5. The effect of a pre- and post-operative exercise program versus standard care on physical activity and sedentary behavior of patients with esophageal and gastric cancer undergoing neoadjuvant treatment prior to surgery (the PERIOP-OG Trial): a randomized controlled trial

Neoadjuvant cancer treatment (NCT) reduces both physical fitness and physical activity (PA) levels, which can increase the risk of adverse outcomes in cancer patients. This study aims to determine the effect of exercise prehabilitation on PA and sedentary behavior (SB) in patients undergoing NCT and surgery for esophagogastric malignancies. This study is a randomized pragmatic controlled multi-center trial conducted across three Irish hospitals. Participants were aged ≥18 years scheduled for esophagectomy or gastrectomy and were planned for NCT and surgery. Participants were randomized to an exercise prehabilitation group (EX) that commenced following cancer diagnosis, continued to the point of surgery, and resumed following recovery from surgery for 6 weeks or to usual care (UC) who received routine treatment. The primary outcome measures were PA and SB. Between March 2019 and December 2020, 71 participants were recruited: EX (n = 36) or UC (n = 35). No significant differences were found between the EX group and UC group on levels of PA or SBs across all measured timepoints. Significant decreases in moderate–vigorous physical activity levels (MVPAs) were found between baseline and post-surgery (P = 0.028), pre-surgery and post-surgery (P = 0.001) and pre-surgery and 6-week follow-up (P = 0.022) for all participants. Step count also significantly decreased between pre-surgery and post-surgery (P < 0.001). Baseline aerobic fitness was positively associated to PA levels and negatively associated with SB. Esophagogastric cancer patients have lower than recommended levels of PA at the time of diagnosis and this decreased further following completion of NCT. An optional home- or group-based exercise intervention was not effective in improving PA levels or behaviors across the cancer treatment journey. [ABSTRACT FROM AUTHOR]

There is currently no consensus as to how to manage esophageal anastomotic leaks. Intervention with endoscopic vacuum-assisted closure (EVAC), stenting, reoperation, and conservative management have all been mooted as potential options. To conduct a systematic review and network meta-analysis (NMA) to evaluate the optimal management strategy for esophageal anastomotic leaks. A systematic review was performed as per the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines with extension for NMA. NMA was performed using R packages and Shiny. In total, 12 retrospective studies were included, which included 511 patients. Of the 449 patients for whom data regarding sex was available, 371 (82.6%) were male, 78 (17.4%) were female. The average age of patients was 62.6 years (standard deviation 10.2). The stenting cohort included 245 (47.9%) patients. The EVAC cohort included 123 (24.1%) patients. The conservative cohort included 87 (17.0%) patients. The reoperation cohort included 56 (10.9%) patients. EVAC had a significantly decreased complication rate compared to stenting (odds ratio 0.23 95%, confidence interval [CI] 0.09;0.58). EVAC had a significantly lower mortality rate than stenting (odds ratio 0.43, 95% CI 0.21; 0.87). Reoperation was used in significantly larger leaks than stenting (mean difference 14.66, 95% CI 4.61;24.70). The growing use of EVAC as a first-line intervention in esophageal anastomotic leaks should continue given its proven effectiveness and significant reduction in both complication and mortality rates. Surgical management is often necessary for significantly larger leaks and will likely remain an effective option in uncontained leaks with systemic features. [ABSTRACT FROM AUTHOR]

Optimal pain control following esophagectomy remains a topic of contention. The aim was to perform a systematic review and network meta-analysis (NMA) of randomized clinical trials (RCTs) evaluating the analgesia strategies post-esophagectomy. A NMA was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)-NMA guidelines. Statistical analysis was performed using Shiny and R. Fourteen RCTs which included 565 patients and assessed nine analgesia techniques were included. Relative to systemic opioids, thoracic epidural analgesia (TEA) significantly reduced static pain scores at 24 hours post-operatively (mean difference (MD): −13.73, 95% Confidence Interval (CI): −27.01–0.45) (n = 424, 12 RCTs). Intrapleural analgesia (IPA) demonstrated the best efficacy for static (MD: −36.2, 95% CI: −61.44–10.96) (n = 569, 15 RCTs) and dynamic (MD: −42.90, 95% CI: −68.42–17.38) (n = 444, 11 RCTs) pain scores at 48 hours. TEA also significantly reduced static (MD: −13.05, 95% CI: −22.74–3.36) and dynamic (MD: −18.08, 95% CI: −31.70–4.40) pain scores at 48 hours post-operatively, as well as reducing opioid consumption at 24 hours (MD: −33.20, 95% CI: −60.57–5.83) and 48 hours (MD: −42.66, 95% CI: −59.45–25.88). Moreover, TEA significantly shortened intensive care unit (ICU) stays (MD: −5.00, 95% CI: −6.82–3.18) and time to extubation (MD: −4.40, 95% CI: −5.91–2.89) while increased post-operative forced vital capacity (MD: 9.89, 95% CI: 0.91–18.87) and forced expiratory volume (MD: 13.87, 95% CI: 0.87–26.87). TEA provides optimal pain control and improved post-operative respiratory function in patients post-esophagectomy, reducing ICU stays, one of the benchmarks of improved post-operative recovery. 25. A study of the effectiveness of an explicit information processing technique (IKR) for interpreting examination essay questions at university level

This thesis presents the results of fourteen experiments designed to test the effect of an explicit information processing technique (IKR) on students' interpreting of examination essay questions. The experiments which took place in the University of Glasgow from February 1990 till December 1992, involved undergraduate students in civil engineering and chemistry, post-graduate students in adult education, and graduate civil engineers preparing for their professional examination. Chapter One provides an extensive report on some of the existing knowledge about essay-writing, and explains how the researcher developed the "Instruction Key Words, Knowledge Key Words and Relevance Key Words" technique (IKR) for interpreting essay questions. In addition, a summary of current information processing theory is given, and IKR explained in terms of it. Some of the experiments reported in this thesis examined the effect of IKR on students': essay examination results; abilities to identify correctly the number of parts required in the answer to questions; criteria for deciding on the number of parts; perceptions of question difficulty when questions contain statements and when questions vary in length; understandings of what different instruction key words (IKWs) entail and the relative difficulty in terms of mental effort that different IKWs imply; criteria for deciding that one question entails a more difficult answer than another; perceptions of the difficulty they experience in undertaking question interpretation exercises; times taken to complete various interpretation exercises; quality of descriptions when asked to describe how they would answer a question if asked to do so; and, time taken to complete interpretation exercises. In general, the results show that IKR enhances the information processing skills (question interpretation skills) of many students. A few individuals show considerable improvement. This is evident in the increase of correct responses after a workshop on IKR and in the change of perceptions away from those that will mislead students, to those that are likely to give more accurate interpretations. However, in several experiments it was found that 20% - 30% of students still gave erroneous responses even after an IKR workshop, indicating that more than one workshop is necessary to alter deep-seated traditional perceptions held by some students. Some of the more revealing findings were that before a workshop on IKR many students: thought (erroneously) that longer questions and questions containing statements were easier because they provided information to help with the answer; thought (erroneously) that vague, less specific, less direct questions were easier because they allowed one to "waffle", and to use one's imagination and personal experiences to make up for lack of knowledge; had an intuitive understanding of the different degrees of mental effort required by different instruction key words; perceived that of sixteen instruction key words, the five most difficult were "analyse", "assess", "evaluate", "justify" and "compare & contrast", with "analyse" being perceived as particularly difficult; have unjustified and erroneous perceptions of what some instruction key words require them to do. Considerable information is presented on how students perceived question difficulty, and the researcher has been able to construct a list of sixty one classifications into which students' perceptions of difficulty can be grouped. It has also been possible to provide a ranking of sixteen instruction words according to the degree of inherent difficulty which students perceive in them. It has also been possible to provide a list of thirty two classifications into which students' reasons for identifying a particular number of parts in a questions, could be grouped. In Chapter Twelve, examiners are offered six recommendations which could contribute to the designing of more effective essay questions and in turn, to students providing more adequate answers to essay questions. Eight recommendations are offered for future research, including for example, further tests on whether enhanced information processing skills leads to enhanced performance in essay examinations. These recommendations for further research reflect the pioneering nature of some of the experiments, the researcher not having encountered similar work in the literature. It is recommended in Chapter Twelve that lecturers introduce their students to IKR and eight guidelines are offered for the successful teaching of IKR. This recommendation is based on the results of this research. It is also offered on the grounds of IKR being a more effective way (than traditional scribing-out of essays in full) of developing the thinking skills necessary for solving complex problems in students' personal and professional lives.

Drought and high evapotranspiration demands can jeopardise trees and shrubs in windbreaks and habitat corridors, where they are more exposed to the effects of extreme weather than in the forest. This study utilised chlorophyll fluorescence to assess how the leaf-level physiological processes of 13 woody species typically planted in Czech habitat corridors responded to the effects of naturally occurring drought and their ability to recover after rain. Linear electron flow (LEF) responded only weakly to the drought, indicating high levels of photorespiration. Trees and shrubs increased the proportion of energy which was dissipated in a harmless way (Φ
NPQ ) during drought and decreased the proportion of energy dissipated through non-regulated processes (ΦNO ). In this way, they reduced processes potentially leading to the production of reactive oxygen species. All species except Tilia cordata Mill. maintained high ΦNPQ even after its release from drought. Tilia cordata was potentially the most susceptible tree to drought due to its low LEF and high ΦNO . The most drought-resistant tree species appeared to be Acer campestre L. and shrubs such as Prunus spinosa L., Viburnum lantana L, and Crataegus monogyna L. These shrubs may be planted at the sunny edges of habitat corridors. The woody species identified as resistant to drought in habitat corridors may also be considered resistant in a warming climate or suitable for planting in the urban environment which is generally warmer and drier than in a forest. - Abstract
Work safety in the forestry industry, where chainsaws are used for tree felling, continues to be a top priority. The mobility of workers involved in chainsaw operations between Europe and Asia has become more common in today’s global workplace. Therefore, sharing knowledge about the types of work safety issues found in both regions can be beneficial. Increased knowledge and safety awareness in the workplace can contribute to a reduction in chainsaw accidents. This paper identifies and addresses four key related areas, namely: regulatory frameworks; chainsaw accidents; personal protective equipment and chainsaw training. Information for both regions was evaluated via interviews, questionnaires, direct observation, desk studies, field studies and descriptive statistical analysis. A total of 234 participants responded to the main research questionnaire, which resulted in data analysis of significant questions related to the use of personal protective equipment (PPE) and possible causes of accidents. Key findings included a need for more information relating to the effect of regulations in individual countries and chainsaw accident and fatality statistics within Asia. A requirement for further research into the suitability of PPE used in Asia was identified. Inadequate training was seen as a primary factor causing accidents in Asia, while in Europe, it was due to chainsaw operators taking shortcuts. Inadequate workplace supervision and a lack of uniform and affordable training provision were common issues identified within both regions. Field tests carried out in accordance with the International/European chainsaw (ICC/ECC) qualification standards of the »non-profit« Awarding Body Association (ABA) International were successful in demonstrating the benefits of uniform training to participants in Europe and Asia. BACKGROUND: The School Health Policies and Programs Study (SHPPS) 2006 examined 8 components of school health programs: health education, physical education and activity, health services, mental health and social services, nutrition services, healthy and safe school environment, faculty and staff health promotion, and family and community involvement. All 8 components were assessed at the state, district, and school levels. Two components, health education and physical education and activity, also were assessed at the classroom level. METHODS: Computer-assisted telephone interviews or self-administered mail questionnaires were completed by state education agency personnel in all 50 states plus the District of Columbia and among a nationally representative sample of school districts (n = 538). Computer-assisted personal interviews were conducted with personnel in a nationally representative sample of elementary, middle, and high schools (n = 1103), with a nationally representative sample of teachers of required health education classes or courses (n = 912), and with a nationally representative sample of teachers of required physical education classes or courses (n = 1194). RESULTS: This article provides a detailed description of the development of the questionnaires; sampling; data collection; and data cleaning, weighting, and analysis. CONCLUSIONS: SHPPS 2006 is the largest and most comprehensive study of school health programs ever conducted. Fielding a study of this magnitude provides many challenges, and several recommendations for future studies emerged from the experience. Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p p Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.
