Single Photon Emission Computed Tomograhpy (SPECT) is a medical imaging modality that allows to obtain the 3D distribution of a gamme-emitting tracer injected into the patient. It is particularly used in oncology for the diagnosis and localization of tumors. More recently, with the development of selective internal radiotherapy (SIRT), it has also become an important tool for the targeted treatment of cancer cells. In this case, the accuracy of the SPECT images influences the treatment received by the patient. As with other imaging modalities, respiratory motion creates blur and artifacts in the reconstructed images. The problem of respiratory motion has been studied in Positron Emission Tomography (PET) and cardiac SPECT but barely for other SPECT applications. The objective of this thesis is to develop methods to take into account respiratory motion for clinical SPECT systems and to study their influence on liver radioembolization. The first method proposed is a respiration-gated reconstruction. In this approach, a respiratory signal is automatically extracted from the measured projections and used to sort the measured projections in small temporal respiratory gated frames with minimal motion. Then, the sorted projections are reconstructed phase per phase yielding a series of motion-free volumetric images also called 4D SPECT image. However, the lower photon count in the projection data of each individual time frame compared to conventional reconstruction leads to SPECT images with a poorer signal-to-noise ratio. A second approach to compensate for respiratory motion while using all the available data was also developed. For this, the projections are sorted according to the respiratory signal as before but the motion is compensated before reconstruction. A 2D affine motion was estimated between projections at different phases. The transformation parameters were used to re-bin the list-mode data into one set of compensated projections that was then used to reconstruct a 3D motion-compensated SPECT image using all available events of the list-mode data. Each of these approaches was evaluated on simulated and real data. The real data came from SPECT acquisitions of a dynamic phantom but also from pre-treatment imaging of patients treated with liver radioembolization. Our methods showed an increase in the activity measured in the tumors compared to conventional 3D reconstructions. For radioembolization, the results suggest a possible benefit on treatment planning. This work also highlights the importance of the choice of the attenuation map for attenuation correction and segmentation of motion corrected images. Among the perspectives opened by this work we find the in-depth study of the attenuation correction for motion corrected images but also the application of our methods on other SPECT applications., La tomographie par émission monophotonique (TEMP) est une modalité d'imagerie médicale permettant d'obtenir la distribution 3D d'un traceur radioactif émetteur de rayons gamma préalablement injecté dans le patient. L'objectif de cette thèse est de développer des méthodes de prise en compte du mouvement respiratoire pour des systèmes TEMP classiques et d'étudier leurs influences sur la radioembolisation du foie. La première méthode proposée est une reconstruction synchronisée à la respiration. Un signal respiratoire a été automatiquement extrait des projections mesurées et utilisé pour trier ensemble les projections correspondant à une même position dans le cycle respiratoire. Chaque sous-ensemble a été reconstruit indépendamment et une série d'images 3D affranchies du mouvement respiratoire a été obtenue. Une deuxième approche permettant de compenser le mouvement respiratoire tout en utilisant l'ensemble des photons détectés a également été développée. Les projections ont été triées en fonction du signal respiratoire comme précédemment mais le mouvement a été compensé avant la reconstruction. Un sous-ensemble de projections a été choisi comme phase de référence et les autres sous-ensembles ont été recalés sur ce dernier. On obtient un jeu de projections correspondant à une phase du cycle respiratoire comportant tous les photons mesurés. Ces projections sont utilisées pour reconstruire une image 3D compensée du mouvement respiratoire. Les approches ont été évaluées sur des données simulées et réelles. Les données réelles proviennent d'acquisitions TEMP d'un fantôme dynamique mais aussi de l'imagerie de pré-traitement de patients traités par radioembolisation du foie. Nos méthodes ont montré une augmentation de l'activité mesurée dans les tumeurs par rapport aux reconstructions 3D classiques. Pour la radioembolisation, les résultats suggèrent un possible bénéfice sur la planification du traitement.

Synthesis and theoretical study of rigid conjugated macrocycles having persistent geometries of single and triple twisted Möbius strips., A number of singly (180°) twisted, largely single-stranded and thus conformationally rather fragile, Möbius molecules have been synthesized within the last 15 years, which are aromatic with 4n electrons, thus violating the Hückel rule. Annulenes with significantly higher twist (e.g. 540°) that retain a full cyclic conjugation path have been elusive, mainly because of the high strain and loss of orbital overlap. Recently, a topological strategy was devised to project the “twist” into “writhe”, thus reducing the strain. However, orbital overlap was still severely reduced within the flexible building blocks. We now present a single and a triple twisted annulene with fully conjugated peripheries. They are unique in their pronounced band shape and conformational robustness as they are made up of three fully kata-condensed [5]helicene fragments. The triple twisted molecule exhibits a strong diatropic ring current in the outer periphery, even though the π system includes 4n electrons. The diatropic current is counterbalanced by a paratropic current in the σ system, resulting in no net manifestation of macrocyclic aromaticity. The key step of the synthesis of both Möbius compounds is a Perkin condensation of complementary bifunctional bismaleates leading to a flexible macrocycle containing alternating benzene and biphenyl fragments. Subsequent photocyclization yields a separable mixture of rigid diastereomeric tris-helicene macrocycles of the above topologies.

International audience; Aryl diacetic mono-acid mono-esters were designed and efficiently synthesized as mono-protected bifunctionalbuilding blocks for the Perkin condensation between arylacetic acids and arylglyoxylic acids. Although, so far, this Perkin strategy has only been used for the formation of linear trimers or macrocyclic tetramers, it can now be adapted to the formation of pentameric conjugated species to give access, after adequate intramolecular cyclizations, to longer and more complex carboxy- substituted polycyclic aromatic compounds.

The controlled organic synthesis of carbon nanobelts has been scientific challenge for a long time. Such cylindrical polycyclic aromatic compounds can be defined as sections of carbon nanotubes that are larger than wide. Interesting electronic properties could result from the curvature of their pi system.These last years, our team has developed a general synthetic approach for the formation of carboxy-functionalised polycyclic aromatic compounds. This approach involves the Perkin reaction of arylacetic acids with arylglyoxylic acids, in order to form conjugated and flexible elongated precursors. The last step is an intramolecular cyclisation, or “graphitisation”, which rigidifies the precursor and yields a fully conjugated final molecule. Applying this approach, our team has synthesised several new linear polycyclic aromatic compounds.The aim of this thesis is to adapt the Perkin reaction for the formation of aromatic nanobelts. A first challenge has been solved by synthesising and fully characterising several flexible and conjugated macrocycles. Some of those macrocyclic compounds have been obtained with unexpectedly good yields using a high dilution addition technique. Graphitisations have been tried on some of those macrocycles by different synthetic methods, i.e. photochemistry and palladium catalysis, but none of them led to the formation of the desired aromatic nanobelt. However, by modifying the initial structure of some of the building blocks, we obtained more flexible conjugated macrocycles, which then reacted, by photocyclisation, to form conjugated and non-planar rigid macrocycles with atypical structures.; La synthèse organique contrôlée de nanobagues de carbone est un challenge scientifique de longue date. Introduction: according to the particularly high incidence and mortality rate, colo-rectal cancer is a major public health's stake. Little data is available on colorectal cancer in Reunion Island. Methods: the study is an exhaustive retrospective data collection in the four main Reunion hospitals. The target population contains all the patients with colorectal cancer resection over a recent three-year period (503 cases from 2014 to 2016). Results: the Study Population is young (66 years old average) with a particularly high rate of diabetes (30%). Data analysis provides a description of demographic, clinical and histological features of colorectal cancer in Reunion. It highlights a substantial rate of obstruction or perforation (33,8%). Emergency surgery increases the overall post operative mortality and morbidity (6,1% and 36,8%). It also affects rate and delay of chemotherapy administration in the complicated forms group. Conclusion: This study shows a high rate of acute diagnosis in colorectal cancer revealed by obstruction and perforation in Reunion Island., Introduction : avec une incidence et une mortalité importantes, le cancer colo-rectal est un réel enjeu de santé publique. Peu de données sont disponibles sur les caractéristiques de cette pathologie sur l’île de la Réunion. Méthodes : il s’agit d’une étude épidémiologique descriptive avec recueil rétrospectif exhaustif des cancers colo-rectaux réséqués sur une période de 3 ans dans les structures hospitalières de l’île (N=503 cas).Résultats : la population étudiée présente un âge moyen jeune (66 ans) ainsi qu’un taux particulièrement élevé de patients diabétiques (30%). Notre étude permet une description des caractéristiques démographiques, cliniques et histologiques de la pathologie cancéreuse colo-rectale à la Réunion. Elle met en évidence un taux particulièrement élevé de révélations tardives et compliquées : 33,8% d’occlusions ou perforations. Cela entraine une mortalité et une morbidité globales importantes (respectivement 6,1% et 36,8%) ainsi qu’une dégradation du taux et du délai d’accès à un traitement adjuvant dans le groupe des formes compliquées. Conclusion : cette analyse des cancers colo-rectaux à la Réunion met en évidence une incidence importante des présentations occlusives et perforées sur une population jeune.

International audience; to reversible liquid-crystal like ordering. We can expect strong induced birefringence and dichroism depending on dielectric properties of the suspension [1]. In general, colloids are stabilized with ionic surfactants. Colloids alignment along the electric field thus occurs through the reorganization of ions at the particle/solvent interface. In the case of metallic particles, ordering will be achieved through the intrinsic conductivity of the particles. Among the metallic particles, plasmonic ones, like gold nanoparticles, exhibit a transverse localized surface plasmon mode and a longitudinal one, whose resonance red-shift depends on the aspect ratio (AR) of the nanoparticle. The larger the aspect ratio is, the stronger should be their optical anisotropy [2]. It is thus of interest to create reversible ordering of gold nanoparticles as their localized surface plasmon properties make them appealing for applications like bio-sensing or metamaterials.In this work, we study two kinds of particles: gold nanorods (GNRs) dispersed in water with an aspect ratio AR ≈10 (Φ=10nm) and new ultrathin gold nanowires (GNWs) dispersed in hexane with an AR up to 1000 (Φ=1.5nm) [3].With our recently developed electro-optic setup [1], we measure the birefringence induced in the sample by short pulses of high-frequency electric field. We study also the field-induced dichroism of the suspensions, using the same sample, sealed in a flat optical glass capillary, and the same experimental setup, mounted either on a polarizing microscope or on a UV-Vis spectrophotometer. In our experimental setup, the electrodes are placed outside the sample. In this configuration, they do not interact with the suspension avoiding electrophoretic effect.We observed alignment of both GNRs and GNWs along the applied electric field. Time responses of GNRs to the electric field are of the order of the millisecond and of more than 10 ms for GNWs. The saturated voltage is 4 times smaller in this last case compared to GNRs. Those results are in agreement with a change in the aspect ratio. In the visible range, birefringence appears negative as theoretically predicted [2]. More surprising, in both cases, a positive linear dichroism is revealed in the optical visible range. On the contrary, we expect a negative dichroism (ie extinction higher for polarization perpendicular to the electric field), while in this frequency range we are sensitive to the transverse mode of the plasmon band [4]. Spectroscopic measurements of the absorbance confirm this result but show in addition that the longitudinal mode is well polarized along the electric field: absorbance increases (decreasing) with the applied electric field for a polarization parallel (perpendicular) to this field. This suggests that the transverse mode is less sensitive to polarization than the longitudinal one (figure 1). This may arise from partial alignment causing light depolarization.Figure : 1. Cette thèse s'intéresse à améliorer les codeurs vidéo actuels tels que H.264 MPEG-4/AVC en utilisant avantageusement des informations structurelles contenues dans les images codées. Dans ce contexte, on observe que tous codeurs vidéo utilisent une étape de transformation permettant de décorréler mathématiquement les informations traitées afin d'en diminuer le coût de codage entropique. D'autre part, on remarque que toutes les images traitées qu'elles soient prédites ou non, possèdent des structures géométriques très marquées. Une étude des transformées existantes et possibles pour ces codeurs vidéo montre que peu d'entre elles (DCT, en ondelettes, à recouvrement, DCT sous forme lifting...) permettent de représenter efficacement ces structures géométriques des images. L'état de l'art de ces transformées exploitant les structures géométriques est porté historiquement par les ondelettes de seconde génération comme les contourlets, les bandelettes ou les directionlets. Mais, plusieurs études plus récentes utilisent des approches DCT, basées blocs, avec des orientations afin de mieux représenter ces structures géométriques. L'objectif de notre étude est d'améliorer le codage des images résiduelles H.264/AVC, issues de prédictions spatiales (Intra) ou temporelles (Inter), en utilisant leurs structures géométriques. Une première approche de ce travail de thèse nous a conduits à analyser et exploiter des méthodes connues de l'état de l'art. Pour cela, nous avons déni un schéma sous forme lifting réalisant les opérations de la DCT H.264/AVC. Ce schéma permet de voir cette DCT comme une transformée en ondelettes et donc de disposer d'une approche commune. On peut alors lui appliquer des outils de seconde génération an qu'elle représente au mieux les structures géométriques des images. Et, une version de transformée à recouvrement en pré- et post-traitements a été utilisée dans le codeur H.264/AVC. Une seconde approche de nos recherches a été de dénir une méthode d'orientation par pré- et post-traitements associée à un parcours adapté des coefficients quantifiés produits. Le pré-traitement de cette méthode d'orientation réalise des pseudo-rotations permettant de redresser les blocs des images vers l'horizontale ou la verticale. Cette opération est réalisée par cisaillements, soit par permutations circulaires des pixels, améliorant la décorrélation de la DCT qui suit sans présenter les défauts inhérents aux approches de l'état de l'art. Cette méthode, insérée dans un codeur H.264/AVC, présente de bonnes performances de codage. Cependant, le coût des informations d'orientation, sélectionnées selon un critère débit-distorsion, est élevé dégradant ces performances dans les bas débits, la méthode restant plus efficace que H.264/AVC dans les hauts débits (QP < 30). Les coefficients quantifiés issus de la méthode d'orientation précédente sont ensuite parcourus à la verticale, à l'horizontale ou en zigzag suivant les redressements appliqués ou le type de partitions. Cette adaptation de parcours permet de légèrement conserver du débit améliorant ainsi notre méthode globale qui devient plus efficaces que H.264/AVC dans les moyens débits (QP < 35).

Current American Thoracic Society (ATS) community-acquired pneumonia treatment guidelines recommend azithromycin monotherapy for a limited subset of hospitalized patients. We evaluated the effectiveness of azithromycin monotherapy in a more generalized population of patients hospitalized with mild-to-moderate community-acquired pneumonia.We reviewed medical records from a Veterans Affairs facility for patients admitted with community-acquired pneumonia between December 1, 1997, and June 30, 2001, comparing those receiving azithromycin monotherapy, other ATS-recommended antibiotics, and non-ATS-recommended antibiotics. We excluded patients with immunosuppression, metastatic cancer, or hospital-acquired pneumonia. Outcome measures included times to stability, meeting criteria for change to oral therapy, and eligibility for discharge; length of stay; intensive care unit transfer; and mortality. Outcomes were adjusted for pneumonia severity, skilled nursing facility status, and processes of care.A total of 442 patients were eligible for the study (221 in the azithromycin monotherapy group, 129 in the ATS group, and 92 in the non-ATS group). Times to clinical stability and to fulfilling early switch criteria were not statistically significantly different among the 3 groups. Mean time to fulfilling early discharge criteria was 2.48 days for patients receiving azithromycin monotherapy vs 2.84 days for those receiving ATS antibiotics (P =.008) and 2.58 days for those receiving non-ATS antibiotics (P =.64). Overall mean length of stay was shorter in the azithromycin monotherapy group (4.35 days) vs the ATS (5.73 days) (P =.002) and non-ATS (6.21 days) (P.001) groups. Mortality, intensive care unit transfer, and readmission rates were similar across the groups.Azithromycin monotherapy is equally as efficacious as other ATS-recommended regimens for treating hospitalized patients with mild-to-moderate community-acquired pneumonia.
