Abstract

Bis(1,2,3,5-dithiadiazolyl) radicals (S2N2C)-X-(CN2S2) containing a heterocyclic bridging group X = piperazine,thiophene,bithiophene were prepared and characterized. The results indicated that piperazidine- and bithhiophene-bridged diradicals crystallize in the monoclinic space group P2(sub 1/n) and associate as cofacial dimers. The latter feature a herringbone stacking of close interdimer S-S interactions along the backbone.

One of the consequences is that a growing number of telemedicine applications are emerging in different health care domains. Also in the area of speech-language (SL) disorders, particularly neurogenic disorders, telemedicine is rapidly gaining interest. In this paper, we place applications for neurogenic SL disorders in a telemedicine taxonomy in order to establish common features along the dimensions of functionality, application, and technology, and their components. Thus, we aim at identifying common features in a wide variety of telemedicine applications and to establish common interests of stakeholders in health care for classified groups of telemedicine applications. This may facilitate decision-making with regard to expansion of innovative products, and give directions to measures needed for upscaling and structural embedding of feasible and effective SL telemedicine applications in health care. Common interests of stakeholders in health care, established using telemedicine taxonomy, is a key factor in decision-making with regard to which telemedicine applications should be given priority for genuine utilization. Priorities of health care institutions, patients, and reimbursement companies are also leading for researchers aiming at solid scientific evidence for the beneficial effects of target applications. That is, although research results tend to indicate the potential of telemedicine in the area of SL pathology, the alleged benefits of most applications have not been confirmed according to the accepted standards for clinical outcome testing as yet. Methodologic obstacles and the lack of adequate speech materials and suitable outcome measures for efficacy and effectiveness testing partially account for this. From the perspective of scientific evidence, the benefits of asynchronous SL telemedicine applications concern data storage and data analyses. To facilitate implementation of telemedicine, there is a call for development of information and communication technology infrastructures that allow feasible applications which meet requirements with regard to licensure and medical privacy laws. For applications with evidence for beneficial effects, we are challenged to develop new business models that apply to a new health care environment. 26. Direct versus Indirect Treatment for Preschool Children who Stutter: The RESTART Randomized Trial

Abstract

Objective Stuttering is a common childhood disorder. There is limited high quality evidence regarding options for best treatment. The aim of the study was to compare the effectiveness of direct treatment with indirect treatment in preschool children who stutter. Methods In this multicenter randomized controlled trial with an 18 month follow-up, preschool children who stutter who were referred for treatment were randomized to direct treatment (Lidcombe Program; n = 99) or indirect treatment (RESTART-DCM treatment; n = 100). Main inclusion criteria were age 3-6 years, >= 3% syllables stuttered (%SS), and time since onset >= 6 months. The primary outcome was the percentage of non-stuttering children at 18 months. Secondary outcomes included stuttering frequency (%SS), stuttering severity ratings by the parents and therapist, severity rating by the child, health-related quality of life, emotional and behavioral problems, and speech attitude. Results Percentage of non-stuttering children for direct treatment was 76.5% (65/85) versus 71.4% (65/91) for indirect treatment (Odds Ratio (OR), 0.6; 95% CI, 0.1-2.4, p = .42). At 3 months, children treated by direct treatment showed a greater decline in % SS (significant interaction time x therapy: beta = -1.89; t(282.82) = -2.807, p = .005). At 18 months, stuttering frequency was 1.2% (SD 2.1) for direct treatment and 1.5% (SD 2.1) for indirect treatment. Direct treatment had slightly better scores on most other secondary outcome measures, but no differences between treatment approaches were significant. Conclusions Direct treatment decreased stuttering more quickly during the first three months of treatment. At 18 months, however, clinical outcomes for direct and indirect treatment were comparable. These results imply that at 18 months post treatment onset, both treatments are roughly equal in treating developmental stuttering in ways that surpass expectations of natural recovery. Abstract

Rapidly evolving technological developments have been influencing our daily lives for a few decades now. In particular information and communication technologies enable more sophisticated and faster ways of communication than ever before. These developments have far reaching consequences for professional, domestic and leisure activities. Use of computers, whether or not with an internet connection, has become very common also in the field of education and health care. The primary benefits in these areas are believed to lie in cost reduction and enhanced efficiency. In this chapter we will focus on the possibilities of technological developments in health care, particularly for patients with Parkinson’s disease. This patient group is believed to benefit considerably from innovative applications of information and communication technologies since the number of parkinsonian patients is dramatically growing due to demographic developments. Moreover, Parkinson’s disease pre-eminently concerns a chronic and progressive illness, increasingly disabling these patients in almost all domains of their lives. In this chapter we will explore how telehealth technology and speech technology relates to the maintenance of their communicative competence.
