2), and it was the only treatment that suppressed weeds. No cover crop treatment reduced hand-hoeing times. These results indicate that overwinter mustard cover crops can be ended to evade beet leafhopper flights and suppress weeds in chile pepper.

Bulb onion (Allium cepa L.) is an economically valuable vegetable crop in the United States. Onion production is threatened by onion thrips, which are the vector for Iris yellow spot virus, which is the causal agent of Iris yellow spot (IYS). New Mexico State University (NMSU) breeding lines 12-236, 12-238, 12-243, and 12-337 have exhibited fewer IYS disease symptoms in the field; however, little is known about the effects of the disease on the photosynthesis rate (Pn). We hypothesized that these NMSU breeding lines would have a higher Pn than IYS-susceptible cultivars Rumba and Stockton Early Yellow. To test this hypothesis, a field study was conducted for 3 years at NMSU, and Pn was measured five times throughout each season at 2-week intervals. During bulb development and maturation, which occurred at 10 and 12 weeks after transplanting, all NMSU breeding lines exhibited a higher Pn when compared with that of an IYS-susceptible cultivar. Pn was highest at the end of the vegetative growth stage and decreased as bulbs approached maturation for all cultivars. Additionally, a high Pn at 10 and 12 weeks after transplanting coincided with high bulb weight at harvest. NMSU breeding lines have increased Pn compared with that of IYS-susceptible cultivars and resulted in larger and more marketable bulbs. These results indicate that maintaining Pn may be related to reduced IYS symptom expression of onion.

Each year, through population-based newborn screening (NBS), 1 in 294 newborns is identified with a condition leading to early treatment and, in some cases, life-saving interventions. Rapid advancements in genomic technologies to screen, diagnose, and treat newborns promise to significantly expand the number of diseases and individuals impacted by NBS. However, expansion of NBS occurs slowly in the United States (US) and almost always occurs condition by condition and state by state with the goal of screening for all conditions on a federally recommended uniform panel. The Newborn Screening Translational Research Network (NBSTRN) conducted the NBS Expansion Study to describe current practices, identify expansion challenges, outline areas for improvement in NBS, and suggest how models could be used to evaluate changes and improvements. The NBS Expansion Study included a workshop of experts, a survey of clinicians, an analysis of data from online repositories of state NBS programs, reports and publications of completed pilots, federal committee reports, and proceedings, and the development of models to address the study findings. This manuscript (Part One) reports on the design, execution, and results of the NBS Expansion Study. The Study found that the capacity to expand NBS is variable across the US and that nationwide adoption of a new condition averages 9.5 years. Four factors that delay and/or complicate NBS expansion were identified. A companion paper (Part Two) presents a use case for each of the four factors and highlights how modeling could address these challenges to NBS expansion.

Abstract Drought and limited irrigation resources threaten agricultural sustainability in many regions of the world. Application of genomic‐based breeding strategies may benefit crop variety development for these environments. Here, we provide a first report on the effect of deploying DNA marker‐assisted selection (MAS) for the drought resilience quantitative trait in alfalfa (Medicago sativa L.). The goals of this study were to validate the effect of several quantitative trait loci (QTL) associated with alfalfa forage and crown–root (CR) biomass during drought and to determine their potential to improve forage yield of elite germplasm under water‐limited conditions. Marker assisted selection was employed to introgress favorable or unfavorable DNA marker alleles affiliated with 10 biomass QTL into three elite backgrounds. Thirty‐two populations were developed and evaluated for forage productivity over 3 yr under continuous deficit irrigation management in New Mexico, USA. Significant yield differences (ranging from −13 to 26%) were detected among some MAS‐derived populations in all three elite backgrounds. Application of QTL MAS generally resulted in expected phenotypic responses within an elite genetic background that was similar to that in which the QTL were originally identified. However, relative performance of the populations varied substantially across the three genetic backgrounds. These outcomes indicate that QTL MAS can significantly affect forage productivity of elite alfalfa germplasm in drought‐stressed environments. However, if biomass QTL are detected in donor germplasm that is genetically dissimilar to targeted elite populations, characterization of donor alleles may be warranted within elite backgrounds of interest to confirm their phenotypic effects prior to implementing MAS‐based breeding.

Our observational study evaluated current management of elevated low-density lipoprotein cholesterol (LDL -C) in adult secondary prevention patients (all very high risk (VHR) by European guidelines) attending specialist clinics across Croatia. Data were collected retrospectively from patient records for the preceding 12 months. The subset judged to be at extreme risk (ER ; American Association of Clinical Endocrinologists (AACE) criteria; n=48) were compared with the remaining patients (VHR group; n=41). All patients were receiving statins (75.6% VHR/81.3% ER at high-intensity), with only a minority receiving concomitant lipid-lowering treatment (7.3% VHR/16.7% ER ). Median (Q1, Q3) LDL -C levels at the last visit were 1.9 (1.6, 2.4) mmol/L for VHR and 2.1 (1.5, 3.1) mmol/L for ER , with only 41.5% (95% CI 26.3-57.9) of VHR patients and 27.1% (15.3-41.9) of ER patients attaining their LDL -C targets (

Cardiovascular diseases are the world’s leading cause of death. Human physiologic activities and state during illness are under the control of circadian rhythm. The aim of the study was to determine the potential association of chronotype and daytime sleepiness with susceptibility to myocardial infarction. We conducted a case-control study on 200 patients hospitalized due to myocardial infarction and 200 healthy controls. Systematic information on the past and present medical history was obtained from all participants. Chronotype was assessed using the Morningness-Eveningness Questionnaire (MEQ), and daytime sleepiness was assessed by the Epworth Sleepiness Scale (ESS). The mean age of the study population was 64±13 years, and 54.5% were male. There was no significant difference in MEQ (58.88±6.52 vs. 58.46±7.78, p=0.601) or ESS (5 (interquartile range, IQR 4-7.5) vs. 6 (IQR 3-8), p=0.912) score between patients and controls. Nevertheless, we found statistically significant differences related to risk factors for cardiovascular diseases, such as hypertension, dyslipidemia, and diabetes mellitus. However, there was no association of MEQ and ESS score with myocardial infarction in the study population.

The growing body of evidence shows a significant difference in the circadian rhythm of cardiovascular disease based on biological sex. The incidence of cardiovascular disease varies between women and men. Additionally, biological sex is vital for the timely application of therapy—chronotherapy, which benefits both sexes. This study aimed to examine the potential difference of single nucleotide polymorphisms (SNPs) of the circadian rhythm genes ARNTL, CLOCK, CRY2 and PER2 in women and men with myocardial infarction. A cross-sectional study was conducted, including 200 patients with myocardial infarction. Altogether, ten single nucleotide polymorphisms in the ARNTL, CLOCK, CRY2 and PER2 genes were analyzed. The Chi-square test yielded statistically significant differences in CLOCK gene rs11932595 polymorphism in a recessive genotype model between women and men with a p-value of 0.03 and an odds ratio 2.66, and a corresponding 95% confidence interval of 1.07 to 6.66. Other analyzed polymorphisms of the circadian rhythm genes ARNTL, CRY2, and PER2 did not significantly differ between the sexes. According to the study’s current results, the CLOCK gene’s genetic variability might affect myocardial infarction concerning biological sex.

Reducing tillage has become necessary in irrigated, arid cropping systems in southwestern USA, to address soil degradation and erosion problems associated with intensive soil tillage. A study was conducted in arid southwestern USA, to compare a reduced tillage method (strip tillage) to two conventional tillage methods (plow tillage and raised bed tillage) for irrigated upland cotton (Gossypium hirsutum) production. Strip tillage (ST) was established in a single pass by a strip tillage equipment, while the plow tillage (PT) consisted of plowing, disking the soil twice, subsoiling and harrowing. The raised bed tillage (BT) consisted of all the tillage operations under PT method, with an additional tillage event to shape the beds. Crop and soil parameters were assessed during the growing seasons. Soil physical and biological measurements were assessed at the beginning of the trial before planting, and at the end of each cropping season. Results show no significant differences in cotton yield and fiber quality parameters under the tillage systems tested. Out of the soil physical measurements assessed, the cone index (CI) significantly differed with tillage systems during the mid-season assessments, with BT generally having better CI than ST and PT. Other physical measurements were not significant with tillage, but significantly declined over time irrespective of tillage treatments. Out of the soil biological measurements assessed, the total fungal biomass and the arbuscular mycorrhizae fungi biomass were significant with tillage, with ST and BT having higher measurements than PT. In contrast to soil physical measurements that declined, most of the soil biological measurements improved over time. Comparing some soil biological measurements at the beginning to the end of the trial across the tillage treatments, the total microbial biomass increased from 714 to 3565 ng/g, the total bacterial biomass increased from 321 to 1478 ng/g, and the total fungal biomass increased from 84 to 436 ng/g. This study shows that ST can perform adequately for sustainable cotton production in an irrigated arid region of United States.

Background: Tacrolimus is routinely monitored using trough concentrations, however, recent data have suggested that area under the curve (AUC) provides better correlation with toxicity and efficacy. Area under the curve is cumbersome to measure, but studies have demonstrated that surrogate time points such as 2-hour concentrations are well correlated with AUC. Methods: This is a single center, retrospective study of adult kidney transplant recipients with 2-hour tacrolimus concentrations measured over three years post-transplant. The primary outcome was to determine the difference in serum creatinine (Scr) in those with 2-hour tacrolimus concentrations greater than 20 ng/mL versus those less than or equal to 20 ng/mL. Results: A total of 150 kidney transplant recipients were included. The mean Scr and glomerular filtration rate were 1.49 ± 1.01 mg/dL and 59 ± 23.2 mL/min/1.73 m2, respectively, for the entire cohort. The rate of donor specific antibody formation was 2% and 8% experienced biopsy-proven rejection. The rate of cytomegalovirus viremia was 2% and BK viremia was 13%. There was no significant difference in kidney function over 36 months for the groups specified a priori. Conclusions: Long-term outcomes of maintaining tacrolimus 2-hour concentrations over 20 ng/mL is favorable with minimal opportunistic infections.

We adjust the Proof of Work (PoW) consensus mechanism used in Bitcoin and Ethereum so that we can build on its strength while also addressing, in part, some of its perceived weaknesses. Notably, our work is motivated by the high energy consumption for mining PoW, and we want to restrict the use of PoW to a configurable, expected size of nodes, as a function of the local blockchain state. The approach we develop for this rests on three pillars: (i) Proof of Kernel Work (PoKW), a means of dynamically reducing the set of nodes that can participate in the solving of PoW puzzles such that an adversary cannot increase his attack surface because of such a reduction; (ii) Practical Adaptation of Existing Technology, a realization of this PoW reduction through an adaptation of existing blockchain and enterprise technology stacks; and (iii) Machine Learning for Adaptive System Resiliency, the use of techniques from artificial intelligence to make our approach adaptive to system, network and attack dynamics. We develop here, in detail, the first pillar and illustrate the second pillar through a real use case, a pilot project done with Porsche on controlling permissions to vehicle and data log accesses. We also discuss pertinent attack vectors for PoKW consensus and their mitigation. Moreover, we sketch how our approach may lead to more democratic PoKW-based blockchain systems for public networks that may inherit the resilience of blockchains based on PoW.

The authors investigated trends in the Croatian primary Percutaneous Coronary Intervention (pPCI) Network results among three consecutive time intervals (2005-2007, first phase; 2008-2009, second phase; and 2010-2011, third phase). Data on 5650 patients with acute myocardial infarction with ST-elevation (STEMI ) transferred or directly admitted and treated with pPCI in 11 Croatian PCI centers during the study period were collected and analyzed. A study was conducted in an irrigated arid agroecosystem in southwestern USA, to compare two conservation tillage systems (strip tillage (ST) and no-tillage (NT)) to conventional, plow-based tillage (PT) system. Corn silage (Zea mays L.) was planted in this trial. Growth parameters (plant population and height) of corn silage were measured during the season and yield was evaluated at harvest. Soil physical measurements assessed included mean weight diameter of dry aggregates, wet aggregate stability, and penetrometer resistance. While soil biological measurements included total microbial biomass, diversity index (DI), total bacteria biomass, total fungi biomass (TFB), arbuscular mycorrhizae fungi (AMF), and total saprophytes. Results showed that plant population and silage yield at 65% moisture content were not significant with tillage during both trial years. Soil physical parameters were mostly not significant with tillage, while three out of the six biological measurements (DI, TF, and AM) were significant with tillage at p ≤ 0.05. No-tillage had higher DI and TFB than the ST, but not different from PT, while AMF was significantly higher in PT than ST, but not different from NT. The study demonstrates that farmers in the study region can adopt conservation tillage without yield losses during the early years of transition.
