China is experiencing a rapid increase in the number of HIV-infected women. In this study, we describe the development and preliminary evaluation of an intervention tailored for Chinese HIV-infected women and caregivers to improve their self- and family management, with goals of enhancing their physical quality of life (QOL) and decreasing their depressive symptomatology. Forty-one HIV-infected women and their caregivers were recruited from two premier Chinese hospitals from July 2014 through March 2016. Participants were randomized to either the control or intervention arm for the Self- and Family Management Intervention (SAFMI). Each study dyad in the intervention arm received three counseling sessions with a nurse interventionist. At baseline, immediate post-intervention (month 1) and follow-up (month 3), the participants were assessed by a self-reported survey. Generalized Hierarchical Linear Modeling was used to evaluate the efficacy of the intervention. Chinese HIV-infected women in the intervention arm had significantly higher probability of higher physical QOL at month 1 and lower probability of clinically meaningful depressive symptomatology at month 3 compared with women in the control arm. In contrast, the effects of the intervention were less salient for caregivers. This study represents one of the first in China to include family caregivers in HIV management. Feasibility and acceptability were high, in that family members were willing to join the study, learn about HIV, and practice new skills to support the HIV-infected women in their lives. A larger trial is needed to fully evaluate this intervention which shows promising preliminary effects in promoting physical QOL and decreasing depressive symptomatology among Chinese HIV-infected women.

Aim: To evaluate knowledge and performance levels regarding intravenous (IV) medication administration to patients among nurses working in paediatric centers of Nepal. Design: A cross‐sectional descriptive observational study. Methods: Enumerative sampling method was used for participant selection from the four hospitals having paediatric units. Data were collected using knowledge survey and observational checklist on four phases of IV medication administration. Results: Of 115 nurses, 14 (12.2%) had adequate knowledge about IV medication administration whereas none had good level and only 20 nurses (17.4%) had fair level of performance. There was a weak negative correlation between knowledge and practice scores. Nurses working 8‐h shifts had better performance than those working 6‐h shifts; however, the recommended nurse–patient ratio was not maintained in 80% of observed shifts. The findings highlight the importance of upgrading nurses' knowledge and professional competencies on medication administration to improve the quality of patient care. [ABSTRACT FROM AUTHOR]

The Middle East and North Africa (MENA) today is an epicenter for the theorization of absence. Scholars, in efforts to counter the politics that have erased and continue to erase humans and places from the region, frequently assert the microdynamics of resistance, resilience, and memory. The editors of the special section, "Following Absence," acknowledge the urgent need for narratives of overcoming destruction and for acts of care and repair. However, this introduction articulates a different intellectual labor—one that examines the ways absences linger and perdure across the MENA. Rather than restoring or recuperating modes of presence, the editors ask, in what ways might absence be told and accounted for? How might new plotlines widen the scope of what can be perceived as absence, its work in the world, and the forms of agency and injury it shapes? In dialogue with the environmental humanities, this thematic section approaches human lives in embodied proximities to the nonhuman. It also relinquishes the need for storytelling to begin from a moment when annihilating violence has ceased, as though erasure could be safely contained in the past. Repetitive and accumulating, recursive and anticipatory, absences inhabit the present and future. Acknowledging these branching, seeping plotlines, the editors urge against any unified theory of absence for, or from, the MENA. Absence is defined by negation, fragments, and residues. By refusing its systematization in scholarship, this introduction calls for modes of attention that refuse the logics of power and foreground instead the flourishing of ongoing life. ObjectiveRegular HIV RNA testing for all HIV-positive patients on antiretroviral therapy (ART) is expensive and has low yield since most tests are undetectable. Selective testing of those at higher risk of failure may improve efficiency. We investigated whether a novel analysis of adherence data could correctly classify virological failure and potentially inform a selective testing strategy.DesignMultisite prospective cohort consortium.MethodsWe evaluated longitudinal data on 1478 adult patients treated with ART and monitored using the Medication Event Monitoring System (MEMS) in 16 US cohorts contributing to the MACH14 consortium. Because the relationship between adherence and virological failure is complex and heterogeneous, we applied a machine-learning algorithm (Super Learner) to build a model for classifying failure and evaluated its performance using cross-validation.ResultsApplication of the Super Learner algorithm to MEMS data, combined with data on CD4 T-cell counts and ART regimen, significantly improved classification of virological failure over a single MEMS adherence measure. Area under the receiver operating characteristic curve, evaluated on data not used in model fitting, was 0.78 (95% confidence interval: 0.75 to 0.80) and 0.79 (95% confidence interval: 0.76 to 0.81) for failure defined as single HIV RNA level >1000 copies per milliliter or >400 copies per milliliter, respectively. Our results suggest that 25%-31% of viral load tests could be avoided while maintaining sensitivity for failure detection at or above 95%, for a cost savings of $16-$29 per person-month.ConclusionsOur findings provide initial proof of concept for the potential use of electronic medication adherence data to reduce costs through behavior-driven HIV RNA testing.

BackgroundEffective and easy to implement interventions to improve adherence to antiretroviral therapy are needed.ObjectiveTo compare site nurse-initiated adherence and symptom support telephone calls for HIV-positive individuals starting antiretroviral therapy to the study site's standard of care.MethodsA randomized controlled trial of site nurse-initiated adherence and symptom support telephone calls for HIV-positive individuals starting antiretroviral therapy. Subjects were randomized to receive site nurse-initiated telephone calls (intervention) or no additional calls to the site's standard of care (control). Subjects received calls 1 to 3 days after initiating antiretrovirals, on weeks 1, 2, 3, 6, 10, 14, 18, 22, and 26, and every 8 weeks thereafter. Self-reported adherence was captured during study visits.ResultsA total of 333 subjects starting antiretrovirals as part of ACTG 384 were co-enrolled into ACTG 5031. Subjects were followed for up to 160 weeks and were contacted for 74% of scheduled calls. There was no significant difference in proportion of patients with ≯95% mean total adherence (87.9% and 91.2%; P = .34) and mean self-reported total adherence (97.9% and 98.4%) in the intervention and control groups, respectively, or in symptom distress and clinical endpoints.ConclusionsIn the context of a clinical trial where self-reported adherence was exceptionally high, the site nurse-initiated telephone calls did not further improve self-reported adherence, symptom distress, or clinical outcomes.

Supervised physical activity can increase functional capacity in persons with HIV (PWH); however, aerobic interventions have shown little improvement in overall physical activity in PWH. In response, we sought to assess the effect of wearing a fitness tracker (FitBit) paired with walk step reminders delivered through an mHealth application to improve physical activity and decreasing body mass index among PWH in New York City. There was no significant difference in the frequency of walk steps between participants in the control group and intervention group from baseline to 6-month follow-up. These findings show that walk step reminders alone were inadequate for sustained improvement of physical activity. This study highlights the need to develop and test the comparative efficacy of physical activity interventions that are tailored to the unique needs and capabilities of PWH. Future interventions should incorporate fitness tracking with tailored interventions focused on the promotion of physical activity. [ABSTRACT FROM AUTHOR]
