Abstract

In hadron therapy, the accelerated ions, interacting with the body of the patient, cause the fragmentation of both projectile and target nuclei. The fragments interact with the human tissues depositing energy both in the entrance channel and in the volume surrounding the tumor. The knowledge of the fragments features is crucial to determine the energy amount deposited in the human body, and - hence - the damage to the organs and to the tissues around the tumor target.

Abstract

Riptide is a detector concept aiming to track fast neutrons. It is based on neutron--proton elastic collisions inside a plastic scintillator, where the neutron momentum can be measured by imaging the scintillation light. More specifically, by stereoscopically imaging the recoil proton tracks, the proposed apparatus provides neutron spectrometry capability and enable the online analysis of the specific energy loss along the track. In principle, the spatial and topological event reconstruction enables particle discrimination, which is a crucial property for neutron detectors. In this contribution, we report the advances on the Riptide detector concept. In particular, we have developed a Geant4 optical simulation to demonstrate the possibility of reconstructing with sufficient precision the tracks and the vertices of neutron interactions inside a plastic scintillator. To realistically model the optics of the scintillation detector, mono-energetic protons were generated inside a $6\times6\times6$ cm$^3$ cubic BC-408 scintillator, and the produced optical photons were propagated and then recorded on a scoring plane corresponding to the surfaces of the cube. The photons were then transported through an optical system to a $2\times2$ cm$^2$ photo sensitive area with 1 Megapixel. Moreover, we have developed two different analysis procedures to reconstruct 3D tracks: one based on data fitting and one on Principal Component Analysis. Abstract

Introduction: Charged Particle Therapy plays a key role in the treatment of deep-seated tumours, because of the advantageous energy deposition culminating in the Bragg peak. However, knowledge of the dose delivered in the entrance channel is limited by the lack of data on the beam and fragmentation of the target. Methods: The FOOT experiment has been designed to measure the cross sections of the nuclear fragmentation of projectile and target with two different detectors: an electronic setup for the identification of Z ≥ 3 fragments and a nuclear emulsion spectrometer for Z ≤ 3 fragments. In this paper, we analyze the data taken by exposing four nuclear emulsion spectrometers, with C and C₂H₄ targets, to 200 MeV/n and 400 MeV/n oxygen beams at GSI Helmholtzzentrum für Schwerionenforschung (Darmstadt, Germany), and we report the charge identification of produced fragments based on the controlled fading induced on nuclear emulsion films. Results: The goal of identifying fragments as heavy as lithium has been achieved. Discussion: The results will contribute to a better understanding of the nuclear fragmentation process in charged particle therapy and have implications for refining treatment planning in the presence of deep-seated tumors.

Abstract

In Charged Particle Therapy (PT) proton or 12C beams are used to treat deep-seated solid tumors exploiting the advantageous characteristics of charged particles energy deposition in matter. For such projectiles, the maximum of the dose is released at the end of the beam range, in the Bragg peak region, where the tumour is located. However, the nuclear interactions of the beam nuclei with the patient tissues can induce the fragmentation of projectiles and/or target nuclei and needs to be carefully taken into account when planning the treatment. In proton treatments, the target fragmentation produces low energy, short range fragments along all the beam path, that deposit a non-negligible dose especially in the first crossed tissues. On the other hand, in treatments performed using 12C, or other (4He or 16O) ions of interest, the main concern is related to the production of long range fragments that can release their dose in the healthy tissues beyond the Bragg peak. Understanding nuclear fragmentation processes is of interest also for radiation protection in human space flight applications, in view of deep space missions. In particular 4He and high-energy charged particles, mainly 12C, 16O, 28Si and 56Fe, provide the main source of absorbed dose in astronauts outside the atmosphere. The nuclear fragmentation properties of the materials used to build the spacecrafts need to be known with high accuracy in order to optimise the shielding against the space radiation. The study of the impact of these processes, which is of interest both for PT and space radioprotection applications, suffers at present from the limited experimental precision achieved on the relevant nuclear cross sections that compromise the reliability of the available computational models. The FOOT (FragmentatiOn Of Target) collaboration, composed of researchers from France, Germany, Italy and Japan, designed an experiment to study these nuclear processes and measure the corresponding fragmentation cross sections. In this work we discuss the physics motivations of FOOT, describing in detail the present detector design and the expected performances, coming from the optimization studies based on accurate FLUKA MC simulations and preliminary beam test results. The measurements planned will be also presented.

Abstract

The main goal of the FOOT (FragmentatiOn Of Target) experiment is the measurement of the differential cross-sections with respect to the energy and angle of the produced fragments in nuclear interactions between a light ion beam, such as proton, helium, carbon, and different targets (proton, carbon, oxygen). Two important fields can benefit from these kinds of measurements: firstly, in the 150-400 MeV/nucleon beam energy range, the data will be used to evaluate the nuclear fragmentation occurring in a standard hadrontherapy treatment and thus to estimate potential secondary effects. Secondly, in the 700-1000 MeV/nucleon beam energy range, the FOOT experiment aims at studying novel shields for spacecrafts involved in long-term missions within the Solar System. The experiment has been funded by INFN and it is currently in its data taking phase. A description of the magnetic spectrometer of the experiment and a first approach to the track reconstruction of detected fragments will be presented.

Abstract

Introduction: Septic shock remains amongst the leading causes of childhood mortality. Therapeutic options to support children with septic shock refractory to initial resuscitation with fluids and inotropes are limited. Recently, the combination of intravenous hydrocortisone with high dose ascorbic acid and thiamine (HAT therapy), postulated to reduce sepsis-related organ dysfunction, has been proposed as a safe approach with potential for mortality benefit, but randomized trials in paediatric patients are lacking. We hypothesize that protocolised early use of HAT therapy ("metabolic resuscitation") in children with septic shock is feasible and will lead to earlier resolution of organ dysfunction. Here, we describe the protocol of the Resuscitation in Paediatric Sepsis Using Metabolic Resuscitation-A Randomized Controlled Pilot Study in the Paediatric Intensive Care Unit (RESPOND PICU). Methods and Analysis: The RESPOND PICU study is an open label randomized-controlled, two-sided multicentre pilot study conducted in paediatric intensive care units (PICUs) in Australia and New Zealand. Sixty children aged between 28 days and 18 years treated with inotropes for presumed septic shock will be randomized in a 1:1 ratio to either metabolic resuscitation (1 mg/kg hydrocortisone q6h, 30 mg/kg ascorbic acid q6h, 4 mg/kg thiamine q12h) or standard septic shock management. Main outcomes include feasibility of the study protocol and survival free of organ dysfunction censored at 28 days. The study cohort will be followed up at 28-days and 6-months post enrolment to assess neurodevelopment, quality of life and functional status. Biobanking will allow ancillary studies on sepsis biomarkers. Ethics and Dissemination: The study received ethical clearance from Children's Health Queensland Human Research Ethics Committee (HREC/18/QCHQ/49168) and commenced enrolment on June 12th, 2019. International audience; The FOOT experiment is a fixed target experiment aiming for high precision (better than 5%) measurement of fragmentation cross section for hadrontherapy and space radioprotection purposes. Both target and projectile fragmentation are studied by using mainly proton, Carbon and Oxygen ion beams in both direct and inverse kinematic regime, alternatively. A good track reconstruction and momentum measurement is of fundamental importance for precise fragment identification and cross section measurement.

Abstract

International audience; The main goal of the FOOT (FragmentatiOn Of Target) experiment is the measurement of the differential cross sections as a function of energy and direction of the produced fragments in the nuclear interaction between a ion beam (proton, helium, carbon, ...) and different targets (proton, carbon, oxygen, ...). Depending on the beam energy, the purpose of the measurements is twofold: in the [150-400] MeV/u range, the data will be used to evaluate the side effects of the nuclear fragmentation in the hadrontherapy treatment, while in the [700-1000] MeV/u range it will be used to optimize the shielding of spaceships for long term space missions. The experiment has been funded by the INFN since September 2017 and it is currently in the construction phase. An overview of the detector, of the results obtained in several beam tests and of the expected performances will be presented.

Abstract

Cytokines play a crucial role in the host's immune response. In melanoma patients, cytokine profiles seems to be related to the clinical course and their imbalance could be associated to tumour progression. Thus, we studied a panel of baseline cytokines and their behaviour during treatment in order to verify their correlation with clinical outcomes. Interleukin-6, interleukin-8, interleukin-10, interleukin-12 and soluble receptor of interleukin-2 were evaluated in 90 out of 176 metastatic melanoma patients enrolled in a phase III study comparing chemotherapy and biochemotherapy. We divided patients into three different groups according to their own cytokine levels (low, intermediate and high) and then we correlated these groups with some clinical features. We also monitored the cytokines during the treatment in a subgroup of 37 patients. In univariate analysis, higher values of interleukin-6 (P = 0.005), soluble receptor of interleukin-2 (P = 0.001) and interleukin-12 (P = 0.010) were correlated with a worse survival. Conversely, interleukin-8 was unable to discriminate patients with different prognoses, and interleukin-10 was undetectable in the majority of patients. In multivariate analysis, only soluble receptor of interleukin-2 maintained its independent role in survival. The impact of baseline cytokines on response was insignificant. Regarding the behaviours of cytokines during treatment, the most remarkable aspect was a progressive increase of interleukin-12 and soluble receptor of interleukin-2 in patients with a better survival. In our metastatic melanoma patients, higher basal levels of interleukin-6, interleukin-12 and soluble receptor of interleukin-2 were associated with a worse survival. In contrast, a progressive increase of interleukin-12 and soluble receptor of interleukin-2 was observed during treatment in patients with a better survival.

Abstract

In a carbon ion treatment the nuclear fragmentation of both target and beam projectiles impacts on the dose released on the tumor and on the surrounding healthy tissues. Carbon ion fragmentation occurring inside the patient body has to be studied in order to take into account this contribution. These data are also important for the development of the range monitoring techniques with charged particles. The production of charged fragments generated by carbon ion beams of 115–353 MeV/u kinetic energy impinging on carbon, oxygen, and hydrogen targets has been measured at the CNAO particle therapy center (Pavia, Italy). The use of thin targets of graphite (C), PMMA (C2O5H8) and polyvinyl-toluene [plastic scintillator (PS), ${\text{C}}_{b}{\text{H}}_{a}$ ] allowed to measure fragments production cross sections, exploiting a time-of-flight (ToF) technique. PS detectors have been used to perform the ToF measurements, while LYSO crystals have been used for the deposited energy measurement and to perform particle identification. Cross sections have been measured at 90° and 60° with respect to the beam direction. The measured proton, deuteron, and triton differential production cross sections on C, O, and H, obtained exploiting the target subtraction strategy, are presented here as a function of the fragment kinetic energy.

Abstract

Introduzione. Diversi studi hanno evidenziato un aumento della sopravvivenza dei nati pretermine con età gestazionale estremamente bassa (e.g. ≤28 settimane) che costituisce un fattore di rischio per lo sviluppo poiché espone il sistema nervoso immaturo e in evoluzione a stimolazioni spesso innaturali e inadeguate del reparto ospedaliero (Als et al., 2004). Inoltre alcuni studi hanno messo in luce che i bambini di e.g. estremamente bassa presentano difficoltà cognitive e di apprendimento in età prescolare e scolare (Wood et al., 2000; Marlow et al., 2005). L’obiettivo è indagare longitudinalmente lo sviluppo psicomotorio dei nati con e.g. estremamente bassa per evidenziare precocemente le difficoltà di questi bambini, confrontati con un gruppo di nati a termine. Metodo. Sono stati esaminati, a 6, 12, 18 e 24 mesi (età corretta per i pretermine), 17 nati pretermine di e.g. ≤28 settimane (e.g. media=27, DS=.79, range 25-28; peso medio=1299g, DS=239, range 555-1310) e 10 nati a termine, tutti esenti da danni neurologici, motori, sensoriali e di madrelingua italiana, mediante l’uso delle Scale Griffiths (1996) che forniscono un punteggio del quoziente di sviluppo generale risultante da 5 sottoscale (locomotoria, personale-sociale, udito-linguaggio, oculomotoria, performance). Risultati. Le analisi della varianza multivariata (RM-MANOVA) hanno mostrato che i nati pretermine presentano punteggi più bassi rispetto ai nati a termine nel quoziente di sviluppo generale [F(1,25) = 6.1; p = .021, ŋ2 = .20] e nelle sottoscale locomotoria [F(1,25) = 10.1; p = .004, ŋ2 = .29], oculomotoria [F(1,25) = 11.1; p = .003, ŋ2 = .31] e di performance [F(1,25) = 4; p = .058, ŋ2 = .14]. Discussione. L’elevata prematurità costituisce un fattore di rischio per lo sviluppo nei primi due anni vita poiché, anche quando si tiene conto del livello di maturazione neurobiologica mediante l’uso dell’età corretta, i bambini di e.g. ≤28 settimane mostrano difficoltà nello sviluppo psicomotorio, che emergono in modo più rilevante nelle competenze motorie, oculo-motorie e cognitive. Questi risultati indicano l’importanza di includere i bambini di e.g. estremamente bassa in programmi di follow-up continuativi nei primi due anni di vita e, se necessario, programmare interventi abilitativi precoci.

Abstract

La nascita pretermine caratterizzata da età gestazionale estremamente bassa (ELGA) costituisce un rischio per lo sviluppo poiché espone il neonato a stimolazioni ambientali inadeguate per un lungo periodo (Als, 2004). Diversi studi hanno evidenziato come sia aumentata la sopravvivenza dei nati pretermine ELGA, ma anche la probabilità di ritardi nello sviluppo (Marlow et al, 2005). Tale sviluppo nei primi due anni di vita non è stato sufficientemente indagato tenendo conto del ruolo dall’età gestazionale. Obiettivo del presente lavoro è valutare gli effetti dell’età gestazionale sullo sviluppo psicomotorio a 18 mesi, confrontando bambini con e.g estremamente bassa (ELGA, < 28 sett.) e molto bassa (VLGA, 29-33 sett.) e bambini nati a termine (NT). Metodo Sono stati esaminati 19 ELGA (e.g media=26.9, range 24-28; peso medio=846g, range 555-1310), 70 VLGA (e.g media=30.6, range 29-33; peso medio=1398g, range 735-2150) e 16 NT. Sono stati esclusi bambini con danni neurologici e sensoriali e/o di madrelingua non italiana. Sono state somministrate ai bambini a 18 mesi (età corretta per i pretermine) le Scale Griffiths (1996) dalle quali si ottiene un punteggio del quoziente di sviluppo generale (GQ). Risultati. Il GQ medio degli ELGA (GQ=91.1, ds=18.6) e dei VLGA (GQ=101.7, ds=12.8) a 18 mesi rientra nella norma. Tuttavia, l’analisi della varianza (ANOVA), utilizzata per confrontare il GQ degli ELGA, dei VLGA e dei NT (GQ=109.9, ds=8.4), ha evidenziato differenze significative tra i tre gruppi con punteggi significativamente inferiori negli ELGA rispetto ai VLGA e ai NT. Discussione. I risultati hanno mostrato come l’e.g. estremamente bassa costituisca un fattore di rischio nello sviluppo, come già evidenziato a 6 e 12 mesi di età corretta (Savini et al., 2006; 2007; 2008), e come all’aumentare del livello di maturazione neurobiologica neonatale (e.g.) si incrementino i punteggi relativi allo sviluppo psicomotorio nei primi due anni di vita. Sul piano clinico, tali risultati indicano l’importanza di attuare programmi di follow-up per monitorare ripetutamente le competenze psicomotorie di questi bambini in fase di acquisizione e programmare, ove necessario, interventi abilitativi precoci finalizzati ad un adeguato sviluppo mentale.

Abstract

The aim was to investigate whether intra-arterial infusion of chemotherapy improves response to treatment in unresectable liver metastases from colorectal cancer. We treated 14 patients (pts) with intra-arterial chemotherapy. Arterial catheters were placed via percutaneous access. Treatment schedule was: 5-FU and mitomycin-C on day 1 every 21 days. Six pts also received from day 3 for 5 days, a continuous intra-arterial 24-hr infusion of interleukin-2 (IL-2). We had only one case of toxicity drug-related > grade 2 (neutropenia). We observed 2 partial response (PR) and 5 stable disease (SD). Median time to disease progression (TTP) and median survival (OS) were, respectively 4 and 15 months.

Abstract

The FOOT (FragmentatiOn Of Target) experiment is an international project designed to carry out the fragmentation cross-sectional measurements relevant for charged particle therapy (CPT), a technique based on the use of charged particle beams for the treatment of deep-seated tumors. The FOOT detector consists of an electronic setup for the identification of Z≥3Z\ge 3 fragments and an emulsion spectrometer for Z≤3Z\le 3 fragments. The first data taking was performed in 2019 at the GSI facility (Darmstadt, Germany). In this study, the charge identification of fragments induced by exposing an emulsion detector, embedding a C2H4{{\rm{C}}}_{2}{{\rm{H}}}_{4} target, to an oxygen ion beam of 200 MeV/n is discussed. The charge identification is based on the controlled fading of nuclear emulsions in order to extend their dynamic range in the ionization response.
