The purpose of our integrative review was to synthesize the literature examining relationships between depressive symptoms, silencing the self (STS), sexual relationship power (SRP), and HIV vulnerability amongwomenin the United States. Literature searches were conducted through CINAHL, MEDLINE, PsycINFO, SCOPUS, Sociological Abstracts, and SocINDEX. Integrative review methodology of Whittemore and Knafl guided the review process, and 37 articles met inclusion criteria. The Theory of Gender and Power, modified by Wingood and DiClemente, was the guiding framework to organize and synthesize findings. Primary findings suggest that depressive symptoms, STS, and SRP in relationships may individually influence women's vulnerability for acquiring HIV, yet research lags behind. Evidence documenting relationships between these factors is insufficient to draw generalizable conclusions. Findings suggest that the current literature on this topic does not reflect those women most highly affected by HIV and those who identify as African American or Black in the Southeast region of the United States. Purpose: The purpose of this qualitative study was to discover rural family dementia caregivers' lived experience in a virtual dementia simulation and how it affected their understanding of their family members' daily challenges. To our knowledge, this study is the first to document its impact and value in this understudied, under-reported, and under-represented population. Sample: A volunteer sample of 10 rural family caregivers of persons with AD/ADRD following participation in a rural eastern North Carolina community delivered AD/ADRD simulation. Method: In-depth interviews lasting 45-110 minutes were conducted. The Utrecht phenomenology analytic method was used to sort, analyze, and interpret data. Richness of data drove the sample size. Results: The volunteer participants were female familial caregivers ages ranged from 49 to 81. Their care recipients' ages ranged from 62 to 93 years. Eighty percent of the caregivers provided care for their loved ones 6-7 days a week. Only one caregiver had formal (paid) assistance for care provision. The themes Now I Understand, Opened My Eyes, and Making Changes emerged from the participants' statements. Conclusion: Rural family caregivers found the AD/ADRD simulation profoundly impacted their perceptions of AD/ADRD and expressed new understandings and eye-opening epiphanies about their family members' daily challenges with dementia and their planned changes to improve their loved ones' quality of life. The AD/ADRD simulated experience provided participants with a unique opportunity for self-discovery about their loved ones' daily challenges. This original study addresses the paucity of literature and research about AD/ADRD simulations for rural caregivers of persons with AD/ADRD. This study further demonstrates the value of AD/ADRD simulation to rural nursing practice and science. These findings may prompt rural health nurses to encourage family caregiver participation in person-in-context simulations to enhance their understanding of the loved ones' lived experience. The improvement of early intervention is a tremendous need, especially for older adults who are isolated in rural areas. Nurses were deployed across 41 counties in eastern North Carolina to increase access to screening and improve the possibility of early intervention for rural community-dwelling older adults. The screenings identified at-risk older adults, who nurses then educated and referred for early intervention of falls risk, depression, and cognitive impairment. Through this grassroots intervention, nurses provided community-dwelling older adults with resources for early detection, early intervention, and cost savings. Improving problem-solving skills and expertise in complex clinical care provision requires engaging students in the learning process--a challenging goal when clinical practicums and supervisors are limited. High-fidelity simulation has created many new opportunities for educating healthcare professionals. Because addressing malodorous wounds is a common problem that may be difficult to "teach," a descriptive, quasi-experimental simulation study was conducted. Following completion of a wound care simulation and Laerdal's Simulation Experience Evaluation Tool by 137 undergraduate nursing students, 50 control subjects were randomly selected and 49 volunteer students (experimental group) participated in a wound care simulation after one of three cheeses with a strong odor was added to simulate a malodorous wound. Compared to the control group, study group responses were significantly better (P <0.001) for eight of the 12 survey variables tested and indicated the addition of odor was beneficial in enhancing the perceived realism and value of the simulation. Students responded that the addition of odor in the simulation laboratory improved realism and they felt better prepared to handle malodorous wounds in a clinical setting. An unanticipated outcome was the enhanced feeling of involvement associated with paired care teams as opposed to working in larger groups. The results of this study indicate that wound care education outcomes improve when nursing students are able to practice using a multi-sensorial wound care simulation model.
