- Abstract
In recent years, traditional power systems have undergone a significant transition, mainly related to the massive penetration of renewable generation. More specifically, the transformation of residential consumers into prosumers has been challenging the existing operation of the electricity market. This transition brings new challenges and opportunities to the power system, leading to new business models. One widely discussed change is related to a consumer-centric or prosumer-driven approach, promoting increased participation of small consumers in power systems. The present paper aims at discussing the recent business models as enablers of the increasing prosumers’ role. To do so, it defines the main features of prosumers and their related regulation as well as possible market designs within power systems. In addition, it discusses enabling technologies to properly create the conditions that sustain new prosumer-driven markets. Then, it presents a comprehensive review of existing and innovative business models and a discussion on their future roles in modern power systems. Moreover, a set of recommendations for promoting these business models in the power system is provided. An important conclusion is that, even though economically possible, not all innovative business models can spread around the world due to regulatory obstacles.

- Abstract
The yellow fever virus (YFV) epidemic in Brazil is the largest in decades. The recent discovery of YFV in Brazilian Aedes species mosquitos highlights a need to monitor the risk of reestablishment of urban YFV transmission in the Americas. We use a suite of epidemiological, spatial, and genomic approaches to characterize YFV transmission. We show that the age and sex distribution of human cases is characteristic of sylvatic transmission. Analysis of YFV cases combined with genomes generated locally reveals an early phase of sylvatic YFV transmission and spatial expansion toward previously YFV-free areas, followed by a rise in viral spillover to humans in late 2016. A Estratégia Saúde da Família (ESF) incorpora os princípios gerais que visam a reorganização da Atenção Básica no Brasil e, neste contexto, o Agente Comunitário de Saúde (ACS) é um trabalhador que faz parte da equipa de saúde da comunidade de uma dada área de residência, realiza orientações às famílias sobre cuidados com a sua própria saúde e também com a saúde da comunidade. Diante da importância do ACS na ESF, o objectivo do presente estudo é conhecer o perfil sócio-demográfico dos Agentes Comunitários de Saúde no município de Goiânia-GO. Estudo quantitativo, realizado no município de Goiânia-GO, teve como base os 475 ACS dos Distritos Sanitários Leste, Campinas-Centro e Noroeste. A colecta de dados para o estudo foi realizada através da aplicação de um questionário de auto-preenchimento elaborado com perguntas abertas e fechadas. A análise das variáveis baseou-se em estatística descritiva, através do cálculo de frequências absolutas e relativas. Os resultados demonstram que o perfil do ACS no município de Goiânia é caracterizado pelo predomínio do sexo feminino, casadas ou em união de facto, com ensino médio ou curso técnico profissionalizante, idade entre os 30 e os 39 anos e de 1 a 5 anos de tempo de profissão.

Por meio de um enfoque integrado da visão sistêmica, terapia familiar, abordagem intergeracional e leitura instrumental mítica, o presente estudo buscou compreender a família de um sujeito com deficiência auditiva, reconhecer mitos familiares, bem como estabelecer relações entre estes mitos e o significado atual por ele atribuído a sua deficiência. Para coleta de dados foram utilizadas entrevistas semi-estruturadas, as quais foram analisadas qualitativamente. Foram identificados os mitos da união, da deficiência auditiva e da luta pela sobrevivência. A repetição da deficiência auditiva em vários membros da família gerou rigidez das pautas de ajuda e cuidado determinadas pelo mito da união e das fronteiras com o mundo externo. Ao identificar os conteúdos que atravessam intergeracionalmente a família, pôde-se garantir a possibilidade de encontrar novos caminhos para lidar com a deficiência, ampliar a visão do contexto e confirmar a necessidade da participação familiar na reabilitação global do sujeito.

OBJETIVO: A real magnitude do problema das mortes maternas em Uberlândia, MG, Brasil, é pouco conhecida, sendo que o objetivo do estudo é conhecer o perfil da mortalidade materna desse município. MÉTODO: Utilizaram-se, como fonte de dados, as declarações de óbito de mulheres falecidas entre 10 e 49 anos de idade, residentes no município de Uberlândia, MG. Entrevistaram-se as famílias dessas mulheres para identificar o estado gestacional no momento do óbito. As mortes caracterizadas como maternas foram investigadas nos serviços de saúde, em prontuários e em entrevistas médicas. Foram analisados os óbitos maternos ocorridos até um ano após o término da gestação, considerando-se: pré-natal, período gestacional, tipo de parto, complicações em gestações anteriores, causa básica da morte, local de ocorrência e possibilidade de se evitar o óbito. Calcularam-se os coeficientes de mortalidade materna (CMM) por 100.000 nascidos vivos. RESULTADOS: Do total de 204 óbitos, analisaram-se 173 de residentes no município. Ocorreram 6 mortes maternas, sendo 4 (66,7%) até 42 dias após o parto e 2 (33,3%) de 43 dias a 1 ano; 5 (83,3%) foram devidas a causas obstétricas diretas (toxemia- 60% e hemorragia - 40%) e 1 (16,7%), indireta (cardiopatia). O CMM a partir das mortes maternas declaradas foi de 22,2 e o corrigido elevou-se para 66,6. CONCLUSÕES: O CMM encontrado está acima do aceitável. Faz-se necessária uma análise acurada da qualidade do pré-natal e do parto, bem como do registro das declarações de óbito.

- Abstract
Yellow fever (YF) presents a wide spectrum of severity, with clinical manifestations in humans ranging from febrile and self-limited to fatal cases. Although YF is an old disease for which an effective and safe vaccine exists, little is known about the viral- and host-specific mechanisms that contribute to liver pathology. Several studies have demonstrated that oxidative stress triggered by viral infections contributes to pathogenesis. We evaluated whether yellow fever virus (YFV), when infecting human hepatocytes cells, could trigger an imbalance in redox homeostasis, culminating in oxidative stress. YFV infection resulted in a significant increase in reactive oxygen species (ROS) levels from 2 to 4 days post infection (dpi). When measuring oxidative parameters at 4 dpi, YFV infection caused oxidative damage to lipids, proteins, and DNA, evidenced by an increase in lipid peroxidation/8-isoprostane, carbonyl protein, and 8-hydroxy-2'-deoxyguanosine, respectively. Furthermore, there was a significant reduction in the activity of the antioxidant enzymes superoxide dismutase (SOD) and glutathione peroxidase (GPx), in addition to a reduction in the ratio of reduced to oxidized glutathione (GSH/GSSG), indicating a pro-oxidant environment. However, no changes were observed in the enzymatic activity of the enzyme catalase (CAT) or in the gene expression of SOD isoforms (1/2/3), CAT, or GPx. Therefore, our results show that YFV infection generates an imbalance in redox homeostasis, with the overproduction of ROS and depletion of antioxidant enzymes, which induces oxidative damage to cellular constituents. 20. Characterizing the Severity of SARS-CoV-2 Variants at a Single Pediatric Center.

Since March 2020, SARS-CoV-2 has plagued the world with COVID-19 and individuals of all ages have experienced varying symptoms of disease. Older adults were experiencing more severe disease compared to children and were prioritized by vaccination efforts. While biologic therapies and vaccinations were implemented, there were changes in public health restrictions with subsequent surges resulting in more infected children. During these surges there was a rise of different SARS-CoV-2 variants with the dominant variant initially alpha (B.1.1.7 and other Pango lineages) and epsilon (B.1.427/B.1.429) in early 2021 and a dramatic shift to delta (B.1.617.2 and other Pango lineages) by mid-summer 2021. In this study we aimed to characterize the clinical severity and host factors associated with disease by SARS-CoV-2 variant and evaluate if there are differences in disease severity by circulating variant. We retrospectively included all individuals 0-25 years of age who presented to our center and had a positive SARS-CoV-2 RT-PCR, SARS-CoV-2 variant mutation testing, and documented clinical notes from 1 January 2021 through 31 December 2021. We identified 745 individuals who met inclusion criteria and found the delta variant was associated with severe/critical disease compared to the other variants studied. The results of the model showed that underlying respiratory disease and diabetes were risk factors for progression to severe disease. Background: Allergen-specific immunotherapy is a potentially disease-modifying therapy that is effective for the treatment of patients with allergic diseases. Although the pain caused by the administration of subcutaneous immunotherapy with allergens (SCITA) is considered to be minimal, no studies assessing that pain for the treatment of only pediatric patients have been reported. Objectives : This research aimed to evaluate the pain associated with SCITA for pediatric patients followed at our Immunoallergology Department., Methods: During four consecutive weeks, the nurse who administered the injection completed a questionnaire recording the child's assessment of the pain associated with SCITA; these questionnaires were randomized before any analyses were done. Two different pain evaluation scales were used, with the choice of scale being determined based on the child's age: the self-reporting faces scale (score: 0-10; 5 to 8 years old) and the numeric scale (score: 0-10; >8 years old). Demographic and clinical data, as well as any adverse reactions, were documented., Results: We included 46 pediatric patients (mean age: 12.3 ± 2.6 years; 69.5% male), most of whom were suffering from rhinitis/rhinoconjunctivitis and undergoing subcutaneous immunotherapy with mites. Seven local adverse reactions were recorded, and all were mild. Ten patients did not mention any pain associated with SCITA. Of the 36 patients who mentioned some pain, 33 mentioned mild pain (scores between 1 and 3); only three mentioned moderate pain (scores between 4 and 6). For both scales, the median score obtained was 1. The maximum pain reported had a score of 6. No significant differences were observed between different groups of patients., Conclusions: In this study, SCITA was shown to be a mildly painful procedure that is associated with only a few local reactions. Therefore, SCITA should be considered as a safe option for the treatment of most pediatric patients suffering from allergies.

- Abstract
Background: Inconclusive results of serological diagnosis in Chagas disease have an important impact on blood banks worldwide, reflecting in the high number of discarded bags or in an increased transmission by blood transfusion. Molecular techniques such as qPCR have been used for diagnosis and to monitor Trypanosoma cruzi load in peripheral blood samples. A promising perspective refers to the possibility of parasite DNA detection in serum, taking advantage in using the same samples collected for serological screening., Methods: In order to evaluate the effectiveness of a qPCR strategy for detecting and quantifying T. cruzi DNA in serum, we selected 40 chronic Chagas disease patients presenting different clinical manifestations: Cardiac (23), Digestive (4), Mixed form [cardiodigestive] (7), and asymptomatic (6). Twenty seronegative individuals from non-endemic areas were included as controls. Samples were extracted using QIAamp DNA mini kit (QIAGEN) and qPCR was performed in a multiplex format with TaqMan probes for the nuclear satellite DNA of T. cruzi and for the human RNase P gene. In addition, DNA migration to serum during blood coagulation was assessed using a commercial exogenous control (Exo IPC, Applied Biosystems) in a separate qPCR reaction., Results: The comparative duplex qPCR analysis revealed that, even with an increase in Ct values, it was possible to detect all DNA targets in serum. In addition, the same linearity range for T. cruzi quantification (from 10(5) to 0.5 par. eq./mL) between serum, blood or culture samples (T. cruzi epimastigotes - Cl Brener strain) was found. When patient samples were evaluated, no significant differences in parasite load between the distinct clinical manifestations were found for both blood and serum samples. Moreover, median values of parasite burden were 1.125 and 1.230 par. eq./mL for serum and blood, respectively. Using serology as gold standard, we found 95% sensitivity for T. cruzi detection in serum and 97.5% for blood, and 100% specificity for both samples., Conclusions: Taken together, our data indicate the potential of using serum samples for molecular diagnosis and parasite load quantification by qPCR, suggesting its use in reference laboratories for the diagnosis of Chagas disease patients.

24. Effects of electroacupuncture on stress-related symptoms in medical students: a randomised placebo-controlled study.

- Abstract
Objective: To assess the effects of electroacupuncture (EA) on relieving stress-related symptoms-sleep disorders, anxiety, depression and burnout-in medical students., Methods: Eighty-two students were randomised into an EA treatment group (n=30), a sham TENS group (n=18) and an untreated control group (n=34). EA was applied at a continuous frequency of 2 Hz to the limbs, face, ears and scalp for 20 min once a week, over 6-8 weeks. Sham transcutaneous electrical nerve stimulation (TENS) was performed on similar sites for the same number of times in each session and for the same length of time. Outcome measurements included a comparison of the indices obtained by different self-applied questionnaires before and after treatment. The surveys used were the Mini-Sleep Questionnaire (MSQ), Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), the Beck Depression and Anxiety Inventories (BDI and BAI) and the Maslach Burnout Inventory-Student Survey (MBI-SS), in addition to the Quality of Life Questionnaire-abbreviated version (WHOQOL-bref)., Results: EA significantly improved scores on the MSQ, PSQI, BDI and the cynicism and academic efficacy (AE) dimensions of the MBI-SS in relation to the control. Sleep quality (MSQ) improved from 36.9 (SD 7.6) to 25.0 (5.7) with EA, 37.6 (6.0) to 32.1 (6.9) with sham TENS, and 36.5 (5.9) to 33.6 (6.7) in the controls (p=0.0000). Compared with the sham TENS group, EA significantly reduced scores on the PSQI. Score improvements in the sham TENS group in relation to control group were significant in the MSQ, BDI and AE. In the EA group, the number of students with better scores after intervention was significantly higher for the MSQ, PSQI, ESS and BAI. This only occurred for the MSQ in the sham TENS group and for the MSQ and ESS in the control group., Conclusions: EA significantly reduced stress-related mental symptoms. The improvement obtained by sham TENS compared with the control group confirmed the presence of a placebo effect resulting from the treatment ritual.

- Abstract
Introduction: The real magnitude of maternal mortality in the city of Uberlândia, Brazil, is fairly unknown. The aim is to identify the characteristics of maternal mortality in the city during 1997., Methods: Death certificates of women between 10-49 years old from Uberlândia were used as a main data source with additional interviews with family members. Only women between 10- 49 years of age at time of death were included. Maternal deaths were further confirmed at the health services level by checking medical records and interviewing physicians. All maternal deaths occurring up to one year after delivery were investigated. Data was collected on delivery conditions, frequency of prenatal care visits, gestational age at death, previous pregnancy complications, site of death, and any avoidable conditions. Maternal mortality rates (MMR) per 100,000 newborns were calculated., Results: There were a total of 204 deaths, but only 173 were from Uberlândia residents. Six maternal deaths were registered, four (66.7%) up to 42 days after delivery, and two (33.3%) up to 43 days to 1 year. Direct obstetric causes (toxemia- 60%; haemorrhage- 40%) were associated with 5 deaths (83.3%), and indirect (16.7%) with 1 death (cardiac disease). The MMR calculated from death certificates was 22.2 but the adjusted rate was 66.6., Conclusions: The MMR is above the expected. It is necessary to pay more attention to the quality of prenatal and delivery care as well as to the correct way of filling out death certificates.

26. [Vitamin A deficiency in children of rural areas of the semi-arid region in Bahia].

- Abstract
The distribution and magnitude of vitamin A deficiency and dietary consumption of 161 children at 6 to 72 months of age in rural zones in Cansancao-Bahia-Brazil were evaluated. The serum retinol levels were measured by the spectrophotometric method (Bassey-Lowry modified by Araujo and Flores). The serum retinol average was found to be distributed homogeneously throughout the different age groups. Inadequate serum retinol levels (<20,0 mu g/dl) were detected in 44.7% of the children, which characterized the deficiency as constituting a public health problem. The serum retinol levels showed no statistically significant association as between the sex and age of the children; however the children of less than 24 months showed a higher prevalence of inadequate serum retinol levels. The main available source of vitamin A for these children was represented by carotenoids, especially beta-carotene. Foods regarded as being rich in vitamin A were consumed by all age groups. The greatest diversification of consumption of foodstuffs with moderate and low vitamin A content was observed in the group of children of from 24 to 72 months of age, through this was no guarantee of adequate serum retinol levels in this group however.
