Naturally processed peptides, obtained by acid extraction of tumor cells, contain Ags able to activate specific CTL in vitro. We recently reported that the nonprofessional APC, RMA-S, expressing the B7.1 molecule (RMA-S/B7), pulsed with naturally processed peptides from the nonimmunogenic B16F1 melanoma (B16F1a.e.) primed syngenic CD8+ T cells against the tumor in vitro. Here, we show the rejection of B16F1 melanoma by C57BL/6 mice after immunization with RMA-S/B7 cells pulsed with B16F1a.e. This response is critically dependent on both CD4+ and CD8+ cells, but not on NK cells. However, only CD8+ T cells exert anti-B16F1 cytolitic activity in vitro. Moreover, RMA-S/B7 cells pulsed with B16F1a.e. can be used to prevent the growth of 24-h preestablished melanomas. These results may have important implications for the clinical use of natural peptide fractions of tumor cells as therapeutic cancer vaccines.

The clinical efficacy of SLIT has been established in a number of clinical trials and meta-analyses. However, because SLIT is self-administered by patients without medical supervision, the degree of patient adherence with treatment is still a concern. The objective of this study was to evaluate the perception by allergists of issues related to SLIT adherence.Methods: We performed a questionnaire-based survey of 296 Italian allergists, based on the adherence issues known from previous studies. The perception of importance of each item was assessed by a VAS scale ranging from 0 to 10.Results: Patient perception of clinical efficacy was considered the most important factor (ranked 1 by 54% of allergists), followed by the possibility of reimbursement (ranked 1 by 34%), and by the absence of side effects (ranked 1 by 21%). Patient education, regular follow-up, and ease of use of SLIT were ranked first by less than 20% of allergists.Conclusion: These findings indicate that clinical efficacy, cost, and side effects are perceived as the major issues influencing patient adherence to SLIT, and that further improvement of adherence is likely to be achieved by improving the patient information provided by prescribers.Keywords: adherence, sublingual immunotherapy, efficacy, cost, side effects

The immunogenic Friend-Moloney-Rauscher (FMR) virus-induced tumors have been used extensively to clarify the cellular and molecular mechanisms responsible for tumor rejection and to develop immunotherapeutic strategies. We characterize here the trimolecular complex MHC class I-antigenic determinant-T cell receptor involved in the induction of a protective CTL response against the RMA thymoma. This complex is mainly composed by the D(b) molecule interacting with a Rauscher virus antigen (Ag) determinant and the Vbeta5+ T cell receptor. We also show that the chemically induced EL-4 thymoma acquires the susceptibility to recognition by anti-RMA CTLs and the ability to elicit a protective anti-RMA CTL response only upon infection by a virus of the FMR family and that RMA and FMR virus infected EL-4 cells share tumor-associated Ag. The data strongly support the hypothesis that the high immunogenicity of virus-induced or infected tumors is determined by the expression of immunodominant virus-encoded Ag. The demonstration of a different outcome in the immune responses elicited in the presence or in the absence of viral Ag further open the contention of the molecular requirements for immunogenicity and should stimulate a more careful revision of unexpected cross-reactivity among tumors.

In this work, we investigate the correlation between ragweed pollen concentration and conjunctival, nasal, and asthma symptom severity in patients allergic to ragweed pollen using ambient pollen exposure in the Milan area during the 2014 ragweed season We calculate the pollen/symptom thresholds and we assess the effectiveness of ragweed allergen immunotherapy (AIT). A total of 66 participants allergic to ragweed (Amb a 1) were enrolled in the study and divided into two groups: AIT treated (24) and no AIT treated (42). Pollen counts and daily symptom/medication patient diaries were kept. Autoregressive distributed lag models were used to develop predictive models of daily symptoms and evaluate the short-term effects of temporal variations in pollen concentration on the onset of symptoms. We found significant correlations between ragweed pollen load and the intensity of symptoms for all three symptom categories, both in no AIT treated (τ = 0.341, 0.352, and 0.721; and ρ = 0.48, 0.432, and 0.881; p-value $$\tau$$ τ = 0.46, 0.610, and 0.66; and ρ = 0.692, 0.805, and 0.824; p-value

The present study is an empirical evaluation of a clinical setting from a particular point of view, able to integrate the vocal dimension-used in different fields of psychology as an indicator of rational phenomena-with the linguistic dimension of contents. Starting from the "interpersonal recognition" extracted from the contents of the verbatim transcripts of some diagnostic processes using the Strumento di Valutazione del Riconoscimento Interpersonale (Evaluation Tool for Interpersonal Recognition), the variation of vocal parameters both in the clinician and in the patient were analyzed. The goal consists in identifying possible nonverbal vocal micro indicators used in the dyadic process of interpersonal recognition. Specific and bidimensional nonverbal vocal patterns and strategies are hypothetically matched with different efficacy levels of recognition, both for the clinician and the patient.

Aim. Eating disorders (EDs) are complex conditions associated with disability and a high rate of mortality. Typical characteristics of these diseases are dissociation, alexithymia and impulse dysregulation, all strategies dealing with negative emotions and regulate negative affect and anxiety. Our study aimed to assess the effectiveness of intensive psychological treatment for EDs, with particular reference to the above mentioned clinical characteristics. Methods. Eight outpatients with eating disorders in psychotherapeutic treatment were evaluated in two stages after one year (T1 and T2), using the Eating Disorder Inventory II, the Toronto Alexithymia Scale 20, and the Dissociative Experiences Scale. Results.Wilcoxon test showed significant reductions in DES score as well as in two subscales of the EDI-2, Impulse Regulation and Body Dissatisfaction (I-EDI2 and BD-EDI2), while alexithymia levels did not show any difference. Conclusions. We can confirm the effectiveness of psychotherapy in people with EDs as regards dissociative moments, impulsivity and body dissatisfaction. However, alexithymia remains unchanged, possibly because of its deep emotional nature.

La difficoltà che pazienti anoressiche ritrovano nella “talking cure” deriva dal loro peculiare assetto metapsicologico e viene riflesso in un linguaggio spesso artificioso e senza parole per l’esperienza: l’esame del linguaggio può essere un elemento cruciale per capire la patologia e le possibilità di trattamento. I modelli teorici assunti come riferimento comprendono le teorie dell’alessitimia, della Regolazione Affettiva, e la teoria del Codice Multiplo di Bucci. Lo studio ha lo scopo di osservare le modifiche dell’espressione emozionale attraverso il linguaggio nel corso di 61 sedute di psicoterapia di una serie di 8 pazienti con DCA. Le variazioni sono state studiate mediante l’applicazione delle misure italiane computerizzate del processo referenziale - I-DAAP (Italian Discourse Attribute Analyses Program) e l’andamento temporale del pattern narrativo nella successione delle sedute è stato testato mediante l’indice di correlazione di Spearman. Per monitorare regole ed eccezioni sono state effettuate analisi qualitative del campione ponendo attenzione all’andamento dei singoli contributi. Gli indicatori linguistici sottolineano il problema dell’elaborazione degli stati affettivi mostrando che un assetto difensivo razionale inattiva l’emergere degli schemi emotivi dissociati e che difese di intellettualizzazione circoscrivono la narrazione rispetto alla paura di uno stato di sopraffazione emotiva. Il pattern narrativo, poco funzionale rispetto a un progetto evolutivo, risulta efficace se si considera la riduzione immediata del disagio psichico che il soggetto ha l’esigenza di soddisfare. Conoscere l’andamento prototipico del processo psicoterapico aiuta nella gestione degli interventi tecnici, nella regolazione del setting, nella comprensione delle difese e dei momenti di possibile connessione emotiva rispetto agli schemi emotivi dissociati.

Concettualizziamo i disturbi del comportamento alimentare come disturbi dell’autoregolazione, la cui caratteristica fondamentale è una menomazione della regolazione degli affetti: le dinamiche anoressico-bulimiche esprimono tentativi di contenere le emozioni su un piano concreto e la psicoterapia può essere intesa come strumento regolatore, in cui implementare l’integrazione tra cognizione e affetti laddove l’alessitimia ha lasciato aree cieche e dis-associazione tra gli stati del sé. Lo scopo del lavoro è descrivere in una serie di 8 pazienti con DCA in psicoterapia la relazione tra la modificazione dell’espressione emozionale attraverso il linguaggio e la variazione della sintomatologia, degli atteggiamenti relativi ad alimentazione, peso, forma, di tratti psicologici caratteristici e di aspetti relazionali. La valutazione di efficacia è stata effettuata mediante la ripetuta somministrazione di 9 scale psicometriche (TAS20, EDI2, BUT, SCL90, MPS, WDQ, PBI, DES, WAI), mentre il processo è stato studiato con l’applicazione delle misure italiane computerizzate del processo referenziale - I-DAAP a 61 trascritti di sedute. Per tre successivi tempi di valutazione nel corso di un anno è stato calcolato l’indice di correlazione per ranghi di Spearman tra le variabili testate. La lettura qualitativa del processo nel nostro campione ha evidenziato l’interrelazione tra gli aspetti del DCA, l’influenza delle cure parentali sull’alessitimia, il ruolo del perfezionismo, il significato della connessione emotiva e le difese strutturare rispetto ad essa. Le modalità di funzionamento adottate vanno pensate anche nei loro significati economici: nella cura, restituire al paziente una forma di comprensione di sé lo aiuta nel non irrigidire modalità di cui, al momento, necessita. Lavori futuri dovranno valutare se e come il miglioramento della comunicazione emotiva vada inteso come principale focus del trattamento.

Le problematiche del trattamento psicoterapico dei pazienti con DCA sono dovute al peculiare assetto cognitivo, emotivo, comportamentale e personologico generato dalla specificità metapsicologica e alla tendenza al riattivarsi delle angosce di abbandono e intrusione nel contesto di cura. Alla luce di queste difficoltà e dell’importanza di feedback sull’efficacia per migliorare i trattamenti, presentiamo alcuni risultati di uno studio process-outcome su una serie di otto soggetti con DCA. Tra i suoi scopi, il progetto si è interrogato sull’efficacia della psicoterapia proponendone una misurazione: il campione clinico è stato confrontato con un campione controllo non clinico e l’efficacia della psicoterapia è stata valutata in termini di miglioramenti sintomatologici, atteggiamenti relativi ad alimentazione, peso, forma, capacità di lettura emotiva e tratti psicologici caratteristici mediante somministrazione di una batteria di scale psicometriche in tre valutazioni ripetute. Mentre il campione controllo non ha riportato variazioni nel corso dell’osservazione, il campione clinico ha mostrato cambiamenti: si sono osservate una significativa riduzione di momenti dissociativi, ascetismo, impulsività e insoddisfazione corporea e altri trend di miglioramento passibili di una futura significatività. La psicoterapia è efficace: il paziente “impara” l’uso di una qualità linguistica che diviene lo strumento privilegiato per regolare gli affetti, oggettivare la malattia e soggettivare sé. Le caratteristiche più difficilmente modificabili sono quelle cognitive, come rimuginio e perfezionismo; l’alessitimia è rimasta pressoché stabile: sarà necessario verificare l’effettiva stabilità di questi tratti su tempi di cura più lunghi.

Introduction: Since we know the difficulties in the “talking cure” of patients with eating disorders (Farrell, 2000) and the importance of the concept of alexithymia (Taylor, 1996; Bruch, 1973), we present the first results of the study “Alexithymia and emotional regulation in the psychotherapy of EDs patients” taking place at the Specialist services for eating disorders, San Gerardo Hospital, Monza. This project, in the field of psychotherapy research, aims to evaluate, in a series of 8 patients with EDs, changes in emotional expression through the language using the application of Italian computer measures of referential process-I-DAAP (Maskit, Bucci, et al., 2004) - developed on the Bucci’s Multiple Code Theory of Bucci (Bucci, 1997). Through an evaluation of the outcome of the therapeutic process and a qualitative analysis of the same, we will examine the modification and/or the influence of the alexithymic condition in the course of psychotherapy of anorexic-bulimic frameworks. Method: The effectiveness of the therapeutic process will be measured by repeated administration of self-report scales related to symptoms, subjective experience, cognitive and relational aspects. The qualitative study will be made with the I-DAAP which allows to evaluate the expression of emotional content through the overall organization of the sentence and the semantic contents. The sample consists of 8 patients with eating disorders in psychotherapy with three experienced therapists. The collection of data (self-report scales and verbatim transcribed of 8 sessions per patient) will be carried out in three stages. In this first analysis on the collected material at time T1 (psychometric scales and 25 transcripts), we have calculated the indices of correlation (r Pearsons) among the variables tested with the self-report instruments (TAS20 for alexithymia, EDI2 for the characteristics of the eating disorder, SCL90 for general psychopathology, PBI for the representation of parental bonding, BUT for body image disorder, WDQ for brooding, MPS for perfectionism, DES for dissociative episodes, WAI for therapeutic alliance), and between these ones and the average narrative indexes of I-DAAP (RA indices - MIWRAD2, MHIWRAD2, HPIWRAD2 -, Reflection - MIRef, Disfluency - MIDIF, Affect - MIAffP, MIAffN, MIAffZ -, Sensorysomatic MISenS). For all values we have also evaluated the correlation with the index of high-functioning of SWAP200 (AF PD_T, AFQ-T), applied to assess the personality of the patients. Data processing was performed using the statistical program STATA. Results and Discussion: The correlations between the variables measured by self-report scales confirm some expected and confirmed by the literature results. We found: Negative correlation between TAS20 and Maternal Care (PBI) (p =.04) and positive correlation between TAS20 and maternal overprotection (p =.03), confirming the relationship between alexithymia and the parenting style. Because of the important role of the caregiver in the process of emotional regulation, both neglect and overprotection seem to contribute to difficulties of identification and expression of one's emotional experience. This is in agreement with the work of Thorberg (2011) and De Panfilis (2008). A positive correlation between TAS20 and EDI2 Drive for thinness (p =.04) and Interoceptive Awareness (p =.03), confirming the link between lack of emotional expression and the symptoms of eating disorders, as well as a tendency to express misunderstood stimuli by means of the body. This finding is consistent with the one reported by Taylor (1996), Schmidt (1993). A positive correlations between EDI2 and the psychometric scales for the evaluation of brooding, perfectionism and body image disorder, confirming the relationship between the EDs specific traits and the monomorphic tendence induced by the anorexic process on the symptoms and on the psychic functioning. Vogliamo riflettere sull’esperienza di un servizio che, grazie al suo doppio carattere pubblico e universitario, prova a far convergere clinica, teoria e ricerca in tema di psicoterapia. L’invito a fare ricerca è diventato un monito importante perché, per una clinica che ottimizzi i modelli di cura, servono solidi ancoraggi teorici e feedback empirici. Punto di partenza è il bisogno di scardinare il principio per cui la scelta dei trattamenti psicoterapeutici è basata su valutazioni soggettive, opportunità organizzative ed economiche piuttosto che su un giudizio clinico razionalizzato. Si rende, allora, necessario sperimentare modelli clinici che si avvalgano di strumenti innovativi e con un alto livello di affidabilità e trasferibilità, protocolli operazionalizzati che standardizzino le variabili specifiche e aspecifiche responsabili di efficacy ed efficiency, equipe multidisciplinari disposte a scomporre microanaliticamente il loro modo di funzionare per identificare fattori curativi e misure di outcome. Questa interfaccia tra clinica e scientificità struttura la cura come un metodo di lavoro comprovabile. Verranno presentati il protocollo operativo e i primi risultati di efficacia del metodo strutturato a Monza, che pone il focus sulla necessità di implementare tre dimensioni, tra loro in relazione secondo un modello circolare e ricorsivo: assessment diagnostico, valutazione dell’outcome, analisi di processo.

Si vuole studiare in un’ottica dinamica – in vivo – la fluttuazione delle comunicazioni emotive clinico-paziente, analizzare cioè la dimensione regolazione degli affetti a partire dai momenti interattivi della coppia che hanno a che fare con temi emotivi. Questo meccanismo di regolazione interattiva è esaminato in relazione agli effetti prodotti dagli interventi del clinico sulla capacità del paziente di creare una connessione tra i propri stati di arousal fisiologici e viscerali da una parte, e i loro significati simbolici dall’altra parte (Bucci, 2002). Si vuole valutare, all’interno di un setting diagnostico, il livello di sintonizzazione-regolazione affettiva clinico-paziente: studiare, cioè, i cambiamenti emotivi del paziente in relazione agli interventi del clinico e viceverssa, cioè le variazioni tra il primo stato emotivo e il successivo in seguito alla modificazione introdotta dall'intervento di riconoscimento – positivo versus negativo – del clinico. Gli obiettivi della ricerca sono due: - individuare le modalità di comunicazione emotiva (specificità) che caratterizzano i micro-cambiamenti processuali - valutare come il paziente esprime i contenuti emotivi attraverso il linguaggio in relazione alla tecnica (regolatoria) del clinico (co-costruzione di significati e riparazione di quelli dissociati). Le ipotesi da verificare possono essere così schematizzate: - esiste un pattern specifico – diverso ma forse in parte sovrapponibile all’andamento prototipico dei ‘buoni’ colloqui di terapia – di andamento emotivo/affettivo nei colloqui del processo diagnostico (RICORSIVITA’) - esiste una relazione specifica in grado di descrivere l’interazione tra tecnica del clinico e capacità del paziente di regolare le emozioni (esistono degli aspetti ridondanti che fungono da organizzatori della relazione? È possibile identificare pattern ricorrenti di interventi del clinico che facilitano versus ostacolano la regolazione emotiva del paziente?) (BI-DIREZIONALITA’). Verificate queste premesse ci si può poi interrogare sul tema cruciale: l’emergere degli schemi emotivi e la loro comprensione/integrazione – riconoscimento e accettazione – è determinata in misura maggiore, minore o simile dalle due variabili rilevate – componente emotiva/affettiva e componente tecnica?
