Abstract

We study the photocurrent induced by pulsed-light illumination (pulse duration is several nanoseconds) of single-crystal diamond containing nitrogen impurities. Application of additional continuous-wave light of the same wavelength quenches pulsed photocurrent. Characterization of the optically quenched photocurrent and its recovery is important for the development of diamond based electronics and sensing.

Abstract

Pick's disease is often thought of as being a degenerative disease involving the frontal lobes of the brain, and indeed it is often a dementing illness characterized by frontal lobe signs. Yet, even the first case described by Pick himself involved the left temporal lobe and was characterized by aphasia (impaired speech). The authors present the case of a 59-year-old physician with deteriorating ability to use words, although his speech was fluent and his comprehension appeared to be intact. Death resulting from pneumonia provided an opportunity for autopsy and the confirmation of Pick's disease. At the time of his initial presentation, the patient reported noticing a decline in his ability to choose the proper word over a three-year period. Six months later, he retired from his practice because of his difficulty naming things, but there was no sign of deterioration of his other skills. In fact, his skill at performing surgical procedures was intact at the time of retirement. After retirement he played tennis with his friends with no signs of loss of skill, and managed his investment portfolio without error. The patient developed psychiatric problems after a car accident, which occurred about two years after his initial presentation, and he died of pneumonia four months later. Gross examination of the patient's brain revealed the presence of atrophy, which was more pronounced on the left than the right hemisphere. Neurofibrillary tangles, which are common in Alzheimer's disease and other brain diseases, were not seen. Neuritic plaques, a characteristic feature of Alzheimer's disease, were not present. However, Pick's bodies (round structures visible in the cytoplasm of affected neurons) were identified, confirming that this case was indeed Pick's disease. The authors suggest that in cases of aphasia, particularly when anomia (the inability to name objects) is the primary symptom, Pick's disease be considered as a possible diagnosis. Abstract

Soft tissue localization of radiotracer on bone scintigraphy may result in false-positive interpretations, if the activity overlies a bony structure. A case is presented in which special views and follow-up clarified the soft tissue nature of focal uptake in arm and axilla.

Abstract

Dipyridamole has liberalized referrals for stress TI-201 chloride (thallium) studies at the Iowa City Veterans Administration Medical Center. Seventy-five percent of referrals now receive dipyridamole and, unlike patients who tolerate conventional exercise testing, these patients are often quite debilitated. Therefore, the hemodynamic consequences of dipyridamole were reviewed in 120 consecutive, physically-limited patients referred for thallium scintigraphy following an average oral dose of 5.4 mg/kg. Each patient's blood pressure was measured every 5 minutes for 1 hour after dipyridamole and compared with several clinical factors to determine if blood pressure change was predictable. In all patients, blood pressure changed from 136 +/- 21/83 +/- 15 (mean +/- 1 SD) to 117 +/- 25/72 +/- 15 following dipyridamole administration. One hundred nine of the 120 patients had a blood pressure decline from 137 +/- 21/82 +/- 12 to 113 +/- 21/70 +/- 13. Of the 109, 43% (N = 47) had a systolic blood pressure decline greater than 20 mmHg, 16% (n = 18) greater than 40 mmHg, and 13% (n = 14) greater than 50 mmHg. Thirteen percent (n = 14) required emergent reversal of the dipyridamole with aminophylline. Significant hypotension is relatively common but generally unpredictable after oral dipyridamole. Therefore, patient eligibility criteria should be carefully considered; strict hemodynamic monitoring must be routine in the usual patient undergoing thallium scintigraphy after oral dipyridamole challenge.

Abstract

Physicians in postgraduate training are expected to learn research methods but how best to achieve that curricular goal is unclear. This article describes a novel educational approach to develop research skills among infectious disease fellows. Five infectious disease fellows and two faculty members participated in a collaborative research project as a vehicle for active, problem-based learning. During the learning experience several tasks with specific learning objectives were achieved. The authors evaluated the weaknesses and strengths of the collaborative research project as an educational program. This problem-based approach for learning research methods seems more effective than traditional methods and may be applicable to a broad range of training programs. [ABSTRACT FROM AUTHOR]
