1. HPV vaccine recommendation profiles among a national network of pediatric practitioners: understanding contributors to parental vaccine hesitancy and acceptance

Abstract

Background: Practitioner communication is one of the most important influences and predictors of HPV vaccination uptake. The objective of this study was to conduct a latent class analysis characterizing pediatric practitioner HPV recommendation patterns. Methods: Pediatric practitioners of the American Academy of Pediatrics’ (AAP) Pediatric Research in Office Settings (PROS) national network completed an online survey where they were presented with 5 hypothetical vignettes of well child visits and responded to questions. Questions asked about their use of communication strategies, assessments about the adolescent patient becoming sexually active in the next 2 years for decision-making about HPV vaccine recommendation, and peer norms. Latent class analysis characterized practitioner subgroups based on their response patterns to 10 survey questions. Multinomial logistic regression examined practitioner characteristics associated with each profile. Results: Among 470 respondents, we identified three distinct practitioner HPV vaccine recommendation profiles: (1) Engagers (52%) followed national age-based guidelines, strongly recommended HPV vaccination, and perceived peers as strongly recommending; (2) Protocol Followers (20%) also strongly recommended HPV vaccination, but were less likely to engage families in a discussion about benefits; and (3) Ambivalent HPV Vaccine Recommenders (28%) delayed or did not recommend HPV vaccination and were more likely to use judgment about whether adolescents will become sexually active in the next two years. Practicing in a suburban setting was associated with twice the odds of being an Ambivalent Recommender relative to being an Engager (OR = 2.2; 95% CI:1.1–4.1). Conclusions: Findings underscore the importance of continued efforts to bolster practitioner adoption of evidence-based approaches to HPV vaccine recommendation especially among Ambivalent Recommenders.

16. Power to the paediatricians: the Australian Paediatric Research Network is born

Abstract

In late 2007, we established the Australian Paediatric Research Network (APRN) - a 350 member strong research network - which aims to facilitate high quality research into common, child health conditions seen in secondary care settings. Through this network we hope to engage paediatricians working at 'the coal face' to generate research ideas, take part in projects and ultimately contribute to better health care and policy for Australian children. As for adults, tertiary institutions remain the predominant site of paediatric research. Tertiary research however, is biased towards severe illness, isolated conditions, highly selected patients and single interventions. This is not always relevant to the real world that paediatricians confront every day. The APRN is off to a strong start. We have worked with members to establish their research priorities and conducted a prospective audit of their caseload to inform the relevance, feasibility and design of future APRN studies. We have developed a website (http://www.aprn.org.au) which will house useful tools for conducting research including child health measures and project design needs. We plan to call for member-initiated research proposals on an annual basis and to conduct a 'multi-topic' survey addressing up to six research topics at any one time. Although conducting research in secondary care settings poses time, cost and process challenges, all of these are firmly within the APRN's sights.

Abstract

To the Editor. Drs Pathomvanich, Merke, and Chrousos have presented their experience with an instructive patient—a white girl whose pubic hair growth began shortly after her sixth birthday and breast development approximately a year and a half later. An evaluation did not occur until she was age 8.5, when significant insulin resistance was found. The authors go on to state that the child “fell within the normal range according to the data presented by Herman-Giddens et al.”1 We disagree. At least 3 factors about this girl's findings should have alerted her pediatrician to the need for an endocrine referral. First, the age at which her pubic hair growth began was 2.5 standards deviations (SDs) below the mean according to the data presented in our study. Second, it is less common for white girls to begin pubic hair growth before breast development. Third, the long delay between the onset of pubic hair growth and breast growth (greater than a year) is not typical of the normal pubertal sequence of growth. Abstract

Abstract Purpose  The present study was designed to evaluate the efficacy and safety of a novel, 10% liquid formulation of intravenous immunoglobulin, stabilized with 250 mmol/L l-proline (Privigen®), in patients with primary immunodeficiency disease. Materials and Methods  Eighty adults and children diagnosed with common variable immunodeficiency or X-linked agammaglobulinemia received intravenous Privigen® infusions (200–888 mg/kg) at 3- or 4-week intervals over a 12-month period, according to their previously established maintenance dose. The primary endpoint was the annual rate of acute serious bacterial infections. Results  There were six episodes of acute serious bacterial infections, corresponding to an annual rate of 0.08; the annual rate for all infections was 3.55. Mean serum IgG trough levels were between 8.84 and 10.27 g/L. A total of 1,038 infusions were administered, most of them at the maximum rate permitted (8.0 mg kg−1 min−1). Temporally associated adverse events, possibly or probably related to study drug, occurred in 9% of infusions, either during or within 72 h after infusion end. 20. Comparing the Effects of Physical Practice and Mental Imagery Rehearsal on Learning Basic Venipuncture by Medical Students.

Abstract

Effective and efficient techniques to teach basic clinical skills are much needed in medical education. Many of these skills are psychomotor, and mental imagery rehearsal has been effective in learning psychomotor skills in many fields. We conducted a fully randomized experiment to determine if mental imagery rehearsal was effective compared with physical practice in learning venipuncture (blood drawing) skills by medical students. All participants first received a lecture-demonstration on venipuncture and 30 minutes of guided physical practice on artificial plastic arms. One treatment group received an additional 30 minutes of such practice. Another treatment group received a 30-minute session of guided imagery. A control group received no additional training. Both treatment groups demonstrated improved performance on a live venipuncture compared to the control group and did not significantly differ from each other. Hence, imagery rehearsal offers a very cost-effective method of learning this basic clinical skill. [ABSTRACT FROM AUTHOR]

Abstract

Two experimental programs were performed: an in-flight simulation to examine STOL longitudinal characteristics, and a fixed-base simulation to study control power and control usage in hover and low-speed flight. The former experiment was accomplished by the National Research Council of Canada, and the latter by United Aircraft Research Laboratory. A limited analysis of the data obtained was performed by Calspan and recommendations were made for future experiments to expand the data base for existing and additional requirements.

Aerodynamic, inertial and control data for this class of airplane were obtained from a prototype Concorde package supplied by the FAA. The simulation program involved the examination of 20 configurations by four evaluation pilots. The configurations evaluated were based upon a systematic variation of the longitudinal stability characteristics for this class of airplane. These variations were designed to examine the influence of pitch stiffness, backsideness, pitch damping and nonlinear pitching moment effects on pilot acceptability of minimum longitudinal stability for the landing approach task. A total of 61 evaluations was performed.
