Abstract

The logit transform is arguably the most widely-employed link function beyond linear settings. This transformation routinely appears in regression models for binary data and provides, either explicitly or implicitly, a core building-block within state-of-the-art methodologies for both classification and regression. Its widespread use, combined with the lack of analytical solutions for the optimization of general losses involving the logit transform, still motivates active research in computational statistics. Among the directions explored, a central one has focused on the design of tangent lower bounds for logistic log-likelihoods that can be tractably optimized, while providing a tight approximation of these log-likelihoods. Although progress along these lines has led to the development of effective minorize-maximize (MM) algorithms for point estimation and coordinate ascent variational inference schemes for approximate Bayesian inference under several logit models, the overarching focus in the literature has been on tangent quadratic minorizers. In fact, it is still unclear whether tangent lower bounds sharper than quadratic ones can be derived without undermining the tractability of the resulting minorizer. This article addresses such a challenging question through the design and study of a novel piece-wise quadratic lower bound that uniformly improves any tangent quadratic minorizer, including the sharpest ones, while admitting a direct interpretation in terms of the classical generalized lasso problem. As illustrated in a ridge logistic regression, this unique connection facilitates more effective implementations than those provided by available piece-wise bounds, while improving the convergence speed of quadratic ones.

2. Semantic Gaps in the Theory and the Practice of Physical and Sports Education in the Italian Context

Abstract

Nowadays, we face a profound fragmentation of knowledge, which is addressed in distinctive ways, conforming to the specifics of each field of knowledge and having a specific lexicon and ways to interpret reality. To better understand and communicate the complexity of the reality of motor and sports sciences in Italy, it is necessary to study its ontology based on a holistic and comprehensive approach such as System Thinking (ST). So, this preliminary review aimed to verify the coherence of terminology used by Italian academics (who study methodologies related to teaching) and practical communities (who live the reality of daily teaching) in motor and sports sciences, specifically regarding the didactic of physical and sports education. For this purpose, according to Design Based Research (DBR), a first investigation of the interpretative semantics of the discipline's specific terms was accomplished with the support of multiple sources of information such as documents, databases, and brainstorming performed by experts (representing the guiding team and stakeholders from all considered communities). Moreover, the frequency of some terms and keywords considered in European (ERC) and Italian (CUN) legislation was assessed to contextualize better the impact of common practice community keywords on the scientific and regulatory community. Successively, the studies that analyzed the terms from 2000 to 2020 in the leading scientific search engines (SCOPUS and Web of Science) were also investigated. Results indicate that in the CUN and ERC areas, there is a total absence of the most relevant lemma to the community of practice. Furthermore, significant terms such as play, sports education, psychomotricity, and gymnastics are less considered or unconsidered in the specific scientific and didactic fields. Possible solutions for a specific semantic model to reduce the gap are finally hypothesized and presented.

Abstract

This letter reports the first complete observation of magnetized collisionless shock precursors formed through the compression of Biermann-battery magnetic fields in laser produced plasmas. At OMEGA, lasers produce a supersonic CH plasma flow which is magnetized with Biermann-battery magnetic fields. The plasma flow collides with an unmagnetized hydrogen gas jet plasma to create a magnetized shock precursor. The situation where the flowing plasma carries the magnetic field is similar to the Venusian bow shock. Imaging 2$\omega$ Thomson scattering confirms that the interaction is collisionless and shows density and temperature jumps. Proton radiographs have regions of strong deflections and FLASH magnetohydrodynamic (MHD) simulations show the presence of Biermann fields in the Thomson scattering region. Electrons are accelerated to energies of up to 100 keV in a power-law spectrum. OSIRIS particle-in-cell (PIC) simulations, initialized with measured parameters, show the formation of a magnetized shock precursor and corroborate the experimental observables., Comment: 6 pages, 5 figures

5. Bayesian analysis of product feature allocation models

Abstract

Feature allocation models are an extension of Bayesian nonparametric clustering models, where individuals can share multiple features. We study a broad class of models whose probability distribution has a product form, which includes the popular Indian buffet process. This class plays a prominent role among existing priors, and it shares structural characteristics with Gibbs-type priors in the species sampling framework. We develop a general theory for the entire class, obtaining closed form expressions for the predictive structure and the posterior law of the underlying stochastic process. Additionally, we describe the distribution for the number of features and the number of hitherto unseen features in a future sample, leading to the $\alpha$-diversity for feature models. We also examine notable novel examples, such as mixtures of Indian buffet processes and beta Bernoulli models, where the latter entails a finite random number of features. This methodology finds significant applications in ecology, allowing the estimation of species richness for incidence data, as we demonstrate by analyzing plant diversity in Danish forests and trees in Barro Colorado Island.

Abstract

Induced magnetospheres form around planetary bodies with atmospheres through the interaction of the solar wind with their ionosphere. Induced magnetospheres are highly dependent on the solar wind conditions and have only been studied with single spacecraft missions in the past. This gap in knowledge could be addressed by a multi-spacecraft plasma mission, optimized for studying global spatial and temporal variations in the magnetospheric system around Venus, which hosts the most prominent example of an induced magnetosphere in our solar system. The MVSE mission comprises four satellites, of which three are identical scientific spacecraft, carrying the same suite of instruments probing different regions of the induced magnetosphere and the solar wind simultaneously. The fourth spacecraft is the transfer vehicle which acts as a relay satellite for communications at Venus. In this way, changes in the solar wind conditions and extreme solar events can be observed, and their effects can be quantified as they propagate through the Venusian induced magnetosphere. Additionally, energy transfer in the Venusian induced magnetosphere can be investigated. The scientific payload includes instrumentation to measure the magnetic field, electric field, and ion-electron velocity distributions. This study presents the scientific motivation for the mission as well as requirements and the resulting mission design. 10. Bayesian inference for generalized linear models via quasi-posteriors

Abstract

Generalized linear models (GLMs) are routinely used for modeling relationships between a response variable and a set of covariates. The simple form of a GLM comes with easy interpretability, but also leads to concerns about model misspecification impacting inferential conclusions. A popular semi-parametric solution adopted in the frequentist literature is quasi-likelihood, which improves robustness by only requiring correct specification of the first two moments. We develop a robust approach to Bayesian inference in GLMs through quasi-posterior distributions. We show that quasi-posteriors provide a coherent generalized Bayes inference method, while also approximating so-called coarsened posteriors. In so doing, we obtain new insights into the choice of coarsening parameter. Asymptotically, the quasi-posterior converges in total variation to a normal distribution and has important connections with the loss-likelihood bootstrap posterior. We demonstrate that it is also well-calibrated in terms of frequentist coverage. Moreover, the loss-scale parameter has a clear interpretation as a dispersion, and this leads to a consolidated method of moments estimator.

14. Low Concern Rate With Oral Health and Associated Factors Among Older Persons: A Cross-Sectional Study

Abstract

OBJECTIVE: This study aimed to assess the prevalence of concern with oral health and associated factors among older adults. METHODS: Individuals aged ≥60 years were included from the cities Cruz Alta and Veranópolis, Brazil. Oral health examination and a structured questionnaire were applied in 569 individuals. The main outcome was concern with oral health (yes or no), which was determined by a validated questionnaire. Uni- and multivariate analyses were conducted by Poisson regression with robust variance to assess the association between outcome and independent variables. RESULTS: The prevalence of concern with oral health was 30.58% (n = 174). Non-white individuals presented significantly higher prevalence ratio (PR) of being concerned about their oral health (PR = 1.28; 95% confidence interval [CI] 1.01–1.62). Individuals with medium or high level of education, not retired and without any health problems presented higher PR of being concerned with their oral health when compared to their controls (p < 0.05). Moreover, those that reported toothbrushing frequency ≥ 2 times/day (PR = 1.58; 95%CI 1.01 – 2.48) and non-edentulous (PR = 1.50; 95%CI 1.12–2.01) also presented higher PR of being concern with their oral health. Older adults that were not concerned with tooth alignment (PR = 0.57; 95%CI 0.44 – 0.74) or tooth color (PR = 0.41; 95%CI 0.31 – 0.54) demonstrated lower concern with their oral health. CONCLUSION: Prevalence of concern with oral health among the older adults was low and associated with non-white individuals, medium and high level of education, individuals not retired, absence of health problems, higher toothbrushing frequency, mild temporomandibular disorder (TMD) and non-edentulism. Additionally, concern with esthetical aspects may be related with this outcome.

18. Bayesian nonparametric modeling of latent partitions via Stirling-gamma priors

Abstract

Dirichlet process mixtures are particularly sensitive to the value of the precision parameter controlling the behavior of the latent partition. Randomization of the precision through a prior distribution is a common solution, which leads to more robust inferential procedures. However, existing prior choices do not allow for transparent elicitation, due to the lack of analytical results. We introduce and investigate a novel prior for the Dirichlet process precision, the Stirling-gamma distribution. We study the distributional properties of the induced random partition, with an emphasis on the number of clusters. Our theoretical investigation clarifies the reasons of the improved robustness properties of the proposed prior. Moreover, we show that, under specific choices of its hyperparameters, the Stirling-gamma distribution is conjugate to the random partition of a Dirichlet process. We illustrate with an ecological application the usefulness of our approach for the detection of communities of ant workers.

Abstract

The standard class-incremental continual learning setting assumes a set of tasks seen one after the other in a fixed and predefined order. This is not very realistic in federated learning environments where each client works independently in an asynchronous manner getting data for the different tasks in time-frames and orders totally uncorrelated with the other ones. We introduce a novel federated learning setting (AFCL) where the continual learning of multiple tasks happens at each client with different orderings and in asynchronous time slots. We tackle this novel task using prototype-based learning, a representation loss, fractal pre-training, and a modified aggregation policy. Our approach, called FedSpace, effectively tackles this task as shown by the results on the CIFAR-100 dataset using 3 different federated splits with 50, 100, and 500 clients, respectively. Abstract

Abstract Although Essential Climate Variables (ECVs) have been widely adopted as important metrics for guiding scientific and policy decisions, the Earth Observation (EO) and Land Surface and Hydrologic Model (LSM/HM) communities have yet to treat terrestrial ECVs in an integrated manner. To develop consistent terrestrial ECVs at regional and continental scales, greater collaboration between EO and LSM/HM communities is needed. An essential first step is assessing the LSM/HM simulation uncertainty. To that end, we introduce a new hydrological reference dataset that comprises a range of 19 existing LSM/HM simulations that represent the current state-of-the-art of our LSM/HMs. Simulations are provided on a daily time step, covering Europe, notably the Rhine and Po river basins, alongside the Tugela river basin in Africa, and are uniformly formatted to allow comparisons across simulations. Furthermore, simulations are comprehensively validated with discharge, evapotranspiration, soil moisture and total water storage anomaly observations. Our dataset provides valuable information to support policy development and serves as a benchmark for generating consistent terrestrial ECVs through the integration of EO products.

Abstract

Abstract Background Chronic pain and inflammation are common features of rheumatic conditions such as Psoriatic Arthritis (PsA) and Axial Spondyloarthritis (axSpA), often needing prolonged medication treatment for effective management. Maintaining drug retention is essential for both achieving disease control and improving patients' quality of life. This study investigates the influence of pain catastrophizing, a psychological response to pain, on the drug retention rates of PsA and axSpA patients. Methods A two-year prospective multicenter observational study involved 135 PsA and 71 axSpA patients. Pain Catastrophizing Scale (PCS) was employed to assess pain catastrophizing. Univariable and multivariable regression analyses were utilized to identify factors associated with drug retention. Results In the PsA group, patients early discontinuing therapy showed higher baseline disease activity as well as higher incidence of comorbid fibromyalgia. Notably, pain catastrophizing, specifically the domains of Helplessness, Magnification, and Rumination, were significantly elevated in PsA patients who interrupted the treatment. Multivariable analysis confirmed pain catastrophizing as an independent predictor of drug suspension within two years. In axSpA, drug discontinuation was associated with female gender, shorter disease duration, higher baseline disease activity as well as elevated levels of pain catastrophizing. Univariable analysis supported the role of pain catastrophizing, including its domains, as predictors of treatment interruption. However, limited events in axSpA patients precluded a multivariate analysis. Conclusion This prospective study emphasizes the impact of pain catastrophizing on drug retention in patients with PsA and axSpA.

Abstract

The motion of line defects (dislocations) has been studied for over 60 years but the maximum speed at which they can move is unresolved. Recent models and atomistic simulations predict the existence of a limiting velocity of dislocation motions between the transonic and subsonic ranges at which the self-energy of dislocation diverges, though they do not deny the possibility of the transonic dislocations. We use femtosecond x-ray radiography to track ultrafast dislocation motion in shock-compressed single-crystal diamond. By visualizing stacking faults extending faster than the slowest sound wave speed of diamond, we show the evidence of partial dislocations at their leading edge moving transonically. Understanding the upper limit of dislocation mobility in crystals is essential to accurately model, predict, and control the mechanical properties of materials under extreme conditions.

Abstract

Nowadays, we face a profound fragmentation of knowledge, which is addressed in distinctive ways, conforming to the specifics of each field of knowledge and having a specific lexicon and ways to interpret reality. To better understand and communicate the complexity of the reality of motor and sports sciences in Italy, it is necessary to study its ontology based on a holistic and comprehensive approach such as System Thinking (ST). So, this preliminary review aimed to verify the coherence of terminology used by Italian academics (who study methodologies related to teaching) and practical communities (who live the reality of daily teaching) in motor and sports sciences, specifically regarding the didactic of physical and sports education. For this purpose, according to Design Based Research (DBR), a first investigation of the interpretative semantics of the discipline's specific terms was accomplished with the support of multiple sources of information such as documents, databases, and brainstorming performed by experts (representing the guiding team and stakeholders from all considered communities). Moreover, the frequency of some terms and keywords considered in European (ERC) and Italian (CUN) legislation was assessed to contextualize better the impact of common practice community keywords on the scientific and regulatory community. Successively, the studies that analyzed the terms from 2000 to 2020 in the leading scientific search engines (SCOPUS and Web of Science) were also investigated. Results indicate that in the CUN and ERC areas, there is a total absence of the most relevant lemma to the community of practice. Furthermore, significant terms such as play, sports education, psychomotricity, and gymnastics are less considered or unconsidered in the specific scientific and didactic fields. Possible solutions for a specific semantic model to reduce the gap are finally hypothesized and presented. Abstract

Context. Supernova remnants (SNR) are one of the main sources of galactic cosmic rays acceleration. This acceleration, believed to happen at the blast wave front, leads to an energy loss at the shock front. This results in the apparent proximity between the blast wave and the contact discontinuity. Aims. In this article, we study the effect that turbulent-like density perturbations of the interstellar medium (ISM) have on the evolution of young SNRs. We focus on the impact such fluctuations have on the SNRs structure and more precisely on the resulting distance between blast wave and contact discontinuity. Since cosmic rays acceleration is necessary to explain this distance, this study indirectly put into question the cosmic rays acceleration at the blast wave front. Methods. We performed a set of purely hydrodynamic three-dimensional simulations without cosmic ray acceleration in a co-expanding frame. We randomly initialised the density variation of the interstellar medium following a Kolmogorov power law. The resulting ratios of radii between blast wave, contact discontinuity and reverse shock are then compared to the astronomical observations. Results. The addition of density perturbation doesnt significantly change the average ratio of radii. However, the simulations show a higher growth of interfacial instabilities in the presence of a turbulent ISM. The resulting deformation of the contact discontinuity could explain the proximity between contact discontinuity and blast wave. They also explain the plateau in the radial distribution of the line of sight velocity associated with the observations of Tycho. Conclusions. The density perturbation of the ISM should not be neglected in the simulation of young SNR as they have an impact comparable to the cosmic rays acceleration on the SNR structure.

Abstract

Ventilation systems constitute an important piece of the industrial facility ecosystems. Creating proper working environmental conditions for humans is crucial, especially in hazardous sites with presence of various gases, such as underground mines. Combined with the vast amount of space to be ventilated in large mines, designing and maintaining such a system is challenging and costly. To alleviate these issues, the EIT-RM project VOT3D (Ventilation Optimizing Technology based on 3D scanning) proposes conducting advanced airflow modeling in the underground tunnel networks, utilizing computational fluid dynamics (CFD) simulations, modern surveying and 3D modeling approaches to reverse engineer a reliable geometric model of the mine and estimate the 3D airflow field inside it. In this paper, we present the challenges to be solved in this task and the proposed workflow to address them. An example related to an active industrial mine in Poland is reported as a basis for performing experimental data processing using the full, highly automatized procedure. Developments and results of underground mobile mapping (with a drone and a handheld system), point cloud processing and filtering, surface reconstruction and CFD modeling are presented. The detailed results of airflow field estimation show the advantages of the proposed solution and promise its high practical usefulness.

Trata-se de uma abordagem realizada na disciplina “Projeto: investigações teórico-práticas”, no Programa de Pós-Graduação em Arquitetura e Urbanismo da Universidade Federal de Santa Catarina. O trabalho apresenta um estudo na praia do Pontal, em Florianópolis, Santa Catarina. Adota-se uma abordagem fenomenológica fundamentada na investigação de características sensoriais do lugar como base para uma intervenção paisagística representada pelo projeto arquitetônico de uma passarela de conexão entre áreas da praia do Pontal. A proposta elaborada evidencia características sensoriais do lugar que se fundem com o processo de projeto em arquitetura, por meio de elementos que se relacionam: qualidades materiais e táteis, referências de luz e sombra, aspectos visuais, particularidades sonoras, dinâmicas antrópicas e relações espaço-tempo. O estudo demonstra que a perspectiva de Juhani Pallasmaa pode contribuir na formação de estudantes, profissionais e pesquisadores, mediante a imersão do arquiteto no lugar, cooperando no desenvolvimento de estratégias projetuais para a definição das experiências que a arquitetura propiciará na relação entre pessoa e ambiente.

Abstract Background Endoplasmic reticulum (ER) stress-mediated increases in the hepatic levels of the very low-density lipoprotein (VLDL) receptor (VLDLR) promote hepatic steatosis by increasing the delivery of triglyceride-rich lipoproteins to the liver. Here, we examined whether the NAD(+)-dependent deacetylase sirtuin 1 (SIRT1) regulates hepatic lipid accumulation by modulating VLDLR levels and the subsequent uptake of triglyceride-rich lipoproteins. Methods Rats fed with fructose in drinking water, Sirt1 −/− mice, mice treated with the ER stressor tunicamycin with or without a SIRT1 activator, and human Huh-7 hepatoma cells transfected with siRNA or exposed to tunicamycin or different inhibitors were used. Results Hepatic SIRT1 protein levels were reduced, while those of VLDLR were upregulated in the rat model of metabolic dysfunction-associated steatotic liver disease (MASLD) induced by fructose-drinking water. Moreover, Sirt1 −/− mice displayed increased hepatic VLDLR levels that were not associated with ER stress, but were accompanied by an increased expression of hypoxia-inducible factor 1α (HIF-1α)-target genes. The pharmacological inhibition or gene knockdown of SIRT1 upregulated VLDLR protein levels in the human Huh-7 hepatoma cell line, with this increase abolished by the pharmacological inhibition of HIF-1α. Finally, SIRT1 activation prevented the increase in hepatic VLDLR protein levels in mice treated with the ER stressor tunicamycin. Conclusions Overall, these findings suggest that SIRT1 attenuates fatty liver development by modulating hepatic VLDLR levels.

Mesothelioma is a type of late‐onset cancer that develops in cells covering the outer surface of organs. Although it can affect the peritoneum, heart, or testicles, it mainly targets the lining of the lungs, making pleural mesothelioma (PMe) the most common and widely studied mesothelioma type. PMe is caused by exposure to fibres of asbestos, which when inhaled leads to inflammation and scarring of the pleura. Despite the ban on asbestos by most Western countries, the incidence of PMe is on the rise, also facilitated by a lack of specific symptomatology and diagnostic methods. Therapeutic options are also limited to mainly palliative care, making this disease untreatable. Here we present an overview of biological aspects underlying PMe by listing genetic and molecular mechanisms behind its onset, aggressive nature, and fast‐paced progression. To this end, we report on the role of deubiquitinase BRCA1‐associated protein‐1 (BAP1), a tumour suppressor gene with a widely acknowledged role in the corrupted signalling and metabolism of PMe. This review aims to enhance our understanding of this devastating malignancy and propel efforts for its investigation.

Abstract

The propagation of a shock wave in solids can stress them to ultra-high pressures of millions of atmospheres. Understanding the behavior of matter at these extreme pressures is essential to describe a wide range of physical phenomena, including the formation of planets, young stars and cores of super-Earths, as well as the behavior of advanced ceramic materials subjected to such stresses. Under megabar (Mbar) pressure, even a solid with high strength exhibits plastic properties, causing the shock wave to split in two. This phenomenon is described by theoretical models, but without direct experimental measurements to confirm them, their validity is still in doubt. Here, we present the results of an experiment in which the evolution of the coupled elastic-plastic wave structure in diamond was directly observed and studied with submicron spatial resolution, using the unique capabilities of the X-ray free-electron laser. The direct measurements allowed, for the first time, the fitting and validation of a strength model for diamond in the range of several Mbar by performing continuum mechanics simulations in 2D geometry. BackgroundSupportive Care is a person-centred approach encompassing non-pharmacological interventions targeted towards persons with dementia to contain the effects of their behavioural disorders, improving their quality of life.AimsTo investigate the effects of lockdown restrictions during the first wave of COVID-19 pandemic on behavioural symptoms of patients involved in a Supportive Care programme in an Italian nursing home.MethodsAnalysis is based on Neuropsychiatric Inventory (NPI) scores and related symptoms data collected before (October/November 2019) and after (July 2020) the introduction of COVID-19 restrictions on a non-random sample of 75 patients living in two units of the facility: 38 involved in a Supportive Care programme and 37 receiving standard care (Control). Group performances were compared over time according to univariate statistics and Latent Class Analysis (LCA).ResultsNPI scores and number of reported symptoms in NPI evaluations increased over time among Supportive Care patients with dementia and decreased in the Control group. Differences are statistically significant. LCA resulted in 3-classes and 5-classes specifications in the two time-occasions.DiscussionSupportive Care patients showed a worsening in behavioural and psychological symptoms after the first pandemic wave, as opposed to the elderly not involved in the programme. LCA showed that patients in the two groups differed according to the combinations of NPI symptoms.ConclusionsThe discontinuation of a Supportive Care programme due to COVID-19 restrictions had strong negative effects on nursing home persons with dementia involved in the programme: Supportive Care interventions are important in controlling the psycho-behavioural symptoms associated with dementia.

Abstract Objective: to analyze the moral sensitivity of Brazilian emergency care nurses according to their personal and work characteristics. Method: this is a quantitative, descriptive, cross-sectional study with a convenience sample. A total of 422 nurses from emergency care services in the five regions of Brazil took part. Sociodemographic and work-related information was collected, as well as the Brazilian version of the Moral Sensitivity Questionnaire. After approval by the Research Ethics Committee, the data was collected using a self-administered form on the Google Forms Platform, organized using Excel software and analyzed using the R language. Results: nurses with longer experience in emergency care services showed higher levels in the interpersonal orientation, moral conflict and moral significance dimensions, while in the professional knowledge dimension, men showed higher levels, as evidenced by items that include confidence in nursing knowledge, intuition, experience and opinion. Conclusion: the differences in the nurses’ moral sensitivity were due to their professional experience. It should be emphasized that valuing the sharing of intergenerational experiences in service could be a possible strategy for fostering moral competencies in the field of practice.

α -1 antitrypsin (A1AT) is a 52 kDa acute-phase glycoprotein belonging to the serine protease inhibitor superfamily (SERPIN). It is primarily synthesized by hepatocytes and to a lesser extent by monocytes, macrophages, intestinal epithelial cells, and bronchial epithelial cells. A1AT is encoded by SERPINA1 locus, also known as PI locus, highly polymorphic with at least 100 allelic variants described and responsible for different A1AT serum levels and function. A1AT inhibits a variety of serine proteinases, but its main target is represented by Neutrophil Elastase (NE). However, recent attention has been directed towards its immune-regulatory and homeostatic activities. A1AT exerts immune-regulatory effects on different cell types involved in innate and adaptive immunity. Additionally, it plays a role in metal and lipid metabolism, contributing to homeostasis. An adequate comprehension of these mechanisms could support the use of A1AT augmentation therapy in many disorders characterized by a chronic immune response. The aim of this review is to provide an up-to-date understanding of the molecular mechanisms and regulatory pathways responsible for immune-regulatory and homeostatic activities of A1AT. This knowledge aims to support the use of A1AT in therapeutic applications. Successivamente sarà illustrato un progetto formativo in corso di svolgimento basato su un modello integrato multi-sportivo, nella sua fase sperimentale inizialmente orientata all'istruttore federale di rugby, il cui successo, sostenibilità, e trasferibilità didattica nei corsi per istruttori di sport giovanile verrà verificata con metodo scientifico, del quale è descritto il protocollo di studio. L’esito dell’iniziativa potrà confermare l’adeguatezza del metodo per raggiungere gli obiettivi della formazione dei professionisti nell’ambito motorio-sportivo e, in forma più estesa, per la promozione della physical literacy dei destinatari finali di suddetta formazione, vale a dire i bambini.

Abstract

ABSTRACT Objective: To understand the practices developed by nurses in primary care in southern Brazil. Method: Qualitative study, with data collection via online interviews, conducted between October 2020 and May 2021, and recorded. 174 nurses from 24 municipalities in southern Brazil participated. Data analysis used inductive thematic analysis. Results: The activity that stood out among nurses was the nurse consultation, for all age groups and health conditions, especially when dealing with chronic disease, prenatal care, attention to women and children, mental health, home visits, and the management of the nursing team and the health unit. Final considerations: This study demonstrated that an excess of responsibilities associated to care and management, added to a lack of balance in the activities common to the team make it difficult for nurses to develop clinical practices.

Abstract

Summary: Background: Sézary syndrome is an extremely rare and fatal cutaneous T-cell lymphoma (CTCL). Mogamulizumab, an anti-CCR4 monoclonal antibody, has recently been associated with increased progression-free survival in a randomized clinical trial in CTCL. We aimed to evaluate OS and prognostic factors in Sézary syndrome, including treatment with mogamulizumab, in a real-life setting. Methods: Data from patients with Sézary (ISCL/EORTC stage IV) and pre-Sézary (stage IIIB) syndrome diagnosed from 2000 to 2020 were obtained from 24 centers in Europe. Age, disease stage, plasma lactate dehydrogenases levels, blood eosinophilia at diagnosis, large-cell transformation and treatment received were analyzed in a multivariable Cox proportional hazard ratio model. This study has been registered in ClinicalTrials (SURPASSe01 study: NCT05206045). Findings: Three hundred and thirty-nine patients were included (58% men, median age at diagnosis of 70 years, Q1-Q3, 61–79): 33 pre-Sézary (9.7% of 339), 296 Sézary syndrome (87.3%), of whom 10 (2.9%) had large-cell transformation. One hundred and ten patients received mogamulizumab. Median follow-up was 58 months (95% confidence interval [CI], 53–68). OS was 46.5% (95% CI, 40.6%–53.3%) at 5 years. Abstract

Summary: Human genetic studies of critical COVID-19 pneumonia have revealed the essential role of type I interferon-dependent innate immunity to SARS-CoV-2 infection. Conversely, an association between the HLA-B∗15:01 allele and asymptomatic SARS-CoV-2 infection in unvaccinated individuals was recently reported, suggesting a contribution of pre-existing T cell-dependent adaptive immunity. We report a lack of association of classical HLA alleles, including HLA-B∗15:01, with pre-omicron asymptomatic SARS-CoV-2 infection in unvaccinated participants in a prospective population-based study in the United States (191 asymptomatic vs. 945 symptomatic COVID-19 cases). Moreover, we found no such association in the international COVID Human Genetic Effort cohort (206 asymptomatic vs. 574 mild or moderate COVID-19 cases and 1,625 severe or critical COVID-19 cases). Finally, in the Human Challenge Characterisation study, the three HLA-B∗15:01 individuals infected with SARS-CoV-2 developed symptoms. As with other acute primary infections studied, no classical HLA alleles favoring an asymptomatic course of SARS-CoV-2 infection were identified.
