2. Breaking New Ground? An Analysis of the Use of Embedded Occupational Therapy in a Postsecondary Education Programme for Students with Intellectual Disabilities

Background: Despite the growing diversity among students in higher education, nonprogression to postsecondary education persists for many students with intellectual disabilities with low completion rates for those who do progress. Research suggests that occupational therapy (OT) support can be extremely beneficial in assisting students. Less is known however about the impact of OT in higher education. This study aims to examine the use of OT embedded in a postsecondary programme for students with intellectual disabilities. Methods: This study uses a scoping review of relevant literature and a qualitative analysis of OT working in a higher education programme specifically designed for students with intellectual disabilities in Ireland. Findings: This paper highlights how the OT support within this programme uses a four-fold collaborative approach supporting students, staff, business partners and wider organisational structures in the university. Conclusion: The analysis highlights the opportunities for collaboration between OT and curriculum developers, teaching and ancillary staff and students in higher education along with the significant OT input into work placements and internships with business partner mentors. Challenges in educating stakeholders to understand OT in the higher education environment would suggest a substantial need for further research.

BACKGROUND: Intravenous fluids and vasopressor agents are commonly used in early resuscitation of patients with sepsis, but comparative data for prioritizing their delivery are limited. METHODS: In an unblinded superiority trial conducted at 60 U.S. centers, we randomly assigned patients to either a restrictive fluid strategy (prioritizing vasopressors and lower intravenous fluid volumes) or a liberal fluid strategy (prioritizing higher volumes of intravenous fluids before vasopressor use) for a 24-hour period. Randomization occurred within 4 hours after a patient met the criteria for sepsis-induced hypotension refractory to initial treatment with 1 to 3 liters of intravenous fluid. We hypothesized that all-cause mortality before discharge home by day 90 (primary outcome) would be lower with a restrictive fluid strategy than with a liberal fluid strategy. Safety was also assessed. RESULTS: A total of 1563 patients were enrolled, with 782 assigned to the restrictive fluid group and 781 to the liberal fluid group. Resuscitation therapies that were administered during the 24-hour protocol period differed between the two groups; less intravenous fluid was administered in the restrictive fluid group than in the liberal fluid group (difference of medians, -2134 ml; 95% confidence interval [CI], -2318 to -1949), whereas the restrictive fluid group had earlier, more prevalent, and longer duration of vasopressor use. Death from any cause before discharge home by day 90 occurred in 109 patients (14.0%) in the restrictive fluid group and in 116 patients (14.9%) in the liberal fluid group (estimated difference, -0.9 percentage points; 95% CI, -4.4 to 2.6; P = 0.61); 5 patients in the restrictive fluid group and 4 patients in the liberal fluid group had their data censored (lost to follow-up). The number of reported serious adverse events was similar in the two groups. CONCLUSIONS: Among patients with sepsis-induced hypotension, the restrictive fluid strategy that was used in this trial did not result in significantly lower (or higher) mortality before discharge home by day 90 than the liberal fluid strategy. Abstract The energy from waves has a vast untapped potential to contribute to renewable energy supply and diversification. For that reason, wave energy conversion systems have been a topical research area in recent years. In particular, harnessing wave energy with an oscillating water column converter has proved to be one suitable solution, which has also seen a number of successful deployments. Nevertheless, additional research is required for this technology in order to reach full commercial maturity and economic performance. This paper proposes an adaptive second order sliding mode controller to maximise the converted energy. In particular, the proposed adaptive control setup maintains the sliding mode robust features, while reducing high frequency oscillations and abrupt control actions produced by fixed‐gain algorithms. A comparison of energy generation performance shows better energy conversion efficiency of the proposed control strategy over standard speed regulation control strategies, even considering air compression dynamics and hydrodynamics in the tests.

Tidal barrage power plants utilise the tidal range variation to generate clean electricity. Although there are several operating tidal barrage schemes around the globe, there is still potential to expand the installed capacity. Given their inherent storage and the high predictability of the tides, tidal barrages can be operated with more flexibility than many other renewables. This means that the control objective of a barrage operation can vary from energy maximisation to constant power output, or demand-matching objectives. The operation of a barrage also influences its impact on the environment and economic activity of the site where it is located, which is a major cause for the slow deployment of such power plants. The aim of this study is to provide a comprehensive and critical analysis of the different strategies considered to date to optimise the operation of tidal barrages, with a focus on an in-depth analysis of the optimisation schemes employed, the barrage models utilised, and opportunities for further improvement.

13. University Students with Intellectual Disabilities: Empowerment through Voice

- Abstract
People with disabilities have been among the most marginalised groups both within society and within post-secondary/higher education. Over the last two decades, an increasing number of inclusive educational programmes have come into existence both nationally and internationally for this group of learners. The Trinity Centre for People with Intellectual Disabilities (TCPID), School of Education, Trinity College Dublin, offers students with intellectual disabilities a two-year programme entitled Arts, Science and Inclusive Applied Practice (ASIAP). This paper presents a selection of voices from ASIAP students which highlights their experiences of becoming both co-researchers and second language learners. These studies present a variety of ways in which power relationships are negotiated between faculty and students through utilising creative and inclusive approaches to the research process.

14. On the Ratio of Reactive to Active Power in Wave Energy Converter Control

Optimal control of wave energy converters (WECs), while converting wave energy into a usable form, such as electricity, may inject (reactive) power into the system at various points in the wave cycle. Though somewhat counter-intuitive, this action usually results in improved overall energy conversion. However, recent experimental results show that, on occasion, reactive power peaks can be significantly in excess of active power levels, leaving device developers with difficult decision in how to rate the power take-off of the system i.e. whether to cater for these high reactive power peaks, or limit power flow to rated (active) levels. The origins of these excessive power peaks are currently poorly understood, creating significant uncertainty in how to deal with them. In this paper, we show that, using both theoretical results and an illustrative simulation case study, under matched controller conditions (impedance-matching optimal condition), for both monochromatic and panchromatic sea-states, that the maximum peak reactive/active power ratio never exceeds unity. However, under mismatched WEC/controller conditions, this peak power ratio can exceed unity, bringing unrealistic demands on the power take-off (PTO) rating. The paper examines the various origins of system/controller mismatch, including modelling error, controller synthesis inaccuracies, and non-ideal PTO behaviour, highlighting the consequences of such errors on reactive power flow levels. This important result points to the need for accurate WEC modeling, while also showing the folly of catering for excessive reactive power peaks.

Objectives: There is a lack of knowledge about the challenges of researchers who continued in-person research during the early phases of the COVID-19 pandemic. Design: Electronic survey assessing work-related exposure to COVID-19, logistical challenges, and procedural changes during the first year of the COVID-19 pandemic on clinical research. Setting: National Heart, Lung, and Blood Institute-sponsored Prevention and Early Treatment of Acute Lung Injury Clinical Trial Network Centers. Subjects: Research staff at research Network Sites. Measurements and Main Results: The 37-question survey was completed by 277 individuals from 24 states between 29 September 2020, and 12 December 2020, yielding a response rate of 37.7%. Most respondents (91.5%) indicated that non-COVID-19 research was affected by COVID-19 research studies. In response to the COVID-19 pandemic, 20% of respondents were reassigned to different roles at their institution. Many survey takers were exposed to COVID-19 (56%), with more than 50% of researchers requiring a COVID-19 test and 8% testing positive. The fear of infection was 2.7-times higher compared to pre-COVID-19 times. Shortages of personal protective equipment were encountered by 34% of respondents, primarily due to lack of access to N95 masks, followed by gowns and protective eyewear. Personal protective equipment reallocation from research to clinical use was reported by 31% of respondents. Most of the respondents (88.5%), despite these logistical challenges, indicated their willingness to enroll COVID-19 patients. Conclusions: During the first year of the COVID-19 pandemic, members of the research network were engaged in COVID-19 research despite logistical challenges, limited access to personal protective equipment, and fear of exposure. The research network’s survey experience can inform ongoing policy discussions to create research enterprises that can dexterously refocus research to address the knowledge gaps associated with novel public health emergencies while mitigating the effect of pandemics on existing research projects and research personnel.

26. Optimizing the Level of Challenge in Stroke Rehabilitation using Iterative Learning Control: a Simulation

The level of challenge in stroke rehabilitation has to be carefully chosen to keep the patient engaged and motivated while not frustrating them. This paper presents a simulation where this level of challenge is automatically optimized using iterative learning control. An iterative learning controller provides a simulated stroke patient with a target task that the patient then learns to execute. Based on the error between the target task and the execution, the controller adjusts the difficulty of the target task for the next trial. The patient is simulated by a nonlinear autoregressive network with exogenous inputs to mimic their sensorimotor system and a second-order model to approximate their elbow joint dynamics. The results of the simulations show that the rehabilitation approach proposed in this paper results in more difficult tasks and a smoother difficulty progression as compared to a rehabilitation approach where the difficulty of the target task is updated according to a threshold., Comment: Accepted at 10th International IEEE EMBS Conference On Neural Engineering (NER'21)

In the same way that Boethius is a prisoner of his mind more so than the cell that confines him in his De consolation philosophiae (c. 524), Hermione in The Winter's Tale (c. 1609-11) is a mind restricted within a body when she chooses to adopt the guise of a statue. The following article situates Hermione's response to her husband's tyrannical treatment within feminist criticism of the play and new research published on its sources in order to argue that Paulina embodies a version of Boethius's Lady Philosophy. The idea of the body as a prison is a major trope underpinning Hermione's miraculous revivification, but she is not the only prisoner released by Paulina's ministrations. Leontes's recalcitrant jealousy and its aftermaths are also mental afflictions from which he needs to be released. Ultimately, Paulina's embodiment of an insubstantial personification allows her to issue a challenge to the authority of a man in a way that makes her an inspiring feminist, and an icon for life-affirming, regenerative friendship between women., At the end of the final scene in The Winter's Tale, Hermione appears as a statue and the audience shares in her family's wonder at her apparently coming to life. We propose in this White Paper a concept for a space experiment using cold atoms to search for ultra-light dark matter, and to detect gravitational waves in the frequency range between the most sensitive ranges of LISA and the terrestrial LIGO/Virgo/KAGRA/INDIGO experiments. This interdisciplinary experiment, called Atomic Experiment for Dark Matter and Gravity Exploration (AEDGE), will also complement other planned searches for dark matter, and exploit synergies with other gravitational wave detectors. We give examples of the extended range of sensitivity to ultra-light dark matter offered by AEDGE, and how its gravitational-wave measurements could explore the assembly of super-massive black holes, first-order phase transitions in the early universe and cosmic strings. AEDGE will be based upon technologies now being developed for terrestrial experiments using cold atoms, and will benefit from the space experience obtained with, e.g., LISA and cold atom experiments in microgravity. This paper is based on a submission (v1) in response to the Call for White Papers for the Voyage 2050 long-term plan in the ESA Science Programme. ESA limited the number of White Paper authors to 30. BackgroundPediatric Intensive Care Unit (PICU) teams provide care for critically ill children with diverse and often complex medical and surgical conditions. Researchers often lack guidance on an approach to select the best outcomes when evaluating this critically ill population. Studies would be enhanced by incorporating multi-stakeholder preferences to better evaluate clinical care. This manuscript outlines the methodology currently being used to develop a PICU Core Outcome Set (COS). This PICU COS utilizes mixed methods, an inclusive stakeholder approach, and a modified Delphi consensus process that will serve as a resource for PICU research programs.MethodsA Scoping Review of the PICU literature evaluating outcomes after pediatric critical illness, a qualitative study interviewing PICU survivors and their parents, and other relevant literature will serve to inform a modified, international Delphi consensus process. The Delphi process will derive a set of minimum domains for evaluation of outcomes of critically ill children and their families. Delphi respondents include researchers, multidisciplinary clinicians, families and former patients, research funding agencies, payors, and advocates. Consensus meetings will refine and finalize the domains of the COS, outline a battery instruments for use in future studies, and prepare for extensive dissemination for broad implementation.DiscussionThe PICU COS will be a guideline resource for investigators to assure that outcomes most important to all stakeholders are considered in PICU clinical research in addition to those deemed most important to individual scientists.Trial registrationCOMET database (http://www.comet-initiative.org/, Record ID 1131, 01/01/18).
