- Abstract
Reported COVID-19 deaths in Germany are relatively low as compared to many European countries. Among the several explanations proposed, an early and large testing of the population was put forward. Most current debates on COVID-19 focus on the differences among countries, but little attention has been given to regional differences and diet. The low-death rate European countries (e.g. Austria, Baltic States, Czech Republic, Finland, Norway, Poland, Slovakia) have used different quarantine and/or confinement times and methods and none have performed as many early tests as Germany. Among other factors that may be significant are the dietary habits. It seems that some foods largely used in these countries may reduce angiotensin-converting enzyme activity or are anti-oxidants. Among the many possible areas of research, it might be important to understand diet and angiotensin-converting enzyme-2 (ACE2) levels in populations with different COVID-19 death rates since dietary interventions may be of great benefit.

- Abstract
The pharmacological management of asthma has changed considerably in recent decades, as it has come to be understood that it is a complex, heterogeneous disease with different phenotypes and endotypes. It is now clear that the goal of asthma treatment should be to achieve and maintain control of the disease, as well as to minimize the risks (of exacerbations, disease instability, accelerated loss of lung function, and adverse treatment effects). That requires an approach that is personalized in terms of the pharmacological treatment, patient education, written action plan, training in correct inhaler use, and review of the inhaler technique at each office visit. A panel of 22 pulmonologists was invited to perform a critical review of recent evidence of pharmacological treatment of asthma and to prepare this set of recommendations, a treatment guide tailored to use in Brazil. The topics or questions related to the most significant changes in concepts, and consequently in the management of asthma in clinical practice, were chosen by a panel of experts. To formulate these recommendations, we asked each expert to perform a critical review of a topic or to respond to a question, on the basis of evidence in the literature. In a second phase, three experts discussed and structured all texts submitted by the others. That was followed by a third phase, in which all of the experts reviewed and discussed each recommendation. These recommendations, which are intended for physicians involved in the treatment of asthma, apply to asthma patients of all ages. [ABSTRACT FROM AUTHOR]

O quadro clínico associado com a doença pulmonar obstrutiva crônica (DPOC), em especial o sintoma de dispneia, é altamente variável de indivíduo para indivíduo em função da complexa interação entre as alterações sistêmicas e pulmonares e da própria percepção de cada paciente. As consequências múltiplas desta patologia que levam a dispneia, limitação da capacidade de exercício, exacerbações, baixa qualidade de vida relacionada à saúde e alta taxa de morbimortalidade frequentemente não apresentam relação direta com as variáveis da espirometria, técnica de função pulmonar utilizada amplamente no diagnóstico e acompanhamento destes pacientes. Sendo a melhora nos desfechos centrados nos pacientes, em especial sintomas de dispneia e qualidade de vida, um dos alvos principais no tratamento destes indivíduos, a identificação de um método objetivo de avaliação da função pulmonar que melhor reflita estes desfechos é altamente desejável, tanto na clínica diária quanto em pesquisas, para que sua subjetividade seja comparada com parâmetros objetivos de avaliação de gravidade e resposta ao tratamento. O sistema de oscilometria de impulsos (IOS) é uma técnica ainda pouco difundida e estudada de função pulmonar que avalia a resistência e a reatância do sistema respiratório em resposta a várias frequências oscilatórias, permitindo avaliação diferenciada das pequenas e grandes vias aéreas separadamente, com as vantagens de ser de fácil realização e não necessitar de manobras forçadas, despontando como uma técnica de grande potencial na avaliação da função pulmonar destes pacientes. Esta dissertação é composta por uma fundamentação teórica e um artigo original. Na primeira são abordados aspectos inerentes ao tema sobre a DPOC como sua definição, fisiopatologia, diagnóstico, tratamento e monitorização da resposta ao tratamento, destacando a utilidade do IOS neste contexto. Foram selecionados os trabalhos mais relevantes a partir de pesquisa bibliográfica utilizando os seguintes unitermos isolados ou em combinação: doença pulmonar obstrutiva crônica (dpoc) (copd), oscilometria de impulso (impulse oscillometry), função pulmonar (lung function). Os bancos de dados pesquisados foram MEDLINE/Pubmed, Scielo, LILACS. No artigo original 51 pacientes com DPOC estável em tratamento regular foram submetidos a avaliação por meio do IOS, espirometria e feita a correlação das variáveis do IOS com o VEF1 e o FEF25-75% e de todas as variáveis do IOS e do VEF1 com os escores CAT (Copd Assessment Test) que avalia a qualidade de vida relacionada a saúde nos pacientes com DPOC e com a escala do mMRC (escala modificada de avaliação da dispneia do Medical Research Council). Correlações estatisticamente significantes foram encontradas entre os desfechos centrados nos pacientes e as variáveis do IOS, ocorrendo também correlação significativa entre as variáveis do IOS estudadas e o VEF1. Concluímos que os parâmetros do IOS podem ser facilmente obtidos, avaliam a função pulmonar de forma mais detalhada que a espirometria e têm associação com a qualidade de vida relacionada a saúde e dispneia nos pacientes com DPOC. The clinical picture associated with the chronic obstructive pulmonary disease (COPD), particularly dyspnea, varies a lot between patients as a result of the complex interactions between systemic and pulmonary changes induced by the disease and also due to individual perception of symptoms. The multiple consequences of this pathology that leads to breathlessness, exercise capacity limitation, exacerbations, poor health related quality of life and high mortality rate often have no direct relation to the variables measured by spirometry, a lung function technique widely used for the diagnosis and monitoring of these patients. As the improvement in patients focused outcomes, especially symptoms of dyspnea and quality of life, one of the main targets in the treatment of these individuals, the identification of an objective assessment of lung function that best reflects these outcomes is highly desirable, both in daily practice and in research, as their subjectivity could be compared to objective parameters in assessing severity and response to treatment. The impulse oscillometry system (IOS) is a pulmonary function evaluation technique still not widely disseminated that measures the resistance and reactance of the respiratory system in response to various oscillatory frequencies, allowing differentiated assessment of small and large airways separately. It has the advantages that needs little patient’s cooperation and does not require forced maneuvers. IOS is emerging as a technique of great potential in these patients lung function evaluation. This thesis consists of a theoretical grounding and an original article. In the first it is addressed aspects inherent to COPD such as definition, pathophysiology, diagnosis, treatment and monitoring response to treatment, highlighting the utility of IOS in this context. We selected the most relevant papers from the literature and the search adopted the following key words alone or in combination: chronic obstructive pulmonary disease (copd), impulse oscillometry, lung function. Databases searched were MEDLINE / PubMed, SciELO, LILACS. For the original article were studied 51 patients with stable COPD on regular treatment that underwent evaluation by the IOS (R5Hz, R5Hz-R20Hz, Fres and X5Hz) and spirometry (FEV1 and FEF 25%-75%) and correlation between IOS variables with FEV1 and between these two and the Copd Assessment Test scores (CAT) that evaluates the health related quality of life in patients with COPD and the modified scale of dyspnea assessment of the Medical Research Council (mMRC) were done. Statistically significant correlations were found between patient-centered outcomes (CAT and mMRC) and the IOS variables, also significant correlations were found between the studied IOS variables and FEV1. We conclude that the IOS parameters can be easily measured, offer more detail in lung function assessment than spirometry and have good correlation with health related quality of life and dyspnea in patients with COPD.

- Abstract
Objective: To compare BCG vaccination involving a single intradermal dose and that involving multiple doses, one given with the multiple puncture technique, in terms of the protective effect against the subsequent onset of asthma., Methods: A retrospective cohort study involving 2,311 individuals having received the BCG vaccine. The patients were classified according to the number of doses of BCG vaccine administered (one; two; or three or more). The minimum follow-up period in order to determine whether there was any protective effect of BCG vaccination regarding the diagnosis of asthma was 10 years., Results: The sample included 1,317 individuals (56.99%) who had received only one BCG dose, 644 (27.87%) who had received two doses, and 350 (15.14%) who had received three or more doses. The number of patients diagnosed with asthma among those who had received one dose, two doses and three or more doses was, respectively, 216 (16.40%), 107 (16.61%) and 50 (14.28%). There were no significant differences among the groups., Conclusions: In the study sample, the prevalence of asthma among individuals having received multiple doses of the BCG vaccine was no different than that observed among those having received a single dose.

- Abstract
The diagnosis of active infection in bancroftian filariasis continues to pose an important and continuously evolving challenge to filariasis-endemic countries and to health personnel. Sensitivity of the immunochromatographic card test (ICT) relative to detection of adult Wuchereria bancrofti worms by ultrasound was evaluated in a retrospective study conducted in the Center for Teaching, Research and Tertiary Referral Hospital for bancroftian filariasis (Federal University of Pernambuco) in Recife, Brazil. The results showed that among 408 persons tested, the overall sensitivity of the ICT was 84.5% and varied from 52% to 100% when patients were grouped by different criteria (age, sex, presence or absence of living adult worms by ultrasound, microfilaremia status/density). The present study provides evidence that a negative antigen result should be interpreted cautiously and may help to explain the different sensitivities of the antigen test found by different investigators in settings with different transmission intensities.

- Abstract
To describe the principal evidence in the literature regarding the interrelationship among helminth infections, atopy, and asthma, a nonsystematic review of the literature was conducted. Among the publications on the subject, we found a number in which there was controversy regarding the capacity of geohelminth infections to inhibit responsiveness to skin allergy tests and to minimize the symptoms of allergic diseases. However, although small in number, studies of patients infected with Schistosoma spp. suggest that these helminths can inhibit the responsiveness to skin allergy testing and minimize asthma symptoms. Evidence provided by in vitro studies suggests that helminthiases inhibit T helper 1- and T helper 2-type immune responses. This opens new therapeutic possibilities for the treatment of immune system diseases.

Background: Benzalkonium chloride (BKC) has been considered an innocuous preservative for prescription drugs., Methods: We performed a double-blind, placebo-controlled, randomized, crossover, single-center trial with a 3-week washout period in 43 healthy volunteers comparing the effect of 3-week use of saline nasal spray containing BKC 0.01% to preservative-free saline t.i.d. on nasal mucociliary clearance rate. Evaluations were done at the beginning and the end of each period by gamma-scintigraphy with technetium99m-labeled strontium., Results: Nasal mucociliary clearance rate was significantly impaired by BKC with a difference of 1.23 mm x min(-1) (p < 0.01) between periods., Conclusion: BKC in the concentration used in nasal preparations impaired mucociliary clearance in healthy individuals after 3 weeks of use. Presently, when preservative-free alternatives are available, BKC could be a risk without benefit.
