Abstract

The acceptable duration of donor warm ischemia time (DWIT) after cardiocirculatory death (DCD) is still debated. We analyzed the biomolecular profile and function during ex vivo lung perfusion (EVLP) of DCD lungs and their correlation with lung transplantation (LuTx) outcomes. Donor data, procurement times, recipient outcomes, and graft function up to 1 year after LuTx were collected. During EVLP, the parameters of graft function and metabolism, perfusate samples to quantify inflammation, glycocalyx breakdown products, coagulation, and endothelial activation markers were obtained. Data were compared to a cohort of extended-criteria donors after brain death (EC-DBD). Eight DBD and seven DCD grafts transplanted after EVLP were analyzed. DCD's DWIT was 201 [188;247] minutes. Donors differed only regarding the duration of mechanical ventilation that was longer in the EC-DBD group. No difference was observed in lung graft function during EVLP. At reperfusion, "wash-out" of inflammatory cells and microthrombi was predominant in DCD grafts. Perfusate biomolecular profile demonstrated marked endothelial activation, characterized by the presence of inflammatory mediators and glycocalyx breakdown products both in DCD and EC-DBD grafts. Early graft function after LuTx was similar between DCD and EC-DBD. DCD lungs exposed to prolonged DWIT represent a potential resource for donation if properly preserved and evaluated.

Abstract

OBJECTIVES This study compares the uniportal with the 3-portal video-assisted thoracic surgery (VATS) by examining the data collected in the Italian VATS Group Database. The primary end point was early postoperative pain; secondary end points were intraoperative and postoperative complications, surgical time, number of dissected lymph nodes and length of stay. METHODS This was an observational, retrospective, cohort, multicentre study on data collected by 49 Italian thoracic units. Inclusion criteria were clinical stage I–II non-small-cell lung cancer, uniportal or 3-portal VATS lobectomy and R0 resection. Exclusion criteria were cT3 disease, previous thoracic malignancy, induction therapy, significant comorbidities and conversion to other techniques. The pain parameter was dichotomized: the numeric rating scale ≤3 described mild pain, whereas the numeric rating scale score >3 described moderate/severe pain. The propensity score-adjusted generalized estimating equation was used to compare the uniportal with 3-portal lobectomy. RESULTS Among 4338 patients enrolled from January 2014 to July 2017, 1980 met the inclusion criteria; 1808 patients underwent 3-portal lobectomy and 172 uniportal surgery. The adjusted generalized estimating equation regression model using the propensity score showed that over time pain decreased in both groups (P CONCLUSIONS Data from the Italian VATS Group Database showed that in clinical practice uniportal lobectomy seems to entail a higher risk of moderate/severe pain on second and third postoperative days.

Abstract

The aim of this project is to evaluate, during ex vivo lung perfusion EVLP, the effects of triiodothyronine T3 on the ischemia-reperfusion injury IRI of the lungs procured from donor after circulatory death in a rat model. Following the 3R strategy, we randomized 35 rats into 7 different groups Table 1. We performed standardized lung procurement and EVLP protocols developed in our Preclinical Research Centre. During EVLP we evaluated the lung function by measuring compliance, mean pulmonary artery pressure, positive end-expiratory pressure, peak inspiratory pressure and total pulmonary vascular resistance. At 60, 120, and 180 minutes of EVLP, we collected Steen solution samples for gas exchange evaluation and dosage of inflammatory mediators, free mitochondrial DNA mtDNA, freeT3 and freeT4. At last we collected tissuesamples for dosage of ATP, T3, mtDNA, lipid peroxide ROS-induced and gene expression. Furthermore, we analysed the basic composition of Steen solution in order to evaluate changing related to our procedures. The concentration of albumin and fT4 in Steen solution doubles that in human blood, while fT3 concentration is tripled. These concentrations were not modified by EVLP. We find increase concentration of flavin mononucleotide FMN in Perfadex, collected after ischemia, and increase NADH concentration inSteen solution samples from Ischemia and EVLP-1,5T3 group; both these findings validate our IRI model.Steen solution samples from EVLP and EVLP-T3 group show a progressive, time-dependant increase of mtDNA. Our results suggest that IRI could be worsened by T3 present in Steen solution; further addition of T3 leads to mitochondrial dysfunction and inflammation. Time-dependant increase of mtDNA suggests that EVLP does not improves IRI. In conclusion, we think that is mandatory to introduce the use of customized Steen solution with recombinant hormone-free albumin, in order to lowerer T3 concentration and test its real effects on IRI of lungs.
